1.Effects of Gestation Exposure to Dibutyltin Dilaurate (DBTD) on Enzymes Activity in Testis and Spermatogenesis Function of Offspring Rats

Objective To explore the effects of gestation exposure to dibutyltin dilaurate (DBTD) on the activity of enzymes in the testis and the spermatogenesis function in the mature male offspring of Wistar rats. Methods The pregnant rats were treated with DBTD by gavage at the doses of 0, 10, 20 and 30 mg/kg for days, from 12th-20th days of gestation. On 70th postnatal day, 10 male offspring rats were selected randomly from each group for measurements of the viscera coefficient of testis and the sperm count in epididymis, the activity of enzymes of testis was determined by spectrophotometry. Results There were no significant differences seen in the body weight gain of pregnant rats from days 12-20 of gestation, litter size, sex ratio of fetuses and body weight of male offspring rats between the experimental groups and the control group. Objective To Investigate the value of HPV genotyping in diagnosis of cervical intraepithelial neoplasia(CIN).Methods From July 2012 to February 2013,200 women from Tianjin Medical University General Hospital and 244 women from Affiliated Hospital of the Chinese People's Armed Forces Logistics College were selected to be HPV genotyping test and thin liqid-based cytology test.Consequently,132 samples were performed colposcopy test and cervical biopsy.Results HPV prevalence was 26.4% (117/444) in this study.The infection of one type HPV was more common.The top 5 of HPV types were HPV16,58,33,18,and 52.The top 5 of the risk for CIN Ⅱ and above followed HPV16,33,39,52 and 18.There was no significant difference between age and HPV positive rate (x2 =0.948,P > 0.05).Multiple infection and cervical lesions rank correlation analysis(r =0.132,P >0.05).For CIN Ⅱ and above disease,cytology positive rate was 90% (44/49),and HPV positive rate was 96% (47/49) cytology combine HPV positive rate was 98% (48/49,x2 =0.063,P > 0.05).Conclusions HPV infection should increasing trends with age.Cytology test and HPV genotyping test had good consistency.The combination of them can improve the sensitivity for high-grade lesions.
3.Mandibular morphology of adults with skeletal classⅢmalocclusion and mandibular deviation analysed by CBCT

Objective:To analyse the asymmetry of mandibular of adults with skeletal classⅢmalocclusion and mandibular deviation using cone-beam computerized tomography(CBCT).Methods:20 adults with skeletal classⅢmalocclusion and mandibular deviation (deviation group)and 20 adults with individual normal occlusion(normal group)were included.CBCT slices were taken for each sub-ject.INVIVO 5 software was used to make 3D reconstruction and location marks for line distance and volume measurements of mandi-ble and condyle.Results:The measurements of the mandibular ramus length,mandibular total length,hemi-mandibular volume, mandibular ramus volume,posterior part of mandibular body length,condylar unit length,angular unit length,body unit length and an-gular angle in deviation group were significantly different between 2 sides(P<0.05).Compared with the normal group,differences of the asymmetry of mandibular ramus length,mandibular total length,hemi-mandibular volume,mandibular ramus volume,posterior part of mandibular body length,angular angle,condylar unit length and body unit length between 2 sides in deviation group were sig-nificantly different(P<0.05).Conclusion:In adults with skeletal classⅢmalocclusion and mandibular deviation deviated condylar and mandibular ramus are smaller than those of the nondeviated;the posterior part of mandibular body is slightly asymmetric while the anterior part of mandibular body is almost symmetric.
4.Optimization of Recombinant E. coli High-density Fermentation for Expressing Human-like CollagenⅡ

To study and optimize the fermentation parameters for expressing human-like collagenⅡduring E. coli high-density fermentation. The effects of pH, temperature, dissolved oxygen and induction instant on the cell growth and human-like collagenⅡproduction were investigated to optimize the fermentation conditions. The results demonstrated that the following conditions were beneficial for cell growth and foreign gene expression, controlling pH in phase induction at 6.8 and initial pH at 6.5, maintaining fermentation temperature and dissolved oxygen concentration was controlled at 34?C and 20% respectively, and implementing induction at the later logarithmic growth phase. Under the optimized condition, the cell density and human-like collagenⅡyield could reach 88.4 g/L and 14.2 g/L, respectively.
5.bla_(LAP-2),A Novel Subtype of ?-Lactamases Gene in Enterobacter cloacae in China

OBJECTIVE To investigate the resistance genes in HZB9055 strain of Enterobacter cloacae isolated from the PLA 98th Hospital,Huzhou District,Zhejiang Province,China.METHODS HZB9055 Strain of E.cloacae was isolated from the inpatient in May,2004,41 kinds(or groups) of genes were analyzed by PCR and verified by DNA sequencing.RESULTS In HZB9055 strain of E.cloacae,3 kinds of genes were positive including blaLAP,blaMIR and qnrS.The 38 kinds of rest genes were all tested negative.The blaLAP gene sequence including 858 nucleotides,which had the amino acid mutation in position 193 compared with LAP-1 type narrow-spectrum ?-lactamase(GenBank accession number:EF026092),and was nominated LAP-2(GenBank accession number:EU159120).CONCLUSIONS At least 3 kinds of resistance genes exist in HZB9055 strain of E.cloacae,and blaLAP-2 is a novel subtype of ?-lactamases gene.
6.Application of PDCA Cycle Management in Increasing the Dispensing Rate of Hospital Outpatient Automatic Dispensing Equipment

OBJECTIVE: To improve dispensing rate of automatic dispensing equipment, and to shorten dispensing time.METHODS: Continuous quality improvement (CQI) group was established, and COI was conducted by using PDCA (Plan, Do, Check, Action) cycle. The influencing factors for slow drug delivery of automatic dispensing equipment were analyzed to set up the target, adjust multi-package drugs, develop appropriate improvement plans and implementation methods, monitor result and evaluate effect (calculated the first 10 in usage frequency of the multi-package drugs). RESULTS: Through adjusting the specification and list of multi-package drugs, the utilization rate of multi-package drug increased from 63. 91% to 86. 23% of 10 drugs during Jun. -Oct. in 2016; compared with the single-package dispensing mode, and dispensing time shortened 7. 86-13. 73 h per month of 10 drugs. CONCLUSIONS: Through the CQI, the dispensing rate of automation dispensing equipment is greatly increased by drug multi-package dispensing mode.

Enteral nutrition plays an irreplaceable role in the nutritional treatment of critically ill patients. In order to help clinical medical staff to manage the common complications during the implementations of enteral nutrition for critically ill patients, the consensus writing team carried out literature retrieval, literature quality evaluation, evidence synthesis. Several topics such as diarrhea, aspiration, high gastric residual volume, abdominal distension, etc. were assessed by evidence-based methodology and Delphi method. After two rounds of expert investigations, Expert consensus on prevention and management of enteral nutrition therapy complications for critically ill patients in China (2021 edition) developed, and provided guidance for clinical medical staff.
8.Analysis of clinical features and genetic variants in a Chinese pedigree affected with tuberous sclerosis.

OBJECTIVE:
To analyze the clinical features of a Chinese pedigree affected with tuberculosis sclerosis and explore its molecular pathogenesis.
METHODS:
Clinical data of the proband and members of his pedigree were collected. Whole exome sequencing was carried out to detect variants of the TSC1 and TSC2 genes. Candidate variants was verified by Sanger sequencing and bioinformatic analysis.
RESULTS:
The proband and his mother, who also had mild features of tuberous sclerosis, were found to harbor a novel heterozygous c.4183C>T (p.Q1395X) variant of the TSC2 gene, which was absent in the 4 healthy relatives. Bioinformatic analysis suggested the variant to be likely pathogenic.
CONCLUSION
The heterozygous c.4183C>T (p.Q1395X) variant of the TSC2 gene probably underlay the disease in this pedigree. Above finding has expanded the spectrum of TSC2 gene variants. The more severe symptoms in the proband may be attributed to phenotypic heterogeneity of this disease.

9.Coronary artery lesions in infants younger than 6 months with Kawasaki disease: clinical characteristics and prognosis

Objective:To analyze the characteristics of coronary artery lesions in infants under 6 months of age with Kawasaki disease(KD), and to explore their regression and risk factors.Methods:The clinical data of 61 infants with KD[34 boys, 24 girls, aged 2.2 (1.7, 3.1) months] admitted to the department of critical care medicine and neonatology, Children′s Hospital, Capital Institute of Pediatrics from October 2015 to February 2020 were retrospectively analyzed.Persistent coronary artery aneurysm(CAA)was defined as the persistent enlargement of coronary arteries(coronary Z-score≥2.5)on echocardiograms at 12 months after KD onset.Cox proportional hazards mode was conducted to evaluate the potential risk factors of persistent CAA.Results:The incidence of CAA in 61 infants with KD was 52.5% (32/61) and occurred on 5 (4, 8)d of the disease course.During a follow-up of 547 (399, 782)d, five(8.2%, 5/61)infants satisfied the definition of persistent CAA.The median recovery time of CAA was 20 (12, 82)d after KD onset.Cox proportional hazards mode revealed that the maximal coronary Z-score was an independent factor of CAA regression( HR=0.451, 95% CI 0.293-0.694, P<0.001). Receiver operating characteristic curve analysis showed that the best cutoff value of coronary Z-score for predicting persistent CAA was 6.15(sensitivity 80.0%, specificity 97.7%). Conclusion:CAA is common in infants younger than 6 months with KD.The maximal coronary Z-score is an independent factor of persistent CAA.
10.Application of magnetic resonance quantitative score on the assessment of brain maturity in preterm infants with white matter injury

Objective:To study the relationship between white matter injury (WMI) and brain maturity in preterm infants at full-term corrected gestational age (cGA).Methods:A retrospective study was performed in preterm infants [GA≤32 weeks or birth weight (BW) ≤1 500 g] admitted to the neonatal intensive care unit of the First Affiliated Hospital of Xi'an Jiaotong University from January 2017 to August 2018 and the Northwest Women and Children's Hospital from January 2017 to June 2017. The infants received conventional magnetic resonance imaging (MRI) at cGA 37~42 weeks. The infants were assigned into the WMI group and the control group according to the WMI scoring system, including the total maturation scores (TMS) and four sub-item scores.Results:A total of 118 premature infants were enrolled in this study (17 cases in the WMI group and 101 cases in the control group). The GA was (30.3±1.7) weeks, and BW was (1 356±268) g. The proportion of delayed TMS in the WMI group was significantly higher than the control group [58.8%(10/17) vs. 31.7%(32/101), P<0.05]. The TMS of the WMI group were significantly lower than the control group [(10.7±1.8) vs. (11.8±1.5), P<0.05]. The sub-item scores of TMS showed that the myelination [(2.8±0.6) vs. (3.1±0.4), P<0.05] and glial cell migration bands of the WMI group [(1.6±0.4) vs. (2.1±0.6), P=0.004] were significantly lower than the control group and no significant differences existed in cortical folding and involution of germinal matrix tissue scores between the two groups. Conclusions:The brain maturity of preterm infants with WMI is substantially delayed than those without WMI, including delayed myelination and delayed disappearance of glial cell migration bands.
