Objective To investigate the ophthalmological consultation diseases in the duration of 1 year ophthalmic chief resident training,to identify the breadth and depth of knowledge an ophthalnic chief resident should have.Methods A retrospective analysis of ophthalmological consultation patients in the past 1 year,to identify proportion of the source department and constituent ratio of ophthalmologic consultation disease in top five sections,top five eye diseases and to understand five special consultation need routine ophthalmology examination which an ophthalmic chief resident should be confronted with.Results Among 25 departments which request ophthalmic consultation,the top five closely related sections were the Department of Endocrinology (452 cases,20.9％),Department of ENT (296 cases,14.5％),Department of Cardiology (194 cases,9.5％),Neurological department of internal medicine (154 cases,7.6％) and Department of gerontology (144 cases,7.1％).Among 32 ophthalmologic consultation diseases,clearly the top five diseases were cataract (630 cases,30.9％),refractive error (368 cases,18.1％),diabetic retinopathy (222 cases,10.9％),conjunctivitis (154 cases,7.6％),glaucoma (66 cases,3.2％) and pregnancy induced hypertension retinopathy (66 cases,3.2％).With the development of new technology and the new situation and the doctor's self-protection awareness,five special consultation circumstances were Department of ENT preoperative routine ophthalmology examination (196 cases,9.6％),further clarify the diagnosis of traumatic optic neuropathy (26 cases,1.28％),Department of Hematology pre-transplantation of stem cell ophthalmologic routine examination (22 cases,1.08％),glaucoma excluded before the atropine experiment checking (20 cases,0.98％) and cornea K-F ring further examination (16 cases,0.79％).Conclusion Ophthalmology-related consultation departments are widely distributed,involving more sorts of eye disease,and part of the consultation needs an eye check-up,which requires that ophthalmic chief resident should be a calm demeanor,who has ability on quick judgment of emergency and takes corresponding measures,and meanwhile,he should be a good communicator with other section practitioners and have quality of comprehensive,independent thinking and innovative consciousness of be a scholar ophthalmologist.

BACKGROUND:Early diagnosis and accurate treatment of ligament injuries are the premise of avoiding chronic instability of knee joint and promoting functional recovery of knee joint.
OBJECTIVE:To investigate the clinical efficacy of limited incision with autologous tendon graft for knee ligament injury.
METHODS:Eighty patients with knee ligament damage were randomly divided into two groups. The cases of control group were given orthosis treatment, and the cases of observation group were treated with the limited incision plus autologous tendon graft.
RESULTS AND CONCLUSION:At 6 months post-treatment, the excellent efficacy rate and postoperative quality of life score were increased in the observation group, compared with the control group (P<0.05). The active and passive ranges of motion of knee joint in the observation group were significantly lower than that in the control group (P<0.05). At 3 months post-treatment, Lysholmp scores of the observation group was significantly lower than that in the control group (P<0.05). Limited incision plus autologous tendon graft treatment can significantly improve knee function after knee ligament injury.

OBJECTIVE:To establish HPLC method for the content determination of bacteriostatic agent benzyl alcohol in Clindamycin phosphate injection.METHODS:The determination was performed on CAPCELL PAK C8 column using potassium dihydrogen phosphate solution-acetonitrile(775:225)as mobile phase at flow rate of 1.0 mL?min-1.The detection wavelength was set at 210 nm.RESULT:The linear range of benzyl alcohol was 4.175～41.75 ?g?mL-1(r=0.999 9)with an average recovery of 100.5%(n=9),RSD=0.09%.CONCLUSION:The method is accurate and reproducible for the content determination of benzyl alcohol in Clindamycin phosphate injection.

Proteomics was important for the coherent study of human reproduction and animal breeding including human infertility,sperm-egg binding and mutual recognition of the mechanism.It was well known that proteomics had become one of the main branches of life sciences in the future.This provides the technological means and theoretical foundation for the individual dynamic changes in the protein.At the same time,it plays an important role in the drug development,the mechanism of life activities and in the field of livestock breeding.

Objective To evaluate the incidence of postoperative epididymitis by laparoscopic cluster ligation and laparoscopic internal spermatic vein high ligation in the treatment of varicocele. Methods Forty-four cases of bilateral varicocele were retrospectively analyzed , who had undergone laparoscopic spermatic vein ligation. The 22 cases were treated with laparoscopic cluster ligation, and the other 22 cases with laparoscopic internal spermatic vein high ligation. The effects and the reasons of two kinds of surgical procedures on postoperative epididymitis were investigated. Results A total of seven patients were affected with postoperative epididymitis. The incidence of laparoscopic cluster ligation was 27.27%. The incidence of laparoscopic internal spermatic vein high ligation was 4.55%. All cases were affected within 4 weeks after surgery. Conclusion It is important to reserve the spermatic artery for decreasing the incidence of postoperative epididymitis.

Postoperative X-ray films showed that the fracture healing time was averagely 5.4 months. Al of the fractures were recovered wel that subtrochanteric fractures, intertrochanteric fractures and femoral shaft fractures were al healed. There was no fracture displacement, internal fixation loosening and varus deformity. Only one case showed difficulties in nail insertion. Excellence rate of Harris hip functional scores was 88.2%postoperatively. Proximal femoral nail antirotation is a reasonable design, and the helical blade can resist the rotation and stabilize the angle. The proximal femoral nail antirotation has better effects, which is an ideal internal fixation for subtrochanteric femoral fractures.

Objective To study the complications related to and its management after retrieving temporary and retrievable IVC filters.Methods From Jul 2007 to Mar 2012,576 consecutive cases of deep venous thrombosis secondary to bone fractures confirmed by Duplex ultrasonography,received IVCF deployment to avoid fatal pulmonary embolism during perioperative period.There were 192 cases receiving temporary filters and 384 cases with retrievable filters.The temporary and retrievable filters were removed 2 to 4 weeks after implantation.Results The success rate of temporary filter removing was 100％.In 24 cases,the filters captured huge thrombi,and they were removed after thrombolysis using the Uni-fuse catheter.One patient received permanent filter deployment before removing the tempo-filter.Filters were successfully removed in 367 out of 384 cases with retrievable ones with a successful rate of 95.57％.In 7 cases the filters were removed with large thrombi.Double-snare-technique was used to retrieve 15 seriously tilted filters.After a mean 22 months follow-up,no pulmonary embolism occurred,but symptomatic DVT were revealed in 26 patients,and the conditions were improved after anticoagulation treatment.Conclusions The application of Uni-fuse thrombolysis catheter increases the success rate of filters retrieving,double-snare-technique helps retrieve seriously tilted filters and decrease long-term complications of permanent filters.

Objective To investigate the relationship between hyperhomocysteinaemia and unstable angina pectoris (UAP). Methods Selected 159 patients with senile hypertension combined with angina pectoris from April 2013 to April 2014, 78 cases of hyperhomocysteinaemia patients as observation group, 81 cases of simple hypertension patients as control group. All of the patients were performed coronary angiography examination, using the Gensini score method calculated the integral of the coronary lesions (CAS). Detected the level of the serum homocysteine (Hcy), hypersensitive C-reactive protein (hs-CRP), fasting blood glucose (FBG) and blood lipid levels. Results The gender, age, the levels of FBG and lipid in two groups had no significant differences ( P>0.05). The incidence of UAP,CAS and hs-CRP in observation group were 44.9%(35/78), (44.34±10.67) scores,(4.31±1.01)μg/L, in control group were14.8%(12/81), (30.24±8.76) scores, (2.13±0.95)μg/L. The incidence of UAP, CAS and hs-CRP in observation group were significantly higher than those in control group ( P<0.05). The levels of serum Hcy,TC,TG,LDL-C, hs-CRP, HDL-C in UAP patients of observation group and control group were (23.21 ± 7.35) and (8.86 ±2.43) μmol/L, (6.54 ±0.75) and (6.67 ±0.82) mmol/L, (1.84 ±0.95) and (1.62 ±0.83) mmol/L, (4.65±0.78) and (4.42±0.72)mmol/L,(5.15±1.24) and(3.21±1.15) mg/L, (1.02±0.32) and(1.01±0.35) mmol/L. The levels of serum Hcy,TC,TG,LDL-C, hs-CRP, HDL-C in SAP patients of observation group and control group were (12.54±4.37) and (6.52±1.36)μmol/L, (4.23±0.54) and (4.43±0.57) mmol/L, (1.45 ±0.76) and (1.25 ±0.68) mmol/L, (2.67 ±0.45) and (2.43 ±0.43) mmol/L, (3.02 ±1.13) and (1.52±0.98) mg/L, (1.24±0.36) and (1.26±0.34) mmol/L.There were significant differences (P<0.05). Multi-factor Logistic regression analysis showed that the senile H-type hypertension was independent risk factor of UAP (OR=4.324, 95%CI 2.463-6.874, P=0.001). Conclusions The levels of serum Hcy is closely related to the stability of the coronary artery atheromatous plaque and lesion severity of coronary artery. The senile H-type hypertension is independent risk factor of UAP.

Objective To explore the gray matter changes in aggressive patients with schizophrenia,and the relationship between the gray matter and aggression in patients.Methods Eighteen aggressive patients with schizophrenia (SZ1),18 age-and gender-matched un-aggressive patients with schizophrenia (SZ2) and 18 normal controls (NC) were enrolled in the study.Then a 3.0 T magnetic resonance imaging (MRI) scan was conducted for each participant.The voxel-based morphometry (VBM) approach and the Chinese version of Buss & Perry aggression questionnaire (B&P) were used to explore imaging data and to assess the aggression,respectively.Results Compared with NC,patients with schizophrenia showed changes in gray matter volume (GMV) in the frontal,temporal and the occipital lobes (P＜0.05,AlphaSim corrected).Compared with SZ2,SZ1 showed increased GMV in the right supramarginal gyrus,right postcentral gyms,bilateral insula and orbito-frontal gyri (P＜0.05,AlphaSim corrected).The GMV of the right insula,right postcentral gyms and right supramarginal grus were positively associated with B&P scores in patients with schizophrenia (P＜0.01,AlphaSim corrected),respectively.Conclusions These preliminary findings support that the aggression in schizophrenia is associated with GMV changes of brain regions in patients with schizophrenia.The right postcentral gyrus,the right insula and the right supramarginal gyrus may be involved in the neural mechanism of aggression in schizophrenia.

Objective To investigate the clinicopathological characteristics and postoperative prognosis analysis of intra-thoracic localized Castleman disease (LCD).Methods The clinical data of 9 patients with intra-thoracic LCD who accepted surgical treatment were retrospectively analyzed.There were 5 males and 4 females,with age of (32.8 ± 10.9) years.Two patients complained of chest pain,1 patient suffered from paraneoplastic pemphigus,and the rest were diagnosed by physical examination.Four cases were diagnosed with LCD by preoperative CT examination.Results All patients underwent surgical resection.Four patients were performed open surgery and 5 patients had video assisted thoracic surgery.All patients accepted radical surgery.But 2 of these patients had postoperative complications.One patient was the injury of phrenic nerve and another was pericardial effusion.Patho-histological showed hyaline vascular type of Catleman disease in all patients.All patients survived without recurrence during the follow-up for 2-53 months.Conclusions Intra-thoracic is rare and liable to misdiagnosed.For increasing the preoperative diagnosis rate of LCD,the combined application of imaging tests is important,and clinicians and radiologists should also enhance the awareness of this disease.Complete surgical resection of the tumor is the best therapeutic alternative for intra-thoracic LCD.
