3.Practice in reorienting a second-tier hospital towards community health service

In light of the objective circumstances and its subjective conditions, our hospital, a grade A, second tier urban general hospital, made the right choice of reorienting itself towards community health service after making a careful assessment of the current situation. In the course of the reorientation, our hospital mainly focused on such tasks as change of concepts, readjustment of its structure and functions, establishment of the service mode of the family physician, delivery of the "six in one" service in the form of a team, and implementation of standard management. Such efforts proved to be fruitful and won the approval of the community. On the basis of this, our hospital tried hard to exploit to the full its favorable conditions as a second tier hospital in providing community health service and called for attention and solution to such issues as the pricing system, the fundraising and compensation mechanism, cost accounting, and reform of the personnel system in the delivery of community health service.
4.Analysis of anti-nuclear antibody and anti-nuclear antibody spectrum in 2 325 patients

Objective To investigate the clinical value of anti-nuclear antibody(ANA) and anti-nuclear antibody spectrum(ANAs) detection.Methods A total of 2 325 patients with or suspected with autoimmune diseases(AID) were enrolled and detected for ANA and ANAs by using indirect immunofluorescence assay(IIF) and linear immunoblot assay(LIA) respectively.All detected results were analyzed.Results Among 2 325 patients,896 cases(38.54%) were positive with ANA,with positive rate of 45.46% in female patients,which was higher than the 18.46% of male patients(P<0.05),and the common fluorescence patterns were nuclear particle pattern,nuclear homogeneous pattern and the nucleolus pattern.816 cases(35.10%) were positive with ANAs,and the positive rates of anti-Sjogren's syndrome(SS)-B antibody,anti Ro-52 antibody and anti SS-A antibody were relatively higher.The consistency rate of the two methods was 91.66%.Conclusion ANA and ANAs detection could be with certain correlation,but might be not completely consistent,detection could improve the detection rate and reduce the missed detection rate.

Immune response is one of the main reasons causing neurological deficits in the patients with cere -brovascular diseases , which activates microglia , induces inflammatory reaction and finally results in serious neuronal and endothelial injury .MicroRNAs take part in the regulation of immunoreaction , and simultaneously regulates many target genes and induces faster post-transcriptional regulation to its target genes compared with the traditional transcriptional regu -lation.For providing a basis for the clinical use of microRNAs and applying new therapy , this review mainly focuses on the function and mechanism of microRNAs in the regulation of the immunoreaction caused by cerebrovascular diseases .

8.DIFFERENTIATION OF BONE MARROW MESENCHYMAL STEM CELLS TO CARDIOMYOCYTES IN VITRO

Objective To establish a new cell resource of stem cell transplantation for heart failure therapy,by using cardiomyocytes from rat marrow mesenchymal stem cells(MSCs). Methods MSCs were isolated and cultured from rat bone marrow,then induced by 5aza(5,10,15,20 ?mol/L) for 24 hours.The cultured cells were observed with phase-contrast microscopy.The immunohistochemical technique and laser scanning confocal microscopy(LSCM) were applied for revealing the expressions of desmin,?-sarcomeric actin and C-TnT.The induced cells were evaluated by a transmission electron microscope.GATA4 and(?-MHC) expressions were detected by relative quantitative RT-PCR after 7,21,28 days of inducement respectively. Results MSCs induced in vitro by 5-aza could be identified by the positive staining for desmin,?-sarcomeric actin and C-TnT.The number of cells stained positively in the 5 ?mol/L was greater than that of other groups.Transmission electron microscope showed that paralleled myofilaments were formed.RT-PCR assessment showed that the differentiated cells began to express GATA4 from day 7 to day 28 of differentiation and began to express ?MHC from day 21 to day 28 of differentiation.Conclusion Cardiomyocyte-like cells can be generated from MSCs in vitro and the marrow mesenchymal stem cells are one of the best resources of donor cardiomyocytes.
