1.COMPARISON OF BIOPSY UREASE TEST, CULTURE AND HISTOLOGICAL EXAMINATION IN THE DIAGNOSIS OF CAMPYLOBACTER PYLORI INFECTION

A method of biopsy urease test to detect Campylobacter pylori infection is described. In 132 consecutive patients undergoing endoscopy, antral biopsies were taken for urease test, culture and histolo-gical examination. There were 89 patients infected with C. pylori, 86 of whom showed positive urease test: 73% of the patients were positive at 0.5h, 88% at 3h and 97% at 24h. There were no false positive results. Twelve infected patients were rebiopsied after a course of furazolidone and tetracycline. A negative test after antibiotic therapy correlated with clearance of C. pylori, Urease test is a simple, sensitive, specific and inexpensive test that enables the endoscopist to diagnose C. pylori infection in the endoscopy room.

3.A mechanical device for animal impact injury experiment and establish a rodent model of severe thoracic injury

Objective Direct impact is a common mechanism of injury for blunt thoracic injury, and if resulting thoracic injury is severe the mortality may be as high as 10% ~ 25% . We aim to develop a mechanical device for animal impact injury experiment, so as to establish a rodent model of severe thoracic injury. Method A spring operated mechanical device for animal impact injury experiment was developed. The device allowed for accurate controlled delivery of impact force to specific areas of the chest well, at specific velocities and degrees of chest compression. Eghty-four male Sprague-Dawley rats were anaesthetized and underwent left carotid artery cannulation. They were randomly divided into seven groups and given the following treatment: group A (Control group) were subjected to sham impact; group B to G animals were subjected to impacts on the right lateral superior chest at different velocities and degrees of chest wall compression. ( B 3 m/s, 20%; C 3 m/s 40%; D 6 m/s 20%; E 6 m/s 40%; F 9 m/s 20%; G 9 m/s 40%). Arterial blood gas samples were taken just before injury, and at 2 and 12 post injury. All rats were sacrificed at 12 hours and their degree of thoracic injury rated. Pathological examination of injured lung tissue was also performed. Results The device was able to deliver impact forces accurately, with < 4% deviation from desired velocity and < 3 mm deviation from target area of impact. Other than the control group, all animals experienced significant hemodynamic changes immediately post impact. Arterial blood gas analysis detected significant hypocapnia in groups B and C. Significant hypoxemia and hypocapnia was detected in groups D, E and F. In groups B,C,D and F, die impact produced a mild thoracic injury with low mortality rate at 12 hours. In group E, the impact produced severe thoracic injury with mortality rate of 33.33% at 12 hours. Group C animals sustained the most serious thoracic injury with mortality rate of 83.33% at 12 hours. Pathological examination revealed injuries from direct trauma as well as secondary lung injuries. Conclusions Our device was able to repetitively deliver accurate and precise impact forces to rats and allows us to establish a rodent model of severe thoracic injury firm blunt trauma. We found that with our device, impact force at velocity of 6 m/s and 40% chest compression produced the most severe lung injury in rats.This helps us establish a rodent model of severe thoracic injury which can be use for future research in severe blunt thoracic trauma and the secondary lung injuries.

Low back pain caused by intervertebral disc degeneration is a common clinical chronic disease. The regenerative ability of intervertebral disc tissue is extremely poor. Meanwhile, current treating methods can not fundamentally solve such problems. With the increasing awareness of the mechanism of disc degeneration and the rapid development of the fields of cellular and molecular biology, gene and materials engineering, using stem cells and tissue engineering technology to slow down or reverse the progress of disc degeneration may become possible. The author reviewed the application of stem cells for treating degenerative discs from present researching status and concepts for the future in the combination of researches reported both at home and abroad.

5.Effect of Cognitive Behavioral Therapy combined Anti-anxiety drou On Preliminary Hypertension Patients

Objective To evaluate the curative effect of combined cognitive behaviioral therapy with alprazolam anti-anxiety. on patients with preliminary hypertemsion. Methods After evaluating by self-rating anxiety scale (SAS) ,60 outpatients with preliminary hypertension were randomly divided into A and B groups. The A group (n = 29) were accepted the combined cognitive behavioral therapy with alprazolam anti-anxiety for 8 weeks at the basis with medication, the B group ( n = 31 ) given rontine medication only. Meantimes, fifteen healthy volunteers who matched with age and sex were selected as the normal control group ( C group ). The blood pressure of all the subjects were recorded at the begin and after eight-weeks therapy,and the levels of plasma von willebrand factor (VWF) and serum high-sensitive reaction protein(hs-CRP) were measured respectively. The evaluating by self-rating anxiety scale (SAS) were performed in the A and B groups,before and after the treatment at the same time. Results The patients with preliminary hypertension have higher SAS score as well as the levels of plasma VWF and serum hs-CRP. After psychotherapy,in A group,the SAS score,the levels of VWF and serum hs-CRP, and the blood pressure were significantly decreased( P < 0. 01 ),compared with those in B group, however, were still significantly higher than those of C group( P < 0. 01 ). Conclusion The patients with preliminary hypertension showed some-degrees anxious emotion. By combined cognitive behavioral therapy with alprazolam anti-anxiety,we could not only more positively control the patients, blood pressure,but also improve the patients emotion states,as well as decrease the levels of VWF and hs-CRP.

7.Effects of glutamine-enriched early enteral nutrition on intestinal mucosal barrier injury after liver transplantation in rats

Objective To observe the effect of glutamine-enriched early enteral nutrition on intestinal mucosal barrier injury after orthotopic liver transplantation in rats.Method Male Wistar rats,the recipients were randomly divided into three groups:control group (control group,n =10),orthotopic liver transplantation group (OLT group,n =30) and glutamine-enriched early enteral nutrition group (EEN group,n =30).Only dissecting hepatoduodenal ligament was performed in control group,and OLT was performed from Wistar to Wistar rats by modified two-cuff method in OLT group and EEN group.For EEN group,recipients were supplied with Nutrison Fiber (125 ml/kg every day) plus Gln (0.3 g/kg every day) for 3 days before and 3 h after surgery by gastric perfusion.For OLT group,the same volume of normal saline was administered instead of the Nutrison in the same time.No special treatment was given in control group.The levels of plasma endotoxin,D-lactic acid,and TNF-α were determined at different time points in the three groups.The ultrastructural changes of ileal mucosa were observed under the transmission electron microscopy.At the same time,the remaining 5 rats per group were used for observing the lifetime.Result As compared with control group at 12,24 and 72 h,the levels of plasma endotoxin,D-lactic acid,and TNF-α were significantly increased in OLT group and EEN group (P<0.01),and as compared with OLT group,the above-mentioned indexes were obviously decreased in EEN group at 24 and 72 h (P< 0.01).The ileal mucosal epithelial clearance in control was normal,and microvilli arranged neatly under the electron microscope.the ultrastructure damage in OLT group was more serious than in EEN group at 12,24,and 72 h.As compared with control group at 12,24,and 72 h,the expression levels of TNF-α mRNA were significantly increased in OLT group and EEN group (P<0.01),and as compared with OLT group,the expression levels of TNF-α mRNA were obviously decreased in EEN group (P<0.01).The survival time in cntrol group was significantly longer than in OLT group and EEN group (P<0.05),and as compared with OLT group,the survival time in EEN group was obviously extended (P<0.05).Conclusion OLT can lead to the damage of intestinal mucosal barrier,and glutamine-enriched early enteral nutrition is a potent protection against intestinal mucosal barrier injury and prolongs the survival time of rats after OLT.

10.Enteral nutrition related complications in severe brain-injured patients

Objecive:To observe the incidence of early enteral nutrition-related complications in severe brain-injured patients in ICU and its associated factors.Methods:78 nutrition severe brain-injured patients(GCS 4-8) in ICU receiving enteral nutrition were all carried out for GCS scoring.The enteral feeding amount and speed,serum albumin(ALB) and intestinal feeding tolerance were observed.The incidence rate of all kinds of complications related to enteral nutrition was summarized.Results:The patients with lower GCS had lower total feeding amount,prolonged feeding duration and higher incidence rate of complications.The diarrhea was negatively related with serum ALB level and GCS. Conclusion :Enteral nutrition-related complications are related with the severity of disease and Alb level.Some critically ill patients can not sustain total enteral nutririon(TEN),and parenteral nutrition is necessary.
