1.Induction chemotherapy plus concurrent chemoradiotherapy (IMRT) in the treatment of locally advanced nasopharyngeal carcinoma

Objective To investigate the efficacy and side effect of induction chemotherapy plus concurrent chemoradiotherapy ( IMRT) in the treatment of 81 patients with locally advanced nasopharyngeal carcinoma ( NPC) . Methods 81 patients with locally advanced NPC were divided into the vinorelbine group and fluorouracil group . Patients in the vinorelbine group were given vinorelbine 25-30mg/m2 d1,d8,DDP 75mg/m2 ,d1-d3.Patients in the fluorouracil group were given DDP 75mg/m2,d1-d3,fluorouracil 750 mg/m2 d1-d5.The treatment schedules were recycled every 3 weeks.After 2-4 cycles, the patients received concurrent chemoradiotherapy .In the vinorelbine group,IMRT with NVB 25-30mg/m2,DDP 40mg d1,d8,d22,d29,d43,d51 from the first day of IMRT.In the fluorou-racil group,IMRT with fluorouracil 750mg/m2 ,DDP 25/m2 d1,d8,d22,d29,d43,d51 from the first day of IMRT. Results The overall leukopenia and thrombocytopenia decline was 47.5% vs 24.4% in patients with Ⅲ and Ⅳgrade(χ2 =4.73,P<0.05).5-year locoregional relapse-free survival rates were 85.0% vs 65.9%(χ2 =4.05,P<0.05).5-year overall survival rates were 85.0%vs 68.3%(χ2 =3.18,P<0.05).Conclusion NP regiment induc-tion chemotherapy plus concurrent chemoradiotherapy for advanced NPC can achieve better result in clinical response and 5-year locoregional relapse-free survival rate compared with FP and the effect is clinically acceptable .
2.Vitamin E effects on the malondialdehyde concentration and superoxide dismutase in skeletal muscle mitochondria following eccentric exercise in rats

BACKGROUND:It has been shown to exhibit the increase of free radical has relationship with exercise-induced skeletal muscle injury.As a kind of antioxidant,vitamin E has the ability of removing radicals,which can relieve antioxidase damage,and delay fatigue,eventually,improve the motor ability.OBJECTIVE:From the angle of free radical metabolism,to explore the effect of vitamin E on malondialdehyde(MDA)and superoxide dismutase(SOD)of mitochondria in skeletal muscular cell after eccentric exercise,additionally,to clarify the mechanism of vitamin E on reducing the skeletal muscle damage.DESIGN,TIME AND SETTING:The randomized grouping experiment of animal was carried out in the Key Laboratory of General Administration of Sport of China in Shenyang Sport University and the Key Laboratory of China Medical University from May to October in 2007.MATERIALS:A total of 48 male,Sprague Dawley rats,weighing(304?12)g were randomly divided into the of control,sport,physiological saline,and vitamin E groups,with 12 animals in each group.METHODS:Rats in the vitamin E group was administered(i.p.)1-1.2 mg/kg vitamin E capsules once per eight hours,four times one day before experiment,and the amount of injecting volume was 4 mL/kg.The rats in physiological saline were injected by physiological saline,with the same injection methods of the vitamin E group.The models of injury were established by a bout of exhaustive eccentric exercise on a treadmill.After exercise,triceps brachiies were moved from the right side of rats.MAIN OUTCOME MEASURES:The content of MDA and activity of SOD were measured with the reagent-boxes and 6010 UV-seen spectrophotometer.RESULTS:The values of MDA,SOD of mitochondria in triceps brachiies cells in each group were greatly increased after 24 hour eccentric exercise(P 0.05),the value of MDA in the vitamin E group was greatly decreased(P
3.Effect of Fat Emulsion on Blood Lipid and Liver Function in Criticaly Ⅲ Premature Infants

Objective To observe the effect of different doses of fat emulsion on blood lipid and liver function in critically ill premature infants. Methods 40 babies were randomly divided into 4 groups,receiving fat emulsion 0,1,2,3 g?kg~(-1)?d~(-1) for 5 days respectively. The blood levels of cholesterol and triglyceride and the serum biochemical indexes relative to liver functions were measured before and after the period of 5 days. Results Blood lipid and serum ?-GT levels did not change remarkably in 1 or 2 g?kg~(-1)?d~(-1) group, but increased significantly in 3 g?kg~(-1)?d~(-1) group. Serum bilirubin levels decreased in all groups, but it was significantly higher in the group of 3 g?kg~(-1) ?d~(-1) than the others. Conclusion It is suggested that the blood lipid level would increase with 3g?kg~(-1)?d~(-1) of fat emulsion used in critically ill premature infants and that the use of fat emulsion would retard the falling of serum bilirubin level when used in newborn infants with byperbilirubinemia.
4.Experience of treating varicosed great saphnous vein by endovenous laser treatment plus punctiform stripping

Objective To evaluate the efficacy of endovenous laser treatment(EVLT) plus punctiform stripping therapy for lower extremity varicose veins.Methods Sixty-eight limbs in fifty patients with varicosed vein underwent EVLT plus punctiform stripping therapy in department of vascular surgery of the first affiliated hospital of Anhui medical university.Results All patients were followed for 2~27 months and all the patients had successful occlusions of varicosed veins with no recurrence were observed.Local skin paresthesia were found in 2 patients.No local recurrence and deep vein thrombosis were found.Conclusion EVLT plus punctiform stripping therapy is a simple and effective treatment for lower extremity varicosed veins with fewer complications and recurrences.
5.EFFECT OF DIETARY FIBER AND PHYTIC ACID ON 1,2-DIMETHYLHYDRAZINE INDUCED COLORECTAL CARCINOGENESIS

Objective: To evaluate the effect of dietary fiber and phytic acid inositol hexaphosphate (IP6 or Insp6) on 1,2-dimethylhydrazine(DMH) induced colorectal carcinogenesis in Wistar rats. Methods: 86 four-week-old male Wistar rats were divided randomly into 6 groups and fed with either basal fiber free diet or the basal diet supplemented with 10% pectin, 10% cellulose, 2% Na-InsP6 supplemented water, 10% pectin in combination with 2% Na-InsP6 supplemented water, 10% cellulose in combination with 2% Na-InsP6 supplemented water. After four weeks, the rats were given a weekly injection of DMH (20mg/kg bw) for 20 weeks. The rats were killed after 26 weeks. The number of total macroscopically visible neoplasms was counted and the volume of individual neoplasmas was calculated. Proliferation cell nuclear antigen (PCNA) was analyzed. Results: There was no significant difference between the treatment group and the control group in the incidence of tumors. InsP6 significantly reduced the number of tumors in rats and tumor volume. Pectin and Pectin+InsP6 significantly increased the number of tumors in rats. The expression of proliferation marker PCNA was significantly down-regulated by InsP6 and significantly increased by pectin. Conclusion: A treatment regimen of 2% Na-InsP6 in drinking water was effective in significantly reducing the risk of colorectal cancer in Wistar rats. Diet supplemented with pectin could increase the risk of colorectal cancer.
6.Construction and identification of shRNA lentiviral vector targeting TPX2 gene

Objective To construct the lentiviral RNA interference vector targeting TPX2 and to obtain the human cervical cancer HeLa cell strain stably infected by TPX2-shRNA for studying the relationship between human cervical carcinoma and TPX2 gene.Methods By targeting TPX2 gene,four double-stranded DNA hairpin structures corresponding to shRNA were designed,synthesized and connected with Pglv2-U6-Puro to construct the recombinant plasmids.Then these recombinant plasmids were transformed into DH5α competent cells.The positive clone was extracted and transfected into 293T cells for virus packages after sequenced correctly.Human cervical carcinoma HeLa cell infected by these recombinant lentiviral was screened by Puromycin,then stable cell strain was obtained.The silencing effect of TPX2 in HeLa cell was detected by RT-fluorescent quantitative PCR and Western blot.Cell cycle and cell apoptosis wer detected by Flow cytometry.Results Sequencing results confirmed that 5 lentiviral was packaged successfully.The steady cell strain transfered TPX2-shRNA was screened with 0.4 μg/mL puromycin.HeLa cells infected by recombinant lentivirus all play the gene silencing effect especially in the group of TPX2-shRNA-1.In the group of TPX2-shRNA-1,TPX2mRNA (0.21 ± 0.07) and protein (0.19 ± 0.28) rela tive expression level is lower than those in the control group (1.08±0.07) (P<0.01) and(0.64±0.03) (P< 0.01)respectively;G2 and S-phase cells are higher than those in the control group (P<0.05)and the apoptosis rate was significantly more than those in the control group (P<0.05).Conclusions The effective TPX2 genetic interference sequence was obtained,lentiviral vectors carrying TPX2shRNA was successfully constructed,and the HeLa cell strain with TPX2 silenced was successfully screened,which lay the research foundation for the study of the role of TPX2 in cervical cancer.
7.Metabolize characteristic and effects of cephalosporins and fluoroquinolones in ACST in human bile

Objective To investigate the regulation of metabolize characteristic of some antibiotics in ACST in human bile, and provide the theoretic basis for physician selecting antibiotic rationally when biliary tract has been infected. Methods Samples of ACST in human bile were obtained by ERCP + ENBD, contents of Cefradine, Cefoperazone, Ceftriaxone, Ciprofloxacin and Lomefloxacin in human bile were simultaneously measured by HPLC method. Results After antibiotics were given through iv 0. 25 ~ 1. 5 hour, an increase was observed in contents of all antibiotics in bile, and the changes of Ceftriaxone and Ciprofloxa-cin were the most significant ( P <0.01). Ceftriaxone and Ciprofloxacin had a higher Cmax than other antibiotics, and Cefoperazone had a longer tl/2. Conclusion As maintaining a little time to tmax , a higher Cmax and a longer tl/2 in the bile by iv, Cefradine and Ciprofloxacin are recommended in the patient with ACST.
8.Effects of tempol administered via different routes on neuropathic pain in rats

Objective To evaluate the effects of tempol administered via different routes on neuropathic pain (NP) in rats.Methods Thirty-two male Sprague-Dawley rats,weighing 250-280 g,aged 8-10 weeks,were randomly divided into 4 groups (n=8 each) using a random number table:sham operation group (group S),group NP,intrathecal tempol group (group T1),and intraperitoneal tempol group (group T2).Neuropathic pain was induced by chronic constriction injury in chloral hydrate-anesthetized rats.The sciatic nerve was exposed and 4 loose ligatures were placed on the sciatic nerve at 1 mm intervals with 4-0 silk thread.The sciatic nerve was only exposed but not ligated in group S.After successful establishment of the model,a catheter was inserted at L4.5 interspace into the epidural space.In S and NP groups,0.9% normal saline 20 μl was injected intrathecally,and 0.9% normal saline 200 μl was injected intraperitoneally once a day for 7 consecutive days.In group T1,tempol 30 μg (in 20 μl of normal saline) was injected intrathecally,and 0.9% normal saline 200 μl was injected intraperitoneally once a day for 7 consecutive days.In group T2,tempol 30 μg (in 200 μl of normal saline) was injected intraperitoneally,and 0.9% normal saline 20 μl was injected intrathecally once a day for 7 consecutive days.The mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured at 3 days before operation,and at 1,3,5,7,10 and 14 days after operation.The animals were sacrificed after measurement of pain threshold at day 14 after operation.The lumbar segment of the spinal cord was removed to detect malondialdehyde (MDA) content and superoxide dismutase (SOD) activity by enzyme-linked immunosorbent assay.Results Compared with group S,the MWT was significantly decreased,and the TWL was shortened at each time point after operation,the content of MDA in the spinal cord was increased (P<0.05),and no significant difference was detected in SOD activity in group NP (P>0.05).Compared with group NP,the MWT was significantly increased at 5,7,10 and 14 days after operation,the TWL was prolonged at 1,3,5,7,10 and 14 days after operation,the content of MDA in the spinal cord was decreased,and the SOD activity was increased in group T1 (P<0.05),and no significant change was found in the indexes mentioned above in group T2 (P>0.05).Conclusion Intrathecal tempol can reduce NP in rats,and the mechanism is related to inhibition of lipid peroxidation in the spinal cord.
9.Effect of dexmedetomidine on BDNF expression during lidocaine-induced spinal neurotoxicity in rats

Objective To evaluate the effect of dexmedetomidine on the expression of brain-derived neurotrophic factor (BDNF) during lidocaine-induced spinal neurotoxicity in rats.Methods Fifty-six male Sprague-Dawley rats,weighing 250-280 g,aged 2-3 months,were divided into 7 groups (n =8 each) using a random number table:sham operation group (group S),lidocaine group (group L),normal saline group (group NS),3 different doses of dexmedetomidine groups (D1,D2 and D3 groups),and oα2-adrenoceptor antagonist yohimbine group (group Y).An epidural catheter was placed at L5.6 interspace.Ten percent lidocaine 20 μl was injected intrathecally in all groups except group S.Dexmedetomidine 5,15 and 25 μg/kg were injected intraperitoneally at 30 min before lidocaine injection in D1,D2 and D3 groups,respectively.In group Y,yohimbine 1.0 mg/kg was injected intraperitoneally at 15 min before dexmedetomidine 25 μg/kg was injected.Before intrathecal administration and at 24,48 and 72 h after intrathecal administration (T1-4),the Basso,Beattie,Bresnahan (BBB) Locomotor Rating Scale was used,and the tail flick latency to a thermal nociceptive stimulus (TFL) was measured to assess the locomotor function.The rats were sacrificed after the last behavioral test,and the lumbar segments (L3-5) of the spinal cord were removed for pathological examination and for determination of BDNF expression and cell apoptosis.The apoptosis index was calculated.Results Compared with group S,the BBB score was significantly decreased at T2-4,the TFL was prolonged,the BDNF expression was up-regulated,and apoptosis index was increased in L,NS,D1,D2 and D3 groups (P<0.05).Compared with group L,the BBB score was significantly increased at T2-4,the TFL was shortened,the BDNF expression was up-regulated,and apoptosis index was decreased in group D3 (P<0.05),and no significant change was found in the parameters mentioned above in NS,D1,D2 and Y groups (P>0.05).Compared with group D3,the BBB score was significantly decreased at T2 4,the TFL was prolonged,the BDNF expression was down-regulated,and apoptosis index was increased in group Y (P<0.05).The pathological changes of the spinal cord were significantly attenuated in group D3 as compared with group L,and there was no significant difference in pathological changes of the spinal cord between group Y and group L.Conclusion The mechanism by which dexmedetomidine reduces lidocaine-induced spinal neurotoxicity may be related to up-regulation of BDNF expression,and the mechanism by which dexmedetomidine up-regulates BDNF expression is completely related to activation of α2-adrenoceptors in rats.
10.In vitro chemosensitivity testing of primary and recurrent breast carcinomas and its clinical significance.

In this study, in vitro chemosensitivity testing was conducted on primary cultured breast cancer cells from 96 patients with breast cancer, and the results showed that the cells from a few patients with primary breast cancer developed multidrug resistance (MDR) prior to the first chemotherapy exposure. All the cells from the recurrent cancer patients had MDR. The findings suggested that patients having MDR would benefit from high-dose chemotherapy (HDC) regimens. In vitro chemosensitivity screening, which was aimed at improving the therapeutic efficacy and minimizing side effects, helps in choosing individualized treatment for breast cancer.
