1.Clinical result of limited arthroscopic debridement for knee osteoarthritis under local anesthesia

Objective To evaluate the effect of limited arthroscopic debridement under local anesthesia for knee osteoarthritis. Methods A retrospective analysis was made including a total of 63 patients (77 knees ) with knee osteoarthritis.There were 11 male and 52 female,with an average age of 56.5 years old(range from 38~71 years old).The limited arthroscopic debridement included removal of loose bodies hyperplastic synovum,degenerated cartilage and the injured meniscus.The average follow-up period was 2.1 years (from 6 month to 4 year).Outcomes were assessed by JOA knee function score and rate of satisfactory. Results During the follow-up,the patients showed significant improvement of knee function score from 41 point to 78 point.Clinical outcomes of 77 knees were excellent in 31 knee,good in 28 knee,fair in 12 knee,poor in 6 knee.The rate of good and excellent results was 76.6%. Conclusions Limited arthroscopic debridement under local anesthesia for knee osteoarthritis is a relative simple and effective method.It could alleviate pain and improve function of the paticents.
2.Effect of Ondansetron on the Anesthesia of Spinal-epidural Anesthesia in Cesarean Section Surgery

OBJECTIVE:To observe the effect of ondansetron on the anesthesia of cesarean section surgery with spinal-epidur-al anesthesia. METHODS:A total of 60 singletons full-term pregnancy were randomly divided into test group and control group. Test group was given 6% Hetastarch(130/0.4)electrolyte injection 500 ml by intravenous infusion 30 min before anesthesia,and Ondansetreon hydrochloric acid injection 4 ml by intravenous infusion 5 min before anesthesia;control group was given 6% Hetas-tarch(130/0.4)electrolyte injection 500 ml by intravenous infusion 30 min before anesthesia,and 0.9% Sodium chloride injection 4 ml by intravenous infusion 5 min before anesthesia. The clinic data was recorded,including the mean arterial pressure (MAP) and heart rate(HR)before anesthesia puncture(T1),after anesthesia maternal left side(T2),after fetal childbirth(T3)and at the end of surgery(T4),Apgar score and incidence of adverse reactions. RESULTS:MAP in control group at T2,T3 was obviously low-er than T1 and test group,HR was obviously higher than T1 and test group,the difference was statistically significant(P<0.05);there were no significant differences in the MAP and HR in test group at each time point(P>0.05). Apgar score of newborn after 1 min birth in test group was significantly higher than control group,the difference was statistically significant (P<0.05),and there was no significant difference in the Apgar score of newborn after 5 min birth between 2 groups(P>0.05). The ADR incidence in test group was significantly lower than control group(P<0.05). CONCLUSIONS:Ondansetron can effectively reduce the inci-dences of vomit vomitting and hypotension in on the cesarean section surgery with spinal-epidural anesthesia,with good safety.
3.Prognostic value of initial arterial lactate and 6-h lactate clearance rate in acute paraquat poisoning

Objective To investigate the correlation of the initial arterial lactate (Lac) and 6-h lactate clearance rate (LCR) with prognosis in patients with acute paraquat poisoning (APP).Methods The included 132 APP patients (70 males and 62 females,age ranging 15-71,30 (18) [M (IQR)]treated at a single center between January 2009 and December 2013.Patients were divided into two groups:group A,survivors > 28 days after admission (n =64); and group B,those died ≤ 28 days after admission (n =68).The prognostic values of arterial Lac and 6-h LCR during the acute stage of poisoning were evaluated.Results Paraquat dose ranged from 5-200 mL,20 mL (37.25 mL) [M (IQR)].The average time from poisoning to arrival at the emergency department was ranging 4-312 h,6 h (8.75h) [M (IQR)].Total mortality was 51.51%.There were no differences in age,gender,and length of time elapsed from poisoning to diagnosis between two groups.Survivors had a significantly lower dose of paraquat ingested compared with nonsurvivors (P < 0.05).An ROC curve analysis determined that the dose had an area of 0.86 (95% CI:0.80-0.92) and the volume cut-off point was 27.5 mL to predict the prognosis in patients with acute paraquat poisoning (75% sensitivity,85.9% specificity,Youden index 0.609).The initial arterial lactate level was higher in nonsurvivors than that in survivors.The ROC curve analysis indicated that the initial arterial lactate level had an area of 0.99 (95% CI:0.99-1.00) and the concentration cut-off point was 5.050 mmoL/L to predicti prognosis in patients with acute paraquat poisoning (sensitivity 98.5%,specificity 100%,Youden index 0.985).The 6 h LCR was lower in nonsurvivors than that in survivors.ROC curve analysis showed that the 6-h LCR had an area of 0.99 (95 % CI:0.97-1.00) and the concentration cut-off point was 17.28% to predict prognosis in APP patients (sensitivity 100%,specificity 97.1%,Youden index 0.971).Conclusions In the early stages of APP,initial arterial Lac and 6-h LCR are closely related to prognosis and may serve as prognostic factors.
4.Reconstruction of anti-hepatocellular carcinoma scFv pharmacy and its killing effect on hepatocardnoma cell in vitro

objective To observe whether the killing effect on HCC SMMC-7721 cell of the antihepatocellular carcinoma scFv reconstructed by pharmacy was enhanced or not.Methods Prokarycytic expression vector containing PET32a-RC-RNase was induced to express by IPTG.The inclusion body purified and Western-blotting was used.PC.CHOL and CHS was added in chloroform.Dry membrane was formed after chloroform was removed.RC-RNase protein solution was added to dissolute the membrane.Then pass the solution over a Sephadex G-50 column after ultrasound and filtrated to detect the encapsulation efficiency of the liposome.The solution reacted in EDC.SSNHS and MES for 30 minutes.Then add hdscFv to the solution in 4 ℃ over night.MTT method was used to detect the killing effect on HCC cell of immunoliposome RC-RNase,immunotoxin RC-RNase and liposome RC-RNase in vitro.Resuits The killing effect on HCC cell of immunoliposome RC-RNase is the best.but that of Iiposome RC-RNase is the worst.The respective JC50 are:3.28μg/ml,22.44μg/ml and 98.26μg/ml.Conclusion The anti-hepatocellular carcinoma scFv relomtructed by pharmacy can promote the killing effect on HCC cell and may have potential in the treatment of hepatocarcinoma.
5.The research of acetated Ringer solution and succinylated gelatin in the fluid resuscitation of sepsis patients

Objective To research the clinical significance of acetated Ringer solution and succinylated gelatin in the fluid resuscitation of sepsis patients.Method Fifty-nine sepsis patients were divided into two groups:lactic acid Ringer solution and hetastarch (group A,27 cases)and acetated Ringer solution and suecinylated gelatin (group B,32 cases),compared the fluid resuscitation and other parameter.Results There were no significant difference in the central venous pressure,mean arterial pressure and the dose of noradrenaline (P > 0.05).The activated partial thrombeplastin time in group A was longer than that in group B[(58±10),(74±13)s vs(48±7),(54 ±11) s in 6,24 hours],fibrinogen concentration was lower than that in group B [(3.3 ±0.8),(1.6±0.3)g/L vs (4.2±1.1),(2.1±0.2)g/L in 6,24 hours] (P<0.05).Serum lactic acid,creatinine,calcium concentration in group A [(6.9±0.8)retool/L,(289 ~ 27)μ mol/L,(2.1 ±0.3)mmol/L]were higher than those in group B [(3.2 ±1.1)mmol/L, (193 ±42)μmol/L, (1.7±0.2)mmol/L](P<0.05),there was no difference in APACHE Ⅲ scores on the third day between two groups (P>0.05).Conclusions Acetated Ringer solution and succinylated gelatin in the fluid resuscitation of sepsis patients has the same effect,but fewer impacts on serum coagulation,serum lactic acid and renal function than lactic acid Ringer solution and hetastareh.They may be more suitable for fluid resuscitation of sepsis patients,but hypecalcemia is easy to happen,and there is no significant evidences proving the prognosis.
6.The characterisitics of temperature/pH sensitive block copolymer micelles in vitro.

The dialysis method was employed to prepare blank and doxorubicin (DOX) loaded micelles formed by temperature- and pH- sensitive polyhistidine-co-polyDL-lactide-co-glycolide-co-polyethyleneglycol-co-polyDL-lactide-co-glycolide-co-polyhistidine (PHis-b-PLGA-b-PEG-b-PLGA-b-PHis). The critical micelle concentrations (CMC) of the copolymers were measured with Pyrene Fluorescent Probe Technique. The temperature- and pH- sensitive properties of the blank micelles solution were investigated by optical transmittance measurement. The morphology and diameter of DOX micelles were characterized by transmission electron microscopy (TEM) and dynamic light scattering (DLS). The entrapment rate and drug-loading rate were determined with dialysis method. The in vitro release study was further performed to examine the temperature- and pH-responsive drug release behavior from DOX-loaded micelles. The results indicated that the CMC, entrapment efficiency and drug-loaded amount of the micelles were 7.5 x 10(-3) g x L(-1), 85.2 +/- 3.1% and 10.4 +/- 4.5%, respectively. The DOX micelle was globular-shaped with a mean diameter of 91.1 +/- 15.8 nm. The transmittance of micelle solution consistently increased with the increasing temperature or decreasing pH. In comparison to the drug release profile at physiological conditions (37 degrees C, pH 7.4), the DOX-loaded micelles showed faster drug release rate at higher temperature (41 degrees C), lower pH (pH 7.0, pH 6.5, pH 5.0) or higher temperature and lower pH (41 degrees C, pH 5.0). This indicated that the micelles showed a temperature and pH-triggered drug release pattern. Base on the above results, it can be concluded that PHis-b-PLGA-b-PEG-b-PLGA-b-PHis block copolymer micelles which respond to temperature and pH stimuli are promising smart carriers for anti-tumor drugs with the advantages of temperature- and pH- triggered drug release.
7.The role of MHCⅡ + T lymphocytes in immune tolerance induction

Traditionally,antigen presenting cells were shown to express MHC Ⅱ antigens.However,some researchers have demonstrated that a subset of T lymphocytes could express MHCⅡ antigens in some situation.Its regulatory mechanism and biological effects remains challenging to researchers.This review provides an overview of the results of these MHC Ⅱ + T lymphocytes regarding to the generation,mechanisms,and the role in immune tolerance induction,aim to provide some insights in clinical immune tolerance induction.
8.Detection of Chemical Warfare Agents by Differential Mobility Spectrometry and Drift-time Ion Mobility Spectrometry Hybrid Technology

Using a novel hybrid technology combined differential ion mobility spectrometry ( DMS) with drift time ion mobility spectrometry DMS-IMS2 , we detected the typical chemical warfare agent simulants dimethyl methylphosphonate ( DMMP ) and methyl salicylate ( MS) . With carrier gas 800 mL/min and DMS RF voltage 1100 V, the chemical warfare agents DMMP and MS could be detected and characterized by DMS-IMS2 under DIMS mode. In addition, DMS-IMS2 is capable to monitor positive and negative ions of DMMP and MS simultaneously, and provides the two-dimensional separation parameters DMS compensation voltage ( CV) and IMS drift time ( Td ) , which provides more information for the identification of two chemical warfare agents. DMS-IMS2 has potential application in on-site detection and instrument miniaturization due to its advantages including small size, low power consumption and rapid response time.
9.Application of nasointestinal decompression intubation in small bowel obstruction

Objective To evaluate the nasointestinal decompression intubation in acute small intestinal obstruction. Methods Ten patients with acute small bowel obstruction received nasointestinal decompression intubation under x-ray guidance. The nasointestinal decompression tube passing over a guidewire was inserted into small intestine near Tres ligament or further down distally with assistance of patients adopting in multi-physical positions. Results The intubation of nasointestinal decompression tubes into small intestine was technically successful in all patients with average procedural time of 16 min.(10 ~ 35 min). After placement of the tube, all patients obtained various degrees of symptoms relief including abdominal pain, distention, vomiting, etc. Four patients with simple adhesive obstruction recovered completely and the tube was removed 2 weeks later. Three patients were refered to surgical operation, and 3 others gave up for further treatment. There were no complications such as bleeding or perforation related to intubation. Conclusion Nasointestinal decompression intubation under guidance of X-ray is rather simple, less time consuming, especially with high efficiency for preoperative gastrointestinal decompression and treating simple adhesive bowel obstruction; ought to be recommended.
