1.Pedicle screw fixation combined with implantation of artificial bone composite for thoracolumbar fractures:vertebral height and Cobb angle

BACKGROUND:The traditional treatment for thoracolumbar vertebral fractures is posterior short-segment pedicle screw fixation. This method uses vertebral soft tissue stretch through distraction reduction mechanism, makes shift bone reset, and restores shape and height of vertebral body. Long-term folow-up found that part of cases experience complications such as vertebral height loss, loss of spinal correction of Cobb angle, the pedicle screw breakage and loosening. OBJECTIVE:To explore clinical effects of pedicle screw fixation combined with the implantation of artificial bone composite for the treatment of thoracolumbar fractures, and to compare with conventional pedicle screw. METHODS: A total of 80 patients with thoracolumbar fractures, who were treated in the Yantaishan Hospital from March 2011 to May 2014, were randomly divided into two groups. In the experimental group, patients received pedicle screw fixation combined with the implantation of artificial bone composite. In the control group, patients received routine pedicle fixation. Pain visual analog scale score was evaluated before treatment and at 6 months after treatment in patients of both groups, and easement of pain was evaluated after treatment. Vertebral body height (normal anterior flange height and up and down the front cone height and the ratio of the average multiplied by 100%) and sagittal Cobb angle (sagittal measurements of X-ray film) were measured in patients of both groups before treatment and at 1 and 12 months after treatment. According to conventional hierarchical evaluation criteria used al over the world, prognosis was assessed at 6 months after treatment, including basic cure, powerfuly, effectively, and invalid. RESULTS AND CONCLUSION: At 6 months after treatment, significant differences in pain visual analog scale scores were detected in the experimental group (2.4 points) and control group (3.7 points) (P < 0.05). No significant difference in height of the injured vertebra and Cobb angle was detected between the two groups before treatment (P> 0.05). The height of the injured vertebra and Cobb angle were better in the experimental group compared with the control group at 1 and 12 months after treatment (P < 0.05). Significant differences in treatment efficiency were detectable between the experimental group (90%) and the control group (73%) at 6 months after treatment (P < 0.05). These data indicate that pedicle screw fixation combined with the implantation of artificial bone composite in the treatment of thoracolumbar fractures can be used to fix fracture reduction, effectively restore height of injured vertebral body, prevent postoperative Cobb angle loss, and improve the treatment efficiency.
2.Enhancement of NGF-induced neuronal differentiation of PC12 cells by co-culturing with primary neurons and glia cells

Objective To establish an in vitro cell co-culture model that simulates the transplantation environment in vivo ,and to investigate the effect of primary cells from neonatal cortex on the differentiation of PC12 cells. Methods PC12 cells were transfected with pEGFP-N1 vector to express green fluorescent protein (GFP) stably,then these cells were transplanted into primary neurons and glial cells from neonatal cortex. Cultures with PC12 cells alone constituted the control group. Nerve growth factor (NGF) was added into all the other cultures to induce neuronal differentiation of the PC12 cells. Neurite outgrowth of the neuronal PC12 cells was observed with laser confocal microscopy. In this study, in vitro chemosensitivity testing was conducted on primary cultured breast cancer cells from 96 patients with breast cancer, and the results showed that the cells from a few patients with primary breast cancer developed multidrug resistance (MDR) prior to the first chemotherapy exposure. All the cells from the recurrent cancer patients had MDR. The findings suggested that patients having MDR would benefit from high-dose chemotherapy (HDC) regimens. In vitro chemosensitivity screening, which was aimed at improving the therapeutic efficacy and minimizing side effects, helps in choosing individualized treatment for breast cancer.

4.Prevalence of extended-spectrum beta-lactamases-producing Escherichia coli and Klebsiella pneumoniae in community-onset bloodstream infections from county hospitals of Zhejiang Province

Objective To study the epidemiology and genotypes of extended-spectrum beta-lactamases (ESBL)-producing Escherichia coli (EC) and Klebsiella pneumoniae (KP) that caused community-onset bloodstream infections (COBSI) in 9 county hospitals of Zhejiang Province.Methods This is a multi-center, prospective, observational study.The cases and isolates with COBSI caused by EC and KP were consecutively collected from 9 county hospitals in Zhejiang Province between 1st March 2014 and 30th April 2015.The double disk diffusion method was used to confirm the production of ESBL.The ESBL genotypes were determined by polymerase chain reaction(PCR) amplification and sequence analysis.Multi-locus Sequence Typing (MLST) was used to analyze the homology of ESBL-producing isolates.Minimal inhibitory concentration (MIC) of frequently used drugs for ESBL-producing isolates was determined by in-vitro antimicrobial susceptibility tests.Results During the study period, a total of 172 cases with COBSI were collected and 171 cases were eligible, among which 126 were caused by EC and 45 were caused by KP.The overall prevalence of ESBL was 34.5% (59/171),and the prevalence of ESBL-EC and-KP was 41.3% (52/126) and 15.6% (7/45), respectively.CTX-M-type ESBL accounted for 96.6% (57/59) of all the ESBLs-producing isolates, and the most common type was CTX-M-14 (27.1%, 16/59), followed by CTX-M-55 (22%, 13/59).MLST analyses revealed significant genetic diversity among ESBL-EC and-KP.The most prevalent ST of ESBL-EC was ST131 (23.1%).In addition to carbapenems, cefoperazone/sulbactam, piperacillin/tazobactam, moxalactam, amikacin and fosfomycin also showed good in-vitro activity against ESBL-EC and-KP.Conclusions The prevalence of ESBL in EC and KP is high in 9 county hospitals of Zhejiang Province, and the most common genotypes are CTX-M-14 and CTX-M-55.The detection rate of ESBL in EC is higher than in KP.It could be considered adequate empirical therapy according to the results of antimicrobial susceptibility tests.Carbapenems, cefoperazone/sulbactam, piperacillin/tazobactam, moxalactam, amikacin and fosfomycin have good in-vitro activity against ESBL-EC and-KP.
5.Expression of limulus Factor C in silkworm larvae by Bac-to-Bac/BmNPV baculovirus expression system.

Limulus Factor C, a serine protease zymogen from the amoebocytes of the limulus, has high affinity for endotoxin. When Factor C is activated by endotoxin, it hydrolyses artificial tripeptide substrate and measurable products are released, so it can be used as an alternative reagent for endotoxin analysis. Factor C gene of Tachypleus tridentatus was obtained through RT-PCR and the recombinant protein was expressed by Bac-to-Bac/BmNPV baculovirus expression system in silkworm larvae. The activity of Factor C was detected with diluted serum of silkworm larvae, and the sensitivity of endotoxin detected was 0.2 EU/mL when the serum was diluted at 1:500. The silkworm larvae expressed limulus Factor C could be used to develop a new low-cost endotoxin test reagent.

6.Ultrasonic observation of correlation between coronary heart disease and common carotid artery and femoral artery atherosclerosis

Objective To assess the correlation between coronary heart disease and common carotid artery and femoral artery atherosclerosis with two-dimensional color Doppler ultrasound (2D-CDUS). Methods Ninety patients with coronary heart disease were divided into three subgroups according to the extent of coronary artery stenosis observed with coronary angiography, and 50 normal subjects were taken as normal group. Ultrasound examination with high-frequency linear array probe was performed to measure IMT of common carotid and femoral artery in order to observe the plaque and extent of stenosis, the formation of blood dynamics, physical characteristics and hemodynamic environmental parameters. Results Among the coronary disease groups, common carotid artery and the femoral artery IMT, atherosclerotic plaque in the number, rate and all points increased along with the degree of coronary artery stenosis increasing. Physical characteristics and its hemodynamic environmental parameters had some changes with the increase of atherosclerosis. Conclusion The atherosclerosis of common carotid artery and the femoral artery are closely related with coronary atherosclerosis.
7.Influence of specific acupoints moxibustion of Yizhichan on quality of sleep, ability to reaction of patinets with chronic fatigue syndrome

Objective To study the effects of Yizhichan moxibustion on chronic fatigue syndrome (CFS) athletes.Methods 143 patients of CFS were blind randomly devided into3 groups, which were moxibustion of Yizhichan (56 cases) group, general moxibustion group (42 cases), and normal group (45 cases).The normal group was treated by passive massage for relaxation, the moxibustion of Yizhichan group was treated by specific acupoints application of moxibustion of Yizhichan in addition to passive massage;and the general moxibustion groupwas treated by general moxibustion in addition to passive massage.All sample were treated once a day for 6 days in a week.3 weeks was a period of treatment, and the total time of treatment was 15 weeks.Results After treatment, the sleep quality (1.73 ± 0.73, 1.54 ± 0.85 vs.0.83 ± 0.52, F=14.130), time for sleep (1.57 ± 0.60, 1.35 ± 0.60 vs.1.03 ± 0.54, F=5.386), sleep time (1.78 ± 0.71, 1.77 ± 0.61 vs.0.67 ± 0.65, F=19.811), daytime dysfunction (0.78 ± 0.83, 1.64 ± 0.83 vs.1.03 ± 0.83, F=4.798) in the control group, moxibustion of Yizhichan group and a general moxibustion group were statistically significant(P<0.05).The total effective rate of the moxibustion of Yizhichan group was better than the control group and the general moxibustion group (83.8%, 42.2% vs.63.6%;x2=13.973, P=0.001).Conclusion Specific acupoints moxibustion of Yizhichan is a beneficial exploration of moxibustion methods of traditional Chinese medicine, and can improve CFS athletes level of physical health, attention and sleep quality.
8.Impact of peptide binding domain of heat shock protein 72 on epithelial to mesenchymal transition

Objective To investigate the effects of peptide-binding domain (PBD) of heat shock protein (HSP) 72 on epithelial to mesenchymal transition (EMT) in rat renal tubular epithelial cells.Methods The expressions of wild-type HSP72,mutant of HSP72 lacking peptide binding domain (HSP72-△PBD) and HSP72-PBD were induced by plasmid transfection.NRK-52E ceils were stimulated by TGF-β1 for 48 h.The expressions of α-smooth muscle actin (α-SMA),E-cadherin,HSP72 and Smad3/p-Smad3 were detected by Western blot and immunofluorescence.Results After NRK-52E cells were stimulated by TGF-β 1 (10 μg/L) for 48 h,the expression of α-SMA was increased and the protein level of E-cadherin was decreased.Western blotting and immunofluorescence showed that over-expression of both HSP72 and PBD inhibited TGF-β1-induced up-regulation of protein α-SMA expression,down-regulation of protein E-cadherin.However,overexpression of HSP72-△PBD did not change the protein level of E-cadherin and α-SMA.In addition,over-expression of HSP72 and PBD significantly inhibited the phosphorylation of Smad3.Conclusion Inhibition of Smad3 activation and EMT by HSP72 is associated with the function of PBD.
9.Common complications of arm port and chest port:a meta-analysis

Objective To analyze common complications of arm port and chest port using meta-analysis and to investigate the safety of ann port.Methods Randomized controlled trials,cohort studies and well-designed casecontrol trials published in CNKI,Wanfang,VIP,PubMed,CINAHL and Embase regarding complication comparison between arm port and chest port were searched.Review Manager 5.3 software was applied to analyze outcome indicators.Results Nine retrospective cohort studies and 2511 patients were included.The results of meta-analysis showed that the incidence of thrombosis in ann port group was lower than that in chest port group,and the difference was statistically significant [0R=0.51,95%CI (0.28,0.92),P=0.03].However,there was no significant difference in incidence of infection[OR=0.85,95%CI(0.53,1.38),P=0.52],incidence of obstruction[OR=1.05,95%CI(0.37,3.01),P=0.92],incidence of sepsis [OR =0.44,95% CI (0.10,1.87),P=0.27],incidence of skin incision [OR =0.28,95% CI (0.07,1.17),P=0.08].Conclusion The incidence of thrombosis in ann port was lower than that in chest port,and there was no obvious advantage in the incidence of infection,obstruction,sepsis and skin incision.
10.Indoor air quality monitoring and management in primary and secondary schools in China

Objective:
To investigate indoor air quality monitoring and management in primary and secondary schools, so as to provide scientific basis for health protection of students and healthy school environment.
Methods:
Stratified sampling method was adopted to select schools for investigation. Data of daily ventilation and ventilation was collected from relevant principals of schools through questionnaire survey. Chi-square test was used to analyze the difference of ventilation and ventilation among different types of schools.
Results:
Daily indoor air quality testing indicators: 317 schools (13.2%) have tested the concentration of CO2 in the air environment. Daily detection of CO2 in urban schools was significantly better than that in township schools, and the difference is statistically significant(χ2=72.06, P<0.01); Non-boarding schools were superior than boarding schools(χ2=21.89, P<0.01). The proportion of schools that routinely tested for carbon monoxide, particulate matter and volatile pollutants was 6.5%, 7.5% and 9.3%, respectively. Of the schools that participated in the survey, 80.8% had a daily ventilation system. Among them, 925 schools (38.5%) had a cumulative daily ventilation time of more than 90 minutes in cold season, and 331 schools (13.8%) had a daily ventilation time of less than 30 minutes.
Conclusion
Regulations and standards for school air quality monitoring needs to be improved. The Center for Disease Control and Prevention or other qualified institutions are suggested to lead air quality monitoring in schools testing, creating a healthy learning and living environment for primary and secondary school students.
