1.Clinical observation on the therapeutic efficacy of matrine on chronic hepatitis B with hyperbilirubinemia

Objective To study the therapeutic efficaey of matrine on chronic hepatitis B with hyperbilirubinemia.Methods A total of 68 patients were randomly recruited into a treatment and a control groups.Diammonium glycyrrhizinate and glutathione injection were used in both groups.On this basis,the treatment group was treated with matrine injection.while the control group Was treated with Yinzhihuang injection.Both groups were treated for one month.Results The total effective rate was 97%and 85%in the treatment group and the control group respectively,showing significant difference(P<0.01).Indicators such as ALT,AST, TBIL showcd significant difference after treatment compared with the pre-treatment in both groups (P<0.05).Conclusion It is effective of matrine treating chronic hepatitis B with hyperbilirubinemia.
2.Clinical significance of sinus heart rate turbulence and heart rate variability in patients with chronic obstructive pulmonary disease

Objective To explore the clinical significance of sinus heart rate turbulence (HRT)and heart rate variability (HRV) in patients with chronic obstructive pulmonary disease(COPD).Methods The 59 moderate to severe COPD patients and 30 healthy subjects were enrolled in this study. The 24-hour holter monitor was used to screen the HRT onset (TO), turbulence slope (TS)and HRV. Pulmonary function tests and echocardiographic examination were performed for measuring left ventricular ejection fraction (LVEF), right atrial dimension (RAD), right ventricular dimension (RVD), right ventricular wall thickness (RVWT). Then all the parameters were compared between NC group and COPD group, and the relationship between HRT and HRV was investigated. Results Compared with control group, TO was significantly increased [(-0.2±1.1) % vs.(-3.8±2.8) %, t=6. 830,P<0.01] and TS was decreased [(7.0±3.6) ms/RR vs. (11.7±6.1) ms/RR, t =3. 866, P<0.01] in COPD group. In time domain HRV parameters, normal RR intervallerinin standart deviation(SDNN), standard deviation of normal-to-normal beats index (SDNNi), standard deviation of the averages of normal sinus to normal sinus (SDANN), mean squared differences of the successive RR intervals (rMSDD), fraction of consecutive normal sinus intervals that differ by more than 50 ms (PNN50) were significantly lower in COPD group than in control group(P<0. 05). TO was negatively correlated with SDANN and rMSDD (r=-0. 369, P<0. 05; r=-0.472, P<0.01).TS was positively correlated with SDNN, SDANN and PNN50 (all P<0.05), but had no correlation with rMSDD (P>0. 05). Conclusions HRT and HRV are dramatically blunted in COPD patients.Combination of HRV and prognosis. and HRT may be simple and elegant ways for evaluating cardiac autonomic functions.
3.Expression of Toll-like receptor 3 in myocardium in experimental autoimmune myocarditis

AIM: To establish an animal model of experimental autoimmune myocarditis (EAM) in BALB/c mice and to investigate the expression and significance of Toll-like receptor 3 in mouse EAM. METHODS: BALB/c mice were immunized with cardiac myosin extracted from porcine ventricular myocardium covered by complete freund's adjuvant (CFA) on 0 d and 7 d, then divided into immunized with CFA only. Serum and myocardium samples were collected at 14 d and 21 d after the first immunization. HE staining was used to identify the areas of inflammation. The myosin IgG antibody was examined by indirect ELISA assay. The changes of TLR3 protein and mRNA expression in myocardial tissue were measured by immunohistochemistry and real time-PCR. RESULTS: Compared to control group, immunohistochemistry results showed that there was positive expression of TLR3 in the myocardium of mice with EAM and the mRNA of TLR3 were more than 20 times (P<0.05). The expression of interferon beta mRNA in EAM group was more than 14 times as many as basal expression, that of tumor necrosis factor alpha was more than 18 times (P<0.05). CONCLUSION: The expression of Toll-like receptor 3 in myocardium is up-regulated in experimental autoimmune myocarditis. The inflammatory response to cardiac myosin may associate with the TLR3 signal transduction pathway.
4.Effects of Danhong on the serum levels of CD137, high-sensitivity C-reactive protein (hs-CRP) and homocysteine in patients with non-ST elevation acute myocardial infarction complicating metabolic syndrome

Objective To investigate effects of Danhong on the serum levels of CD137, high-sensitivity C-reactive protein (hs-CRP) and homocysteine (Hcy) in patients with non-ST elevation acute myocardial infarction complicating metabolic syndrome. Methods A total of 126 patients with non-ST elevation acute myocardial infarction complicating metabolic syndrome were enrolled and randomly divided into a conventional treatment group and a Danhong treatment group using a random-digit table, with 63 patients in each group. All patients underwent angiography or percutaneous coronary intervention. The patients in the Danhong treatment group treated with intravenous Danhong 20 ml on the basis of conventional treatment for 1 week. The serum levels of CD137, hs-CRP and Hcy were measured at hospital admission and 10 days after treatment. The severity of coronary artery disease was assessed by the Gensini-score. Results The levels of CD137, hs-CRP and Hcy in both groups after treatment were significantly lower than before treatment (conventional treatment group: t 12.393, 17.408 and 9.458; Danhong treatment group: t 16.110, 17.573 and 13.481; all P<0.01), and the Danhong treatment group were significantly decreased than the conventional treatment group (t 2.815, 3.224 and 3.157, all P<0.01). The serum levels of CD137 and hs-CRP before treatment were significantly correlated with Gensini scores in 126 patients (r 0.720 and 0.562,all P<0.01). Conclusions The serum levels of CD137 and hs-CRP are significantly correlated with the severity of coronary artery disease, intravenous Danhong may has protective effect for coronary artery disease via decreasing CD137 and hs-CRP.
5.Clinical study on treatment of poststroke depression by acupuncture combined with conventional therapy of western medicine

Objective To evaluate the curative effect of acupuncture combined with conventional therapy for the treatment of post-stroke depression (PSD). Methods Sixty patients were divided into two groups according to the computer. Acupuncture group was treated with acupuncture combined with conventional therapy of western medicine. Non-acupuncture group was treated with sham acupuncture combined with conventional therapy. Both groups were treated for 4 weeks. The patients was evaluated using the Hamilton depression rating for scale (HAMD). Adverse reactions during treatment were assessed by the Treatment emergent Symptom Scale (TESS). Results The HAMD score at the first week (27.1 ± 6.1 vs. 30.8 ±5.0,t=-5.532),the second week(26.6 ±6.3 vs.15.8 ±7.3,t=2.744),the 4th week(15.8 ±7.3 vs.20.7 ± 5.2,t=3.570),and the 8th week(9.9 ±6.2 vs.15.6 ±5.7,t=6.154)in the acupuncture group were lower than those in the non-acupuncture group (P<0.01). During the treatment, 2 cases of insommia occurred in acupuncture group, 8 cases of insommia occurred in nonacupuncture group, 2 cases of nausea and vomiting occurred in acupuncture group, 9 cases of nausea and vomiting occurred in nonacupuncture group(χ2=10.550, P=0.001).Conclusions Acupuncture combined with conventional therapy can improve the depressive state of PSD patients, reduce the adverse reactions and improve the quality of life of patients.
6.Clinical study of acupuncture combined with repetitive transcranial magnetic stimulation in the treatment of cognitive impairment after traumatic brain injury

Objective:To observe the efficacy of acupuncture combined with repetitive transcranial magnetic stimulation (rTMS) in the treatment of cognitive impairment after traumatic brain injury.Methods:A total of 40 patients were randomly divided into treatment group and control group with 20 cases each. The control group received routine basic therapy and cognitive function training, and the treatment group received rTMS combined with acupuncture based on the control group treatment. Both two groups were treated for 4 weeks. The MMSE, MoCA, latency and amplitude were used before and after treatment to compare the difference between the two groups.Results:The MMSE (23.60 ± 2.16 vs. 21.70 ± 2.87, t=-2.366) and MoCA (21.75 ± 3.35 vs. 19.05 ± 2.46, t=-2.903) scores in the treatment group were significantly higher than those in the control group ( P<0.05). There were no significant difference in latency and amplitude of motor evoked potential between both groups before and after treatment ( P>0.05). Conclusion:Acupuncture combined with rTMS can improve the cognitive function of patients with cognitive impairment after traumatic brain injury.
8.Progress of the application of optical coherence tomography in gastrointestinal tumor surgery.

Optical coherence tomography (OCT) is a real-time, cross-sectional optical imaging technology. It is analogous to ultrasonography, except that OCT uses light waves instead of sound waves, and can provide three-dimensional morphological images of living tissues with a micrometer resolution. Through the use of endoscopes, needles, catheters and laparoscopes, OCT has demonstrated tremendous imaging potential in tumor surgery. The current studies suggest that OCT has potential for clinical applications in the following fields of gastrointestinal tumor surgery: (1) Early tumor detection and diagnosis: OCT can distinguish differences between polyp tissue, normal tissue and malignant tissue. It could possibly identify premalignant lesions or conditions potentially predisposing to malignancy, such as gastric and intestinal metaplasia, gastritis associated with Helicobacter pylori, and early gastric cancer involving the mucosa or submucosa. In addition, OCT can differentiate between adenomatous polyps and hyperplastic polyps. (2) Optical biopsy of lymph nodes: As a high-resolution, near-IR imaging modality, OCT is capable of visualizing microscopic features within tissue, distinguishing lymph node tissue from surrounding adipose tissue, revealing nodal structures such as germinal centers and intra-nodal vessels. Consequently, OCT has the ability to show changes in node microarchitecture during metastatic tumor infiltration. (3) Intraoperative guidance for real-time determination of surgical margins: In other tumors such as oral squamous cell carcinoma and breast cancer, it has been demonstrated that OCT can be used to rapidly scan large areas of tissue, to guide at the cellular level the surgical resection of neoplastic disease, and to scan tumor margins for the presence of residual disease, tumor foci, and potentially even metastasizing tumor cells. It implies that colorectal neoplasms surgeons can possibly use the laparoscopic OCT to detect the intestinal tumor margin and lymph nodes during operation in the future, so as to determine the appropriate range of bowel resection and lymph node dissection. At present, there are few reports about the intra-operative application of OCT in the field of gastrointestinal tumor surgery. Thus there is a tremendous opportunity for further research in this field.

The dental operative microscope has been widely employed in the field of dentistry, particularly in endodontics and operative dentistry, resulting in significant advancements in the effectiveness of root canal therapy, endodontic surgery, and dental restoration. However, the improper use of this microscope continues to be common in clinical settings, primarily due to operators' insufficient understanding and proficiency in both the features and established operating procedures of this equipment. In October 2019, Professor Jingping Liang, Vice Chairman of the Society of Cariology and Endodontology, Chinese Stomatological Association, organized a consensus meeting with Chinese experts in endodontics and operative dentistry. The objective of this meeting was to establish a standard operation procedure for the dental operative microscope. Subsequently, a consensus was reached and officially issued. Over the span of about four years, the content of this consensus has been further developed and improved through practical experience.
