1.Dissolution of Yinxingye Dispersible Tablets by Quantitative Analysis of Multi-components with Single-marker

Objective:To establish a quantitative analysis method for determining the dissolution of Yinxingye dispersible tablets by quantitative analysis of multi-components with single-marker ( QAMS) . Methods:A small glass method was adopted. Hydrochloric acid (0. 1 mol·L-1 ) was used as the dissolution medium, and the stirring speed was at 50 r·min-1 . HPLC-ELSD with gradient elu-tion combined with QAMS was used to simultaneously determine the dissolution of 3 total flavonoids ( artemisinin, quercetin and isorh-amnetin) and 4 total lactones (ginkgo esters and lactones a, b and c) in Yinxingye dispersible tablets. Results:Within a certain line-ar range, the relative correction factors (RCFs) of quercetin and isorhamnetin with the reference of artemisinin was 1. 873 and 0. 324, respectively, and the RCFs of lactones a, b and c with the reference of ginkgo esters was 2. 280, 1. 659 and 1. 429, respectively, and the repeatability was good under the different experimental conditions. There were no significant differences between the calculated val-ues by QAMS and those by the external standard method in 5 batches of Yinxingye dispersible tablets, and the results showed that the RCFs was authentic. The dissolution uniformity of Yinxingye dispersible tablets was good in 0. 1 mol·L-1 hydrochloric acid. Conclu-sion:The method is simple, accurate and reproducible in the dissolution determination of Yinxingye dispersible tablets.
2.Insight on the separated ownership/operation reform of public hospitals in Zhenjiang city

Public hospitals are the backbone of China's healthcare,shouldering critical tasks of ensuring health of the Chinese people at large.Their reform,therefore,should enhance and coordinate the balance between public welfare and efficiency of public hospitals as much as possible.Such a “separated ownership/operation” reform in the management system reform of public hospitals can raise their operational and management efficiency,so as to maximize their performance of public benefits.Based on the basic theory of separated ownership and operation,authors of the paper proposed policy recommendations for such a reform upon the analysis of the four optional patterns separated ownership and operation of public hospitals.
3.Association of CACNA1A gene polymorphism with familial hemiplegic migraine in southern Chinese

BACKGROUND: The involvement of genetic factors in migraine has been under close investigation, and geneiic epidemiological study and segregation analysis have confirmed genetic disposition as an important risk factor for migraine.OBJECTIVE: To analyze the connection between mutations in CACNA1A gene and familial hemiplegic migraine (FHM) in southern Chinese Han patients by examining the three most frequently mutated sites in CACN1A gene.DESIGN: Sampled survey.SETTING: First Affiliated Hospital of Zhongshan University and Baoan Xixiang People's Hospital of Shenzhen City.PARTICIPANTS: All the participants were selected from patients in the above two hospitals and their relatives, including 10 patients with FHM, 12 relatives of the patients in 2 pedigrees, 53 migraine patients with aura without family history, and 10 healthy control subjects.METHODS: The exons 13, 16 and 17 of CACNL1A4 gene were amplified by PCR. Single-strand conformation polymorphism technique was employed to detect the most frequent mutations in the 3 exons (T666M, R583Q and D715E) in these subjects.MAIN OUTCOME MEASURES: ① Results of PCR amplification of the 3 exons of CACNL1A4 gene; ② Results of SSCP for mutation analysis of the 3 exons.RESULTS: Participants completed the study. The target fragment length of exons 13, 16 and 17 were 247 bp, 268 bp and 204 bp, respectively.None of mutations of T666M, R583Q and D715E were detected in the subjects, including FHM patients and their relatives, migraine patients without family history and the healthy control subjects.CONCLUSION: None of the 3 most frequent mutations (T666M, R583Q and D715E) can be detected in southern Chinese FHM pedigrees or migraine patients without family history.
4.Clinical evaluation of non small-cell luny cancer operated by autofluorescence bronchoscopy

ObjectiveTo explore the role of autofluorescence bronchoscopy in lung cancer operation and the meaning of choice lung cancer operation mode.MethodsTo retrospectively analyze 32 non-small cell lung cancer patients.Before lung cancer operation,white light bronchoscopy (WLB) and autofluorescence bronchoscopy (AFB) had been done routinely.Compared the different invasive tumor conditions by WLB and AFB,operation modes were decided by the edge of the tumor which were proved by biopsy pathology.ResultsIn 32 cases,19 cases underwent pulmonary lobectomy.One case underwent carinal resection and reconstruction.8 cases underwent sleeve lobectomy.4 cases underwent other therapy because of tumor airway metastasis.In 8 cases which underwent sleeve lobectomy,3 ca ses were found by WLB and AFB together,5 cases were found only by AFB.In 4 cases who had no operation chance,2 cases were found by WLB and AFB together,2 cases were only by AFB.The sensitivity for the detection of bronchial premalignant lesions was extremelyhigher withthe addition of AFB than WLBalone ( P < 0.05 ).ConclusionAutofluorescence bronchoscopy is a safe and efficient technique which could improve the sensitivity of diagnosis in lung cancer than WLB.It is important to select operation mode.
5.Study on the Protective Effect Mechanism of Total Flavones from Prunus cerasifera Fruits for Alcoholic Liver Disease in Rats

OBJECTIVE:To study the protective effects of total flavone from Prunus cerasifera fruits(PCE)on alcoholic liver disease(ALD)in rats,and provide experimental basis for developing new medicines for anti-ALD. METHODS:40 rats were ran-domly divided into normal group(distilled water),model group(distilled water),silibinin group [positive control,30 mg/(kg·d)] and PCE high-dose,low-dose groups [80,40 mg/(kg·d)],8 in each group. All rats were intragastrically administrated(10 mL/kg) every morning,once,for 6 weeks;meanwhile,except for normal group,rats in other groups received 50% alcohol(10 mL/kg) once intragastrically every afternoon to induce ALD model. After administration,heart,liver,spleen and other organ indexes,sub-cutaneous fat,brown fat,abdominal fat indexes were determined,as well as serum biochemical indexes [glutamate transaminase (ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IBIL),gamma-glu-tamyl transpeptidase(GGT),and total cholesterol(TC),triglyceride(TG),high-density lipoprotein(HDL),low-density lipopro-tein (LDL)] level and liver biochemical indexes [superoxide dismutase (SOD),malondialdehyde (MDA),glutathione peroxidase (GSH-Px),TC,TG] and TC,TG levels in feces;pathological changes of liver and kidney tissues were observed. RESULTS:Compared with normal group,heart,liver,spleen indexes,subcutaneous fat,abdominal fat indexes in model group were in-creased(P<0.05 or P<0.01),brain index and brown fat index were decreased(P<0.05);HDL level and HDL/TC ratio in serum were decreased,other serum indexes were increased(P<0.05 or P<0.01);SOD,GSH-Px levels in liver tissue were decreased, other above-mentioned liver biochemical indexes were increased(P<0.01);TC,TG levels in feces were increased(P<0.01);liv-er and kidney showed obvious lesions. Compared with model group,the above-mentioned indexes in silibinin group and PCE high-dose group were significantly improved (P<0.05 or P<0.01);ALT,AST in serum and MDA level in liver tissue in PCE low-dose group were significantly decreased(P<0.01),and SOD,GSH-Px levels in liver tissue were significantly increased(P<0.05);lesion degree of liver and kidney and lipid accumulation in liver were reduced in administration groups. CONCLUSIONS:PCE may play a role in anti-ALD by anti-oxidation,promoting liver cell regeneration and regulating lipid metabolism.
6.The clinical features, neuroimaging findings and pathological characteristics of 26 patients with pathologically proven tumor-like inflammatory demyelinating diseases

Objective To summarize the clinical features, neuroimaging findings and pathological characteristics of 26 patients with tumor-like inflammatory demyelinating diseases (TIDD) confirmed by histopathology for better diagnosis and differential diagnosis. Methods The clinical features, neuroimaging findings and pathological characteristics of 26 patients (14 male, 12 female) with pathologically proven TIDD(24 brain-type and 2 spinal cord-type ) were retrospectively analysed. Results The mean onset age was 6-69 (36.7±13.8) years. Twenty-one patients had good prognosis with a median followed-up duration of 51.0 months. Two patients were died of post-operative complication and pulmonary infection respectively and the remaining 3 patients were lost to followed up. The TIDD patients almost showed monophasic clinical setting. Headache, indifference accompanied with hypomnesis were the commonest initial symptoms. The positive or abnormol rates of cerebrospinal fluid oligoclonal bands (OCB) and myelin basic protein (MBP)in TIDD patients were high. The involvements of bilateral and multi-lesions were commonest in TIDD (61.5%, 65.4% respectively). Twenty-two patients with CT unenhanced scanning showed hypodense lesions. Long T1 and long T2 signal intensity was showed on MRI and most cases apeared round-like lesion in shape. Acccording to the shape of enhancement of the 23 patients performed with contrast agents, 11 were shown with open-ring enhancement, 4 cases (including 2 accompanied with open-ring enhancement) with complete ring enhancement, 3 with asymmetrical dotted enhancement, 2 with diffused even enhancement,and no enhancement was seen in the other 6. Furthermore, 14 cases with DWI and 12 with FLAIR all appeared hyperdensity. The typical pathological changes were demyelinating, perivascular inflammatory cells infiltration and reactive gliosis. Occationally, the Creutzfeuldt cells were also found in brain tissue of some patients. Conclusions TIDD is a distinct demyelinating disease entity. In spite of being apt to be confused with the neoplasm in brain and spinal cord. TIDD has its own-features, for example, OCB is frequently positive in patients with TIDD and the level of MBP may be significantly increased. Furthermore, the involvements of bilateral and multi-lesions are the common in TIDD, and most cases showed open-ring enhancement or complete rim enhancement on MRI. In addition, all cases present hypodense lesions on unenhanced CT and patients with hyperdense seemed not to be considered as TIDD.
7.Role of external anal sphincter electromyography for differential diagnosis of multiple system atrophy, Parkinson's disease and late-onset spinocerebellar ataxia

Objective To observe the electrophysiological changes, especially in the examination of external anal sphincter electromyography ( EAS-EMG), with those patients diagnosed as multiple system atrophy ( MSA), Parkinson's disease (PD) and late-onset spinocerebellar ataxia (LOSCA) and explore its clinical diagnostic value as well as differential diagnostic value for the three diseases. Methods The clinical data, cranial magnetic resonance imaging (MRI) data as well as results of EAS-EMG for 3 groups patients, including 88-cases MSA, 69-cases PD and 18-cases LOSCA, were analyzed retrospectively.Results EAS-EMG showed that 84 cases (95.5%) in MSA group had varying degrees of neurogenic injury. Meanwhile, mean motor unit potentials (MUPs) duration (( 12.92 ± 2.59)ms), mean MUPs amplitude ( ( 648.6 ± 251.0 ) μV ), and MUPs polyphasicity ( percentage of polyphasic MUPs; 42. 6% ±21.2% ) in MSA group were significantly different from those in PD ( ( 8. 99 ± 0. 47 ) ms, (470. 0 ±91.9) μV, 24.2% ±11.0%) and LOSCA groups ((9.04 ±0.62)ms, (493.1 ± 113.7)μV,22.0% ±12. 1%; Welch:94. 240,18. 093,26. 710,all P =0. 000). The spontaneous potentials and satellite potentials showed more common in MSA group, but not in other groups. Conclusions MSA and PD and LOSCA are easily mutually misdiagnosed because of some similar syndromes, but the method of EAS-EMG could be effective and helpful to enhance accurate diagnostic rate of MSA and its differential diagnosis with PD and LOSCA.
8.Clinical and neuroimaging features of gliomatosis cerebri confirmed by neuropathology

Objective To study the clinical and neuroimaging features of gliomatosis cerebri(GC)so as to enhance the accuracy of diagnosis. Methods The clinical manifestations and cranial MRI characteristics of 10 cases of GC confirmed by neuropathology of brain biopsy were analyzed. Results Main clinical features of the 10 patients were not so severe and varied with headache, dizziness, dementia,aphasia, epilepsy, etc. Cranial MRI scan showed diffuse long T2 signals in frontal lobes, temporal lobes,parietal lobes, occipital lobes bilaterally, as well as corpus collosum, thalamus, paraventricular white matter, caudate nucleus and putamen. Brain stem and cerebellum were also involved. The borders of the lesions were unclear and no obvious mass effect was found. Contrast enhancement was positive only in 3 patients after Gd-DTPA injection. The lesions were always bilateral without cystic formation or hemorrhage.DWI signals of 8 cases were in accordance with pathological grades. Cho/Cr and Cho/NAA ratio increased in all 7 cases with MRS imaging. Cho/Cr in tumor area versus normal area was 1.28 ±0. 15 vs 0. 92 ±0. 17(t=4.201,P=0.0012,95% CI:0.17-0.57).The value of Cho/NAA was 3.21 ± 1.19 vs 0.61 ±0.18(t = 5.716,P=0.0001,95 % CI:1.61-3.59).Biopsy pathological examination showed diffuse infiltrative growth of astroglioma cells of 8 cases with low-grade while 2 cases with high-grade. Conclusions Mismatch between relatively slight clinical manifestations and heavy diffuse imaging features are important in the diagnosis of GC, with the aid of brain biopsy. MRS would contribute to diagnosis and DWI signal may be correlated with GC grade and help judging prognosis.
9.Cephal CT scanning for distinguishing tumor-like inflammatory demyelinating diseases from glioma or primary central nervous system lymphoma

Objective To explore the value of brain CT scanning for distinguishing tumor-like inflammatory demyelinating diseases (TIDD) from glioma or primary central nervous system lymphoma.Methods The brain CT features in 20 patients with TIDD(10 female,10 male;mean age (35.6±14.0)years;range,6-51 years)and 32 gliomas(16 female,16 male;mean age(42.0±19.8)years;range,12-75 years)and 6 lymphomas(3 female,3 male;mean age(53.8±11.8)years;range,32-68 years)were retrospectively reviewed and compared between brain tumors and TIDD.Results (1)Among the 38 primary brain tumors,there were 19 cases(50%,14 gliomas,5 lymphomas)with hyperdense lesions,10 cases(26.3%,9 gliomas,1 lymphomas)with isodense lesions,and 9 glionms (23.7%)with hypodense lesions.In contrast,the brain unenhanced CT manifestation of 20 TIDD all showed with hypodense lesions.(2)On unenhanced CT the lesions of 6 lymphomas all were hyperdense or isodense,like 90% of 20 high grade gliomas(WHO grade Ⅲ and Ⅳ),but this rate for grade Ⅱ was only 41.7%.(3)According to the shape of hyperdense lesions of the 19 primary brain tumors with,7 cages(6 gliomas,1 lymphomas)manifested with asymmetric hyperdense small-patches,4 cases(1 gliomas,3 lymphomas)with symmetric hyperdense large-patches,4 cases(3 gliomas,1 lymphomas)with diffused hyperdensed lesions,and 4 cases(4 gliomas)with ring-shaped hyperdensed lesions.Furthermore,4 primary brain tumors(4 lymphomas)underwent CT enhanced scanning and all the cases showed strong enhancement(3 cases with hyperdense lesions and 1 with isodense lesions on unenhanced CT),but only 3 cases of 7 TIDD showed mild enhancement in contrast.(4)By Spearman's relevant analysis,hyperdense and isodense on unenhanced CT was proved to have significant positive correlation between the grade of gliomas(r=0.435,P=0.013).Therefore,the frequency of hyperdense and isodense lesions in lymphomas and WHO grade Ⅲ and Ⅳ astrocytoma was higher in contrast with low grade astrocytoma.Conclusions Brain CT as a simple,economical and practical examination method has significant meaning for differentiating TIDD from glioma or PCNSL and could be used as an adjuvant method for MRI and magnetic resonance spectroscopy.Patients with hyperdense or isodense on unenhanced CT or strong enhancement could be excluded from TIDD.
10.Investigation of inductive effect of triptolide on cytochrome P450 s in rat hepatocytes and HepG2 cells and possible mechanism

Aim Toinvestigatingtheinductionof CYPs in hepatocytes or HepG2 cells by triptolide(TP) andthepossiblemechanism.Methods AfterTPtreat-ment,the expression of CYPs in rat primary hepato-cytes or human HepG2 cells was detected by real-time PCR and Western blot assays.Specific inhibitors or gene knockdown method were employed to analyze the possiblemechanism.Results Theexpressionof CYP1A2,2C7,2C11,2C12,2D2,2E1 and 3A1 in rat primary hepatocytes was induced by TP.The fold was 19,2,31,3,21,88 and 34 at 50 nmol·L-1, respectively while at 100 nmol·L-1 it was 20,5,30,23,61,83 and 38,respectively.In HepG2 cells,the expression of human CYP1A1,2B6,2C9,2C19, 2D6,2E1 and 3A4 was also induced by TP.The ac-tivities of nuclear receptor PXR and CAR were inhibi-ted.TP upregulated p53 expression,and the induction of several CYPs caused by TP was blocked when p53 wasinhibited.Conclusions TPinducesCYPsexpres-sion in rat hepatocytes and HepG2 cells.Nuclear re-ceptors may not be involved in TP induced CYPs, while the mechanism may partly attribute to p53.
