1.Analysis of 197 Cases of Adverse Drug Reactions in the Nervous System

Objective:To observe the factors of adverse drug reactions in the nervous system.Methods:197 cases of adverse drug events in the nervous system were collected and analyzed in our hospital from January,2007 to December, 2008.Results:In the 197 cases,80 patients(about 40.61%) were above 60 years old,and 149 patients(about 75.63%) were medicated by i.v.183 cases belonged to the scope of ADRs,and the other 14 cases,to ADEs.The main drugs were anti -bacterials which caused ADR/ADE of the nervous system,and the second were traditional Chinese drug preparations. Conclusion:The ADRs in the neurological system were involved in many kinds of drugs,and had many clinical manifestations. The clinicians should pay attention to them.
2.Analysis of Influential Factors and Improvement on the Medication Compliance of Patients with Diabetes

Objective:To analyze the influential factors on the medication compliance of the patients with diabetes, and investigate the measures to improve it.Method:A total of 200 patients were investigated with questionnaires and interview in the study.Result:There was no significant difference in the compliance among the patients with different sexes, ages and payment modes,while some significant results emerged with respect to their culture,incomes,courses and their initiatives,conceptions and knowledge in the drug use.Conclusion:In order to improve the compliance of patients, clinical pharmacists could do jobs in many ways by means of combinative measures.
3.Adverse Drug Reactions Induced by Anti-infectives:Analysis of 657 Cases

OBJECTIVE:To investigate the characteristics and regularities of adverse drug reactions(ADR) induced by anti-infective drugs in our hospital.METHODS:657 ADR cases induced by anti-infective drugs collected by the ADR monitoring center in our hospital in 2006 were classified and analyzed.RESULTS:13 categories(49 kinds) of anti-infective drugs were involved in the total 657 ADR cases.Of which,those patients aged above 60 years accounted for 30.14%.The ADR were chiefly induced by quinolones,followed by cephalosporins and penicillins etc,which chiefly manifested as lesion of skin and its accessories(53.12%),followed by the lesions of digestive system and nervous system etc.CONCLUSION:Multiple factors contributed to the ADR,to which great importance should be attached so as to reduce the incidence of ADR induced by anti-infectives.
4.Effects of dexmedetomidine on mRNA expression of triggering receptor expressed on myeloid cells 1 in peripheral blood mononuclear cells of rats with acute obstructive suppurative cholangitis

Objective To investigate the effects of dexmedetomidine (DEX) on the mRNA expression of triggering receptor expressed on myeloid cells 1 (TREM-1) in peripheral blood mononuclear cells of rats with acute obstructive suppurative cholangitis (AOSC).Methods Sixty healthy male Wistar rat models of AOSC induced by complete common bile duct ligation and injection of E.coli into the bile duct through an intubation tube were replicated successfully.After modeling,the peripheral blood was collected and mononuclear cells were isolated and cultured.According to random number table method,the mononeuclear cells were divided into model group (no drug added in culture of mononuclear cells) and low,medium and high dose DEX groups (final concentrations 0.4,0.8,1.2 μg/L DEX were in low,medium and high DEX mononuclear cell cultures,respectively).After the mononuclear cells were cultured for 24 hours,the levels of tumor necrosis factor-α (TNF-α),interleukins (IL-1 and IL-6) in the supernatant of the cultured mononuclear cells were detected by enzyme linked immunosorbent assay (ELISA).The level of C-reactive protein (CRP) was detected by immunity transmission turbidimetry.The expression of TREM-1 mRNA in the mononuclear cells was detected by reverse trantscription-polymerase chain reaction (RT-PCR).Results Compared with the model group,the levels of TNF-α,IL-1,IL-6,CRP were decreased,the TREM-1 mRNA expressions were down-regulated in the different DEX dose groups,and the degrees of descent in medium and high dose groups were more significant than those in low dose group [TNF-oα (ng/L):95.5±8.6,88.9±5.3 vs.131.1 ± 14.2;IL-1 (ng/L):53.5±8.3,48.3 ± 6.7 vs.73.7 ± 12.8;IL-6 (ng/L):266.9±26.2,252.1 ± 17.7 vs.349.9±40.4;CRP (ng/L):4.3 ± 1.1,3.9 ±0.7 vs.5.6 ± 1.7;TREM-1 mRNA (A value):0.43 ± 0.18,0.39 ± 0.16 vs.0.65 ±0.25,all P < 0.05].Conclusion DEX can down-regulate the expression of TREM-1 mRNA and inhibit the formation and secretion of inflammatory factors TNF-α,IL-1,IL-6 and CRP in peripheral blood mononuclear cells of rats with ASOC.
5.Effects of low-dose recombinant human growth hormone on severity of illness in patients with systemic sepsis

Objective To investigate the effects of growth hormone on illness severity and prognosis of septic patients. Methods Thirty septic shock patients were randomized into control group (10 cases) and growth hormone treatment group (20 cases) , to receive 8 U recombinant human growth hormone every day for 7 consecutive days, control group received NS. Serum concentration of procalcitonin ( PCT) , C-reactive protein ( CRP) , tumor necrosis factor-? ( TNF-?) and interleukin-6 ( IL-6 ) were measured on the 1st,4th,and 7th day. The severity of illness was assessed daily with the Acute Physiology and Chronic Health Evaluation-Ⅱ (APACHE-Ⅱ ) scoring system and the Elebute & Stoner's Sepsis scoring system. The efficacy also were evaluated on day 15 postoperation. Results The serum concentration of PCT, CRP, TNF-?, IL-6 decreased gradurally in both group after study starting. 6.Research on entity relation extraction of Chinese adverse drug reaction reports based on deep learning method

Adverse drug reaction(ADR)reports are acting as primary sources for post-marketing drug safety evaluation, which have important reference value for drug safety evaluation. In this article, bidirectional gated recurrent unit, a kind of deep learning method, was applied as the model for relation extraction of drugs and adverse reactions in free-text section of ADR descriptions in Chinese ADR reports, with attention as well as character embedding and word segmentation embedding added into the network. The experimental results showed that our model achieved good performance in the classification task of confirming the relationship of “Drug-ADR” pair(denial, likely, direct and post-therapy)in the ADR description, and the final model achieved an F-value of 87. 52%. The extracted information can assist in evaluating ADR reports and at the same time be utilized in tasks like statistical analysis of certain drugs and adverse events and ADR knowledge base construction to provide more research techniques for drug safety researches.
7.Cohort Study of Therapeutic Drug Monitoring on Meropenem in Intensive Care Unit Patients

Objective To evaluate whether individualized dosage regimen based on therapeutic drug monitoring (TDM) is beneficial for improving the rate of pharmacokinetics/pharmacodynamics (PK/PD) reaching the standard and anti-infection efficacy. Methods Totally, 36 cases in the intensive care unit ( ICU ) of Nanjing Drum Tower Hospital using meropenem during January 2015 to December 2015 were collected, and divided into intervention group and control group. Results On the fourth day of administration, meropenem concentration in intervention group was significantly higher than that of the control group (22.5 μgmL-1 and 17.5 μgmL-1, respectively, P=0.007).With minimal inhibitory concentration (Cmin)>8 μgmL-1 serving as target, the rate of reaching the standard was both 22. 2% on day 2, 100. 0% and 72. 2% on day 4 in intervention group and control group, respectively (P=0.015). With minimal inhibitory concentration (Cmin) >32 μgmL-1 serving as target, the rate of reaching the standard was both 0 on day 2, 38. 9% and 5. 5% on day 4 in intervention group and control group, respectively (P=0.015). The clinical curative rate of the intervention group and control group was 83.3% and 72.2%, respectively (P=0.437), and the failure rate was 16.7% and 27.8%, respectively.Bacteria clearance rate was 88.9%and 55.5% in the intervention group and control group, respectively(P<0.05). Conclusion Individualized dosage regimen based on TDM in ICU patients is beneficial to improving the rate of PK/PD reaching the standard and anti-infection efficacy.
8.Determination of Phosphates, Tetrabromobisphenol A and Sudan Ⅰ in Children Products by Ultra Performance Liquid Chromatography-Mass Spectrometry

A reliable UPLC-MS method was developed for the simultaneous determination of 4 chemicals (Sudan Ⅰ, tetrabromobisphenol A, tris ( 1, 3-dichloroisopropyl ) phosphate and tris ( chlorisopropyl ) Phosphate) in children products. The samples were ultrasonic extracted with acetonitrile, and then the four chemicals were separated on a C18 column in 3 min. Results showed that the limit of quantification of the method was between 5 and 500μg/kg. The calibration curves were linear within 2-3 orders of magnitude with typical correlation coefficient above 0 . 9995 . The recoveries ranged from 83 . 7% to 97 . 8% with three addition levels. The sensitivity, recovery and selectivity of the method could fully meet the requirements of practical work.
9.Effect of Xinfeng Capsule on lung function in rats with adjuvant-induced arthritis and its mechanism.

OBJECTIVETo investigate the effects of Xinfeng Capsule (XFC) on pulmonary function and related mechanism in adjuvant-induced arthritis (AA) rats.
METHODSThe rats were randomly divided into five groups: normal control (NC), model control (MC) groups, methotrexate (MTX), tripterygium glycosides tablet (TPT) and Xinfeng capsule (XFC) treatment groups. The adjuvant-induced arthritis model was established by intracutaneous injection of 0.1 mL Freund ' s complete adjuvant in the right paw of rats; the drugs were given 19 d after model establishment. The toe swelling degree (E), arthritis index (AI), pulmonary function, peripheral blood Treg levels, pathological changes of lung tissue and expression of Foxp3, TGF-β1, Smad3, Smad7 proteins in lung tissue were measured 30 d after drug administration.
RESULTSCompared to NC group, the levels of E, AI, alveolitis score, TGF-β1 and Smad3 were significantly increased (P <0.05 or P <0.01); maximum expiratory flow 25% of vital capacity (FEF25),50% maximal expiratory vital capacity flow (FEF50), maximum expiratory flow at 75% of vital capacity (FEF75), maximum mid-expiratory flow (MMF), force peak expiratory flow (PEF), CD4+ CD25+ Treg, Foxp3 and Smad7 were significantly decreased in MC group (P <0.05 or P < 0.01). Compared to MC group,the expression of E, AI, TGF-β1 and Smad3 were reduced, while FEF50, FEF75, MMF, PEF, Treg, Foxp3 and Smad7 were elevated in XFC group (P <0.05 or P <0.01). Compared to XFC group, the level of body mass,FEF25,FEF50, FEF75, MMF and Treg were lower in MTX and TPT groups (P <0.05 or P <0.01).
CONCLUSIONThere are inflamed joints and reduced pulmonary function in rats of adjuvant-induced arthritis. XFC can inhibit paw edema degrees, reduce arthritis response, and improve pulmonary function, which is associated with up-regulating expression of Treg and Foxp3, down-regulating the expression of TGF-β1 and adjusting TGF-β1/Smads signal pathway.

10.Epidemiology of intussusception related hospitalizations in children aged <2 years in Suzhou, 2007-2013

Objective To understand the epidemiological and clinical characteristics of < 2 years old children hospitalized due to intussusceptions.Methods Clinical and demographic data of <2 years old children hospitalized due to intussusception between January 2007 and August 2013 were retrospectively collected in Affiliated Children' s Hospital of Soochow University in Suzhou.The incidence data,age distribution,seasonality and clinical characteristics of hospitalized intussusceptions cases were analyzed.Results A total of 594 intussusception-related hospitalizations were identified during this period in children aged <2 years,no death occurred.The crude incidence of hospitalized intussusception was 57.3 per 100 000 in children aged <2 years (95%CI:52.8-62.1),and 100.6 per 100 000 in children aged <1 year (95%CI:92.1-109.8).The male to female ratio was 1.90 ∶ 1.Up to 85.4% (507/594) of the cases were aged < 1 year,and 66.2% (393/594) of the cases were aged 3-8 months.The incidence peaked in age group 5-8 months.The median age of the cases was 6.8 months (QR=4.4),and increased from 6.3 months (QR=4.2) in 2007 to 7.3 months (QR=4.0) in 2013.No obvious seasonality was observed.Main symptoms or signs included vomiting (83.2%,494/594),abdominal mass (81.1%,482/594),and bloody stool (64.5%,383/594).Abdominal ultrasonic testing was the most frequently used diagnostic approach (98.7%,586/594).Up to 86.2% (512/594) of patients were successfully treated by surgical intervention.The main sites for acute intussusception in children aged <2 years were ileo-colic (34.5%,183/530),ileo-ileo (30.8%,163/530) or ileo-ileo-colic (27.9%,148/530).Conclusion The incidence of hospitalized intussusception in children aged <2 years was high in Suzhou.It is necessary to establish an active surveillance system to provide baseline data for the evaluation of rotavirus vaccine safety.
