1.Precision medicine in castration-resistant prostate cancer

Most prostate cancer (PCa) patients in China are diagnosed at an advanced stage. Many PCa patients are initially sensi-tive to hormonal therapy and experience temporary tumor regression, but nearly all of the patients will finally reach a state of castration-resistant prostate cancer (CRPC). CRPC is difficult to cure and thus has poor prognosis. Identification of new therapies to treat CRPC remains an urgent need. Precision medicine individualizes CRPC treatments by tailoring them to the genomic characteristics of CRPC. In this review, we summarize the recent progress of pathogenesis, molecular targeted therapy, and clinical trials of CRPC as well as dis-cuss precision medicine for CRPC.

Precision medicine is to develop the most appropriate individualized treatment for each patient based on the level of individual differences. Genomic, proteomics, metabolomics data, and other big data analysis methods are the essence of precision medicine. Precision medicine brings the hope to overcome cancer. In this review, we summarize the connotation of precision medicine in CRPC, the application of second generation sequencing and genome sequencing, and clinical molecular targeted therapy of CRPC as well as discuss clinical precision medicine for CRPC.
3.Research progress on PLK1 in castrate-resistant prostate cancer

Polo-like kinase 1 (PLK1) is a highly conserved serine/threonine protein kinase that has attracted research attention be-cause it plays a critical role in mitosis regulation. PLK1 is overexpressed in 80%of human tumors, which indicates a poor prognosis in most tumors. PLK1 is one of the most promising targets for antitumor therapy because it is upregulated in castrate-resistant prostate can-cer (CRPC). This review focused on the basic structure and function of PLK1, the relationship between PLK1 and CRPC occurrence and progression, and CRPC treatment by inhibiting PLK1. This study provides a theoretical basis for the targeted molecular therapy of CRPC.
4.Diagnostic value of dynamic contrast-enhanced magnetic resonance imaging in the detection of residual or recurrent prostate cancer after radical prostatectomy:A meta-analysis

Objective:To determine the diagnostic performance of dynamic contrast-enhanced MRI (DCE-MRI) in patients with recurrent or residual prostate cancer after radical prostatectomy. Methods:Studies were systematically searched in the PubMed, EMBASE, Cochrane library, SCI, CBM, CNKI, VIP, WanFang, and other databases. Additional studies were manually searched using the references of the retrieved articles. The retrieved deadline time was June 6th, 2013. Studies were eligible for inclusion based on the inclusion and exclusion criteria, and the qualities of the studies were reviewed based on the QUADAS criteria. The Meta Disc 1.4 software was used for meta-analysis, and a summary receiver operating characteristic (SROC) curve was constructed. The patient-based pooled weighted estimates of the sensitivity, specificity, diagnostic odds radio, and 95%confidence interval were calculated. Results:Among the 118 eligible studies, 7 articles (12 studies) were included in the meta-analysis. The pooled weighted estimates of the sensitivity, specificity, and the area under the curve were 0.88 (95%confidence interval:0.84 and 0.91), 0.87 (95%confidence interval:0.81 and 0.92), and 0.939 1, respectively. The pooled diagnostic radio (dOR) was 50.4 (95%confidence interval:26.0 and 97.6) and Q*was 0.876 4. Conclusion:DCE-MRI has high sensitivity and specificity in the evaluation of locally recurrent or residual prostate cancer after radical prostatectomy, in which the diagnostic efficiency of DCE-MRI combined with magnetic resonance spectroscopy is much better.
5.Biomechanical changes of spine after thoracolumbar fracture and dislocation treated with pedicle screw internal fixation

BACKGROUND:Anterior decompression, bone graft fusion, plate and screw fixation are commonly used in clinic for the treatment of thoracolumbar burst or comminuted fractures. The posterior surgery including decompression laminectomy and pedicle screw internal fixation.
OBJECTIVE:To explore the biomechanical changes of spine after thoracolumbar fracture and dislocation treated with pedicle screw internal fixation.
METHODS:Forty-six patients with thoracolumbar fracture and dislocation were treated with pedicle screw internal fixation, and then the recovery of spinal cord injury and fracture reduction were observed after internal fixation, immediately after internal fixation and 1-year fol ow-up.
RESULTS AND CONCLUSION:Al the 46 cases were fol owed-up. Frankel function classification assessment was used to evaluate the spinal cord injury grade. There was no significant difference in the percentage of A grade between periods (P>0.05), and there were significant differences in the percentage of B-E grades when compared between immediately postoperative period, 1-year fol ow-up period and preoperative period (P<0.05), but there was no significant difference when compared between immediately postoperative period and 1-year-fol ow period (P>0.05). The anterior height percentage, posterior height percentage and Cobb angle indicators were used to assess the fracture reduction, and the results showed there was no significant difference in posterior height between periods (P>0.05), and there was significant difference in Cobb angle when compared between immediately postoperative period, 1-year fol ow-up period and preoperative period (P<0.05), but there was no significant difference between immediately postoperative period and 1-year fol ow-up period (P>0.05). The results indicate that pedicle screw internal fixation system for the treatment of thoracolumbar spinal fracture and dislocation is conducive to the fracture reduction and functional recovery.

7.Diagnosis and treatment of renal angiomyolipoma with hemorrhage (a report of 18 cases)

Objective To investigate the diagnosis and treatment of renal angiomyolipoma with hemorrhage. Methods Eighteen cases of renal angiomyolipoma with hemorrhage were diagnosed by imaging such as ultrasound and CT from June 2000 to December 2007.Open operations were adopted in 15 cases. Expectant treatment was adopted in 3 cases. Results Hemorrhage stopped in 3 cases with expectant treatment.All the operations succeeded.Pathological diagnosis after operations was renal angiomyolipoma.The follow-up time was 3 to 60 months.Ultrasound,CT and intravenous pyelography were mainly methods.There was no case of relapse or fistula.Tumor size of 3 cases with expectant treatment changed little. Conclusions Diagnosis of renal angiomyolipoma with hemorrhage should be based on clinic and imaging.CT and ultrasound have better diagnosis effect.Expectant treatment,selective embolization of renal artery,tumor excision and heminephrectomy are main methods.Nephrectomy can also be adopted when there is serious hemorrhage.
8.Phosphodiesterase 4 as a target for the development of novel drugs against cognitive impairment:research advances

Alzheimer′s disease(AD)is one of the most common causes of cognitive impairment.“Aβhypothesis”and“tau protein aggregation hypothesis”are two representative hypotheses in relation to AD pathology. But recently,therapeutic strategy target?ing on reducing Aβdeposition failed in clinical trials. On the other hand,as the phosphorylation of tau protein is regulated by multiple upstream kinases,inhibition of a single kinase usually cannot effectively suppress the aggregation of the tau. While blocking multiple kinases at the same time will produce serious side effects. Currently,targeting on Aβand tau protein get into awkward situations. In view of this,researchers are looking for new drug targets for improving cognitive function. Phosphodiesterase 4(PDE4 4)is an enzyme responsible for the hydrolysis of cAMP in the body. There are four subtypes for PDE4,and PDE4A,B and D are highly expressed in the central nervous system. Inhibition of PDE4 causes activation of cAMP/PKA/CREB/BDNF signal pathway,which is beneficial for the strengthening and consolidation of learning and memory. This review will focus on the most recent evidence regarding the role of PDE4 in learning and memory.
9.Changes of NO and iNOS expression in lung tissues and injury of lungs in traumatic shock rats of early stage preconditioned with heat stress

Objective To research nitric oxide( NO) concentration, induced nitric oxide synthase( iNOS) mRNA expression and the pathological changes of lung tissue in traumatic shock rats of early stage preconditioned with heat stress. Methods A total of 98 male SD rats were equally divided randomly into heat stress untreated group and heat stress treatment group, and each group were equally divided into 7 subgroup: control group and shock 0 h group, 0. 5 h group, 1. 0 h group, 1. 5 h group, 2. 0 h group, 3 h group. To established the model of traumatic shock after heat stress pretreatment and remained lung tissue, observed the changes of pulmonary pathology, meas-ured the changes of the NO concentration and iNOS expression in lung tissues. Results The lesions of lung tissue structure were more lighter in heat stress treatment group than the corresponding time point of heat stress untreated group and lung histopathology score lower. The NO concentration of heat stress treatment group were lower than the corresponding time point of heat stress untreated group, the two groups have a rise at shock 0 h,the peak value of NO concentration in heat stress treatment group appeared later than the heat stress untreated group. INOS mRNA expression quantity of heat stress treatment group were lower than the corresponding time point of heat stress un-treated group, the peak value of heat stress untreated group in 2.0 h and heat stress treatment group in 1. 5 h, it was positively correlated with lung injury score. Conclusion Heat stress pretreatment can drop NO concentration and iNOS mRNA expression in lung tissues significantly, delayed and reduced the lung injury in traumatic shock rats of early stage.
10.Progress in molecular prognostic biomarkers of castration-resistant prostate cancer

Prostate cancer is one of the most common malignant tumors among males, and nearly all patients will inevitably de-velop castration-resistant prostate cancer (CRPC). The prognosis of CRPC patients is very poor. How to implement the individualized treatment plan with minimal side effects and optimum benefit as well as how to prolong the patients' lives have recently become impor-tant issues. Current staging and grading systems fail to provide accurate prognoses of most CRPC, which have complicated biological behaviors. Over the past few years, a host of tumor molecular markers have been discovered and can provide prognostic information for CRPC. This review summarizes the molecular prognostic biomarkers that affect the prognosis of CRPC.
