1.Application of European five-dimensional health questionnaire for the quality of life among the population over 60 years old in Dalian city.

OBJECTIVEUsing European five-dimensional health questionnaire (EQ-5D) to study the quality of life among the population over 60 years old in Dalian city and analyze the influence factors.
METHODIn 2013, multi-stage stratified cluster random sampling method and EQ-5D questionnaire were used to investigate the status of 4 081 participants over the age of 60 in Dalian. The questionnaire contained EQ-5D health describing system and the VAS. The EQ-5D index and EQ-VAS score of the study objects were calculated and the participants' quality of life and impact factors were analyzed.
RESULTWomen's percentage of "having difficulty" in 5 dimensionalities was 18.1% (373/2 059), 12.6% (264/2 059), 16.2% (334/2 059), 32.0% (659/2 059), 17.5% (371/2 059) respectively, men' percentage of "having difficulty" in 5 dimensionalities was 13.3% (269/2 022), 10.2% (218/2 022), 12.5% (253/2 022), 23.1% (467/2 022), 13.7% (77/2 022) respectively, and the difference was significant (P < 0.05). The EQ-5D index and VAS score of men was (0.82 ± 0.30), (72.75 ± 16.26), higher than those of women ( (0.79 ± 0.33), (70.79 ± 16.64 )) and the difference was significant (P < 0.05). The score of quality of life who were married was (0.82 ± 0.28) and (73.27 ± 16.60).With age increasing, the elders' quality of life score decreased, and with education level increasing, the elders' quality of life score increased (P < 0.05).
CONCLUSIONThe quality of life of the elders in Dalian city was kind of good, and it could be impacted by genders, marital status, education levels and the chronic diseases.

2.Research on evaluation of comprehensive ability and influencing factors of graduates in clinical medicine

Objective:To investigate the comprehensive ability and influencing factors of clinical medicine graduates from a medical college in Xi'an, and provide a scientific basis for improving the training model and comprehensive ability of clinical medicine students.Methods:Based on the Chinese Undergraduate Medical Education Standard-Clinical Medicine Specialty (2016 Edition), we conducted a self-designed questionnaire, the Clinical Medicine Graduate Comprehensive Ability Evaluation Questionnaire, containing 62 items in 4 dimensions, selected the clinical medicine graduates as the research objects by random sampling method, used SPSS 22.0 to make data analysis, and performed multiple linear regression to analyze its influencing factors.Results:Clinical medicine graduates had the highest score in the field of clinical competence (3.05±0.84), followed by the field of professionalism (2.48±0.81), and the lowest score was in the field of science and academics (1.89±0.68). The top three scores of each factor were clinical practice ability (3.39±0.76), theoretical knowledge mastery (3.55±0.48) and medical knowledge mastery (2.98±0.81). The three factors with the lowest scores were literature search ability (1.31±0.64), global health issues (1.49±0.82) and self-improvement willingness and ability (1.81±0.73). Using the multiple stepwise regression method to analyze the influencing factors of comprehensive ability, the optimal equation was obtained: Y=8.412+0.063 X6+0.190 X8+0.266 X10+0.031 X11+0.187 X12 ( X6, working level; X8, academic performance ranking; X10, practical teaching satisfaction; X11, mode of teaching satisfaction; X12, Re-education satisfaction after graduation). Among them, the higher the practical teaching satisfaction, the satisfaction of re-education and the satisfaction of teaching mode after graduation, the higher the student's performance ranking, and the higher the comprehensive ability score of clinical graduates. Graduates whose employment level was in a general hospitals had higher comprehensive ability scores than those of grassroots hospitals such as community/township hospitals. Conclusions:In the process of training clinical medicine students, we should improve the quality of practical teaching and the teaching model, increase the training of science and academic fields, increase the investment in re-education of primary medical institutions, and raise the satisfaction of graduates in education, so as to improve the clinical medicine profession comprehensive ability of graduates.

5.Effect of ultrasound-guided stellate ganglion block on intraoperative fentanyl dosage in patients undergoing open thyroidectomy

Objective To investigate the effect of ultrasound-guided stellate ganglion block(SGB)on intraoperative fentanyl dosage in the patients undergoing open thyroidectomy.Methods A total of 70 patients with elective open thyroidectomy under general anesthesia in the Affiliated Hospital of North Sichuan Medical College from November 2021 to April 2022 were selected as the study subjects and divided into the SGB group(group S,n=35)and the control group(group C,n=33).The group S conducted ultrasound-guided SGB at 15 min before anesthetic induction(injection of 0.25%marcaine 6-8 mL),and group C conducted the stellate ganglion recognition under the ultrasound guidance in 15 min before anesthetic induction without conducting other operations.All patients all received the anesthesia induction and maintenance under the same BIS moni-toring.The fentanyl dosage,recovery time,anesthetic drugs dosage,fluid infusion amounts,bleeding volume,use rate of atropine and ephedrine,operation time and postoperative complications as well as the VAS scores in PACU 30 min,at postoperative 3,6,12,24 h were recorded.Results Compared with group C,the intraopera-tive amount of fentanyl in group S was significantly decreased[(247.9±65.4)μg vs.(295.7±61.5)μg,P=0.003].The propofol dosage,cisatracurium dosage,fluid infusion amounts,bleeding amounts,use rate of atro-pine and ephedrine,recovery time and incidence rate of complications had no statistical differences between the two groups(P>0.05).The VAS scores at various time points in group S all were lower than those in group C(P<0.05).Conclusion Ultrasound-guided SGB could reduce the fentanyl use amounts during operation in the patients with open thyroidectomy.
6.Combination of AAV-delivered tumor suppressor PTEN with anti-PD-1 loaded depot gel for enhanced antitumor immunity.

Recent clinical studies have shown that mutation of phosphatase and tensin homolog deleted on chromosome 10 (PTEN) gene in cancer cells may be associated with immunosuppressive tumor microenvironment (TME) and poor response to immune checkpoint blockade (ICB) therapy. Therefore, efficiently restoring PTEN gene expression in cancer cells is critical to improving the responding rate to ICB therapy. Here, we screened an adeno-associated virus (AAV) capsid for efficient PTEN gene delivery into B16F10 tumor cells. We demonstrated that intratumorally injected AAV6-PTEN successfully restored the tumor cell PTEN gene expression and effectively inhibited tumor progression by inducing tumor cell immunogenic cell death (ICD) and increasing immune cell infiltration. Moreover, we developed an anti-PD-1 loaded phospholipid-based phase separation gel (PPSG), which formed an in situ depot and sustainably release anti-PD-1 drugs within 42 days in vivo. In order to effectively inhibit the recurrence of melanoma, we further applied a triple therapy based on AAV6-PTEN, PPSG@anti-PD-1 and CpG, and showed that this triple therapy strategy enhanced the synergistic antitumor immune effect and also induced robust immune memory, which completely rejected tumor recurrence. We anticipate that this triple therapy could be used as a new tumor combination therapy with stronger immune activation capacity and tumor inhibition efficacy.
