1.The changes of serum homocysteine, C-reactive protein levels in acute cerebral infarction patients and its clinical significance

Objective To investigate the changes of serum homocysteine ( Hcy),C-reactive protein ( hsCRP) levels in acute cerebral infarction patients and its significance.Methods 64 patients with acute cerebral infarction were selected as research group,and 60 healthy volunteers were selected as control group.Detected the acute cerebral infarction patients after 1d,the first 7d,14d Hcy,hs-CRP,total cholesterol,triglycerides,high density lipoprotein,low density lipoprotein levels and the control group the level of these indicators.Results Of total cholesterol,triglycerides,low density lipoprotein of the study group was higher than those of the control group of high-density lipoprotein(t =3.073,2.874,2.015,all P <0.05);after admission ld,treatment of the first 7d,14d showed no significant difference between the two groups ( t =2.223,2.268,all P < 0.05 ).Conclusion Hcy,hs-CRP and the occurrence of acute cerebralinfarction was closely related to the development,and testing to determine the severity of the disease for the guide had important clinical significance.
2.The effects of rehabilitation training on microvessel nltrastructure and expression of CD31 in rats with focal cerebral infarction

Objective To study the effects of rehabilitation training on angiogenesis and its ultrastructure and expression of CD32 in the peri-infarction region of rats with focal cerebral infarction. Methods Sixty-six Spra-gue-Dawley rats with experimental left middle cerebral artery occlusion (MCAO) were used as subjects in this study. All the rats were randomly divided into three groups: a rehabilitation training group (n=30), which was given bar rotating, balancing and rolling exercises everyday after 48 hours post MCAO; a control group (n = 30) and a sham-operated group (n=6), which were fed in cages with no special training exercises. Then brain tissues were fixed on the 3rd d, 7th d, 14th d after MCAO, for observing the morphological alterations of microvessels in peri-infarction re-gion using transmission electron microscope, immunohistochemistry and Western blotting were used to measure the ex-pression of CD31, which acted as the marker of the neogenetic microvessels. Results (1) It showed that the capil-lary endothelial cells were less edematous in the rehabilitation training group, and there were less pinocytosis bullae in basal membrane more integral nucleus of endothelial cells in rehabilitation training group when compared with those in the control group. (2) Expression of CD31 can be observed in peri-infarction region in both groups from the 3rd d on-wards, and peaked on the 7th d, and then gradually went down after the 14th d. Comparison between the 2 groups showed that the expression of CD31 in rehabilitation training group was higher than that in the control group at every time point, but statistical difference between the 2 groups in this regard could be revealed only on the 7th d (P< 0.05), Conclusion Rehabilitation training could promote ultrastruetural recovery of microvessels and induce an-giogenesis in peri-infaretion region, and it might be one of the mechanisms of neural functional recovery in rats after MCAO.
3.Adverse reactions of Nedaplatin and Cisplatin in concurrent chemoradiotherapy for locally advanced nasopharyngeal carcinoma

Objectives To analyze comparatively the adverse reactions of Nedaplatin(NDP)and Cisplatin(DDP)in concurrent chemoradiotherapy for locally advanced nasopharyngeal carcinoma and summarize the nursing points as well.Methods From March 2012 to March 2013,112 patients with locally advanced nasopharyngeal carcinoma were randomly divided into NDP group and DDP group.Besides intensity modulated radiotherapy for both groups,NDP group were treated with intravenous drop infusion of NDP by 100 mg/m2 and the control group with intravenous drop infusion of DDP by 100 mg/m2 both for three courses of once every three weeks (e.g.day one,day 22 and day 43 during the course).The two groups were compared in terms of therapeutic effects and incidences of adverse reactions.Results The complete remission rates of the NDP group and DDP group were 87.5%and 85.7%,respectively (P>0.05).The incidences of adverse reactions like gastrointestinal reactions and radioactive mucositis in the NDP group were significantly lower than those in the DDP group(P<0.05)and the index of platelet decrease was significantly higher(P<0.05).There were no significant differences between the two groups in terms of liver and kidney dysfunction and white blood cells decrease(P>0.05). Conclusions Chemotherapy with NDP combined with radiotherapy for locally advanced nasopharyngeal carcinoma has fewer adverse effects and is easy to be accepted by patients so that their quality of life can be improved.In the application of the two kinds of chemotherapy,we should pay attention to the adverse reactions on patients in order to give pertinent care.
4.The pathologic mechanism underlying hypertension-induced decline in arterial elasticity

Objective To investigate the effect of hypertension on the large artery elasticity index (C1),the small artery elasticity index (C2) and the medial structure of the ascending aorta as well as the relationship between artery elasticity and the medial structure of the ascending aorta.Methods Sixty patients with CHD receiving coronary artery bypass graft surgery at our hospital were divided into two groups:30 patients in the hypertension group and 30 patients in the non-hypertension group.C1 and C2 were measured using the CVProfilor DO-2020 system.Sections of tissues taken from the anterior wall of the ascending aorta during the surgery were subjected to Masson's trichrome staining for the detection of vascular smooth muscle and collagen fibers and Weigert's resorcin-fuchsin staining for the detection of elastic fibers.The relative areas of vascular smooth muscle fibers,collagen fibers and elastic fibers of the ascending aorta were measured by a computer image analysis system under the light microscope.The linear correlations of C1 and C2 with the medial structure of the ascending aorta were analyzed.Results C1 in the non hypertension group was higher than that in thehypertensiongroup[11.9±1.8 (ml/mmHg×10) w 13.1±2.5 (ml/mmHg×10),t 2.22,P <0.05].In the media of the ascending aorta,the relative content of collagen fibers was higher,while the relative content of elastic fibers was lower in the hypertension group than in the non-hypertension group [(46.0±3.8)% w (42.2±3.0)%,(17.5±3.5)% vs.(19.3 2.7)%,respectively,t=4.24 and 2.20,P<0.01 or 0.05].C1 was positively correlated with the relative content of elastic fibers but negatively correlated with the relative content of collagen fibers in both groups (r=0.52 and 0.39,respectively,P<0.05 or 0.01).Conclusions The main pathogenic basis of hypertension-induced decline in arterial elasticity in CHD patients is increased collagen fibers and reduced elastic fibers with disorganization of the two types of components.C1 may accurately reflect the effect of hypertension on medial collagen fibers and elastic fibers in the ascending aorta.
5.The effect of CPB with or without blood cardioplegia on hemodynamics and myocardial function

Objective To evaluate the effect of CPB with or without cardioplegia on hemodynamics and myocardial function. Methods Thirty NYHA class III patients undergoing mitral valve replacement were randomly divided into three groups of ten each: group Ⅰ received no blood cardioplegia; group Ⅱ received tepid blood cardioplegia solution and group Ⅲ received cold blood cardioplegia solution. The patients were premedicated with pethidine 50mg and scopolamine 0.3mg. Swan-Ganz catheter was inserted via right interval jugular vein into pulmonary artery and radial artery was cannulated under local anesthesia before anesthesia. Anesthesia was induced with midazolam 0.05-0.01 mg.kg-1, fentanyl 15-20?g. kg-1 and pancuronium 0.12mg.kg-1 and maintained with fentanyl, midazolam and pancuromium. Patients in group Ⅰ received no cardioplegic solution. Ascending aorta was not cross-clamped. Body temperature was reduced to 32℃-33 ℃ (naso-pharyngeal T) . The empty heart was beating at 40-60 bmp. In group D and Ⅲ cardioplegic solution (modified St. Thomas solution) was added to blood from oxygenator (in the proportion of 1:4).In group Ⅱ body temperature was reduced to 32℃-33℃.Tepid (32℃ ) hyperkalemia blood cardioplegic solution was infused at 200-250ml.min-1 after ascending aorta was cross-clamped. After cardioplegic arrest hypokalemic blood cardioplegia solution was infused. In group Ⅲ body temperature was reduced to 28℃-29℃. Cold (8℃) hyperkalemic blood cardioplegic solution was infused at 200-250ml. min-1, after ascending aorta was cross-clamped. After cardioplegia arrest cold hypokalemic blood cardioplegia solution was infused every 20 min. Hemodynamic parameters (MAP, MPAP, CO, CI, SVRI, PVRI, LVSWI and RVSWI) were recorded before anesthesia (T0), before CPB (T1), 10, 30, 60 min after termination of CPB (T2-T4 ) and at the end of surgery (T5 ) . Results Demographic data including age, gender, body weight and body surface area were comparable between the three groups. CI in group 1 and Ⅱ was significantly higher at T2 -T5 than that in group Ⅲ . SVRI after CPB in group Ⅰ was significantly lower than that in group Ⅲ . Conclusion The myocardial function after CPB without cardioplegia is better than that after CPB with cold cardioplegia, but is not significantly different from that after CPB with tepid blood cardioplegia.
6.The effects of rehabilitative training on motor function and expression of GAP-43 and SYN in rats with local cerebral infarction

Objective To study the effects of rehabilitative training on motor function and expression of GAP-43 and SYN in rats with local cerebral infarction. Methods A total of 76 male adult Sprague-Dawley rats were randomly divided into a rehabilitative training group(n=32),a control group(n=32),and a sham-operated group(n=1 2).All the rats were subjected to left middle cerebral artery occlusion(MCAO)with the suture occlu sion.Motor training programs including balancing,grasping,rotating and walking exercises were administered to the rats of the rehabilitative training group at 48 hours post-operation,while those of other two groups were reared in their original living status without any special training.The animals were given behavioral tests with Bederson test,balancing wood test,net screen test to assess the functional outcome,and immunohistochemistry staining was employed to evaluate the exDression of GAP-43 and SYN in peri-infarction cortex at the 3rd,7th,21 st,35th days after MACO,respectively. Results The scores of behavioral tests in the rehabilitative training group was better than those in the control group(P<0.05)at the 7th,21 st,35th day after MCAO,and the immunostaining showed that expression of GAP-43 was higher in the rehabilitative training group than that in the control group(P<0.05)and the sham operated group(P<0.01)at the 7th and 21 st days post-operation,respectively,and that the expression of SYN was higher in the rehabilitative training group than that in the control(P<0.05)and the sham operated groups(P<0.05)at the 21 st and the 35th days post-operation,respectively. Conclusion Rehabilitative training can improve func tional recovery in rats with local cerebral infarction,and the function enhancement may be partially attributed to the up-regulation of expression of GAP-43 and SYN in peri-infarction cortex.
7.Analysis on molecular typing and clinical characteristics of Staphylococcus aureus in ICUs and general wards

Objective To conduct the molecular epidemiologic analysis of Staphylococcus aureus (S.aureus) in the intensive care units(ICUs) and general wards and to compare their clinical characteristics.Methods Ninety-six clinically isolated strains of S.aureus(43 strains from the emergency intensive care unit(EICU) and neurosurgical intensive care unit(NICU) and 53 strains from the general wards) collected from Sepetember 2015 to April 2016 were performed the bacterial identification and antibiotic susceptibility test.The molecular typing was performed by adopting staphylococcal protein A (spa) typing method.Results Among 96 strains of S.aureus,the detection rate of methicillin-resistant S.aureus(MRSA) was 40.6%(39/96),which among 43 strains in ICU was 62.8%(27/43) and which among 53 strains in the general words was 22.6%(12/53).The resistance rates of strains from ICUs to gentamicin,levofloxacin,clindamycin,fosfomycin and minocycline were 23.3%,48.8%,46.5%,32.6% and 32.5% respectively,while which from the general wards were 7.5%,24.5%,18.9%,2.1% and 0% respectively.The Spa typing results showed that the main types of ICUs were t002,t091 and t311.The major epidemic strain was t002(n=16,37.2%) and mainly isolated from EICUs(12 strains),26 spa types were identified among the general wards trains,mainly were t189,t377,t571,t034,t091,t127.Conclusion The detection rate of MRSA in ICUs is higher than that in the general wards,these strains have high resistant rate to routine antibacterial drugs.t002 is the major epidemic strain.The general wards have more spa types with higher genetic diversity.
8.Clinical value of preoperative mark for the submucosal tumor originating from the muscluaris prop- ria around the cardia in submucosal tunnel

Objective To evaluate the clinical value of preoperative mark with methylene blue for the submucosal tumor originating from the muscluaris propria around the cardia in submucosal tunnel.Meth-ods A total of 27 patients with cardiac tumors originating from muscularis propria diagnosed by endoscopy and endoscopic ultrasonography underwent endoscopic submucosal tunnel dissection from June 2011 to May 2014.Eighteen cases were marked by methylene blue,and 9 others were not.The operation time and the in-cidence of complications were compared between the two groups.Results All lesions were resected success-fully.The time of lesion location of non-mark group was 14.7 minutes(9-32 min),and that of mark group was 8.1 minutes(7-10 min).The incidence of subcutaneous emphysema of thorax and cervix of non-mark group was 2 /9(2 cases),and that of the mark group was 1 /18(1 case).The incidence of pneumoperitone-um of non-mark group was 1 /9(1 case),while that of the mark group was 2 /18(2 cases).There was no pneumothorax or mediastinal emphesema in all cases.Conclusion Marking with methylene blue before op-eration can shorten operation time effectively and lower incidence of complications.
9.In vitro activity of artemether against Demodex folliculorum

Objective To assess the in vitro antimite activity of artemether against Demodex folliculorum, and to provide evidence for the use of artemether in the treatment of skin diseases caused by Demodex folliculorum infection. Methods Artemether was diluted to different concentrations(20, 10, 5 and 2.5 g/L)with peanut oil. The pH values of working solutions of artemether and peanut oil were measured. Demodex folliculorum mites were divided into several groups(32 mites in each group)to be treated with artemether(20, 10, 5 and 2.5 g/L, artemether groups) or peanut oil(control group). Results There were significant differences in the time required for killing of Demodex folliculorum among the 20?, 10?, 5?and 2.5?g/L artemether groups and control group(Median[P25-P75]:3.00[2.00-3.88]vs. 6.00[4.13- 7.25]vs. 13.00[11.63- 14.50]vs. 17.00[15.25- 20.75]vs. 34.00[23.50- 39.50]hours, H=133.954, P<0.001). Additionally, the time required for killing of Demodex folliculorum was significantly shorter in these artemether groups than in the control group(all P<0.001), and was gradually shortened with the increase of artemether concentrations, but was similar between the 10? and 20?g/L artemether groups(P > 0.05). Moreover, the pH values of working solutions of artemether and peanut oil ranged between 7.0 and 7.1, and were close to neutral. Conclusion Artemether at 20, 10, 5 and 2.5 g/L can kill Demodex folliculorum in vitro, so artemether may serve as an alternative drug for the treatment of Demodex folliculorum infection.
10.The immunoregulatory effect of CD8+CD28-T lymphocytes in an experimental bone marrow mesenchymal stem cell treatment of autoimmune encephalomyelitis

Objective To study the immune system regulatory effects of CD8+CD28-regulatory T lymphocytes in an experimental bone marrow mesenchymal stem cell treatment of autoimmune encephalomyelitis.Methods A model of autoimmune encephalomyelitis (EAE) was established in twenty-eight C57BL/6 female mice,6 to 8 weeks old weighing 16 to 20 g using myelinoligodendrocyte glycoprotein 35-55 amino acid peptide (MOG35-55).The mice were randomly divided into a phosphate-buffered solution (PBS) group (n =7),an MSCs-Low group [n =7 which received an injection of 2× 105 mesenchymal stem cells (MSCs)],an MSCs-Med group (n=7,1× 106 MSCs),and an MSCs-High group (n=7,5×106 MSCs).Their clinical condition was then evaluated daily.On the 15th day after the MOG35-55 immunization,the different MSC doses were administered intravenously via the tail.On the 30th day the mice were sacrificed to observe any neuropathology of the spinal cord.At the same time,FACS flow cytometry was used to assay CD8+CD28-T cells in the spleen.Results Compared with the PBS control group,the MSC treatment effectively alleviated illness among the mice by the 15th day after the immunization.The maximum and average disease scores and clinical illness scores had all improved significantly.The medium dosage worked best.Neuropathological analysis showed that the MSC treatment could significantly reduce the invasion of inflammatory cells and minimize demyelination in the spinal cord.Furthermore,the CD8+ CD28-regulatory T cells in the spleens of the MSCtreated animals increased compared with the PBS control group,though the secreted levels of IL-10 showed no obvious difference.Conclusions Treatment with MCSs can promote the recovery of neural function in autoimmune encephalomyelitis,at least in mice.CD8+CD28-regulatory T cells may not be the main effector cells,playing only a synergistic therapeutic role.
