1.Onset risk of lung cancer and short tandem repeat polymorphisms

Objective The onset and progression of lung cancer correlate closely to individual genetic background .This study evaluated the correlation between the onset risk of lung cancer and short tandem repeat (STR) polymorphisms in order to find a new pathway for investigating the pathogenesis of lung cancer . Methods Using PCR and electrophoresis on 15 STR loci , we studied the gene and genotype frequencies of peripheral vein blood specimens from 120 lung cancer patients and 156 healthy individuals in Nan-jing area.According to the differences in the allele distribution of the 15 STR loci, we evaluated the susceptive and/or resistant factors relevant to lung cancer . Results There were statistically significant differences between the lung cancer patients and healthy controls at alleles 10, 15.2, and 16 of the loci CSF1PO, D19S433, and D3S1358 (P<0.05, OR>1), as well as at alleles 14 and 14.2 of the loci CSF1PO and D19S433 (P<0.05, OR<1). Conclusion It is possible that alleles 10, 15.2 and 16 of the loci CSF1PO, D19S433, and D3S1358 are susceptive factors and alleles 14 and 14.2 of the loci CSF1PO and D19S433 are resistant factors relevant to lung cancer .
2.Correlation between cerebral microbleeds and warfarin-related intracerebral hemorrhage

Objective To investigate whether there is a correlation between cerebral microbleeds and warfarin-related intracerebral hemorrhage (ICH). Methods Thirty-four patients developed ICH after taking warfarin outside the hospital were used as microbleed group, and 54 patients without developing ICH after taking warfarin at the same period were used as control group. The general information, cerebrovascular risk factors, and laboratory tests, such as coagulation function and imaging findings of cerebral microbleeds between the two groups were compared. Results The detection rate of cerebral microbleeds (82.4% vs. 22.2%, P<0.001)and the number of lesions (10.40±25.60 vs.0.60±1.09, P<0.001) in the microbleed group were significantly higher and more than those in the control group. Multivariate logistic regression analysis showed that the international normalized ratio was too high (OR 4. 241,95% CI 1. 421-12. 198; P =0. 012), and cerebral microbleeds (OR 79. 827, 95% CI 6. 113-368. 13; P =0. 001 ) were the independent risk factor for warfarin-related ICH. Conclusions Cerebral microbleeds were the independent risk factors for warfarin-related ICH.
3.Expression of Mitochondrial Motility-related Gene mirol during an Acute Bout of Prolonged Exercise

Objective The purpose of this study was to investigate the expression of mitochondrial motility-related gene mirol and mitochondrial energy metabolism during an acute bout of prolonged exercise. Methods CS7 BL/6 mice underwent a moderate intensity treadmill running with the slope of 0° and at the speed of 13m/min, and they were randomly divided into 5 groups:resting group(R),exercise groups running for 30min(E30),60min(E60),90min (E90) and 120min(E120),respectively. Respiratory control index and ATP synthesis activity in isolated mitochondria were detected. Skeletal muscle H_2O_2 concentration,mirol mRNA expressions were also measured. Results (1)mirol mRNA contents were significantly increased in groups E30 to E120,as compared with the group R;and mirol mRNA expression increased 32.8%, 107.6%,63.8% and 44.8% respectively in groups E30 to E120. (2)H_2O_2 contents of skeletal muscle were clearly increased in group s E30 to E120. (3)ATP synthesis activity elevated at 30 min of exercise and returned to the baseline thereafter,whereas respiratory control index without remarkable change in groups E30 to E120. Conclusion Muscular mirol mRNA expression significantly increased during 120min of exercise as compared with the resting status and thus facilitated the mitochondrial respiratory and ATP synthesis for matching the energy demand during exercise.
4.Impact of plasma homocysteinemia on contrast-induced nephropathy after percutaneous coronary intervention in patients with coronary syndrome.

OBJECTIVETo explore the impact of plasma homocysteinemia(Hcy) on contrast-induced nephropathy (CIN) after percutaneous coronary intervention (PCI) in acute coronary syndrome (ACS) patients.
METHODSConsecutive 684 ACS patients undergoing first PCI in our department between January 2013 and December 2014 were prospectively enrolled.Patients were divided into 2 groups according to the pre-procedural plasma Hcy level: high-Hcy group (Hcy≥10 μmol/L, n=404) and control group (Hcy<10 μmol/L, n=280). The CIN was defined as serum creatinine ≥ 44.2 μmol/L or 25% increase compared to baseline within 48-72 h after PCI.The baseline clinical data and the ratio of CIN were compared between the 2 groups.Multivariate logistic regression analysis was used to define the independent risk factors for CIN.
RESULTSCIN occurred in 133(19.4%) out of 684 enrolled patients, and the incidence of CIN was significantly higher in high Hcy group than in the control group (22.0%(89/404)vs. 15.7%(44/280), P=0.040). After adjusting the confounding factors, including age, acute myocardial infarction, co-morbidities(hypertension, diabetes mellitus, and old myocardial infarction), laboratory examination (level of cystatin C and uric acid), glomerular filtration rate, left ventricular ejection fraction, angiographic and procedural characteristics (3 diseased vessels, multiple stent implantation), treatment at admission (spironolactone, digoxin), multivariate logistic regression analysis showed that high Hcy was independently associated with the development of CIN (OR=1.70, 95%CI 1.60-2.64, P=0.021).
CONCLUSIONElevated Hcy prior PCI is an independent risk factor of CIN in ACS patients undergoing first PCI.

5. Evaluation of the effectiveness of specialized nursing team in the management of critical patients

Objective:
To explore the application effect of a specialized team consisting of specialist nurses and some emergency care′s or intensive care′s head nurses of critically ill patients in the hospital.
Methods:
From 2016 to 2018, the Critical Care Professional Group of implemented a provincial and above specialist nurses and the backbone of the specialist care departments of the acute and critical departments. A total of 23 members were composed of core members. A total of 56 liaison officers from each ward were selected to participate. The training and assessment activities presided over by the core staff of the professional group; at the same time, the professional group liaison officer is also the leader of the critical care quality control team in this ward, and implements the quality control of critical care patients; the core members are responsible for the guidance of the nursing care of critical patients in the hospital. The liaison staff carries out the training and assessment of the intensive care knowledge, skills and related nursing standards, norms, as well as the sharing of new technologies and new projects or research topics for acute and critical care, and the training of intensive care posts in the hospital. Before and after the operation of the specialized nursing team, the quality control scores of critically ill patients in 2015-2018, the satisfaction survey of nursing, the number of patents published by nurses in 2-25 years, the mortality rate of patients, and the results of unplanned extubation in 2016-2018 Compare.
Results:
The quality control scores of critical care patients in 2015-2018 were 93.91±1.23, 94.07±1.38,94.33±1.24, 95.42±1.56. The difference was statistically significant (

OBJECTIVETo explore the relationship between hyperhomocysteinemia (H-Hcy) and long-term outcome of coronary artery disease (CAD) patients after drug-eluting stent (DES) implantation in a single centre.
METHODSA total of 1 408 consecutive patients implanted with DES in our department between March 2011 and January 2013 were enrolled in this prospective study. Patients were stratified into H-Hcy (Hcy≥10 µmol/L, n=798, 56.7%) and non-H-Hcy group (Hcy<10 µmol/L, n=610, 43.3%). The clinical characteristics, coronary artery lesions, SYNTAX score and 1-year major adverse cardiac and cerebrovascular events (MACCE) were compared between the two groups.
RESULTSCompared with non-H-Hcy group, coronary artery stenosis was severer as shown by higher diseased arteries (2.11±0.87 vs. 1.91±0.82, P<0.001), higher incidence of three diseased arteries (39.7% vs. 29.5%, P<0.001) and higher SYNTAX score (36.99±29.66 vs. 27.39±22.70, P=0.001) in H-Hcy group. The 1-year incidence of MACCE was also higher in H-Hcy group compared with non-H-Hcy group(18.4% vs. 8.9%, P<0.001). Multivariate Cox analysis showed that diabetes mellitus (OR=1.530, 95%CI 1.142-2.050, P=0.004), age (OR=1.065, 95%CI 1.038-1.093; P<0.001), and Hcy (OR=1.019, 95%CI 1.005-1.033, P=0.009) are the independent predictors for 1-year MACCE.
CONCLUSIONHigh Hcy level is correlated with the severity of coronary artery stenosis, and serves as an independent predictor of MACCE after stenting in CAD patients.
