1.Effects of Compound Sophorae Flavescentis Radix Injection on Immune Function and Quality of Life in Ovarian Cancer Patients with Chemotherapy

Objective To observe the effects of compound Sophorae Flavescentis Radix (Kushen) Injection on immune function and quality of life in ovarian cancer patients with chemotherapy. Methods Sixty cases of ovarian cancer patients with chemotherapy were randomly divided into the observation group and the control group. Patients in the observation group were treated with paclitaxel plus carboplatin chemotherapy plus compoundKushen Injection for intravenous injection, while the patients in the control group were treated with paclitaxel plus carboplatin chemotherapy for three cycles. Changes in peripheral venous blood, the level of T cell subsets and NK cell, body weight, quality of life of 1 day before chemotherapy and 1 week after chemotherapy were observed.Results The mean values of CD3+, CD4+, CD4+/CD8+ and NK cells in the observation group improved to certain different degrees, with statistical significance compared with the control group (P<0.05). The body weight of observation group after chemotherapy increased than before chemotherapy (P<0.05);the control group had no significant difference (P>0.05). Karnofsky score of the observation group increased after chemotherapy (P<0.05);the control group had no obvious change (P>0.05).Conclusion CompoundKushen Injection can effectively enhance the immune function of ovarian cancer patients with chemotherapy, increase the weight of patients, and improve their quality of life.
2.Study on the Effect of Astragalus Polysaccharide on the Proliferation of Human Neuroblastoma SH-SY5Y Cells

OBJECTIVE:To investigate the inhibitory effect of astragalus polysaccharide(APS)on the proliferation of human neuroblastoma SH-SY5Y cells. METHODS:After the cells were cultured with 0(blank control),25,50 and 100 mg/ml APS for 6,12 and 24 h,MTT method was used to determine cell viability and calculate inhibition rate. Following cell cultured with 0 (blank control),25,50 and 100 mg/ml APS for 24 h,Hoechst 33258 fluorescent staining was performed,and then cell nucleus morphology was observed under the fluorescence microscope;flow cytometer was used to detect the distribution of cell cycles and apoptosis;western blot was employed to determine the expression of extracellular regulated protein kinases (ERK) 1/2 protein in cells. Enzyme linked immunosorbent assay (ELISA) was conducted to determine the contents of interleukin 2 (IL-2),IL-6 and IL-12 in the cells. RESULTS:Compared to the blank control,those cultured with 100 mg/ml APS for 6 h,50 and 100 mg/ml APS for 12 h and 25,50 and 100 mg/ml APS for 24 h demonstrated higher inhibition rate. After the cells were cultured with 50 and 100 mg/ml APS for 24 h,those in G0/G1 phase increased and those in G2/M and S phases decreased,and the contents of IL-2 and IL-6 increased. After cells were cultured with 25,50 and 100 mg/ml APS for 24 h,the apoptosis rate was higher,densely hyperchromat-ic fragments in cell nuclei and apoptotic bodies appeared,the phosphorylation level of ERK1/2 protein in the cells was lower,and the content of IL-12 was higher. There was statistically significance (P<0.01 or P<0.05). CONCLUSIONS:APS can inhibit the proliferation of SH-SY5Y cells by arresting cell cycle and inducing cell apoptosis through a mechanism which may be correlated to the decrease in the phosphorylation of ERK1/2 and increase in cytokine.
3.Analysis on feasibility of C14-urea breath test in diagnosis of child chronic cough

Objective To explore the feasibility of C14-urea breath test in the diagnosis of child chronic cough.Methods One hundred and twenty children cases of chronic cough were collected as the research subjects and divided into the C14 group(adopting C14-urea breath test for diagnosis) and combined group(C14-urea breath test combined with routine diagnostic method) according to different diagnostic means,60 in each group.Then the positive diagnostic rate and effectiveness were analyzed and compared between the two diagnostic methods.Furthermore the ROC curve was adopted to evaluate its diagnostic value.Results The positive diagnosis rate in the C14 group was 76.67%,which was significantly lower than 95.00% in combined groups,the difference was statistically significant (P<0.001).The sensitivity and positive predictive value in the combined group were obviously higher than those in the C14 group (P<0.01),but there were no statistically significant differences in specificity and negative predictive value between the two methods (P>0.05).The expression levels of serum hs-CRP and IL-6 in the C14 group were significantly higher than those in the combined group,but the expression level of IL-10 was obviously lower than that of the combined group (P<0.05).The AUC in the ROC curve of combined group was significantly higher than that of the C14 group(P<0.05).Conclusion The C14-urea breath test can serve as a supplementary diagnosis means in child chronic cough,and its combining with the routine diagnosis method can significantly increase the diagnosis effectiveness.
4.Construction of learning teaching faculty according to the idea of ‘ school-based’ training

5.Treatment of high energy injury of tibial plateau with Hybrid external fixation

Objective To evaluate the results of less invasive surgical treatment of high energy tibial plateau fracture, which involves limited open reduction, percutaneous screw fixation and Hybrid external fixation. Methods From 1999 to 2002, 12 cases of high energy tibial plateau fracture were treated with Hybrid external fixation. They were 8 males and 4 females. According to Schatzker classification, there were 9 cases of Type Ⅴand 3 cases of Type Ⅵin the group. The average age of the patients was 39.3 years old (ranging from 27 to 48). Results All were followed up for an average of 15 months (ranging from 6 to 27). All the fractures got united, with an average healing time of 3.2 months. All patients achieved 0 to 5 degrees of extension and more than 100 degrees of flexion. The patients had an average knee score of 88.7 and an average functional score of 85.2(Knee Society clinical rating system). 1 case had infection of pin holes, and 2 knees had Grade 2 arthrosis at radiographic check up two years after the operation. Conclusion Hybrid external fixation combined with limited open reduction is appropriate for treatment of the complex tibial fractures, especially those with a poor soft tissue envelope.
6.Comminuted intertrochanteric fractures: Should the lesser trochanteric fragment be fixated?

Objective Unstable intertrochanteric hip fractures are characterized by comminution of the posteromedial cortex and a fragment of variable size containing the lesser trochanter. This paper is to discuss whether it is necessary to perform reduction and fixation for this fragment. Methods 67 cases of intertrochanteric fractures were treated by dynamic hip screw (DHS) fixation between March 2001 and September 2002 and followed up for a minimum of 1 year. Their treatment results were retrospectively analyzed. Results No nonunion, coxa vara or failure of internal fixation occurred in all these patients. Conclusion If DHS provides stability, screw fixation of the lesser trochanteric fragment is unnecessary.
7.Clinical Study of Entecavir Combined with Chemotherapy in the Treatment of Positive HBV in Tumor Pa-tients with Normal Liver Function

OBJECTIVE:To explore the effects of entecavir combined with chemotherapy on positive HBV in tumor patients with normal liver function. METHODS:106 patients were selected and divided into observation group (44 cases) and control group (62 cases) according to therapy plan. Control group received routine chemotherapy according to tumor condition;observa-tion group was given Entecavir tablets orally 0.5 mg,qd,before 1 week of chemotherapy,for consecutive 1 week. HBV reactiva-tion rate,recurrence rate of severe hepatitis,mortality rate,serum level of ALT before and after treatment and the incidence of ADR were compared between 2 groups. RESULTS:4 patients of observation group and 2 of control group withdrew from the study. HBV reactivation rate,recurrence rate of severe hepatitis and mortality were 7.5%,0,0 in observation group and 56.7%, 50.0%,13.3% in control group,with statistical significance between 2 groups(P<0.05). Before chemotherapy,there was no sta-tistical significance in serum levels of ALT between 2 groups(P>0.05);after chemotherapy,the serum levels of ALT in 2 groups were increased significantly,and the control group was significantly higher than the observation group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Entecavir can effectively prevent HBV reactivation and decrease the recurrence rate of severe hepatitis and mortality rate in tumor patients dur-ing chemotherapy, and doesn’t increase the risk of ADR.
8.Nicardipine′s Influence on Post Operation Cognitive Dysfunction in Senior Patient after Hip Joint Replacement Operation

Objective To discuss nicardipin′s influence on post operation cognitive dysfunction (POCD) in senior patient after hip joint replacement operation.Methods 180 senior patient, who received selective unilateral hip joint replacement operation between October 2015 and October 2016 under the condition of combined spinal-epidural anesthesia (CSEA) were randomly divided into Group A (with nicardipine) and Group B (without nicardipine).Nicardipine was only appropriately pumped into vein in time in Group A.MAP was observed and recorded 30 minutes after starting the operation and at the end of the operation.Mini-mental state examination (MMSE) was applied to score the patient one day before, one day, three days and five days after operation, and the number of POCD was recorded.Results Compared with Group B, Group A was obviously lower in MAP level (P<0.05) after 30 minutes.MMSE score of Group A was obviously higher (P<0.01) one day after operation.The number of POCD in Group A was 8 (8.89%) significantly lower than than that of Group B (19, 21.11%).Conclusion Nicardipine could maintain hemodynamic stability of senior patients receiving selective unilateral hip joint replacement operation under the CSEA and prevent POCD to a certain extent.
9.The Value and Path of Fine Traditional Chinese Culture in Cultivating New Era Medical Ethics and Style

Cultivating new era medical ethics and style with the fine traditional Chinese culture (TCC) is foundation of the construction of medical ethics and style in the new era, which can fundamentally rectify the alienation of medical ethics and style in modern medicine. It can cultivate the new era medical staff’s benevolence of "putting the people and the people’s lives as the first place", the dedication spirit of "value justice over profit", the humanistic feelings of "putting oneself in other’s shoes", the medical innovation of "facing people’s health", the world responsibility of building a community of human health. Cultivating medical ethics and style in the new era with fine TCC means achieving creative transformation and innovative development of fine TCC, fundamentally nourishing and nurturing medical ethics and style in the whole process. It is important to firm cultural confidence, continue to make great efforts to inherit and promote the fine TCC, especially the traditional Chinese medicine culture; strengthen cultural education, build an "antique" education system of medical ethics and style, and focus on integration and innovation to improve the effectiveness of medical ethics and medical style cultivation in the new era.
10.Anatomical study of motor branches from tibial nerve transfer to restore the deep fibular nerve

Objective To explore the feasibility of tibial nerve motor branches transfer to the deep fibular nerve in an anatomical study.Methods Twenty-three sides lower limbs from 12 adult cadavers which preserved in Formalin were used for dissection of the tibial nerve and its all motor branches,and the proximal deep and superficial fibular nerve.Experimental measurement were performed for the parameters of each branch such as length,diameter,the location of original point relative to the level of the fibular head.The diameter of proximal part of the deep fibular nerve was measured simultaneously.Finally,the length from original point of each branch to the fibular neck was also measured during simulation of nerve transfer procedure.Results The average length of motor branches to the flexor digitorum longus muscle,to the flexor hallucis longus muscle and the superficial branches to the soleus muscle were (95.70 ± 13.40)mm,(96.90± 13.60)mm and (73.60 ± 12.00)mm respectively.Their average diameter were (0.63 ± 0.16)mm,(0.65 ±0.20)mm and ( 1.56 ± 0.26)mm respectively.The average diameter of proximal deep fibular nerve was (2.54± 0.26)mm.Based on length,branches to the flexor digitorum longus muscle and flexor hallucis longus muscle were adequate for direct nerve transfer to the deep fibular nerve in all specimens without interpositional grafr.And in 22 specimens (95.7 percent),the superficial branches to the soleus muscle were long enough to directly transfer.Other branches of the tibial nerve were not adequate for direct nerve transfer Conclusion This study confirmed the anatomical feasibility of using motor branches from tibial nerve for direct transfer to restore the deep fibular nerve.The superficial branches to soleus muscle were the best donor nerve if considering the branches,length,diameter and the difficulty of surgical procedures.
