1.Clinical analysis of acute invasive fungal sinusitis with orbital infection.

The clinical manifestation of acute invasive fungal sinusitis was associated with facial pain,altered sense of smell, blindness and headache. Physical examinations show that dark brown nasal secretions with bone resorption in paranasal sinus. Radiographi parameters showed uneven density in paranasal sinus and intraorbital extension. Fungus smears and pathological examination can make a definitive diagnosis.

2.A clinical study on obstructive sleep apnea hypopnea syndrome treating by nasal operation.

OBJECTIVE:
To study the treatment of OSAHS with nasal diseases by nasal operation.
METHOD:
Ninety four cases of OSAHS with nasal diseases were analyzed. The patients were operated on nasal cavity to improve ventilation included Septectomy, CPS for inferior turbinate and FESS. The patients were examined by PSG at 2 months and 1 year after operation. All patients were followed up one year.
RESULT:
Among 94 cases, 19 cases had efficient result after nasal operation and had no recurrence one year after operation. The efficient rate was 20. 21% (19/94). These patients were mild degree and I type. The other 75 cases had invalid result.
CONCLUSION
The patients of OSAHS with nasal diseases, especially mild degree and I type one should be undertaken nasal operation.

4.Cytokine changes in community-acquired pneumonia in elderly and intervention of traditional Chinese medicine.

OBJECTIVETo make a study of the cytokine changes in community-acquired pneumonia (CAP) in the elderly and the intervention of traditional Chinese medicine that can clear away the lung-heat and dissipate blood stasis (Qingfeihuayu soup).
METHODThe 82 cases with CAP in the elderly were divided at random into two treatment group and control group. Based on heteropathy, the treatment group was given Qingfeihuayu soup two times a day. The control group was given Rocephin 2 g once daily for 7 days. The clinical effect and the changes in TNF-alpha, IL-6 and IL-10 were observed before and after the treatment. A healthy group was also set up.
RESULTBefore treatment, IL-6 and TNF-alpha in both groups were higher than the healthy group (P < 0.01) and IL-10 lower than the healthy group (P < 0.01). After treatment, IL-6 and TNF-alpha in both groups decreased (P < 0.01) while IL-10 in treatment group increased. There existed a great difference compared with the control group (P < 0.01). The total effective rate in the treatment group is 92.50% while the control group is 85.71%. thus have a great difference (P < 0.05).
CONCLUSIONDuring the process of the development of CAP in the elderly, there existed the phenomenon of the excessive release of TNF-alpha, IL-6 and the too much inhibition of IL-10. The unbalance of TNF-alpha, IL-6, IL-10 can be a monitoring index reflecting the severity of the disease. The Chinese medicine Qingfeihuayu soup has obviously have regulating and clinical effect.

5.Analysis of food allergens in patients with allergic rhinitis in Guangzhou.

OBJECTIVE:
To investigate the distribution of multiple food allergens in patients with allergic rhinitis in Guangzhou area.
METHOD:
Skin prick tests were performed in 1529 patients with allergic rhinitis. The result was discussed by the age of patients, the types of allergens and the response intensity.
RESULT:
Five hundred and eighty-seven cases were positive. The most important food allergens were crab (25.6%), prawn (19.4%). There were statistical differences of positive reactions among three age groups (P < 0. 05). The main food allergen was seafood.
CONCLUSION
Crab and prawn are the main food allergens in patients with allergic rhinitis in Guangzhou. It is helpful for clinical diagnosis and treatment.

6.Effect of deep pharyngeal nerve stimulation combined withXingnao-Kaiqiao acupuncture for the patients with Pharyngeal dysphagia after stroke

Objective To discuss the effect of deep pharyngeal muscle nerve stimulation combined with Xingnao-Kaiqiao acupuncture on patients with Pharyngeal dysphagia after stroke . Methods According to the digital table method, 99 patients with Pharyngeal dysphagia after stroke were randomly divided into A group, B group and C group, 33 cases in each group. A group received deep pharyngeal muscle nerve stimulation
combined with Xingnao-Kaiqiao acupuncture; B group received deep pharyngeal nerve stimulation; and C group received Xingnao-Kaiqiao acupuncture treatment. The conventional treatment were used in all groups After 4 weeks of treatment, the relevant evaluation indexes were compared before and after treatment. Results After treatment, the Grade of aspiration (4.2 ± 0.5, 4.0 ± 0.4 vs. 3.0 ± 0.5, F=6.117) was significantly higher than A group (P<0.05), dysphagia score (5.3 ± 1.3, 5.1 ± 1.1 vs. 7.6 ± 1.4, F=6.541) in B and C groups was significantly lower than A group (P<0.05). The total effective rate [96.4% (27/28), 85.7% (24/28) vs. 82.1% (23/28), χ2=4.766, 7.791] of A group was significantly higher than that of B group and C group (P<0.05). Conclusions The deep pharyngeal nerve stimulation combined with Xingnao-Kaiqiao acupuncture could treatthe patients with dysphagia after stroke.
7.Influence of Weipixiao on Histopathological Changes of Gastric Mucosal Tissue in Rats with Gastric Precancerous Lesions

Objective To observe the effect of Weipixiao, a compound recipe which has the actions of strengthening spleen, resolving stasis and removing toxins, on the histopathological changes of gastric mucosal tissue in rats with gastric precancerous lesions ( GPL) . Methods SD rats were randomly divided into normal group, model group, Vitacoenzyme group (0.2 g·kg-1·d-1), and high-, middle-, and low-dose Weipixiao groups ( in the dose of 15, 7.5, 3.75 g·kg-1·d-1, respectively) . Except for the normal control group, the rats in other groups received spontaneous intake of N-methyl-N’-nitro-nitrosoguanidine ( MNNG) solution combined with irregular diet and oral use of purgative herbs for 18 weeks to induce GPL. From the 9th week, the mediation groups were simultaneously given corresponding medicine for 10 weeks. At the end of the experiment, the histopathological changes of gastric mucosal tissue in all groups were observed. Results Pathological scores of intestinal metaplasia and epithelial dysplasia in rat gastric mucosa of the model group were significantly increased ( P<0.01 compared with those of the normal group) , but were decreased in three Weipixiao groups to various degrees, particularly in low-dose Weipixiao group ( P<0.05 or P<0.01) . Conclusion Weipixiao can block and reverse gastric intestinal metaplasia and dysplasia in GPL rats to certain degrees, and low-dose Weipixiao may have better long-term effect for the prevention and treatment of GPL.
8.Data mining research on traditional Chinese medicine formulas for epilepsy treatment based on national patents

Objective To investigate the medication and compatibility patterns of traditional Chinese medicine(TCM)compound patents in the treatment of epilepsy through data mining research.Methods Data on TCM compound patents for treating epilepsy were retrieved from the China National Intellectual Property Administration's patent publication website.Descriptive analysis,association rule analysis,and cluster analysis were conducted using softwares such as Excel,Origin 2021,R 4.3.1 and Cytoscape 3.9.1,and the relevant topological property values were calculated to construct the core complex network.Results A total of 346 TCM compound patents for treating epilepsy were included,involving 738 different herbs.The top 5 frequently used herbs were Acorus tatarinowii,Gastrodia elata,Buthus martensii,Eupolyphaga seu steleophaga,and Uncaria rhynchophylla.The predominant drug category was liver-soothing and wind-extinguishing drugs.Cluster analysis identified two major categories of drug combinations,and the core combination in the co-occurrence network was Rhizoma acori tatarinowii-Scorpio-Gastrodiae rhizoma-Bombyx batryticatus-Uncariae ramulus cum uncis.A total of 23 association rules were obtained.Conclusion This study reveals the primary characteristics of treating epilepsy,which mainly focus on therapeutic directions such as soothing the liver and suppressing wind,calming wind to arrest convulsions,and calming the mind to tranquilize the spirit.It incorporates individualized dialectical diagnostic and treatment strategies,such as promoting blood circulation and resolving stasis,invigorating the spleen,and transforming phlegm.This discovery aims to provide reference and reference value for the clinical treatment of epilepsy in traditional Chinese medicine.
9.Venetoclax combined with hypomethylating agents in treatment of blastic plasmacytoid dendritic cell neoplasm: report of 5 cases and review of literature

Objective:To investigate the efficacy and safety of venetoclax (VEN) combined with hypomethylating agents (HMA) in the treatment of blastic plasmacytoid dendritic cell neoplasms (BPDCN).Methods:A retrospective case series study was conducted. The clinical data of 5 patients with BPDCN treated with VEN combined with azacitidine (AZA) or decitabine (DAC) in the First Affiliated Hospital of Soochow University and Suzhou Hongci Blood Disease Hospital from February 2017 to July 2023 were collected, and the therapeutic effect, adverse reaction and prognosis of all 5 patients were summarized.Results:All 5 BPDCN patients were male with the median onset age [ M ( Q1, Q3)] of 66 years (51 years, 73 years), of which 4 cases were presented with skin lesions and 1 case was presented with lymphadenopathy as the primary symptom. As for the treatment, 3 patients were initially treated with VEN in combination with AZA induction regimen, among which 2 patients achieved complete remission with incomplete blood count recovery (CRi) after 2 cycles of treatment, survived for 26.5 months and 14.6 months, respectively and finally died, and 1 patient achieved partial remission after 1 cycle of treatment and he still survived after 3-month follow-up; 1 patient was initially treated with VEN in combination with DAC induction regimen, and achieved clinical complete remission of non-active disease with residual skin abnormalities after 2 cycles of treatment followed by allogeneic hematologic stem cell transplantation (allo-HSCT) and he was in the state of disease-free survival for 15-month; and another 1 patient experienced a relapse after treatment with acute lymphocytic leukemia-like regimen in combination with allo-HSCT and again achieved CRi after 2 treatment courses of VEN in combination with AZA regimen, and he was in the state of disease-free survival for 30-month follow-up. Treatment-related haematological adverse effects of VEN combined with HMA were mainly neutropenia with fever, reduction of hemoglobin and thrombocytopenia; and non-haematological adverse effects were mainly gastrointestinal reactions such as nausea and vomiting. These adverse events improved with symptomatic supportive therapy, and no treatment-related deaths occurred. Conclusions:BPDCN patients who are unable to tolerate intensive chemotherapy regimens at initial time of diagnosis may attempt induction therapy with VEN+HMA regimen, which has a manageable adverse reaction and may serve as a bridge to allo-HSCT.
10.Association of poly(ADP-ribose) polymerase activity in circulating mononuclear cells with myocardial dysfunction in patients with septic shock.

BACKGROUNDSevere sepsis and septic shock are the leading causes of morbidity and mortality in hospitalized patients. This study aimed to investigate the association of poly (ADP-ribose) polymerase-1 (PARP-1) activity in circulating mononuclear cells with myocardial dysfunction in patients with septic shock.
METHODSA total of 64 patients with septic shock were divided into the survival group (n = 41) and the nonsurvival group (n = 23) according to mortality at 28 days after enrollments. PARP-1 activity in circulating mononuclear cells, brain natriuretic peptide, Acute Physiology and Chronic Health Evaluation II score, the cardiac index (CI), the cardiac function index (CFI), global ejection fraction (GEF), and the left ventricular contractility index (dp/dt max) were measured after admission to the intensive care unit.
RESULTSPARP-1 activity in circulating mononuclear cells of nonsurvival patients with septic shock was significantly higher than that in survival patients. PARP-1 activity in circulating mononuclear cells was strongly, negatively correlated with the CI, the CFI, GEF, and dp/dt max. Multiple Logistic regression analysis showed that PARP-1 activity in circulating mononuclear cells was an independent risk factor of myocardial dysfunction. The optimal cutoff point of PARP-1 activity for predicting 28-day mortality was 942 nmol/L with a sensibility of 78.2% and specificity of 65.1%.
CONCLUSIONPARP-1 activity in circulating mononuclear cells is significantly associated with myocardial dysfunction and may have prognostic value in patients with septic shock.
