1.Clinical observation of blue-light-filtering intraocular lenses implantation on treating patients with diabetes cataract

Objective To explore therapeutic efficacy of blue-light-filtering intraocular lenses implantation on treating patients with diabetes cataract , and provide a more comfortable choice after phacoemulsification ( PHACO) and intraocularlens ( IOL) implan-tation.Methods A total of 100 cases (100 eyes) of patients with diabetes cataract was collected , and was randomly divided into group I ( n =50, implanted ordinary intraocular lenses ) and group II ( n =50, implanted blue-light-filtering intraocular lenses).Af-ter surgery, the best-corrected vision and postoperative complications were reported .The subjective visual performance was evaluated by questionnaire .Results At 1 week and 1 month after surgery , the best-corrected vision had no significant differences between two groups ( P >0.05 ) .Early complication after surgery was not significantly different between two groups ( P >0.05 ) .The subjective visual performance in the blue-light-filtering intraocular lenses group was significantly better than that in the ordinary intraocular lenses group ( P <0.05 ) .Conclusions When the blood sugar is controlled in certain level , it is possible for the blue-light-filtering in-traocular lenses to provide a better choice for patients with diabetes cataract .
2.Contribution of basal and early phase insulin secretion to plasma glucose level in patients with type 2 diabetes

Objective To investigate the effects of basal and early phase insulin secretion on plasma glucose level in type 2 diabetes. Methods Plasma glucose and true insulin levels were measured at 0, 30, 60, 120 min during standard meal test in 81 patients with type 2 diabetes. Insulin sensitivity index (ISI) and insulin secretion index (?I 30 /?G 30 ) were calculated for evaluating the insulin sensitivity. Contributions of basal and early insulin secretion to plasma glucose level were evaluated by multivariate regression analysis with SAS software. Results ISI and ?I 30 /?G 30 showed nearly equal effects on plasma glucose levels by multivariate regression analysis. Among insulin levels of different time points during standard meal test, basal and postprandial 60 min insulin levels played important roles in changes of plasma glucose levels. The effect of fasting insulin on the area under plasma glucose curve was stronger than that of ?I 30 /?G 30 . Conclusion Both basal and early insulin secretions greatly contribute to glycemic control.

4.Intake of liquid dairy products among the fourth grade studentsin Nanjing City

Objective:
To investigate the intake of liquid dairy products and identify its influencing factors among the fourth grade students in Nanjing City.
Methods:
The fourth grade students were selected as the study subjects in Nanjing City using a multi-stage random cluster sampling method in September 2020. Their general information, frequency of liquid dairy product intake one week prior to survey and mean intake amount per time were collected. According to the 2016 Dietary Guidelines for Chinese Residents, weekly intake of liquid dairy products of 2 100 g and greater was defined eligible. Factors affecting the frequency and amount of liquid dairy product intake were identified using multivariable logistic regression analysis.
Results:
A total of 2 268 questionnaires were allocated and 2 216 valid questionnaires were recovered, with an effective recovery rate of 97.71%. The respondents included 1 199 boys ( 54.11% ) and 1 017 girls ( 45.89% ). The frequency of liquid dairy product intake was (6.41±4.86) times per week, and the median intake amount was 1 250.00 g per week (interquartile range, 1 750.00 g per week). There were 607 students ( 27.39% ) consuming liquid dairy products of 2 100 g and greater a week, and 1 016 students ( 45.85% ) consumed liquid dairy products for 7 times and more a week. Multivariable logistic regression analysis showed that living in urban areas ( OR=1.204, 95%CI: 1.005-1.443 ), knowing nutrition labels ( OR=1.221, 95%CI: 1.021-1.460 ), periodical measurement of body weight (OR=1.486, 95%CI: 1.098-2.011) and restricted intake of sugar-containing drinks ( OR=1.264, 95%CI: 1.005-1.590 ) facilitated the intake of liquid dairy products for 7 times and more a week, and students with periodical measurement of body weight were more likely to consume liquid dairy products for 2 100 g and greater a week ( OR=1.821, 95%CI: 1.240-2.676 ).
Conclusions
Inadequate intake of liquid dairy products is found among the fourth grade students in Nanjing City. Residence, awareness of nutrition labels, periodical measurement of body weight, and parental restriction of sugar-containing drink intake affect the intake of liquid dairy products.
5.Diagnostic value of ultrasound shear wave elastography and portal vein hemodynamic parameters in chronic hepatitis liver fibrosis

Objective:To explore the diagnostic value of ultrasound shear wave elastography and portal vein hemodynamic parameters for chronic hepatitis and liver fibrosis.Methods:The clinical data of 48 hospitalized patients with chronic hepatitis diagnosed in Fuyang Second People's Hospital from May 2019 to July 2020 were collected and analyzed retrospectively. The patients voluntarily received portal vein hemodynamics and ultrasonic shear wave elastography. According to Scheuer's method, 48 patients were classified into 5 stages of liver fibrosis, including 10 patients in S0 stage, 13 patients in S1 stage, 10 patients in S2 stage, 10 patients in S3 stage and 5 patients in S4 stage. The average velocity of portal vein, peak portal vein velocity (PVVmax), portal vein diameter (PVD), liver stiffness measurement (LSM) and Young's modulus of liver were compared. Pathological and liver biopsy was the gold standard to analyze the sensitivity and specificity of various detection methods. The normally distributed measurement data were expressed as xˉ± s, the comparison between multiple groups was performed by one-way ANOVA, and the pairwise comparison was performed by LSD-t test. Spearman method was used to analyze the correlation between liver function classification and various parameters. ROC curve was used to analyze the diagnostic value of ultrasonic shear wave elastography, portal vein hemodynamics and combined detection in predicting liver fibrosis in chronic hepatitis. Results:In the staging of liver fibrosis, the LSM of the patients in the S0 stage was (5.29±0.19) kPa, and the Young's modulus of the liver was (21.65±2.35) kPa; the LSM of the patients in the S1 stage was (6.38±1.25) kPa, and the Young's modulus of the liver ( 22.89±3.19) kPa, LSM (9.76±1.33) kPa and hepatic Young's modulus (23.77±3.52) kPa in S2 group, LSM (15.44±2.44) kPa, hepatic Young's modulus (25.14±2.29) in S3 group, LSM (18.08±1.22) kPa and hepatic Young's modulus (27.94±2.58) kPa in patients with S4 stage, the differences between groups were statistically significant (F values ??were 115.47, 4.84, P values?were <0.001, 0.003), and the difference was statistically significant (all P<0.05). The average flow velocity of patients in S0 stage was (20.56±4.21) cm/s, PVVmax (22.19±4.33) cm/s, the average flow velocity of S1 stage was (18.39±3.79) cm/s, PVVmax (20.69±3.12) cm/s, and the average of S2 stage Flow velocity (13.46±2.21) cm/s, PVVmax (16.65±2.54) cm/s, average flow velocity in S3 stage (10.56±2.85) cm/s, PVVmax (13.42±2.46) cm/s, average flow velocity in S4 stage (8.15±1.65) cm/s, PVVmax (11.89±2.89) cm/s, the difference between the groups was statistically significant (F values were 21.35, 16.96, all P<0.001), and the difference between the two groups was statistically significant (all P<0.05). Correlation analysis by Spearman method showed that liver function grades were negatively correlated with average flow velocity and PVVmax (r values ?were -0.75 and -0.88, respectively; all P<0.001), and were positively correlated with liver Young's modulus and LSM. (r values ??were 0.54 and 0.86, respectively; all P<0.001). According to the ROC curve analysis, the AUC predicted by ultrasonic shear wave elastography was 0.75, AUC predicted by portal vein hemodynamics predicts was 0.68, and AUC predicted by combined detection predicts was 0.94. Conclusion:The combination of portal vein hemodynamics and ultrasonic shear wave elastography has a certain diagnostic power for the assessment of chronic hepatitis and liver fibrosis, with high specificity and sensitivity.
6.Clinical analysis of 185 patients with polycythemia vera.

OBJECTIVETo understand the clinical feature and natural course of polycythemia vera (PV).
METHODSThe clinical symptoms, signs, laboratory examination and prognosis of 185 patients with PV were analysed.
RESULTSThere are 122 males and 63 females. The mean age was (52.7 +/- 14.1) years. The mean hemoglobin level was (208.3 +/- 21.2) g/L. Pancytosis was displayed in 74 (40%) cases, excess of red blood cells in 33 (17.8%), excess of red blood cells and granulocytes in 67 (36.2%) and excess of red blood cell and platelets in 11 (5.9%). Splenomegaly was found in 123 (66.5%) patients and hepatomegaly in 30 (16.2%). Quantitative assess of serum Epo was done in 25 patients. The level was low in 16 (64.2%) and normal in 9 (36.0%). Hematopoietic progenitor culture yields was elevated in 11 patients, endogenous erythroid colonies (EEC) formation was found in 10 cases (90.9%). Eighty two patients (44.3%) had 101 attacks of vascular thrombotic incidents, 7 patients developed myelofibrosis (MF). Secondary cancer occurred in 1 patient. Two patients died of thrombosis.
CONCLUSIONPV is an elderly adult myeloproliferative disease with a high frequency of thrombosis. EEC can be found out in PV patients. The serum Epo level is not increased in PV patients. The main sequelae of PV is MF.

7.Role of Th1 cell in the pathogenesis of aplastic anemia.

OBJECTIVETo detect the quantity, proportion and function of producing cytokines of Th1 and Th2 cells in aplastic anemia (AA) patients and their contribution to the hematopoietic failure.
METHODS(1) Eleven patients with severe aplastic anemia (SAA) at diagnosis were observed by Marsh's method for the CFU-E, BFU-E and CFU-GM before and after depletion of CD(4)(+) T lymphocytes from bone marrow mononuclear cells (BMMNC); (2) Th1 (CD(4)(+) IFN-gamma(+)) and Th2 (CD(4)(+) IL-4(+)) cells in peripheral blood mononuclear cells (PBMNC) of 21 SAA patients and 17 normal controls were counted by FACS. (3) mRNA expression of IFN-gamma and IL-4 gene in unstimulated BMMNC from 16 SAA patients, 11 chronic aplastic anemia (CAA) patients, 26 other hematological diseases patients and 11 normal controls were measured by reverse transcriptase polymerase chain reaction (RT-PCR).
RESULT(1) CFU-E, CFU-GM and BFU-E increased significantly after depletion of CD(4)(+) T lymphocytes from BMMNC of SAA patients. (2) The percentage of IFN-gamma producing CD(4)(+) T cell (Th1) of SAA patients was significantly higher than that of controls, the percentages of IL-4 producing CD(4)(+) T cells (Th2) had no difference between SAA patients and normal controls. (3) IFN-gamma mRNA was detected in unstimulated BMMNC in 13 of 16 SAA patients, 6 of 11 CAA patients and one of 6 paroxysmal nocturnal hemoglobinuria (PNH) patients. The IFN-gamma mRNA was not detected in unstimulated BMMNC of 11 normal controls and other hematological diseases patients.
CONCLUSIONSDisbalance of CD(4)(+) T lymphocytes subsets and increases in quantity and IFN-gamma producing function of Th1 cells might be important for the development of bone marrow failure in AA and in distinguishing AA from other kinds of pancytopenic diseases.

8.Quantity and apoptosis-related protein level of B lymphocyte in patients with immunorelated pancytopenia.

OBJECTIVETo examine the quantity and apoptosis-related protein level of B lymphocyte in the patients with immunorelated pancytopenia (IRP) and explore the role of B lymphocyte in the pathogenetic mechanism of IRP.
METHODSQuantities of all B lymphocytes and CD(5)(+) B lymphocytes and the expressions of Fas and bcl-2 on B lymphocytes in 25 patients with untreated IRP, 15 IRP patients in complete remission (CR) and 10 normal controls were assayed by FACS.
RESULTSThe percentages of B lymphocyte and CD(5)(+) B lymphocytes were significantly higher in untreated IRP patients than in CR IRP patients and normal controls (P < 0.05); there was no significant difference between the latter two groups (P > 0.05). There was no significant difference of Fas expression in B lymphocytes among the three groups (P > 0.05). The expression of bcl-2 on B lymphocytes was significantly higher in untreated patients than in CR patients or normal controls (P < 0.05), and so did in CR patients than in normal controls (P < 0.01). The apoptosis-related index was significantly lower in untreated patients than in CR patients or normal controls (P < 0.01), and was lower in CR patients than in normal controls (P < 0.05). The percentage of B lymphocyte was positively correlated with the duration from the beginning of treatment to response.
CONCLUSIONThe production of auto-antibodies in IRP patients probably has some relationships with the abnormal quantities of B lymphocyte and its subsets, and with the inhibition of B lymphocyte apoptosis.

9.Study on the Mechanism of Sophora tonkinensis in the Treatment of Leukemia Based on Network Pharmacology

OBJECTIVE: To investigate the potential mechanism of Sophora tonkinensis in the treatment of leukemia. METHODS: The active components and their target proteins of S. tonkinensis were searched by the analysis of traditional Chinese medicine system pharmacology platform, and UniProt database and PubMed database were used to query corresponding gene names of target proteins of active components. Cytoscape 3.6.0 software was used to construct compound-target network. The genes related to leukemia were searched by DisGeNET databases, and OmicShare platform was used to screen the intersection genes of the active component targets of S. tonkinensis and leukemia disease targets. STRING database and Cytoscape 3.6.0 software were used to construct PPI network, and topological analysis was performed. GO analysis and KEGG pathway enrichment analysis were performed by using DAVID bioinformatics database. RESULTS: There were 13 active components and 204 target proteins in S. tonkinensis. The components and targets with high node degree included quercetin, kaempferol, PTGS2, PRSS1, CAMKK2, etc. There were 24 intersection genes between the active component target and leukemia target, including IRF1, BCL2, CYP1A1, PIM1, etc. PPI network of the above intersection genes contained 24 nodes and 142 edges, with an average node degree of 6.5 and an average medium of 0.045. The results of GO analysis showed that the biological process of the above-mentioned genes involved in apoptosis signaling pathway in vitro without ligands, negative regulation of apoptosis process, positive regulation of B cell proliferation, etc. Molecule function mainly included that protein homology activity and binding of the same protein. Cell components mainly included extracellular region, mitochondria and so on. KEGG pathway enrichment showed that above-mentioned genes were mainly associated with T cell receptor signaling pathway, JAK/STAT signaling pathway, HTLV-Ⅰ infection. CONCLUSIONS: Through JAK/STAT signaling pathway and HTLV-Ⅰ infection pathway, the active components of S. tonkinensis may act on PTGS2, PRSS1, CAMKK2 and other targets, and then play a therapeutic role on leukemia, showing the characteristics of multi-component, multi-target and multi-channel.
10.Study on the population-genetics of Mycobacterium tuberculosis from Sichuan Basin in China

Objective To explore the population-genetics characteristics of Mycobacterium tuberculosis (MTB) prevailing at the Sichuan basin of China.Methods A total of 413 MTB strains collected from Sichuan basin were genotyped by large sequence polymorphism (LSP) and 15 loci variable number tandem repeat (VNTR).Difference between the distribution of lineage population was analyzed by x 2-test and the discriminatory ability of each VNTR locus was evaluated under the Hunter-Gaston Discriminatory Index (HGI).Both phylogeny on population level and genetic structure were demonstrated through N-J tree and the Minimal Spanning Tree (MST).Genetic differentiation of different lineage strains was analyzed by Analysis of Molecular Variance (AMOVA).Time of the Most Recent Common Ancestor (TMRCA) was calculated based on the Bayesian model.Results Four hundred and thirteen MTBs were divided into two major lineages,in which the Beijing lineage accounted for 56.2% (232/413) and the Euro-American lineage for 43.8% (181/413).There was no significant difference of population distribution between the two lineages (P>0.05).The N-J tree of Beijing lineage MTB presented distinctly "star-like" and 72.4% strains were grouped to one clonal complex in MST.The Euro-American lineages MTB presented "branch-like" in N-J tree and were grouped into multiple clonal complexes in MST.There was significant genetic differentiation in Beijing lineage MTBs between Chongqing and Sichuan (FST=0.018 91,P<0.05),but not in the Euro-American lineage MTB (FST=0.005 19,P>0.05).TMRCA of the largest clonal complex in Euro-American lineage MTB appeared to be 723 (95% CI:517-946) years.Conclusion Both Beijing lineage and Euro-American lineage MTBs were competitively prevalent in the Sichuan basin.There was some difference noticed between the two lineages referring to genetic differentiation.The invasion of Euro-American lineages MTB to the basin area might be associated with a war occurred in this area about 720 years ago.
