1.Comparison of effect of video tracheal intubation and direct laryngoscopy on tracheal intubation in patients with missing teeth

Objective To compare the effect of the application of tracheal intubation with SEESHEEN video intubation endoscope (ENF-20) and direct laryngoscopy in patients with missing teeth.Methods 50 patients with missing teeth were selected,and they were randomly divided into two groups, respectively after induction of general anesthesia in tracheal intubation using ENF-20 (group E) and direct laryngoscopy (SHUCMAN laryngoscopy) (group S) intubation.The time of intubation, the times of intubation and the complication of intubation were recorded.Results The success rate of intubation in group E was 92%, which was significantly higher than 64% in group S (x2=5.711,P=0.017).The intubation time of group E was (27±12)s, which was significantly shorter than (36±13)s in group S (t=-2.620,P=0.012).The total intubation complications of group E was 12%, which was significantly lower than 48% of the group S (x2=6.095,P=0.014).Conclusion The successful rate of tracheal intubation in patients with missing teeth by using video tracheal intubation (ENF-20) is higher and with less complication.
2.Application of DNA barcoding technique of COXⅠ gene in identification of ticks

Objective To investigate the application of the cytochrome C oxidase subunit Ⅰ gene(COXⅠ ) barcoding in the i‐dentification of wild‐caught ticks .Methods The COXⅠ gene of 9 caught tick individuals with different species was sequenced ,the DNAstar software was used to conduct the sequence comparison and homology analysis .Meanwhile the philp3 .6 software was a‐dopted to select the maximal likelihood(ML) method for constructing the evolutionary trees .Results In the phylogenetic trees ,the samples of sh2011040702 ,ra2011042193 and ra2011051168 with Haemaphysalis were in a branch ,sh2011051404 ,sh2011051408 and ra2011041177 with dermacentor were clustered together ,and sh2011051837 ,sh2011041917 and ra2011041175 with hyalomma were in a branch ,which were consistent with the morphological identification results .Conclusion Making the COXⅠ gene sequence as tick DNA barcoding has certain feasibility for identifying the tick species .
3.Feasibility study on application of local anesthesia in PICC

Objective To investigate application of local anesthesia on the success rate of PICC. Methods A retrospective analysis was carried out in patients (120 cases)receiving PICC from March 2004 to January 2006. Randomly selected 120 cases were divided into 2 groups, each with 60 cases and PICC was im-plemented with the conventional method or under local anesthesia respectively in the two groups. Psychological changes of patients and nurses,adverse reactions of pain, success rate and incidence rate of complications were observed. Results The two groups were different in the following aspect, success rate, psychological changes, adverse reactions of pain,etc (P<0.01). Conclusions Local anesthesia during PICC is conducive to raise the success rate of tube insertion and the elimination of other adverse factors, especially for coarse PICC suite.
4.Research in application effect of risk management in obstetric care

Objective To explore the risk management factors and preventive measures in obstetric care. Methods The implementation of risk management in our hospital was retrospectively analyzed. Results Maternal satisfaction degree was significantly increased, complaints and errors of nursing care were significantly decreased. Conclusions Strengthening of risk management in obstetric care can benefit to eliminate and prevent unsafe factors, so that obstetric care quality is improved.
5.The analysis of bearings on measure of gastrointestinal hemorrhagea

Objective To analyse the reason of gastrointestinal hemorrhage,and the relationship between the degree of fever and the quantity of hemorrhage and blood transfusion volume.Methods The patients suffering from the gastrointestinal hemorrhage were selected from the Central Hospital of Xinyang from June 1996 to June 2005. The treatment of 225 patients were analysed.Results The symptoms of fever arose.The number of higher fever ex- ceeding 39.0℃was 16 accounting for 7.1%.The number of secondary fever was 82 numbers,accounting for 36.4%.The number of lower fever (37.3~38.1℃) was 127,accounting for 56.4 %.Conclusion The degree of fever have a remarkable relationship with gastrointestinal hemorrhage and transfusion.Patients with large quantity of hemorrhage and blood transfusion volume usually have a high degree of fever.
6.Clinical efficacy and impact on liver and immune function in ERCP stent implantation for elderly patients with malignant obstructive jaundice

Objective To probe into the clinical application value of ERCP stent implantation in treatment of elderly patients with malignant obstructive jaundice. Methods From January 2014 to December 2015, 97 elderly patients with malignant obstructive jaundice were selected in our hospital as study objects. According to the different treatment, the patients were divided into study group (n = 50) and control group (n = 47) randomly. The patients in study group were treated with ERCP biliary stent implantation. The patients in control group were treated with PTCD biliary stent implantation. The surgical treatment, clinical symptoms, liver function, immunologic function and incidence of complication of the patients between the two groups were compared and analyzed. Results Compared with control group, the patients in study group, the rate of successful operation was 96.00% (48/50), there was increased, but the differences were not significant statistically (P > 0.05); the patients in study group, total efficiency rate was 94.00% (47/50), there were increased significantly, the differences were significant statistically (P < 0.05); the patients in study group after 2 weeks treatment, TBIL were (110.24±60.05) μmol/L, DBIL were (82.43 ± 46.19) μmol/L, ALT were (86.79 ± 39.30) IU/L, AST were (53.65 ± 35.81) IU/L, ALP were (216.83 ± 127.74) IU/L, GGT were (196.52 ± 100.64) IU/L, there were improved significantly, the differences were significant statistically (P < 0.05);the patients in study group after treatment, the levels of CD3+ was (70.24 ± 5.62)%, CD4+ was (37.74 ± 3.85)%, CD4+/CD8+ was (1.35 ± 0.12) and CD8+ was (27.96 ± 3.14)%, there was reduced significantly, the differences were significant statistically (P < 0.05); the patients in study group, the incidence of total complication was 8.00% (4/50), there was decreased significantly, the differences were significant statistically (P < 0.05). Conclusion ERCP stent implantation has very important clinical significance in improving the treatment effect, liver function and immune function for elderly patients with malignant obstructive jaundice.

[Objective] Analyze six differences between TCM and western medicine, which are raised by professor Chen Yi, in order to return the origin of TCM and cal on inheritance. [Methods] This article summarizes insightful views on the inheritance, development and innovation of TCM by professor Chen Yi from fol owing six differences between TCM and western medicine, as theory, thinking, culture, therapy, medication and disease cognition. [Re-sults] TCM, whose theory is based on principle, uses macro-control thinking, roots in Chinese culture, pays attention to syndrome differentiation, chooses one formula for one TCM type, masters herbs by drug properties theory and regards the healthy standard as harmony. While, western medicine, whose theory is based on reality, adopts the micro causal thinking, establishes on western culture, pays attention to disease diagnosing, uses one drug to treat one kind of disease, il ustrates western medicine by molecular formula and copes with causes or lesions of diseases. [Conclusion] Professor Chen Yi analyzes the six differences between TCM and western medicine and emphasizes TCM should be developed and innovated on the basis of inheritance, which is ful of enlightenment in methodology.
8.The protective effect of mesenteric lymph duct ligation on systemic inflammatory responsein rats with severe acute pancreatitis

Objective To observe the role of mesenteric lymphatic pathway in severe acute pancreatitis (SAP)by detection of inflammatory factors levels and mesenteric lymph duct ligation in rats.Methods Twenty-four SD male rats were randomly divided into group A (SAP group),group B (SAP + mesenteric lymph duct ligation group)and group C(sham operation group),with eight rats in each group.Samples of portal vein blood and mesenter-ic lymph were collected for measuring the level of tumor necrosis factor α(TNF-α),interleukin 1β(IL-1β),interleu-kin 6(IL-6)and endotoxin at twelfth hour.The blood CDl b was tested at sixth hour.24h lactulose /mannitol excretion ratios(L/M)were taken as the intestinal mucosal permeability index by gavage of 100mg lactulose and 50mg mannitol solution.The terminal ileums were collected for pathological examination.Results The blood levels of endotoxin and cytokines(TNF-α,IL-1β,IL-6)were significantly increased in group A compared with group C(all P <0.01).Each factor′s concentration in lymph was much higher than that in portal vein blood,especially in group A.The endotoxin and cytokines(TNF-α,IL-1β,IL-6)levels in portal vein blood of group B were significantly decreased compared with group A(all P <0.05),but still higher than that of group C(all P <0.05).CDl b expression in group A was the highest,followed by group B,group C was the lowest (P <0.01).24h L/M in group A was significantly higher than that in group C (P <0.01),and L/Min group B decreased significantly compared with those in A group(P <0.05). Conclusion Lymphatic pathway is an important pathway of systemic inflammatory response induced by SAP,and ligation of mesenteric lymph duct has a protective effect on the organism.
9.Application of Painless Nursing in Postoperative Pain at Night Patients after Calcaneal Fractures Surgery

Objective:To explore the effect of painless nursing in patients with calcaneal fractures postoperative pain at night.Methods:Total 60 cases of orthopaedic hospital patients were randomly divided into control group and experimental group, 30 cases in each group.The control group had routine nursing, the painless nursing was used in experimental group.By updating the paramedics painless concept, clear pain assessment method and pain nursing records, the implementation of procedural pain in health education, will be painless care effectively applied in postoperative patients with calcaneal fractures pain at night.Joint pain evaluation method is used and the night sleep stage method compare two groups of postoperative pain at night is happening.Results:After the painless nurs-ing, the patients pain at night to sleep affect significantly reduce, the incidence of pain at night fell, the experi-mental group effectiveness were significantly higher than that of control group, the difference was statistically signifi-cant ( P<0.05) .Conclusions:Painless nursing can effectively prevent or reduce the occurrence of calcaneal frac-tures patients postoperative pain at night, promote the body′s recovery, improve patient′s satisfaction, is helpful to improve the quality of nursing.
10.Effects of peroxisome proliferator-activated receptor gamma agonists on fibronectin expression after SiO2 stimulation

BACKGROUND: The effect and mechanism of peroxisome proliferator-activated receptor gamma (PPARγ) agonists on transforming growth factor (TGF)-β1-induced fibrotic responses in lung fibroblasts remain unclear, and seldom reported.OBJECTIVE: To study the effects of PPARγ agonists on TGF-β1-induced fibrotic responses in lung fibroblasts, so as to investigate its effects in preventing pulmonary interstitial fibrosis of silicosis. METHODS: The pulmonary alveolar macrophage was prepared with 200 mg/L SiO2 and the supernatants were got. Chinese hamster lung fibroblasts were incubated with the prepared supernatants. Then a cellular model in vitro for studying pulmonary fibrosis induced by SiO2 was developed. Chinese hamster lung fibroblasts were cultured to observe the effects of the PPARγ ligand (15 d-PGJ2) and its agonists (troglitazone and ciglitazone) on the expression of TGF-β1-induced fibronectin. RT-PCR was used to detect the mRNA expression of fibronectin and Western blotting was used to detect the protein expression of fibronectin. RESULTS AND CONCLUSION: (1) The expression of TGF-β1 mRNA showed an effect in a dose- (0-5 μg/L) and time- (0-24 hours) dependent manner. (2) PPARγ ligands and agonists reduced the mRNA and protein expressions of fibronectin. Results suggested that PPARγ agonists can inhibit TGF-β1-induced fibronectin synthesis after stimulation by SiO2 and may play a potential role in preventing pulmonary interstitial fibrosis.
