1.Correlative Factors of Hospital Infection among Children with Infectious Diseases in a General Hospital

OBJECTIVE To investigate the distribution of infectious diseases and diagnostic and therapeutic conditions in pediatric department of a general hospital,and try to find out the measures to decrease hospital infection.METHODS From Jan 2004 to Dec 2005,507 cases of infectious diseases were retrospectively analyzed in pediatric department of our hospital.RESULTS Among them epidemic parotitis,infectious diarrhea and bacillary dysentery were 56.2%,20.1%,and 17.4%,respectively.Others were measles,scarlet fever,rebella,viral hepatitis,epidemic cerebrospinal meningitis,typhoid,and syphilis.Among the cases,the most were outpatients.CONCLUSIONS Because the species of infectious diseases are various and the amount are large,the pediatric department is important in a general hospital for management of hospital infection.Strengthening the standardized management of pediatric infectious diseases and taking strong measures may play the important role in control the cross infection.
2.Gemcitabin plus capecitabine as a second-line chemotherapy for patients with advanced breast cancer

Background and purpose:Chemotherapy including anthracyclin and taxanes is one of the effective regimens for patients with advanced breast cancer, but 20%-30% patients do not achieve a satisfactory curative effect .At present, there is no unified standard second-line chemotherapy regimen for these patients. We studied the efficacy and toxicity of gemcitabin plus capecitabine in the treatment of the patients with advanced breast cancer who had failed in response to the treatment of anthracyclin and taxanes chemotherapy. Methods:Gemcitabin 1 000 mg/m2 iv infusion at d1,8 and capecitabine 950 mg/m2 po tid at d1-14.The interval between the two cycles was 3 weeks. The assessment for clinical effect and side effect was conducted for the patients with completion of 2 cycles of chemotherapy at least.Results:30 female patients were enrolled in this trial ,overall response rate was 46.7%,with 6.7% complete response (2/30)and 40% partial response (12/30). Stable disease was seen in 42.3%(13/30),and disease progression in 10% (3/30).Median time to progression was 9 months, and median overall survival was 12.5 months. The main toxicities were myelosuppression and hand-foot syndrome.Conclusions:Gemcitabin plus capecitabine is effective for the patients with advanced breast cancer who failed in the treatment of anthracyclin and taxanes chemotherapy, and its side effect is tolerable.
3.Construction of pcDNA3. 1-Brugia malayi glyceraldehyde phosphate dehydrogenase eukaryotic recombinant plasmid and study on its role in DNA immunity

Objective To construct the pcDNA3. 1-Brugia malayi (Bm) glyceraldehyde-3-phosphate dehydrogenase (GAPDH) eukaryotic recombinant plasmid and to study its effect on mouse cellular immunity response. Methods Total RNA was prepared from periodic Bm. The target gene fragments were amplified by reverse transcription-polymerase chain reaction (RT-PCR) technique and then were inserted into the cloning vector. pGEM-T Easy, and sub-cloned into pcDNA3. 1. Purified pcDNA 3. 1-BmGAPDH recombinant plasmid and CpG were injected into the anterior tibial muscle of BALB/c mice in order to induce host immunity response. Mice injected with PBS and mice injected with blank plasmid were prepared as controls. The mouse models were immunized for 3 times with an interval of 2 weeks. RT-PCR was utilized to detect target gene expression in the muscle tissue. MTT method was used to measure the immunized mice T lymphocytes stimulation index, while enzyme-linked immunoassay (ELISA) was used to determine the serum interleukin (IL)-4 and interferon (IFN)-γ level. Means were compared using t test with SPSS software. Results The recombinant plasmid pcDNA3. 1-BmGAPDH was constructed sucessfully. The target gene was 1020 bp long and its homology with known gene sequence in database was 99%. BmGAPDH gene in the injected muscle of the immunized mice was detected by PCR. The proliferation of spleen T lymphocytes was higher in pcDNA3-BmGAPDH group than in the 2 control groups which were 1. 398, 1. 006 and 1. 017,respectively (P< 0. 05). The levels of IFN-γ and IL-4 in serums from the immunized mice were significantly higher than those of the PBS control group and blank plasmid control group which were 163.905, 58.589, 51. 317 and 107. 906, 27.111, 34.627, respectively (P<0. 05). Immune adjuvant CpG could accelerate and boost antigen-specific immune responses induced by vaccine, which presented as significant increase of IFN-γ and lymphocyte proliferation at 4 weeks after immunization.Conclusion The recombinant eukaryotic plasmid pcDNA3. 1-BmGAPDH is constructed and could elicit cellular immune responses in immunized mice.
4.Psychological rehabilitation of paraplegic patients with spinal fracture

Objective To study the effects of psychological rehabilitation in the treatment of paraplegic patients with spinal cord injury (SCI).Methods A hundred paraplegic spinal fracture survivors were divided into a research group and a control group with 50 cases in each.Systematic rehabilitation was given to the patients in the control group,while this was combined with individualized psychological rehabilitation protocols for the patients in the research group.The daily treatment lasted 10 weeks,6 days a week.All of the patients were assessed with the Barthel index (BI),a functional independence measure (FIM),the Hamilton depression scale (HAMD) and the Hamilton anxiety scale (HAMA) pre-treatment and post-treatment.Results Before the intervention there was no significant difference between the 2 groups in any of the assessments.After ten weeks the average BI,FIM,HAMD and HAMA results in the research group were significantly better than those pre-treatment and also significantly better than those in the control group.Conclusions Psychological rehabilitation can distinctly improve the functioning and psychological state of paraplegic patients after SCI.
5.Effect of hypertension on vascular contraction and sodium pump activity in rat cerebral basilar artery

Aim To explore the effects of hypertension on the contractibility of rat basilar artery and its inter-actions with the sodium pump activity.Methods The basilar artery was respectively isolated from Wistar and SHR rats,and the isometric tension of arterial rings was measured by Multi Myograph System-610M.The cont-ractibilities of arterial rings induced by KCl or 5-HT were compared between the basilar arteries of the two groups of rats to analyze the effect of hypertension on the cerebral vascular tension and the activity of sodium pump.Results In SHR rats,the concentration-re-sponse curves of the contraction of isolated basilar ar-tery rings induced by KCl and 5-HT were significantly shifted to right,and the relaxation of vascular tone in-duced by K+which was reintroduced from the external was attenuated compared with those in the WR.These results suggested that hypertension could significantly decrease the activity of the sodium pump and the con-tractile responses of KCl and 5-HT.OUA could con-tract the basilar artery in a concentration-dependent manner,and its concentration-response curve was opti-mally fitted by a two-site binding model:Kd was 1.7 ×10 -8 and 1.6 ×10 -5 mol·L-1,respectively.The results indicated that the two different function sodium pumps existed in the rat basilar artery:one with the high OUA affinity and the other with the low OUA af-finity.If the high and low affinity sodium pumps were inhibited by 5 ×10 -7 and 10 -4 mol · L-1 OUA,re-spectively,the concentration-response curves of KCl and 5-HT would shift to left in SHR rats but not in WR rats.It suggested that OUA could enhance the contrac-tion induced by KCl and 5-HT significantly,and a concentration-dependent effect was observed in the SHR vascular contraction induced by 5-HT (r =0.9393 ,P<0.05 ).When the two concentrations of OUA were applied,there was no significant difference in the shift left of the concentration-response curves in-duced by KCl in the SHR cerebral vessels.However, the marked difference was shown in the shift left in-duced by 5-HT.The results implied that only the high affinity sodium pump was involved in the contractile re-sponse of SHR cerebral vascular to KCl,whereas,the contractile response of SHR cerebral vascular to 5-HT was induced by both high and low affinity sodium pumps.Conclusion Hypertension could lower the contractile response of the basilar artery to vasocon-strictors,and the mechanism might relate to the de-creased sensitivity of the sodium pump induced by hy-pertension or the increased sensibility of the sodium pump to OUA.
7.Effects of Total Flavone of A on Post-stroke Depression in Rats

OBJECTIVE: To study the antidepressant effects of total flavone of A( TFA) on post stroke depression ( PSD) in rats. METHODS: Middle cerebral artery occlusion ( MCAO) followed by isolation feed and irritable stimuli was used to induce PSD. Behaviours of rats were detected by open- field test . The effects of TFA on the indexes of hemorheology and brain homogenate lipid peroxidation were also tested. RESULTS: TFA could increase the crossing and rearing scores in open- field test, inhibit the elevation of whole blood viscosity as well as plasma viscosity at high- middle- low shear rates, increase the erythrocyte deformation, enhance the activities of Superoxide Dismutase ( SOD) and Glutathione Peroxidase ( GSH- Px) , and reduce Malondialdehyde ( MDA) content in brain homogenate in PSD in rats. CONCLUSIONS: TFA produces an antidepressant- like effect in PSD rats. This mechanism may be in association with attenuating hemorheological parameters and decreasing lipid peroxidative damage.
8.The relationship among the sense of coherence, empowerment and self-efficacy in peritoneal dialysis patients

Objective To explore the relationship among the sense of coherence,empowerment and self-efficacy in peritoneal dialysis patients.Methods General information,the Sense of Coherence Scale,the Peritoneal Dialysis Empowerent Scale-Short Form and the Self-Efficacy for Managing Chronic Disease Scale were used to investigate a total of 160 peritoneal dialysis patients in a third-class hospital in Tianjin from March to June,2015.Results The mean score of the sense of coherence,empowerment,self-efficacy were (62.01±11.94) points,(4.09±0.68) points and (6.08±2.19) points.Both the sense of coherence and empowerment could positively influence the self-efficacy of peritoneal dialysis patients directly (r=0.429-0.571,P < 0.01),and the sense of coherence could also indirectly influence the self-efficacy through the mediation of empowerment (β decreased from 0.429 to 0.172,P < 0.01),the mediation effect accounted for 60.1% of the total effect.Conclusions Medical staff should take different actions to improve the sense of coherence and empowerment to improve the self-efficacy of peritoneal dialysis patients.
9.Effect of Shuanghuanglian injection on CRP, PCT and IL-6 in pneumonia children

Objective To study the effect of Shuanghuanglian injection on CRP, PCT and IL-6 in pneumonia children.Methods 68 cases of pneumonia children were selected and divided into the control group and the experiment group.33 case in the control group and 35 cases in the experiment group.The two groups were treated with conventional symptomatic treatment and anti infection treatment for the two groups were implemented routine symptomatic treatment and anti infection treatment.The control group were treated with cefotaxime sodium injection, the experiment group were treated on the base with Shuanghuanglian injection.CRP, PCT and IL-6 in serum were compared before and after the treatment.Results Compared with the control group, the level of CRP in serum was lower(P<0.05), the level of PCT in serum was lower(P<0.05), the level of IL-6 in serum was lower(P<0.05).Conclusion Shuanghuanglian injection has a good clinical effect on children’s pneumonia.It is speculated that the mechanism is related to the decrease of serum CRP and IL-6 level.
10.Relationship between genetic mutations and therapeutic effect of glibenclamide in permanent neonatal diabetes

Objective To study the diagnostic significance and clinical value of genetic analysis in children with neonatal diabetes. Methods Gene mutation analysis was performed in four patients from Zhengzhou children ' s hospital with diagnosis of with neonatal diabetes. Therapeutic effect of glibenclamide in patients with or without gene mutation was compared. Results KCNJ11 gene mutation was found in two patients with neonatal diabetes. Glibenclamide was found only effective for blood glucose control in patients with KCNJ11 mutation. Therefore, Insulin remains the best therapeutic choice in patients without the genetic mutation. Conclusions Genetic mutation status may be useful in choosing treatment options of neonatal diabetic patients, therefore, should be performed in all children with neonatal diabetes.
