1.Clinical analysis of treating the developmental dislocation of the hip in old age children by Pemberton operation and femoral shortening

[Objective]To review the treating effect of cases of the developmental dysplasia of the hip in old age children by a one-stage combined Pemberton operation and femoral shortening and investgate if the hip joint stiffness rate and ischemic necrosis rate of femoral head after operation are reduced. [Method]From June 2002 to Decempber 2006, 136 hip congenital dislocation of 101 old age children (7 years to 14 years) were treated by Pemberton operation and femoral shortening,and were followed-up from 1.5 years to 4 years(average, 2 years and 7 months).[Result]Thirty-one cases can only bend their hip joint below 90 degrees, accounting for 22.79% of all. Thirty-eight ischemic necrosis of femoral head occurred which accounted for 27.94%.The rate of ischemic necrosis of the femoral head and hip joint stiffness of old age children after operation were reduced obviously according to the standard by Salter.[Conclusion]Pemberton operation and femoral shortening are preferred in treating the developmental dysplasia of the hip of old age children.It can reduce the rate of ischemic necrosis of femoral head and hip joint stiffness.
2.Studies on physiology of sleeping of swine model with obstructive sleep apnea syndrome

Objective To study the physiology of sleeping in a swine model with obstructive sleep apnea syndrome (OSAS) reproduced by high altitude and hypoxia. Methods Twelve male minipigs were randomly assigned to 2 groups, named A and B. The pigs of Group A were placed in Room 1, which was a double-room altitude chamber, and the pressure in it was adjusted to 53.9 kPa with an oxygen concentration of 10.0%~11.2%. The pigs of Group B were kept in Room 2 with altitude of sea level and oxygen concentration of 21%. All the animals were kept in their respective rooms for 6 hours per day and for 22 consecutive days. On the 23rd day sleeping was monitored, at the same time the movements of chest and abdomen, respiratory airflow, heart rate, and pulse oxygen saturation (SpO_2) were recorded. Apnea/Hypopnea Index (AHI), Hypopnea Index (HI), and average SpO_2 were derived from analysis of the data of sleeping monitoring. Pigs were sacrificed after the experiment, and pharyngeal tissue was harvested for pathological examination. 3.MRI features of Rosai-Dorfman disease in CNS

Objective To summarize the clinical and imaging characteristics of primary Rosai-Dorfman disease (RDD)in central nervous system in order to understand well its features and improve the diagnosis.Methods The clinical and imaging features in 3 patients with RDD in CNS proved by pathology were retrospectively analyzed in combination with the related literatures.Results The lesions in 2 patients occurred in brain and other lesion in 1 occurred outside the brain.Iso-or slight hyperintensity on T1 WI and marked enhancement of the lesion were found.Single lesion was found in 2 patients near the dura mater and located near the superior sagittal sinus or cervical spinal cord.Multiple lesions in 1 patient were located in the lateral ventricle.Conclusion It’s suggested that single or multiple lesions in dural or spinal meninges located in the midline or near the intracephalic venous system would be considered as RDD in CNS.

Objective To research the clinical therapeutic effect and side effect of gesterol- pingyangmycin cream in interventional therapy of hysteromyoma. Methods 110 cases of hysteromyoma underwent interventional therapy who were divided randomly into two groups:group A with polyvinyl alcohol particle and group B with gesterol-pingyangmycin cream .By percutaneous catheterization in bilateral uterine arteriae using the technique of Seldinger,polyvinyl alcohol particle or gesterol-pingyangmyci cream were injected to blodk up the blood supply of hysteromyoma.The volume of hysteromyomas was measured with B-mode ultrasonography prior and post-treatment.The reaction of post-treatment and the changes of menstrual blood volume were observed . Results The menstruation turned to normal and or menstrual blood volume reduced obviously in two groups , the total effective rate was 95.49 . 6.Evaluation of the Cartilage Injury in Osteoarthritis in Rabbits with MR Imaging

Objective To evaluate the value of fat-saturated three-dimensional fast imaging with steady state procession(FS-3D-FISP)sequence in detecting hyaline cartilage defects in osteoarthritis in rabbits.Methods Osteoarthritis was induced in eighteen male rabbits by injecting papain into the left knee joints.The same volume of sterile saline solution was injected into right knee joints as controls.MRI of knee joints was done at 1,2 and 4 weeks after injection using FS-3D-FISP sequence.The the rabbits were killed and pathohistological examinations of the femoral condylars and tibial plateaus were performed.The cartilage changes were observed by MR image according to the Outerbridge method and by pathology using Collins staging.Results There was closely correlation between MR imaging and pathology in grading of cartilage injury in grade I~II and grade III~IV(P<0.001).Conclusion FS-3D-FISP can evaluate accurately the degree of articular injury.
7.Killer cell immunoglobulin-like receptor gene polymorphism in Hashimoto′s thyroiditis patients

Objective To investigate the relationship of the killer cell immunoglobulin-like receptor (KIR) gene polymorphism with Hashimoto′s thyroiditis(HT). Methods One hundred HT patients and 260 randomly matched healthy controls were enrolled to detect the KIR genotype. The genomic DNA were extracted, and 15 selected KIR genes, KIR2DL1-5, KIR3DL1-3, KIR2DS1-5, KIR3DS1 and pseudogene KIR2DP1, were determined by a polymerase chain reaction using sequence-specific primers (PCR-SSP). Results The frequency of KIR2DL5 gene was significantly lower of the patient group than that of the control group (0.200 vs 0.312, RR=0.64, P<0.01). Conclusion There may be an association between pathogenesis of HT and KIR2DL5 gene.

Objective To study the significant immunohistochemical marker to identify lung adenocarcinoma(ADC) and squamous cell carcinoma(SCC).Methods Three hundred and twenty-nine Choose 329 cases of nonsmall-cell lung cancer (NSCLC) were chosen.Analysis of the clinical and pathological features.The expression of cell keratin 7 (CK7),thyroid transcription factor-1 (TTF-1),Napsin A,CK5/6,p40 and p63 were detected by using immunohistochemistry.Results (1) Among 329 specimens,containing 129 cases of resections,195 cases of biopsies and 5 cases of pleural effusion specimens.(2)In these cases,187 cases were classified to be ADC,142 cases were classified to be SCC.(3) CK7,TTF-1,Napsin A,CK5/6,p40,p63 sensitivity were 97.9%,87.2%,81.3%,6.4%,3.7%,18.7% in ADC groups,and 25.4%,11.3%,0,92.3%,95.1%,98.6% in SCC groups,and the differences of two groups were significant statistically (x2 =190.665,187.432,214.542,242.003,274.407,206.818;P< 0.001).(4) In the 3 IHC of ADC,CK7 had the highest sensitivity,Napsin A had the highest specificity.In the 3 IHC of SCC,p63 had the highest sensitivity,p40 had the highest specificity.Conclusion CK7,TTF-1,Napsin A,CK5/6,p40 and p63 can be a markable panel of immunohistochemistry in the differential diagnosis of NSCLC.
9.Valuea of contrast-enhanced imaging and enhanced CT in diagnosis of traumatic spleen rupture

Objective To explore the value of contrast-enhanced ultrasound in the diagnosis and classification of traumatic spleen rupture , as compared with enhanced CT. Methods The manifestations of contrast-enhanced ultrasound on surgically or clinically confirmed spleen rupture in 40 patients were retrospectively analyzed. The performance of contrast-enhanced ultrasound on diagnosis and classification was compared with that of enhanced CT. Results For 40 patients with traumatic spleen rupture , the of accuracy of enhanced CT and contrast-enhanced imaging in the diagnosis of traumatic spleen rupture was 97.5%, with no significant statistical differences (chi-square = 0, P = 1). On contrast-enhanced ultrasound examination, 14 patients were diagnosed as true splenic rupture , 9 as subcapsular spleen rupture , and 16 as central splenicrupture, with a accuracy rate of 92.5% (37/40); and accuracy rate for enhanced CT was 90.0% (36/40), there was no significant statistical difference (chi-square = 1.97, P > 0.05). Conclusions Contrast-enhanced and enhanced CT have good consistency in the diagnosis and classification of traumatic spleen rupture. Contrast-enhanced ultrasound can accurately determine the scope and degree of spleen damage , resulting in more accurate classification; and it has values in the diagnosis of traumatic spleen rupture and choice of therapies.
10.Pedicle subtraction osteotomy for repairing thoracolumbar degenerative kyphosis:correlation of spino-pelvic parameters

BACKGROUND:Thoracolumbar degenerative kyphosis could experience severe lumbar back pain due to sagittal plane imbalance, thereby affecting quality of life. Thus, it is very important to reconstruct spino-pelvic profile in these patients. OBJECTIVE:To explore the relationships between life quality and spino-pelvic parameters fol owing pedicle subtraction osteotomy for thoracolumbar degenerative kyphosis and the clinical significance. METHODS:Between May 2010 and October 2014, 59 patients with thoracolumbar degenerative kyphosis undergoing L2 pedicle subtraction osteotomy in Cangzhou Hospital of Integrated Traditional and Western Medicine were retrospectively reviewed. Anteroposterior and lateral X-ray films of al patients during standing were photographed before and after surgery. The pre-and post-operative thoracic kyphosis, lumbar lordosis, sagittal imbalance, T1 pelvic angle, pelvic incidence, sacral slope and pelvic tilt were measured in al patients. The patients’ quality of life was evaluated using SF-36 preoperatively and postoperatively. RESULTS AND CONCLUSION:(1) Significant differences were observed in the improvement of thoracic kyphosis, lumbar lordosis, pelvic tilt, sacral slope and sagittal imbalance (P<0.01). With respect to SF-36, postoperative SF-36 score was higher than preoperative postoperative SF-36 score (P<0.01). (2) The alteration of lumbar lordosis showed significant correlation with the change of pelvic tilt, sacral slope and sagittal imbalance. The change of pelvic tilt exhibited cardinal correlation with the change of sacral slope, body pain and general health. The improvement of sagittal imbalance significantly correlated with the improvement of lumbar lordosis, body pain and general health. The improvement of T1 pelvic angle significantly correlated with the improvement of lumbar lordosis, sagittal imbalance, body pain and general health. (3) Pedicle subtraction osteotomy can effectively restore spino-pelvic sagittal profile, improve the life quality and relieve pain for the patients with thoracolumbar degenerative kyphosis.
