Objective To investigate clinical effects of methotrexate and laparoscopic salpingotomy in the treatment of non -ruptured tubal pregnancy.Methods 150 children with non -ruptured tubal pregnancy were cho-sen and randomly divided into A group (75 children)with methotrexate treatment and B group (75 children)with laparoscopic salpingotomy.The hospital staying time,the degree of tubal patency,intrauterine pregnancy rate,ectopic pregnancy rate,ovarian volume and number of follicles after treatment of the two groups were compared.Results The hospital staying time of A group and B group were (21.56 ±4.98)d,(7.91 ±1.42)d,the hospital staying time of B group was significantly shorter than A group (t =2.27,P <0.05).The tubal patency rate,pass and not free rate and barrier rate after treatment of A group were 10.66%(8 /75),62.67%(47 /75),26.67%(20 /75),those of B group were 65.33%(49 /75),26.67%(20 /75),8.00%(6 /75),the tubal patency rate and barrier rate after treatment of B group were significantly better than A group (χ2 =12.36,8.94,P <0.05).The intrauterine pregnancy rate and ectopic pregnancy rate of A group were 30.67% (23 /75 ),20.00% (15 /75 ),those of B group were 53.33%(40 /75),8.00%(6 /75),which were significantly better than A group (χ2 =9.33,8.16,all P <0.05).There was no significant difference in ovarian volume after treatment between the two groups (P >0.05).The number of follicles in 1 month,3 months and 6 months after treatment of B group was significantly better than A group (t =2.57,2.06, 2.10,all P <0.05).Conclusion Compared with methotrexate treatment,laparoscopic salpingotomy in the treatment of non -ruptured tubal pregnancy can efficiently shorten hospital staying time,higher the degree of tubal patency, improve the long -term outcome,and is helpful to reduce the effect of ovarian function.

Smoking and exposure to second hand smoke are the deadly killer of human health. Tobacco kills more than one million people each year in China. According to 2015 Chinese adult tobacco survey, the number of smokers in China increased by 15 million during five years, from 301 million in 2010 to 316 million in 2015. Smoking addiction is a chronic recurrent disease, and its pathogenesis is associated with the mesolimbic dopamine system. In recent years, domestic and overseas scholars studied the brain function of smoking addicts through resting state functional magnetic resonance imaging (fMRI), and found that the brain functional activity of smoking addicts has changed. fMRI can help us understand the mechanism of tobacco dependence, and provide the evidence of diagnosis, treatment and health management of tobacco dependence. In this paper, the present study situation and the application in the field of the treatment and management of smoking cessation of fMRI in smoking addicts are reviewed.

Objective To express the recombinant protein VP1-Z, and investigate whether VLP-Z has the physiological functions like as wild-type VLP. Methods The expression plasmid pET15b-VP1-Z was introduced into competent E. coil BL21 (DF3)/pLys cells by transformation, and the expression of re-combinant protein VP1-Z was induced by incubation of the cells with IPTG. The protein was prepared as pre-viously described for wild-type VLP. The morphous of VLP-Z were observed by electron microscopy, and the physiological functions of VLP-Z were investigated by hemagglutination test and by immunofluorescence. Re-sults The purified VLP-Z composed of VP1-Z possessed hemagglutination activity and yielded a prominent band of 50×10~3 on SDS-PAGE and staining with Coomassie Brilliant Blue. The VLP-Z exhibited virus-like particles structure like as wild-type VLP with a diameter of 45-50 nm, which was slightly bigger than that of wild-type VLP(42-45 nm). In immunofluorescence test, VP1-Z was detected within the cytoplasm and nu-cleus after HeLa cells were inoculated with VLP-Z. Conclusion The physiological functions of recombinat-ed protein VLP-Z were comparable with wild-type VLP.

Objective To construct the vector pET15b-Z-VP1 by inserting the Z fragment into amino-terminal of JCV VP1.Methods The VP1 and Z fragment were amplified by PCR from plasmid pET15b and pEZZ18 respectively,and then they were linked by recombinant PCR.The Z-VP1 fragment was inserted into plasmid pET15b by restriction enzyme BamHⅠ and NcoⅠ.Results The VP1 and Z fragment were obtained by PCR and gel purification.The Z-VP1 fragment,which was linked by recombinant PCR from VP1 and Z fragment,was inserted into plasmid pET15b between BamHⅠ and NcoⅠ sites,and confirmed by enzyme digestion and DNA sequencing.The expression of VP1-Z was confirmed by Western blotting.Conclusion The plasmid pET15b-Z-VP1 has been constructed successfully by inserting Z fragment into amino-terminal of VP1.

Objective To explore the relative factors of polycystic ovarian syndrome (PCOS) in infertile women.Methods Ninety-eight infertile women diagnosed PCOS at affiliated hospital of the Chinese People's Armed Force Logistic college from Jan.2008 to Dec.2012 were served as PCOS group.Two hundred and eighty infertile women with normal ovarian structure according to laparoscopic examination result were selected as control group.The general information,factors related infertile and laparoscopic examination of the two groups were recorded.Results Fifty-one cases (52.0％) in PCOS group were with menarche age earlier than 13 years old,higher than that of control group (113 (40.4％),x2 =4.03,P ＜ 0.05).Eleven cases (11.2％) in PCOS group were with abnormal menstruation cycle,higher than that of control group (12 (4.3％),x2 =6.12,P ＜ 0.05).The incidence of pelvic adhesions in PCOS group was lower than the control group(60(21.1％) vs.224(40.4％),x2 =13.70,P ＜0.05).Irregular menstruation cycle was risk factor of PCOS (OR=1.770,95％ CI:1.09-2.88,P =0.002).Conclusion Women with earlier menarche and irregular menstruation cycle should be paid more attention in terms of PCOS,early screening,discovery,prevention and treatment,so as to reduce the incidence of PCOS induced infertility and the long-term complications.

Objective To investigate the pharmacokinetics of propofol when combined with remifentanil in patients with liver cirrhosis.Methods Ten patients (5 males, 5 females) with liver cirrhosis scheduled for endoscopic esophageal varix ligation (test group) and 10 cases (5 males, 5 females) with normal liver function scheduled for gastroscopy (control group), aged 18-55 yr, weighing 40-75 kg, were studied. The patients were unpremedicated. All the patients received iv injection of propofol 1.5 mg/kg and remifentanil 0.5 μg/kg, and 5 min later propofol 0.5 mg/kg and remifentanil 0.2 μg/kg was given again. Blood samples were taken from radial artery before administration and at 2, 5, 10, 15, 20, 30, 45, 60, 80 and 120 min after administration for determination of the plasma propofol concentration using gas chromatography-mass spectrography. The pharmacokinetic parameters were calculated using DAS 2.0 software.Results The pharmacokinetics of propofol was best described by a three-compartment open model. There was no significant difference in the distribution half-life, elimination halflife , terminal half-life, area under the curve and transfer rate constant between the two groups ( P ＞ 0.05) . The apparent volume of distribution of propofol and clearance were significantly increased in test group compared with control group (P ＜0.01) .Conclusion When propofol combined with remifentanil is used in patients with liver cirrhosis, the apparent volume of distribution of propofol and clearance are significantly increased, while no changes in the other pharmacokinetic parameters are found.

Objective To evaluate the effect on treatment of multiple myeloma complicating femoral neck fracture by surgical intervention combined with drugs.Methods Twelve patients with multiple myeloma complicating femoral neck fracture were treated by cemented total hip arthroplasty femoral head shank.All of patients were received zoledronic acid treatment,at the same time,7 cases of them treated by MPT regimen (melphalan,methylprednisolone and thalidomide),3 by bortezomib combined with dexamethasone,and 1 by autologous peripheral blood stem cell transplantation.Results Twelve patients could tolerate surgery,postoperative pain was significantly reduced.Assessed by using Harris hip function score after 6 months operation,2 cases were excellent,7 cases were good,3 cases were common,excellent and good rate was 75％ (9/12).All patients were followed up for 8 months to 3 years,1 case of local recurrence after 13 months.1-year overall survival rate was 100％,2-year overall survival rate was 83％,and 3-year overall survival rate was 67％.Conclusions Multiple myeloma complicating femoral neck fracture,using bone cement in total hip arthroplasty femoral head shank to clear the local tumor lesions,receive reconstruction capability,rapid postoperative recovery,pain relief,and opportunities for further comprehensive treatments.The bisphosphonate can promote new bone formation and prevent further fractures.The surgical intervention combined with chemotherapy could relieve the symptoms,reduce tumor cell burden,improve quality of life and prolong survival time.

Objective To highlight the characteristics of pulmonary sequestration (PS) with combination of practical experience and review of literature. Methods One patient with PS confirmed by biopsy was described and the relevant literatures of 279 cases were reviewed. Results PS can be divided into two types: intralobar type and extralobar type. The percentage of intralobar type is about 78.5%, intralobar type is 13.3%, other unreported is 8.2%. Clinically, PS was often combined by repeated pulmonary infection. The mis-diagnose rate is 72.8%. Etrograde arteriography or enhanced CT scan could find the abnormal artery, from which we can make correct diagnosis. The diagnose rate is 100%. Conclusion For patients with above clinical and X-ray symptoms, PS should be considered. Enhanced CT or retropgrade arteriography examination should be done as soon as possible. Timely diagnosis and surgery treatment are important for curing this kind of disease.

BACKGROUND:Studies have shown that the finite element method could preferably simulate the biomechanical analysis for the object with complicated structures and irregular shapes. The similarities for the finite element model have great influences on the results of the analysis. However, to construct an ideal model is the most time-consuming and complicated portion for the finite element analysis. OBJECTIVE:To construct a finite element model for the maxilary first molar and the periodontal tissue, and to provide a basis of biomechanical researches of the maxilary first molar. METHODS: A volunteer with complete mandibular dentition and healthy periodontal tissue was selected in this study. Cone-beam CT was scanned. The images were saved as DICOM format. These images were imported to the medical modeling software Mimics. The surface model for the maxilary first molar and the alveolar bone was constructed. The model was then imported to GiD for pre-processing. Thus, the complete three-dimensional finite element model for the maxilary first molar and the periodontal tissue was constructed. RESULTS AND CONCLUSION:A finite element model for bilateral maxilary first molar, periodontal ligament and maxilary alveolar bone was constructed, including 896 035 nodes and 4 881 067 elements. This model has restored the geometric shape and the structure of the research object. This study successfuly constructed finite element models of maxilary first molar and the periodontal tissue, which can be a basis of biomechanical researches for the maxilary first molar and the periodontal tissue under the effect of different clinical orthodontic forces.
