1.A Case of Klippel-Trenaunay-Weber Syndrome.

A case of Klippel-Trenaunay-Weber syndrome described originally by Klippel and Trenaunay on 1900, was presented with a brief review of literatures. The patient, 30 year-old male, said that he had noticed a reddish plaque in his right leg at the age of about 10 and had felt the asymmetricity of the limbs about 5 years later. On physical examination, he had an ulcerated sclerosing hemangioma on his right limb which is extending from medial aspect of upper one third of the lower leg to the foot. It was also associated with the varicosities just above the hemangioma, edema and hypertrophy around the right ankle, and a visible elongation of the right limb. A plane X-ray revealed osteohypertrophy of tibia and fibula without any periosteal reaction, and a soft tissue hypertrophy was also noticed around the right ankle. Histological findings of the hemangioma revealed moderate fibrosis around the proliferated vascular tissue and two of the remarkable thrombosis in the median sized vessels.

2.A Case of Erythema Elevatum Diutinum.

A case oof erythema elevatum diutinum in 30-year old male was described. Reviewing literatures, this disease is very rare and is the first case to be reported in Korea. There were persistant elevated hard brownish red nodulo-infiltrated plaques which were symmetrically distributed especia]ly over the shins, elbows, knees, ear lobes, buttocks, and dorsal surfaces of the hands, feet, fingers, and toes, accompanied by occasional mild itching. These skin eruption were started from the erythematous papulo-vesicular macules which developed on the left shin and wrist about 7 years previously. He has also had a long-standing encapsulated pleural effusion with marked pleural thickening in left lower chest which was diagnosed 3 years ago. The serum IgG was noted to be increased. Histological examination of skin lesions revea]ed leucocytoclastic angiitis and sevrer perivascular fibrosis in older lesions.

3.Animal Experiments of Cutaneous Depigmentation by 4-Isoprophlcatechol and Hydroquinone.

The depigmenting activity of 4-isopropylcatechol and hydroquinone, and the behaviour of salicylic acid affecting to the activity were studied. To the hydrophilic ointment base, 4-isopropylcatechol and hydroquinone were incorperated respectively in concentrations of l% and 3%, and to these creams, salicylic acid was also added in concentration of 3% and 5% respectively. These creams were applied topically to the epilated skins of the back of black guinea pigs once a day for 3 weeks. Followings were the results: 1) The depigmenting activity was found to be more potent in 4-isopropylcatechol than in hydroquinone. 2) The depigmenting activity of hydroquinone was found to be remarkably accelerated by the addition of salicylic acid. 3) The skins of guinea pigs were found to be mildly irritated by the application of both 4-isopropylcatechol and hydroquinone.

4.Treatment of Endophthalmitis Associated with Filtering Blebs.

Two Patients having endgphthalmitis associated with filtering blebs were diagnosed clinically when they developed ocular inflammation with hypopyon after Scheie's operation for glaucoma. They had a good response to treatment outlined below with no loss of visual acuity.

5.Posttraumatic Stress Disorder Prevalence, Symptoms, Depressed Mood and Anxiety According to the Presence of Consciousness at the Time of Traumatic Accident.

OBJECTIVES: This study investigated PTSD prevalence, symptoms, depressed mood and anxiety, comparing two groups of the patients in trauma; one who had experienced the loss of consciousness (group "A" here after) and the other group of patients who had not(group "B" here after). METHOD: Subjects were 120 patients(age18-66) who had received trauma(traffic accident, fall down) more than 1 month. Before they consisted of 56 patients who had experienced the loss of the consciousness(46.7%) and 64 who had not(53.3%). Men were 69 and women were 51. Clinician administered PTSD Scale, Beck Depression Inventory, State Trait Anxiety Inventory-I, II were administered. The factors such as sociodemographic variables, unconsciousness at the time of trauma, memory about traumatic accidents were considered. RESULT: Out of 120 subjects, 30 patients(25%) were diagnosed as PTSD: 17 from 56(30.4%) and 13 from 64(20.3%) were found in the group "A" and "B" respectively. Prevalence of PTSD was higher in the group "A". However, there was no significant difference between two groups(X2=1.607, p>0.05). The CAPS of group "A" showed significantly high frequency in the loss of interest, detachment, restricted affect, sleep difficulty, irritability/anger, concentration difficulty(p<0.05), while the group "B" scored high in intrusive recall and increased startle. The scores of BDI, STAI-I, II were significantly higher in the group "A"(t=-3.16, p<0.05)(t=-2.75, p<0.05, t=-3.38, p<0.05). CONCLUSION: PTSD was more frequent and depressed mood and anxiety appeared more often in the group who experienced the loss of consciousness. Thus the loss of the consciousness at the time of trauma prones of the aevelopment PTSD and to symptom of aepression and anxiety.

6.The Reliability and Validity of The Migraine-Specific Quality of Life Questionnaire for Korean Migraine Suffers.

BACKGROUND: Recurrent, often unpredictable, migraine attacks significantly interfere with daily functioning and productivity of patients with migraine. Quality of life measurement represents an important tool for evaluating migraine therapy and judging the impact of headache either on individuals or on society. The objective of this study was to test the reliability and validity of the translated version of MSQoL (Migraine-Specific Quality of Life) in Korean migraine patients. METHODS: Participants, migraineurs from the Samsung Medical Center Migraine Clinic, were screened using the International Headache Society migraine criteria prior to enrollment through direct interview. This survey included the MSQoL, MIDAS (Migraine Disability assessment) and STAI (State-Trait Anxiety Inventory). RESULTS: Sixty-five migraineurs were recruited. For the MSQoL, Cronbach's alpha was 0.93. Frequency and number of accompanying symptoms were predictive of the MSQoL. The negative correlation between the MSQoL scores and the MIDAS score was seen (r=-0.585, p<0.01). Similarly a negative correlation between the MSQoL scores and duration of headache was also observed (r=-0.38, p<0.01). Although a significant negative correlation between the MSQoL and STAI scores was present (r=-0.47, p<0.01), correlation between the MIDAS and the STAI scores was not significant. CONCLUSIONS: The MSQoL is a reliable and valuable migraine-specific quantitative tool to assess the degree of quality of life in Korean migraineurs. It has a potential to provide valuable information on migraineur's disability and anxiety level. The MSQoL has acceptable psychometric properties and can be used to estimate long-term outcome of migraine therapy in Korean population.

8.Cardiac Hypertrophy in Transgenic Mice with Overexpressed Small-Subunit of Human Myosin Light Chain Phosphatase.

BACKGROUND AND OBJECTIVES: Recent reports have demonstrated that perturbation of the balance between myo-sin light chain (MLC) phosphorylation and the dephosphorylation status is associated with the development of cardiac hypertrophy. Myosin light chain phosphatase (MLCP) is a key enzyme that regulates the phosphorylation status of the MLC, but its functional roles in cardiac muscle have not been well investigated. Especially, the functions of the small-subunit of MLCP in cardiac muscles are not well elucidated. Here, whether the human heart-specific small-subunit (M21) of MLCP is associated with hypertrophic responses in a transgenic mice model were assessed. MATERIALS AND METHODS: The transgenic mice, overexpressing the human M21, were generated from a cardiac-specific transgenic construct. Cardiac tissues from the transgenic mice were subjected to histology for their morphological examination. The echocardiographic parameters of the murine heart were examined with transgenic mice aged 1, 2 and 3 months, and compared with their non-transgenic littermates. To determine whether the transgenic heart was sensitive to stress, the echocardiographic examination was also performed at the baseline, both before and after the administration of isoproterenol, at a dosage of 30 microgram/g/day, for 2 weeks. RESULTS: The histological analysis of the transgenic heart revealed myocyte disarray and nuclear hypertrophy. No significant differences were observed between the transgenic and non-transgenic mice in relation to the echocardiographic determinants, such as the left ventricular dimensions and the wall thickness. Chronic cardiac stress, induced by isoproterenol infusion, also failed to show any significant differences in relation to the same determinants. CONCLUSION: Overexpression of the human M21 in the murine heart induced myocyte hypertrophy. However, the overall cardiac functions were not affected under normal and stressed conditions.
