1.THE CLINICAL SIGNIFICANCE OF DETERMINATION OF SERUM PEPSINOGENS Ⅰ AND Ⅱ

The results of determination of SPgⅠ and SPg Ⅱ in healthy subjects, some cases of gastrointestinal diseases and other diseases were reported. The level of SPgI was lower in the intestinal type of gastric cancer, but not in gastric type of gastric cancer. The severer the degree of atrophy in the gastric fundus glandular mucosa, the lower the level of SPgⅠ. levels of SPgⅠ, SPgⅡ were high in peptic ulcer, especially in the active stage of duodenal ulcer. In addition, low SPgⅠ, SPgⅡ levels were found in patients after gastrectomy or with cirrhosis, and very high levels were found in chronic renal failure. It is suggested that SPgⅠ, SPgⅡ values may be used as a new serological marker for the initial screening test for intestinal type of gastric cancer, for evaluation of the degree of atrophy of gastric glandular mucosa and for follow-up study of atrophic gastritis. It might also serve as a new serological parameter in the etio-logical as well as epidemiological study for duodenal ulcer.
2.Effect of OT in resuming upper extremity function of the hemiplegia

Objective:To study the effect of occupational therapy(OT),for the improvement upper extremity function of the hemiplegia patient.Methods:67 hemiplegia patients have recevied OT,and assessed with Fugl-Meyer and ADL method.Results:After treatment,improvement for upper extremity and hand 78%,ADL ability 87%.Conclusion:Application OT for training the function of upper extremity and hand of hemiplegia,may recover the all kinds of delicate and coordinate action of hand and upper extremity and improve the quality of daily living.
3.Observation on standing balance of the patients with cervical vertebra syedrome

73 patients with cervical vertebra syndrome have been examined by body balance function with Japanese centre of gravity vacillate instrument. In six basis indexes. 58 patients with two or more abnormal, (80%). 11 Patients, one abnormal, (15%) only 4 had normal indexes (5%), but the centre of gravity chart showed two of them at middle pattern, one at disperse,another at polycentric pattern. The rate of difference remarkble cheek was significant (p>2.58, P<0.01)The examination of body balance function and the index has quantitative analysis significance. It can be effective method for assistant diagnosis and healing effective observation on certebra syndrome
4.Treatment of noninfectious nonunion of tubular fractures with compression and distraction technique by external fixators

Objective To explore the effect and safety of compression and distraction technique by external fixation without grafting in treatment of noninfectious nonunion of tubular fractures.Methods 43 patients were randomly divided into 2 groups by visiting sequence with random number,odd into the observation group,even into the control group.The observation group(22 cases) adopted the external fixation retractor pressure treatment,the control group(21 cases) was given extemal fixation combined with locking plate treatment.The efficacy of two groups was compared.Results 43 patients were followed up for 6 ~ 15 months,average 7.5 months.The bone healing time was 3 ~ 14 months,average 5.1 months.43 patients were all healed.The total effective rate of the observation group was 81.8%,which was significandy higher than 66.7% in the control group (Z =9.654,P =0.001).There were no significant differences in adverse reactions and complications between the two groups (x2 =0.068,P =0.795).Conclusion The compression and distraction technique by external fixation in the treatment of noninfectious nonunion of tubular fractures has obvious curative effect,it can be widely used in clinical practice.
6.Epidemiological characteristics of occupational musculoskeletal disorders among nurses in three tertiary grade A hospitals

Objective To understand the prevalence condition of occupational musculoskeletal disorders in nurses,so as to supply scientific evidence for reducing and preventing nurses′occupational injuries.Methods 582 nurses from three level three class -A hospital in Wenzhou were investigated,which were selected by convenience sampling method.Results The prevalence of musculoskeletal disorders was 76.3%.The prevalence of low back pain was highest,which was 58.9%.Among the nurses who had lower back pain,in the past 1 year,43.4%of them had a total duration of illness more than 30 days,18.7% of them reduced daily activities time for more than 30 days,and 22.6% of them had received examination and treatment.The main factors which related to low back pain among nurses were age (OR =2.18,95% CI:1.36 -3.67 ),stoop time (OR =2.24,95% CI:1.08 ~4.36 ), occupational stress(OR =1.82,95%CI:1.14 ~2.89),work fatigue(OR =2.45,95%CI:1.27 ~3.15)and physical exercise(OR =0.63,95%CI:0.44 ~0.89).Conclusion The condition of occupational musculoskeletal disorders is serious,especially low back pain.Interventions related to influencing factors can be taken to reduce the incidence of low back pain.
7.A qualitative study on the work experience of enterostomal therapist in Shanghai

Objective To describe the work experience and needs of enterostomal therapist in Shanghai.Methods Phenomenological methodology was adopted in this study.In-depth interview was conducted on 9 enterostomal therapists.Data were analyzed by inductive codes and coding methods.Results Five themes were extracted,including responsibility and mission,pride,accomplishment,helplessness,and satisfaction.When they began their new jobs,they confronted many challenges,but they conquered them with responsibility and mission.Finally,they felt growing up and harvested a lot during this time.Conclusions Hospital managers should pay attention to problems they encountered in their work and provide necessary supports for their training,work and development.

9.Risk factors for postoperative reintubation in patients undergoing general anesthesia

Objective To identify the risk factors for postoperative reintubation in patients undergoing general anesthesia.Methods Forty-six thousand five hundred and seven patients,aged 18-83 yr,requiring reintubation after planned extubation in the postanesthesia care unit (PACU) of our hospital from January 2010 to December 2012,served as reintubation group.Patients in a 1∶5 ratio,aged 18-83 yr,admitted to the PACU of our hospital from January 2010 to December 2012,with successful extubation,served as control group.The general data of patients and operation-related factors including type of operation (emergency operation/elective operation),operative sites (head and neck,airway,within the chest,upper abdomen,lower abdomen,other sites) and operation time and anesthesia-related factors including requirement for opioids and muscle relaxants within 30 min before operation,and for neostigmine at the end of operation were recorded.The risk factors of which P values were less than 0.05 would enter the logistic regression analysis to stratify reintubation-related risk factors.Results Thirty-two patients were reintubated after operation and the incidence was 0.069%.There was significant difference in age,gender,body mass index,ASA physical status,preoperative SpO2,complication with upper respiratory infections within 2 weeks before operation,chronic obstructive pulmonary disease (COPD),or systemic inflammatory response syndrome (SIRS) and hypoproteinemia,operative sites and operation time between the two groups (P < 0.05 or 0.01).The logistic regression analysis showed that ASA physical status ≥ Ⅲ,complication with COPD or SIRS,and thoracic surgery were closely correlated with postoperative reintubation in patients undergoing general anesthesia.Conclusion ASA physical status ≥ Ⅲ,complication with COPD or SIRS,and thoracic surgery are risk factors for postoperative reintubation in patients undergoing general anesthesia.
10.Research advances in proton therapy for pediatric cancer

Pediatric cancer is one of the leading causes of death in children around the world.Although radiotherapy is an important means of treatment for pediatric cancer,it causes acute or chronic adverse events that may affect patients' survival time and quality of life.As a novel and advanced radiation technique,proton therapy allows for precise dose delivery in target volume,significantly reducing the irradiation to surrounding tissues and organs.Studies have shown that proton therapy is well tolerated in the treatment of pediatric cancer,and it achieves good tumor control;proton therapy is superior to traditional radiotherapy in improving quality of life,protecting intelligence,and reducing the risk of secondary cancer.This article reviews the research advances in the application of proton therapy in the treatment of pediatric cancer.
