1.Effect of Chaihu Shihuang soup on blood serum level TNF, IL-6 and IL-10 of severe acute pancratitis (SAP).

OBJECTIVETo make a study of the effect of Chaihu Shihuang soup on blood serum level TNF, IL-6 and IL-10 of severe acute pancratitis.
METHODSixty SAP cases were divided at random into treatment group and control group (30 cases each). The treatment group were given chai-hu-shi-huang-soup once a day; the control group were given Dachaihu soup once daily, with both treatment period for 7 days. The tumor necrosis factor-alpha (TNF-alpha), interleukin level 6 (IL-6) and 10 (IL-10) of both groups were determined before and after the treatment. We also had 30 healthy cases.
RESULTIL-10 level in both groups was lower than healthy group while TNF-alpha and IL-6 were higher than healthy group (P < 0.01). After treatment, the treatment group's TNF-alpha, IL-6 level were comparatively lower than that in the control group (P < 0.01), and IL-10 level were clearly higher than that of the control group (P < 0.01), comparison of the treatment group before treatment (P < 0.01).
CONCLUSIONChaihu Shihuang soup can significantly regulate cytokine and improve early recovery of SAP.

2.Cytokine changes in community-acquired pneumonia in elderly and intervention of traditional Chinese medicine.

OBJECTIVETo make a study of the cytokine changes in community-acquired pneumonia (CAP) in the elderly and the intervention of traditional Chinese medicine that can clear away the lung-heat and dissipate blood stasis (Qingfeihuayu soup).
METHODThe 82 cases with CAP in the elderly were divided at random into two treatment group and control group. Based on heteropathy, the treatment group was given Qingfeihuayu soup two times a day. The control group was given Rocephin 2 g once daily for 7 days. The clinical effect and the changes in TNF-alpha, IL-6 and IL-10 were observed before and after the treatment. A healthy group was also set up.
RESULTBefore treatment, IL-6 and TNF-alpha in both groups were higher than the healthy group (P < 0.01) and IL-10 lower than the healthy group (P < 0.01). After treatment, IL-6 and TNF-alpha in both groups decreased (P < 0.01) while IL-10 in treatment group increased. There existed a great difference compared with the control group (P < 0.01). The total effective rate in the treatment group is 92.50% while the control group is 85.71%. thus have a great difference (P < 0.05).
CONCLUSIONDuring the process of the development of CAP in the elderly, there existed the phenomenon of the excessive release of TNF-alpha, IL-6 and the too much inhibition of IL-10. The unbalance of TNF-alpha, IL-6, IL-10 can be a monitoring index reflecting the severity of the disease. The Chinese medicine Qingfeihuayu soup has obviously have regulating and clinical effect.

3.Comparison of patient-specific distal femoral cutting guide and conventional instruments in total knee arthroplasty

Objective To design a patient-specific distal femoral cutting guide for total knee arthroplasty using rapid prototyping and 3D printing technology and compare with conventional instrumentation.Methods A prospective self-control study was performed in 32 patients who underwent bilateral total knee arthroplasty between March 2015 and November 2015 in our department.The bilateral knee joints were randomly divided into patient-specific guide group and traditional surgery group.The patient-specific guide group obtained CT data of the lower extremities preoperatively.Patient-specific distal femoral cutting guides were designed and manufactured using rapid prototyping and 3D printing techniques.The traditional surgery group were performed using conventional instrumentation.The operation time of the two groups was recorded,and the amount of distal femoral osteotomy was measured.Postoperative alignments were measured including the angle of the lower limb mechanical axis angle (hipknee-ankle Angle,HKA),the mechanical lateral distal femoral angle (mLDFA),and the mechanical proximal tibial angle (mMPTA).In different follow-up points Hospital for Specific Surgery scores of the knee were recorded.Results In the coronal position,the postoperative mLDFA was 90.34°± 1.6° in the patient-specific guide group and 91.37°± 1.8° in the conventional surgery group.The difference between the two groups was statistically significant (t=-2.452,P=0.020).In the patient-specific guide group,the HKA angle was 0.36°±2.35°,and the conventional surgery group was 0.87°±1.85°.The difference between the two groups was statistically significant (t=-2.332,P=0.043).If preoperative mLDFA≤93°,there was no significant difference in postoperative mLDFA between the two groups (t=-1.409,P > 0.05).If preoperative mLDFA > 93°,there was a significant difference in postoperative mLDFA between the two groups (t=-4.145,P=0.004).In addition,the operation time of the patient-specific guide group was significantly shorter (t=-2.425,P < 0.05),but the two groups did not have significant functional differences in the early postoperative period.Conclusion The 3D-printed patient-specific distal femoral cutting guide can significantly shorten the operation time and improve postoperative alignments.It is simple to operate.However,large sample sizes and long-term follow-up studies are still needed to verify their long-term effects.
4.Disposal of a COVID-19 outbreak caused by imported case of an Omicron variant strain

Abstract: Objective To retrospectively analyze the investigation and disposal of the COVID-19 outbreak caused by the transmission of the Omicron variant in infected imported cases, and provide basis for COVID-19 outbreak management. Methods The description epidemiological method was used to describe the COVID-19 outbreak in Sanya City from March 31 to April 15, 2022. The propagation chain was mapped and the experience gained and shortcomings identified in emergency responses were analyzed. Results The outbreak resulted in 95 reported locally transmitted COVID-19 cases with a incubation period M(P25, P75) of 4 (3, 5) d. In the 95 cases, the proportion of cases detected through close contact screening, centralized isolation, community screening, control area screening, active treatment (examination), and key population screening were 33.68%, 22.11%, 18.95%, 12.63%, 6.32%, 4.21% and 2.11%, respectively. The epidemic spread for 6 generations, causing 5 clusters of outbreaks and 12 cases of cluster disease. The epidemic affected 12 villages/neighborhood committees, 1 bar, 1 hospital, 1 small clinic, 1 farmer's market, 1 large shopping mall and 1 restaurant in 2 districts of Sanya City. The result of gene sequencing was Omicron variant BA.1.1. Through the immediate launch of emergency plans, nucleic acid and antigen testing, controlling close contact between infected persons and close contacts, suspending indoor business sites, central urban control, and temporary suspension, COVID-19 was controlled within 16 days. Conclusions The transmission chain of this outbreak was clear and was caused by imported cases. Strengthening the management of the pass, doing a good job in information sharing and docking, timely screening for cases, screening, pushing, controlling high-risk groups, and implementing comprehensive control measures, can effectively prevent the spread of the epidemic, providing a reference for the control of epidemic situations in relevant scenarios.
5.Influence of intervene of traditional Chinese medicine on cytokine level in benign prostatic hyperplasia.

OBJECTIVETo make a study of the influence of the intervene of traditional Chinese medicine on cytokine level in benign prostatic hyperplasia.
METHODThe patients were divided into treatment group (group A), control group (group B) and healthy group (group C) with the randomly compared researching method. Group A were given Qianliening Tang (QLNT), daily potion, two times a day. Group B were given Qianliekang, three times a day, three pills each time. The course of both treatment lasted for 8 weeks.
RESULTBefore treatment, TNF-alpha, IL-17/IL-10, IL-4 in group A and B are obviously out of order compared with group C (P < 0.01). After the treatment, TNF-alpha, IL-17 in group A decreased dramatically and IL-10, IL-4 increased, which shows great differences compared with those before treatment and with group B (P < 0.01), but still can't reach to group C level.
CONCLUSIONQian liening Tang can efficiently regulate the level of suppressive inflammatory cytokines and proinflammatory cytokines. It provides scientific experimental basis for clinical diagnosis and treatment of the disease.

6.Influence of exogenous taurine addition in amino acid content in brains of hypoxic-ischemic brain injury rats during lactation

Objective To investigate the effect of exogenous taurine (TAU) on amino acid content in the brains of rats suffered hypoxic-ischemic brain injury (HIBD).Methods The 7-day-old SD rats were randomly divided into sham-operated group, control group and TAU treatment group.HIBD models in the later two groups were established following Rice-Vannucci method.On the basis of breastfeeding, TAU at 300 mg/(kg ·d) via tube feeding to the TAU treatment group and an equal volume saline solution to the sham-operated group and control group were given.The aspartic acid (ASP), glutamic acid (GLY), glycine (GLY), TAU and γ-aminobutyric acid (GABA) contents in the rat brains were measured by high-performance liquid chromatography at 48 h and 14 d after HIBD.Results At 48 h after HIBD, the TAU, ASP and GLU contents in the TAU treatment group were significantly increased as compared with those in the control group (P<0.05);the GLU/TAU ratio in the TAU treatment group was significantly decreased as compared with that in the control group (P<0.05).Fourteen d after HIBD, the TAU, GABA and GLY contents were significantly increased, the GLU content was significantly decreased, and the ASP/GLY, GLU/GLY and GLU/TAU ratios were significantly deceased in the TAU treatment group as compared with those in the control group (P<0.05).Conclusion Exogenous TAU addition during lactation of rats suffered HIBD may improve contents of inhibitory amino acids as TAU, GABA and GLY, and modulate the ratio of main excitatory/inhibitory amino acids neurotransmitters.
7.Diagnosis and clinical characteristics analysis in elderly patients with severe pneumonia caused by Chlamydia psittaci

Objective:To investigate the diagnosis and clinical characteristics of five elderly patients with Chlamydia psittaci-caused severe pneumonia.Methods:Through retrospective analysis, diagnosis and treatment process and clinical characteristics of five elderly inpatients with severe pneumonia caused by Chlamydia psittaci were summarized in the Department of Critical Care Medicine, Quanzhou First Hospital East Street Branch Area, affiliated to Fujian Medical University between January to June 2021.Results:Five patients with severe pneumonia caused by Chlamydia psittaci were aged from 64 to 74 years, with various underlying diseases such as coronary heart disease, chronic heart failure, chronic obstructive pulmonary disease, etc.All patients had an established history of poultry exposure.These cases had high fever, cough, spitting, and dyspnea as the main clinical manifestations.Some of them also had systemic symptoms or weakness of the limbs as the prodromal symptoms.The disease progressed rapidly, with severe respiratory failure, acute kidney injury, and shock appearing soon, accompanied by different degrees of muscle injury, and damage to the heart, liver, blood coagulation, and immune systems at the same time.Laboratory examination showed that levels of inflammatory indicators were increased: at 3, 5, 7 d after admission, the level of C reactive protein was 214.6(153.9-256.3)mg/L, 199.2(115.8-333.8)mg/L, 151.0(11.19-173.7)mg/L, respectively; and interleukin 6 level was 1 241.0(912.1-6822.0)ng/L, 779.1(451.2-7122.0)ng/L, 631.2(7.0-4 321.0)ng/L, respectively.And monitoring results of nutritional index indicated a high metabolic state.The imaging examinations showed that consolidation and ground-glass shadows spreaded to both lungs, may accompany the miliary and nodular shadows, and may also involve pleural which caused pleural effusion.After the clinical use of metagenomic next-generation sequencing(mNGS), mNGS has been confirmed as an important method for the diagnosis of Chlamydia psittaci infection.The disease course and prognosis were related to the severity of the disease, advanced age, underlying diseases, and timely diagnosis and effective treatment.Conclusions:The progression of Chlamydia psittaci pneumonia to severe disease may be related to advanced age, more basic diseases such as chronic cardiopulmonary disease, smoking, and timely diagnosis and treatment.Generally, laboratory and imaging examinations have no diagnostic specificity, but mNGS is of great significance for early diagnosis, transition to target treatment and improvement of prognosis.
8. Establishment and practice of a point-of-care testing management system in hospitals

With the market-oriented application of new technologies and the development of various subjects in hospitals, more and more point-of-care test(POCT) devices appear in medical institutions, as an important method for rapid clinical acquisition of test results. As introduced by the authors, the hospital since 2017, established based on the hospital′s total quality management system, the POCT management committee, and built the hospital′s POCT quality management system. Thanks to its efforts, the hospital has fully standardized the POCT management from equipment procurement, test, maintenance, personnel training and examination, thus further ensuring medical quality.
