1.Biomechanical progress of fatigue effect on non-contact anterior cruciate ligament injury

BACKGROUND:Anterior cruciate ligament injury can trigger knee instability, knee osteoarthritis which decreases human beings’ life quality. Exploration on non-contact anterior cruciate ligament injury mechanism can prevent the occurrence of anterior cruciate ligament injury.
OBJECTIVE:To investigate the mechanism of fatigue effect on non-contact anterior cruciate ligament injury and to introduce the progression of fatigue effect on non-contact anterior cruciate ligament injury in jump-landing activities.
METHODS:The author retrieved the PubMed database from 1988 to 2013 by computer. The key words were set as:ACL, landing, and fatigue. A total of 42 articles were included which related to biomechanics research on anterior cruciate ligament injury, fatigue landing and low-limb injury.
RESULTS AND CONCLUSION:In jump-landing activities, knee valgus, moment, knee joint rotation and smal er knee flexion are main biomechanics underlying non-contact anterior cruciate ligament injury. In addition, fatigue effect is another critical factor for non-contact anterior cruciate ligament injury. However, the cause-effect between fatigue effect and anterior cruciate ligament injury is not clearly investigated and summarized, especial y on fatigue level, fatigue induction and movement control. Introduction of fatigue induction is crucial for better understanding how fatigue effect results in anterior cruciate ligament injury. Thus, the quantification of fatigue level, fatigue models and landing ways wil provide new ideas for exploring biomechanical mechanism underlying anterior cruciate ligament injury in fatigue state, as wel as provide more information on intervention design and injury rehabilitation.
2.Alendronate use for 3 to 4 years reduces incidence of fragile fractures in community postmenopausal women: a meta-analysis

Objective:To evaluate systematically the outcomes of prevention of fragility fracture with alendronate use for 3 to 4 years.Methods:We searched CENTRAL, MEDLINE, EMBASE and CBM for relevant randomized controlled trials published before 16 July, 2020. The quality of included studies was evaluated according to the Cochrane tool for assessing risk of bias. Fix- or random-effects were taken for meta-analysis depending on the magnitude of heterogeneity. Sensitivity analysis was used for high risk studies to assess the robustness of results. The results were reported according to The PRISMA 2009 Check-list.Results:A total of 8 reports from 4 studies were included. Two reports from 2 studies were rated as high-risk while the other 6 reports from the other 2 studies as low-risk. The meta-analyses showed that use of alendronic acid for 3 to 4 years effectively prevented new vertebral fracture ( RR=0.54, 95% CI: 0.44 to 0.66, RD=-0.03), clinical fracture ( RR=0.82, 95% CI: 0.73 to 0.92, RD=-0.03), non-vertebral fracture ( RR=0.84, 95% CI: 0.75 to 0.95, RD=-0.02), clinical vertebral fracture ( RR=0.51, 95% CI: 0.34 to 0.76, RD=-0.01) and hip fracture ( RR=0.56, 95% CI: 0.37 to 0.87, RD=-0.01), but did not prevent wrist fracture ( RR=0.85, 95% CI: 0.67 to 1.09), serious adverse event ( RR=0.95, 95% CI: 0.80 to 1.14) or upper gastrointestinal adverse event ( RR=1.02, 95% CI: 0.96 to 1.07). By the sensitivity analysis of the 6 results from the high-risk reports, the HRs for clinical fracture, non-vertebral fracture, vertebral fracture, hip fracture, wrist fracture, and serious adverse event were, respectively, 0.81, 0.85, 0.49, 0.62, 0.94 and 0.94. Conclusions:Alendronate use for 3 to 4 years can effectively prevent fragility fractures in postmenopausal women with low bone mass or osteoporosis, leading to better prevention effect on vertebral fracture than on non-vertebral fracture.
3.Effects of Aerobic Exercise on Calcium Release Related Genes Expression in Sarcoplasmic Reficulum of Skeletal Muscle in Mice

Objective To study the effects of 6-week aerobic treadmill exercise on the genes expression in skeletal muscle of C57BL/6 mice.Gene array was performed and the data was analyzed to evaluate the adaptive changes at gene level caused by exercise.Methods Twenty male C57BL/6 mice were randomly divided into two groups:control group(C) and aerobic exercise group (E).The mice from group E were forced to run on treadmill for 6 weeks.After 6 weeks,all mice were fasted for 16 hours,and then sacrificed.Their flexor muscle was removed.4 samples from each group were analyzed with gene chips to dissect the differentially expressed genes between the two groups.Results Our results showed that 723 genes were differentially expressed in E group as compared with C group after the 6 weeks aerobic exercise,among which 510 genes were up-and 213 down-regulated.There were 6 differentially expressed calcium signaling genes and 2 ofwhich(Gnall and Pdgfra) were up-regulated,while the others(Phka1,Picd4,RYR1 and Ppid)were down regulated.
4.Analysis on Composition Principles of Prescriptions for Sequela of Apoplexy by Traditional Chinese Medicine Inheritance Platform

This study was aimed to analyze the composition principles of prescriptions for sequela of apoplexy collected in the CNKI by the Traditional Chinese Medicine Inheritance Platform (TCMIP), in order to provide a reference for clinical selection of medication. Prescriptions for the sequela of apoplexy treatment in CNKI were collected. Based on the TCMIP, the medication rules of prescription were analyzed by data mining method after data entry. A total of 190 prescriptions involving 204 herbs for sequela of apoplexy treatment were screened and studied. The herbs for blood-activating andqi-tonifying were used with higher frequency. There were 36 commonly used herbal pairs. The herb pair of Dang-Gui(Radix Angelicae Sinensis) andHuang-Qi (Radix Astragali) was with the highest frequency. The herbal combination with three types of herbs wasDi-Long (Pheretima),Dang-Gui andHuang-Qi. The herbal combination with four types of herbs wasChuan-Xiong (Rhizoma Chuanxiong),Di-Long,Dang-Gui andHuang-Qi. The analysis on association rule showed thatChuan-Xiong,Dang-Gui,Tao-Ren (Semen Persicae),Hong-Hua(Flos carthami) andHuang-Qiwere often combined with each other. A total of 48 core combinations and 11 new prescriptions were mined. It was concluded that the prescriptions for sequela of apoplexy were mainly composed with herbs for blood-activating,qi-tonifying and collateral-dredging. According to the flavor and meridian tropism theory of Chinese medicine, the herbal medication selection in the prescription was mostly warm and hot in nature, sweet and pungent flavor, and the meridian tropism of the liver and spleen meridian.
5.Effect of prefilling blood reservoir with MAP solution on damage to erythrocytes in intraoperative salvaged blood

Objective To evaluate the effect of prefilling blood reservoir with mannitol-adeninephosphate MAP) solution on the damage to erythrocytes in intraoperative salvaged blood in patients.Methods One hundred and fifty blood samples were collected from 150 patients who were scheduled for elective spinal surgery requiring blood salvage,and were equally and randomly divided into 5 groups (n =30 each) using a random number table:group N,group N1,group N2,group M1 and group M2.The blood reservoir was not prefilled before surgery in group N,while the blood reservoirs in N1,N2,M1 and M2 groups were prefilled with normal saline (NS) 100 ml,NS 200 ml,MAP solution 100 ml and MAP solution 200 ml,respectively.Blood sauples were obtained for erythrocyte osmotic fragility test after the salvaged blood was washed,and hemolysis rates in different concentrations of hypotonic NaCl solution were calculated.The concentration of free hemoglobin in the clear supernatant liquid (FHb) of washed blood placed for 0 h (T0),1 h (T1) and 2 h (T2) were detected.Results Compared with N and N1 groups,the hemolysis rate of washed erythrocytes under 0.48% 0.68% NaCl solutions was significantly decreased,the concentration of FHb at T1 was decreased,and no significant change was found in FHb at T2 in group M1.Compared with N and N2 groups,the haemolysis rates of washed erythrocytes under 0.48%-0.68% NaCl solutions were significantly decreased,and the concentrations of FHb at T1,2 were decreased in group M2.The concentration of FHb was significantly lower at T2 in group M2 than in group M1.Conclusion Prefilling blood reservoir with MAP solution can mitigate the damage to erythrocytes in the intraoperative salvaged blood in patients,and the efficacy of prefilling of 200 ml is superior to that of prefilling of 100 ml.
6.Case report of WHIM syndrome with cardiac malformation as the first symptom

The clinical data of a WHIM syndrome child with cardiac malformation as the first symptom in December 2017 in Beijing Children′s Hospital Affiliated to Capital Medical University was retrospectively analyzed.A 5-year-old female patient presented with cardiac malformation, neutropenia and recurrent infection.Heterozygous mutation(c.1000C>T) was detected in CXCR4 gene.Echocardiography and CT exhibited cardiac malformation.WHIM syndrome is very rare, and it was the first case with cardiac malformation as the first manifestation in China, thus hoping to improve clinicians′ understanding of this disease.
7.Determination of individualized PEEP during lung-protective ventilation in patients undergoing general anesthesia: comparison of pulmonary electrical impedance tomography and dynamic lung compliance

Objective:To compare the efficacy of individualized PEEP determined by lung electrical impedance tomography (EIT) and dynamic lung compliance (Cdyn) during lung-protective ventilation strategies in the patients undergoing general anesthesia.Methods:Sixty patients of both sexes, aged 18-64 yr, of American Society of Anesthesiologists physical status Ⅰor Ⅱ, with body mass index of 18.5-28.0 kg/m 2, undergoing elective surgery with general anesthesia and endotracheal intubation in the Second Affiliated Hospital of Soochow University, were selected.Lung-protective ventilation strategy was applied in supine position after general anesthesia.The peak value of PEEP did not exceed 10 cmH 2O, with an increment/decrement of 2 cmH 2O for titration.The corresponding Cdyn value and lung EIT data were collected during titration.The patients were divided into 2 groups ( n=30 each) using a random number table method: titration first increased and then decreased group (group A) and titration first decreased and then increased group (group B). The determination method of individualized PEEP: Cdyn method was the PEEP corresponding to the maximum Cdyn value; EIT method was obtained through PV500 PC software analysis.The level and success rate of individualized PEEP determined by the Cdyn and EIT methods were compared, and the ICC consistency analysis of the determined individualized PEEP was performed. Results:Compared with the Cdyn method, the success rate of individualized PEEP determined by EIT method was significantly increased, and the level of individualized PEEP was decreased in the two group ( P<0.05). In group A, the individualized PEEP titrated by the EIT and Cdyn methods showed good agreement (the ICC value of the increment-Cdyn and increment-EIT methods was 0.761, P<0.05; the ICC value of the decrement-Cdyn and decrement-EIT methods was 0.763, P<0.05). In group B, the individualized PEEP titrated by the EIT and Cdyn methods showed good agreement (the ICC value of the increment-Cdyn and increment-EIT methods was 0.809, P<0.05; the ICC value of the decrement-Cdyn and decrement-EIT methods was 0.797, P<0.05). Conclusion:The agreement between the individualized PEEP determined by lung EIT method and Cdyn method during lung-protective ventilation is good in the patients undergoing general anesthesia, and the success rate of EIT method is higher, and the level of individualized PEEP is lower.
8.Genetic transformation of Bt gene into sorghum (Sorghum bicolor L.) mediated by Agrobacterium tumefaciens.

Sorghum (Sorghum bicolor L.) was one of the most important crops in the world next to wheat, rice, maize, soybean and barley. Using the callus derived from immature inflorescence as the recipients, we efficiently transformed sorghum varieties 115, ICS21B and 5-27 with the insecticidal Bacillus thuringiensis (Bt) cry1Ab gene carried in the T-DNA of binary vectors which contained hygromycin resistance gene and gus gene via Agrobacterium tumefaciens. After gradient selection with hygromycin, a total of 21 independent transgenic plant lines, 52 transgenic plants were regenerated, and the average stably transformation efficiency was 1.9%. The integration and transcription of cry1Ab gene in transgenic sorghum was confirmed by PCR analysis, Southern blotting and RT-PCR analysis. The Bt proteins were expressed in most transgenic plants with different level from plant to plant by Western blotting and ELISA assay. According to insect bioassay in laboratory, the transgenic plants with a relatively high level of Bt gene expression displayed insect-resistance to pink rice borer (Sesamina inferens).

9.The clinical value of mean platelet volume in predicting the therapeutic effect of TACE for HCC

Objective To explore the potential clinical value of mean platelet volume (MPV) in predicting the curative effect of TACE for hepatocellular carcinoma (HCC). Methods The clinical data of 263 HCC patients, who were treated with TACE at authors' hospital during the period from January 2012 to June 2016, were collected. The MPV data before initial TACE, before and after each repeated TACE were recorded. The time of tumor progression (TTP; referring to mRECIST standard), was documented. Based on the MPV determined before initial TACE, the patients were divided into low - MPV group and high - MPV group, and the TTP between the two groups was compared. Results A total of 263 patients were enrolled in this study. In HCC patients, the MPV determined before initial TACE was (9. 45±1. 24) fL, while the MPV determined after initial TACE was (9. 01±1. 11) fL, the difference between the two was statistically significant (t=4. 344, P<0. 05). The MPV determined at the time when HCC lesion first developed progression was (9. 38±1. 16) fL, which was significantly different with the MPV of (9. 01±1. 11) fL that was obtained after initial TACE (t=3. 498, P<0. 05). Taking the median value of MPV determined before initial TACE (9. 3 fL) as the cutoff value, the patients were divided into the low - MPV group and the high - MPV group according to patient' s MPV determined before initial TACE. Statistically significant difference in the time when HCC lesion first developed progression existed between the low - MPV group and the high - MPV group (P<0. 05). COX regression analysis showed that BCLC stage B and C, MPV value before initial TACE were the independent risk predictors of tumor progression. Conclusion The pre-TACE and post-TACE MPV values in HCC patients treated with TACE has certain clinical significance in evaluating the curative effect of TACE. Moreover, MPV value before initial TACE has some value in predicting the time of HCC progression after TACE. (J Intervent Radiol, 2018, 27:257-262)
10.Optimal PEEP for volume-controlled ventilation in patients undergoing surgery with general anesthesia: pulmonary electrical impedance tomography monitoring

Objective To determine the optimal positive end-expiratory pressure (PEEP) for volume-controlled ventilation using pulmonary electrical impedance tomography in the patients undergoing surgery with general anesthesia.Methods Fifty patients of both sexes,aged 18-64 yr,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,with body mass index of 18.5-28.0 kg/m2,scheduled for surgery for ureteral calculi under general anesthesia,were enrolled in this study.The patients were tracheally intubated after anesthesia induction and mechanically ventilated in volume-controlled mode,with tidal volume 6 ml/kg,mean arterial pressure was recorded at 3 min of ventilation and served as the baseline value,and then PEEP was increased with an increment of 3 cmH2O every 3 min until PEEP reached 15 cmH2 O.The percentage of dorsal pulmonary ventilation and peak airway pressure were recorded at 3 min of ventilation with different PEEPs.When the decrease in mean arterial pressure was more than 20% of the baseline value during ventilation,deoxyepinephrine 0.1 mg was injected intravenously,and the consumption of deoxyepinephrine was recorded within 3 min of ventilation with different PEEPs.Results Peak airway pressure was gradually increased with the increase of PEEP (P<0.05),the percentage of dorsal pulmonary ventilation was gradually increased when PEEP was 6 cmH2O (P< 0.05),and the consumption of deoxyepinephrine was gradually increased when PEEP was 15 cmH2O (P<0.05).Conclusion The optimal PEEP is 12 cmH2O during volume-controlled ventilation with tidal volume of 6 ml/kg in the patients undergoing surgery with general anesthesia.
