Objective:To evaluate the efficacy and safety of combined cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) with lobaplatin and docetaxel for treatment of synchronous peritoneal carcinomatosis (PC) from gastric cancer. Methods:Fifty patients with synchronous PC from gastric cancer were treated by 52 CRS+HIPEC procedures with 100 mg of lobaplatin and 120 mg of docetaxel in 12000 mL of normal saline at (43 ± 0.5)℃for 60 min. The primary and secondary endpoints were overall survival (OS) and perioperative safety profiles, respectively. Results:At a median follow-up of 22.5 months, the median OS rate was 14.3 (95%CI:7.6-21.0) months, and the 1-, 2-, and 3-year survival rates were 58%, 40%, and 32%, respectively. No perioperative deaths or serious adverse events occurred in 12 cases (23.1%). Multivariate analysis indicated that completeness of cytoreduction, nor-mality of perioperative tumor markers, and adjuvant chemotherapy of more than six cycles were independent predictors for improved survival. Conclusions:CRS+HIPEC with lobaplatin and docetaxel could improve the OS and ensure perioperative safety of patients with synchronous PC from gastric cancer.

Chinese Journal of Thoracic and Cardiovascular Surgery 2011;27(3):165-167
Objective There is a paucity of data regarding hepatic dysfunction (HD) following type A and B aortic dissection repair with deep hypothermic circulatory arrest (DHCA). We determine the incidence and outcomes for postoperative HD, and analyze the risk factors of death for HD. Methods Between January 2006 and June 2008, 208 patients have undergone open repairs of aortic dissection with DHCA. Indications for surgical intervention were type A aortic dissection in 181 patients and type B in 27 patients. 18 patients had postoperative hepatic dysfunction with abnormal hepatic enzyme and bilirubin. The mean patient age was 43 years and one third were women. Perioperative data including age, sex, type, surgery intervention, CPB time, aortic-clamp time and ICU retention time were collected. Complications were classified as bleeding, low cardiac output, acute renal failure, hypoxemia, infection, temporary neurologic dysfunction, multiple organ dysfunction and death.Serum GPT, LDH and TBIL were assayed and recorded before and after operation, as well as 12 h, 1 d, 3 d, 5 d, 7 d. Risk factors for death of hepatic dysfunction were ascertained by univariate and multivariable analysis. Results The incidence of hepatic dysfunction within one week following surgery is 8.7%. The mortality associated with HD was 39% compared with 1.6% (P<0.0001) in patients without HD. ICU retention time were significantly different (P<0.001) between HD grorp (11.9days) and non-HD group (4.2days). In this group, intraoperative and postoperative 24 hours blood transfusion volume (PRBC) >20 U occurred in 6 patients, reopen for bleeding in 3 patients, low cardiac output in 6 patients, sepsis in 1 patients, acute renal failure in 7 patients, hypoxemia in 5 patients, severe infection in 2 patients, temporary neurologic dysfunction in 5 patients, multiple organ dysfunction in 10 patients. Bleeding( P = 0. 024 ), low cardiac output (P = 0. 024 ), acute renal failure ( P = 0. 024), MOD ( P = 0.002) are the risk factors of death for hepatic dysfunction. And independent determinants were bleeding (P= 0.019) and MOD ( P = 0.001 ). Conclusion Multiple risk factors impact the onset of postoperative hepatic dysfunction. Bleeding and MOD after aortic dissection surgical repairs were associated with an increased mortality.

Chinese Journal of Pharmacology and Toxicology 2010;24(1):69-72
OBJECTIVE To investigate the effect of NF-κB on cardiomyopathy induced by doxorubicin(Dox) in rats and the effect of melatonin(MT). METHODS Thirty male Sprague-Dawley rats were randomly divided into 3 groups: normal control group; Dox model group; Dox+MT group. The content of nitric oxide (NO) and the activity of inducible nitric oxide synthase (iNOS) in cardiac tissues were examined by spectrophotography method; the activity of NF-κB in myocardial cell was observed by immunohistochemistry method; the apoptosis information of myocardial cell was detected by TUNEL method. RESULTS There was significant difference in activity of NF-κB between normal control and the Dox model group. MT could inhibit the activation of NF-κB(P<0.05). There was significant difference in the content of NO and the activity of iNOS between normal control and Dox model group, MT could decrease content of NO and the activity of iNOS(P<0.05); apoptotic rate in Dox model group was significantly increased compared with that in normal control group, MT could significantly decrease myocardial cell apoptosis (P<0.05). CONCLUSION The activation of NF-κB plays an important role in cardiomyopathy induced by Dox, NF-κB seems to increase the activity of iNOS, and then significantly increase the content of NO in cardiac muscle; NF-κB possibly enhances myocardial cell apoptosis in cardiomyopathy induced by Dox. MT may inhibit the activation of NF-κB, and prevent excessive oxidative stress and suppress myocardial cell apoptosis, MT has protective effect against cardiomyopathy induced by Dox.
