1.Comparison of different methods for detecting mycoplasma pneumoniae

Objective To investigate the sensitivity and specificity of different methods for the detection of mycoplasma pneu‐moniae .Methods The serum and throat swab specimens were collected from 199 pneumonia patients .ELISA and real‐time fluores‐cence quantitative polymerase chain reaction (FQ‐PCR) assays were used to detect the serum mycoplasma pneumoniae IgM antibod‐y and DNA .Results Among 199 pneumonia patients ,61 cases were diagnosed as mycoplasma pneumoniae pneumonia according to the diagnosis standard .The sensitivity and specificity of ELISA and FQ‐PCR for detecting mycoplasma pneumoniae were 86 .9%and 96 .7% ,and 78 .3% and 97 .1% ,respectively .Conclusion The sensitivity and specificity of FQ‐PCR for detecting mycoplasma pneumoniae are higher than those of ELISA .
2.Improving effect of tertiary rehabilitation training on upper limb motor function after cerebral hemorrhage

Objective: To study effects of tertiary rehabilitation training on upper limb motor function recovery in patients with cerebral hemorrhage.Methods: A total of 120 patients with cerebral hemorrhage, who were treated in our hospital from May 2013 to May 2015, were selected.According to random number table, they were randomly and equally divided into routine rehabilitation group and tertiary rehabilitation group, and both groups received the same routine medical treatment and nursing.Simple test for evaluating hand function scale (STEF), Fugal-Meyer motor assessment (FMA) and Brunnstrom limb motor function assessment were used to assess and compare limb recovery after rehabilitation training between two groups.Results: At the end of first month of rehabilitation training, there were no significant difference in STEF and FMA scores between two groups, P>0.05 both.On the end of second month of rehabilitation training and later, compared with routine rehabilitation group, there were significant rise in STEF score[end of second month: (3.3±0.5) scores vs.(4.2±1.1) scores, end of sixth month: (6.3±1.2) scores vs.(8.5±1.1) scores], total STEF score[(20.2±4.2) scores vs.(38.2±3.5) scores]and FMA score[end of second month: (28.4±3.6) scores vs.(33.4±2.7) scores, end of sixth month: (38.2±6.6) scores vs.(46.3±5.8) scores]in tertiary rehabilitation group, P<0.05 all.Compared with routine rehabilitation group after six-month rehabilitation training, there were significant rise in percentages of Brunnstrom stage V (23.33% vs.43.33%) and stage VI (6.67% vs.20.00%) in tertiary rehabilitation group(P<0.05 both).Conclusion: Tertiary rehabilitation training can significantly improve upper limb motor function in patients with cerebral hemorrhage, so it's worth extending.
3.Complicated Hospital Infection by Mucor in Liver Transplant Recipients:A Clinical Analysis

OBJECTIVE To investigate the incidence,symtoms and prognosis of hospital infection by Mucor after liver transplantation.METHODS The clinical data of all the patients with complicated hospital infection orthotopic by Mucor after orthotopic liver transplantation were analyzed restrospectively.RESULTS There were 9 cases with complicated hospital infection by Mucor from in 750 liver transplant recipients.Seven cases developed superficial incisional infection with delayed healing and rapid increased secretion on the surface of incisions.All of them recovered after antifungal therapy.Two cases had pulmonary Mucor infection combined with bacteria and died of multi-organ failure.CONCLUSIONS The incidence of hospital infection by Mucor in liver transplant recipients is about 1%.The most common symtoms are superficial incisional infection and the patients recover well.However,the cases with complicated pulmonary Mucor infection are difficult to treat and have a poor prognosis.
4.The Influence of Stents Selection on Curative Effect of Stents Placement in The Treatment of Benign And Malignant Esophageal Strictures

Objective To assess clinic value of esophageal stent placement in the benign and malignant esophageal strictures,and the influence of stents selection on curative effect. Method 36 cases of esophageal strictures, 5 cases of benign strictures,31cases of malignant strictures,including 4 cases with esophagotracheal fistula and 3 cases with esophagomediastrinal fistula. 26 domestic Nickel-Titanium alloy memorial netlike stents and 14 membrane-covered stents were used for the treatment. All patients were treated by placing esophageal stent perorally under fluoroscopic guidance. Results 40 stents were planced successfully in 36 cases, only one case showed membrane covered stent migrating. Symptoms of dysphagia were considerably relieved after stent insertion. The total effective rate was 97.2%. Conclusion The esophageal stent is an effective and safe treatment for benign and malignant esophageal stenosis: The nutritive condition and the quality of life of patients are improved, which is favourable for tumor further treatment. Appropriate stent selection can keep esophageal patent rate and decrease sliding rate.
5.Clinical Value of Interventional Treatment for Obstructive Jaundice

Objective To observe the clinical value and the effect of interventional treatment on obstructive jaundice. Methods 24 patients with obstructive jaundice, including 21 cases of malignant biliary obstruction and 3 cases of chololithiasis post operative biliary stricture of bile duct, were treated after percutaneous transhepatic biliary moulding, 6 cases with percutaneous transhepatic cholangic drainage(PTCD), 18 cases with insertion of biliary stents, 13 net-type stents and 5 Z-type stents were used, according to the patient's state and financial condition. Results The total technical success rate was 100%. Background and purpose: Docetaxel/cisplatin are widely used in chemo-naive patients with advanced non-small cell lung cancer(NSCLC),but the standard 3-weeks'project of docetaxel caused significant toxicity.We performed this study to compare the effect and toxicity of modified and standard 3-weeks'docetaxel/cisplatin as first line chemotherapy for advanced NSCLC.Methods:68 patients with stage ⅢB or Ⅳ NSCLC(proven by histology or cytology) were randomly divided into two groups,modified(A) and standard(B) chemotherapy.Group A: docetaxel 75 mg/m2,divided into 2 days,ivgtt d1 and d 8,cisplatin 25 mg/(m2?d),ivgtt d 1-d 3,q3w;Group B: docetaxel 75 mg/m2,ivgtt d 1,cisplatin was administered as Group A,q3w.The effect and toxicity were assessed after two cycles and one-year survival was followed up.Results:There was no CR in both groups.10 PR,20 SD,4 PD were found in group A,the overall response rate is 29%;whereas 11 PR,20 SD,3 PD were found in group B,the overall response rate is 32%.The one-year survival rate were 38% and 35% in group A and B,respectively.There were no significant difference about the overall response rate(P=0.793) and one-year survival rate(P=0.801) between group A and B.The rates of grade Ⅲ/Ⅳ neutropenia were 18% and 47% in group A and B respectively.The difference was statistically significant(P=0.010).Conclusions:In comparison with the standard 3-weeks'docetaxel/ cisplatin chemotherapy,the modified one has similar response rate but lower hematologic toxicity,and thus it was well tolerated.

MicroRNA-494 (miR-494) is one of the microRNAs from 14q32.31 miR-gene cluster.Recently, miR-494 was found to closely relate with tumors and other diseases .This article reviews the expression changes , roles and possible regulatory mechanisms of miR-494 in multiple tumors and other hypoxia/ischemia diseases .Recent studies demon-strate that the expression of miR-494 is affected by many factors , and miR-494 could be a biomarker of diagnosis , staging and prognosis in tumors and other diseases , and a target of therapy in future .
8.EXPRESSION OF c-Jun PROTEIN IN INJURED CULTURED SPINAL CORD NEURONS

Objective To establish a model of injury to primarily cultured spinal cord neurons,mimicking the neuronal injury after complete transactional spinal cord trauma,for the sake of exploring changes in expression of an immediately-early gene,c-jun,in central nervous system injury. Methods Spinal cords were removed form fetal Wistar rats at the 14th gestation day and the neurons were cultured for 10 to 12 days.Then,mechanical injury was applied to the neurons by making regular scores on the culture disk under direct vision with the aid of a self-made standard template.Morphology of the injured neurons and changes in expression of c-Jun protein were observed before and at different intervals after injury. Results c-Jun expression was noted in neuronal nuclei 10 min after injury and its peak appeared at 2 hrs.Besides,the density of positive neurons bore evidently an inverse proportion with their distance from the scores.Conclusion\ Positive expression in injury neurons show that c-jun gene enters the nucleoli of injured neurons and takes the role of “the third messenger” at early time after neuronal injury.
9.Effects of dexamethasone on expression of glucocorticoid receptor in human monocyte cell line THP-1

Objective To investigate the effects of dexamethasone on the expression of glucocorticoid receptor (GR ?, GR ?) in human monocyte cell line THP-1.Methods Cultured THP-1 cells were stimulated by dexamethasone with different concentrations and for different durations. The GR ? and GR ? protein expressions were examined by Western blotting. Results The expressions of GR ? and GR ? were observed in stimulated and unstimulated THP-1. The quantity of GR ? expression was reduced by dexamethasone treatment in a time-dependent manner. The quantity of GR ? expression was increased by dexamethasone treatment in time- and dose-dependent manner. Conclusion Dexamethas one stimulation time-dependent reduce GR ? expression in THP-1 cell. Dexamethasone stimulation time- dependent and dose-dependent increase GR ? expression in THP-I. The expressions of GR a and GR ? were regulated by glucocorticoid.
10.Rat spinal cord primary neuron injury-regeneration-related genes cloned by improved subtractive hybridization

Objective To clone injury-regeneration-related genes after primary neurons of spinal cord were injured in rat, and to elucidate the molecular mechanism of injury and regeneration in CNS. Methods An in vitro spinal cord primary neuron injury model was replicated in rat. The differentially expressed genes during injury and regeneration periods of spinal cord primary neurons were cloned by improved subtractive hybridization. Results Twenty-seven differentially expressed sequences were obtained, 23 were known genes and the others were unknown. Among these known sequences, cDNA for synuclein and clusterin, together with several other gene sequences, were closely related with nerve degenerative diseases. Conclusion Primary neuron injury-regeneration-related genes of the spinal cord can be successfully cloned by improved subtractive hybridization.
