1.Maspin gene and the invasion and metastasis of gastric cancer

The maspin gene,a tumor suppressor gene and a member of the serpin superfamily,is isolated from mammary epithelial cells.Maspin gene plays an important role in the process of invasion and metastasis of gastric cancer.Researches show that maspin gene suppresses the occurrence,development and metastasis of gastric cancer mainly by inducing tumor cell apoptosis,inhibiting tumor angiogenesis,methylation modification,promoting homogeneity adhesion and inhibiting heterogeneous adhesion.
2.Expression and significance of secretory phospholipase A2 in gastric adenocarcinoma

Objective To investigate the expression and signficance of secretory phospholipase A2(sPLA2>) in gastric adenocareinoma. Methods Sixty-five samples of gastric adenocarcinoma (11 with high differentiation, 10 with median differentiation and 44 with low differentiation) and 11 samples of normal gastric mucosa had been obtained from the Second Affiliated Hospital of Jilin University from January 2006 to April 2007. Fifty samples of gastric adenocarcinoma were found with lymph node metastasis. The expression of sPLA2> in gastric adenocarcinoma and normal gastric mucosa was detected. The relationship between sPLA2> and the differentiation of gastric adenoearcinoma, lymph node metastasis and helicobacter pylori infection was detected. All the data were processed with chi-square test or Spearman rank correlation. Results The positive rates of the sPLA2> expression in normal gastric mucosa and gastric adenocarcinoma were 36% (4/11) and 78% (51/65). The positive rates of sPLA2> expression in low, median and high differentiated gastric adenocarcinoma were 84% (37/44), 70% (7/10) and 64% (7/11), respectively. The expression of sPLA2> was positively correlated with the malignancy of gastric adenocarcinoma (r =0.272, P <0.05). The positive rates of sPLA2> expression in gastric adenocarcinoma with and without lymph node metastasis were 88% (44/50) and 47% (7/15), respectively. The expression of sPLA2> was correlated with lymph node metastasis (X2 = 9. 347, P < 0.05). The positive rates of sPLA2> expression in gastric adenocarcinoma with and without helicobacter pylori infection were 79% (38/48) and 76% (13/17), respectively. The expression of sPLA2> was not correlated with helicobacter pylori infection (X2 = 0. 000, P > 0.05). Conclusions The activation of sPLA2> gene may be correlated with the genesis of gastric adenocarcinoma. sPLA2> may influence the invasion of gastric adenocarcinoma, and can be used as an indicator in predicting poor prognosis.
3.Transabdominal intramediastinal esphagogastrostomy for the treatment of gastric cordial cancer

Objective To evaluate a surgical approach and anastomosis for the treatment of carcinoma of the gastric cardia. Methods Transabdominal intramediastinal esophagogastric anastomosis covered by sero-muscular flap of gastricwall for cardial carcinoma in 187 cases. Results The method reached the satisfactory surgical result in terms of tumor free cut edge on esophagus end of the resected samples. And the morbidity rate was 5.8%. Conclusion The technique enables lymphadenectomy within the lower mediastinum and a sufficiently long enough resection of esophagus. Transabdominal incision of the crus dextrum of the diaphragm makes a clear operative field for the purpose of radical operation for carcinoma of the gastric cardia. The anastomosis effectively prevents anastomotic leakage. This procedure is indicated for cardial carcinoma cases in which the esophaged involvement is within 2cm.
4.Fabrication of acellular nerve allograft through chemical extraction and efficacy with the use of the graft in repair of rat sciatic nerve defect

Objective To evaluate regenerative nerve and functional recovery of target muscle in rats with sciatic nerve defect bridged by acellular nerve allograft made through chemical extraction.Methods Sciatic nerve of SD rats was processed in a volume fraction of 3% Triton X-lO0 solution and 40 g/L sodium deoxycholate solution.Morphology of myelin sheath,axons and basal lamina tubes of sciatic nerve segments was observed under the light microscopy before and after the chemical processing.Twenty-five Wistar rats were divided into acellular nerve allograft group (n =10),autograft group (n =10) and normal control group(n =5) according to the random number table.A 1 cm sciatic nerve defect was created in acellular nerve allograft group and autograft group,and was respectively bridged by acellular nerve allograft and autograft.Sciatic nerve function index (SFI) was measured every two weeks.Twelve weeks after surgery,nerve conduction velocity (NCV),recovery rate of compound muscle action potential (CMAP) and recovery rate of muscle force were measured in each group.Results Cellular components including myelin sheath and axons were removed thoroughly,but the basal lamina tubes were preserved completely.At postoperative 2,4,6,8,10 and 12 weeks,SFI in normal control group (-1.7±5.9,-0.3 ±2.5,0.8 ±4.1,-1.4±3.6,-2.5 ±5.7 and-2.1±3.2) was superior over autograft group (-94.3±3.7,-90.1±4.1,-63.7±7.8,-51.9±8.2,-48.8±8.6 and -44.3 ± 10.5) and acellular nerve allograft group (-97.1 ± 5.3,-91.2 ± 6.1,-70.6 ± 5.5,-60.4±6.2,-58.2 ±10.2 and-56.4 ±8.0) (P <0.01).At postoperative 6,8,10 and 12 weeks,SFI in autograft group were better than those in acellular nerve allograft group (P <0.05).NCV [(61.6 ± 8.1) m/s],recovery rate of CMAP[(98.7 ± 5.9) %] and recovery rate of muscle force [(101.8 ± 6.6) %] in normal control group were higher than those in acellular nerve allograft group [(22.3 ± 4.7) m/s,(40.3 ± 9.2) % and (43.8 ± 9.3) %] and those in autograft group [(29.0 ±5.5) m/s,(52.5 ± 10.6) % and (54.3 ± 10.5) %] (P < 0.01).NCV,recovery rate of CMAP and recovery rate of muscle force in autograft group were better than those in acellular nerve allograft group (P < 0.05).Conclusions Acellular nerve segments are harvested satisfactorily by chemical extraction.Sciatic nerve defect in rats can be cured with the acellular nerve allograft,but the repair effect of autograft is relatively better.

As an important measure to reduce casualties and prevent secondary pollution, decontamination is an impor-tant link in the process of emergency response during chemical accidents.The decontamination effect is closely related to decontamination technology and equipment.Decontamination agent selection and development are an important part of a decontamination technology.In this paper, the development and use of cleaning agents, such as alkaline, oxidation and chlorination, adsorption (degradation), metal oxide and oxygen acid salt, chemical compounds, biological (enzymatic), and individual disinfection package, light decontamination equipment, multifunctional integrated large-scale decontamination equipment at home or abroad, are reviewed.By laying bare the gap between China and advanced countries in the related field, we hope to raise the concern of relevant professional counterparts and promote the development of domestic decontami-nation technology and equipment with decontaminant agents at the core.
6.Influence of thoracic sympathetic block on patients with diabetes mellitus complicated heart failure

Objective: To observe curative effect of thoracic sympathetic block on patients with diabetes mellitus (DM) complicated cardiac chamber enlargement and heart failure (HF). Methods: A total of 76 DM patients with cardiac chamber enlargement and HF were randomly divided into routine treatment group (n=36) and epidural nerve block group (nerve block group, n=40, received thoracic epidural nerve block based on routine treatment), and four weeks were a course of treatment. Changes of left ventricular ejection fraction (LVEF), left ventricular end-diastolic dimension (LVEDd), left atrial diameter (LAD), levels of tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), brain natriuretic peptide (BNP) and glycosylated hemoglobin (HbA1c) were observed in two groups. Results: There were no significant difference in all indexes between two groups before treatment (P>0.05). After treatment, total effective rate of cardiac function improvement in nerve block group was significantly higher than that of routine treatment group (95% vs. 66.7%, P<0.05). Compared with before treatment, there were significant decrease in LVEDd [(71.63±9.26)mm vs. (65.4±8.12)mm] and LAD[(45.62±7.10)mm vs. (37.14 ±6.28)mm] (P<0.05 both), significant increase in LVEF [(31.3±11.46)% vs. (42.12± 10.42) % , P< 0.01], and significant decrease in levels of TNF-α [(73.55±16.36)ng/L vs. (28.35±10.08) ng/L], IL-6 [(17.62±5.47) ng/L vs. (7.88±2.29) ng/L], BNP[(13.49±2.94) ng/L vs. (6.28±2.28) ng/L] and HbA1c [(7.9±2.6) % vs. (6.8±1.9) %] in nerve block group after treatment, P<0.05 or <0.001; and levels of LVEF, TNF-α, IL-6 and BNP in nerve block group after treatment were significantly improved than those of routine treatment group, P<0.05 or <0.01. Conclusion: Thoracic sympathetic block can significantly decrease levels of neuroendocrine and inflammatory factors, improve cardiac function, and elevate quality of life in patients with diabetes mellitus complicated heart failure.
7.Pattern and related factors of lateral cervical lymph node metastasis in patients with cN0 papillary thyroid carcinoma

Objective To evaluate the pattern of lateral cervical metastases and to investigate the risk factors for lateral cervical lymph node metastases in paoiuary thyroid carcinoma patients with clinical negative lateral neck lymph node.Methods 73 patients with paoiuary thyroid carcinoma who underwent prophylactic lateral neck dissections(level Ⅱ-Ⅵ or level Ⅱ-ⅣandⅥ) were reviewed retrospectively on their medical records paoiuary thyroid carcinoma.None of patients in this study had a clinically positive lymph node.Neck dissection specimens were obtained for histological analysis for node metastasis with respect to the individual neck levels.Results Occult metastases in lateral neck were observed in 12(16.4 %)patients.9.6 %,0,13.6 %,9.6 %,0,4.8 % and 42.4 % patients had histologically positive lymph nodes in levels Ⅱa,Ⅱb,Ⅲ,Ⅳ,Va,Vband Ⅵ respectively.In multivariate analysis,lymphatic metastases in level Ⅵ was associated with lateral neck metastasis(OR=7.3,P=0.020)in cN0 patients with paoiuary thyroid carcinoma.Conclusion Levels Ⅲ,Ⅱa and Ⅳwere the most common stages showing occult lymph node metastases.Prophylactic lateral neck dissections may be omitted in the treatment of cN0 PTC patients ff level Ⅵ lymphatic metastases are not found on histological exam.
8.Treatment outcome comparation of surgery plus radiotherapy and radiotherapy or chemoradiotherapy plus salvage surgery for stage Ⅲ/Ⅳ laryngeal carcinoma

Objective To compare the treatment outcome of underwent surgery plus radiotherapy and radiotherapy/chemoradiotherapy plus salvage surgery for stage Ⅲ/Ⅳ laryngeal carcinoma,to investigate an optimized treatment for the patients of stage Ⅲ/Ⅳ laryngeal carcinoma.Methods Clinical data from 103 patients with stage Ⅲ (39 cases) or stage Ⅳ (64 cases) laryngeal carcinoma were retrospectively analyzed.The patients were divided into surgery plus radiotherapy group (S±R,46 cases) and radiotherapy/chemoradiotherapy plus salvage surgery group (R±S,57 cases).Overall survival,relapse free survival,and laryngeal preservation rate were used to compare the treatment outcome between two groups.Multivariate regression models were used to analyze the independent factors for survival and laryngeal preservation rate.Results Survival rate was higher in S±R group than in R±S group [2 year overall survival/relapse free survival 74.7 % (34/46) / 72.4 % (33/46) vs 46.4 % (26/57) / 40.9 % (23/57),P < 0.05].Laryngeal preservation rate was higher in R±S group than in S±R group [93.0 % (15/46) vs 32.6 % (53/57),P < 0.05].Multivariate analysis demonstrated that treatment modality and T stage were independent factors for long-term survival,while treatment modality was the only an independent factor for laryngeal preservation rate.Conclusions Surgery plus radiotherapy result in better survival and lower laryngeal preservation rate than radiotherapy/chemoradiotherapy plus salvage surgery in treatment of stage Ⅲ/Ⅳ laryngeal carcinoma.Surgery plus radiotherapy should be the first choice for treatment of locally advanced laryngeal carcinoma.Improvement of the quality of life could be achieved by laryngeal preservation surgery and phonation reconstruction procedures.
9.Effect of probiotics on patients with severe acute pancreatitis: A Meta analysis

Objective To explore the effect of probiotics on patients with severe acute pancreatitis.Methods Randomized controlled trials of probiotics in patients with severe acute pancreatitis were searched in Cochrane Library(2010),Medline(1966 to September 2010),China Academic Literature Full-text Database,Chinese Science and Technology Journal Database and WanFang Database between 1966 and September 2010.Evaluation of literature quality and data extraction were independently completed by two researchers.Data were analyzed with RevMan4.3 software.Results A total of 313 papers had been searched.After excluded duplicated and low-level paper,six articles were included in this study.The result showed that the infection rate(OR=0.88,95%CI[0.56,1.38])and mortality rate(OR=1.44,95%CI[0.78,2.64])of patients with severe acute pancreatitis in intervention group were not lower than control group,however the length of stay in hospital was shorter in intervention group than control group(WMD=-4.69,95% CI[-7.73,-1.65]).There was no significant difference between intervention group and control group in systemic inflammatory response syndrome(OR =1.33,95 % CI[0.63,2.83]),multiple organ failure (OR=1.50,95 % CI[0.89,2.52])and transfering to surgery department for further treatment(OR=0.92,95%CI[0.56,1.51]).Conclusion So far the studies indicated that probiotics could not lower the infection rate and mortality rate in patients with severe acute pancreatitis.Large sample and highquality randomized controlled trials are needed for further evaluation.
10.Neck dissection for thyroid carcinoma patients who received nonstandard operation

Objective To study the values of neck dissection for thyroid carcinoma who received nonstandard operation and help to choose good re-operation methods. Methods Retrospective investigation was carried out in 38 cases of neck dissection for thyroid carcinoma patients who received nonstandard operation during 1997 to 2005, and 32 cases were treated with neck dissection. Results Pathological results confirmed there were 73.68 % with residual tumor, positive rate of thyroid was 47.38 %, and that cervical lymph node was 57.89 %. Conclusion The residual rate of cervical lymph nodes of reoperated patients with thyroid carcinoma was relatively higher, so neck dissection for thyroid carcinoma who received nonstandard operation was necessary.
