2.Depression in systemic lupus erythematosus

Objective To investigate the level of depression in patients with systemic lupus erythematosus (SLE).Methods Fifty six in patients and out patients with SLE were investigated with questionnaire of self rating depressive scale (SDS).Results Of these patients 41% had clinical features of depression.Tiredness,hebetude,hyposexuality,despair,irritability,depression,thinking difficulty,insommia,uneasiness and vacancy were more common.Most of these patients are manual worker,poorly educated ( P 0 05).Conclusion Patients with SLE have symptoms of depression to a certain extent.Doctors should pay close attention to their mentality and emotion in diagnosis and treatment of the prtients with SLE.
3.Effects of protein-enriched meal on triglyceride, uric acid and renal function among overweight and obese adults

Objective To investigate the effects of protein-enriched meal on uric acid and renal function among overweight and obese Chinese adults during weight loss. Methods People, 18-55 yearsold, with body mass index 25-35 kg/m2 and triglycerides 1.7-5.4 mmol/L were enrolled. Major exclusion criteria were: type 2 diabetes, weight reducing surgery or use of weight loss drugs, chronic kidney disease, and hepatic dysfunction. A high-protein (HP) diet (protein energy ratio was 30%) or a standard-protein (SP) diet (protein energy ratio was 15%) was provided to eligible subjects for 12 weeks (84 days). Energy intake was calculated by Katch-McArdle formula:basal metabolic rate=370+21.6 × lean body mass (kg) and coefficient of variation. All the subjects were followed up at day 0,week 4,week 8,and week12. The changes of triglyceride (TG), total cholesterin (TC), low density lipoprotein (LDL), uric acid (UA), blood urine nitrogen (BUN) and creatinine (Cre) were determined at day 0, week 4, and week 12. Results Ninety-four subjects with HP (n=49) or SP (n=45) were enrolled, and seven subjects were withdrawn for refusing the following visit. The average age was (42.68±8.98) years in HP group, and (41.09±9.67) years in SP group (P>0.05). At week 12, the BMI drop was significant in both groups from baseline, but with no significant difference between the two groups. The decreases of TG (mmol/L) at 4 weeks and 12 weeks were significant compared with the baseline (0 d, 4weeks, 12 weeks:HP:2.18±1.18, 1.77±0.82, 1.86±0.83;SP:2.60±2.67, 2.03±1.28, 1.92±0.79) (P<0.05),but with no significant difference between the two groups. Compared with the baseline, the decrease of UA (mmol/L) at 4 weeks in HP group was significant (0 d, 4 weeks, 12 weeks:HP:326.12± 88.31, 294.76 ± 106.57,307.15 ± 94.70;SP:325.62 ± 87.16,321.51 ± 101.19,315.71 ± 86.77) (P<0.05). The BUN and CRE in both groups increased slightly compared to baseline level, but remained in the normal range. Conclusion Comparison of HP and SP groups showed that renal function did not change significantly, the levels of UA in HP group decreased significantly than that in SP group.
4.A study on the clinical application of all-in-one-mixed and pre-mixed parenteral nutrition preparations

ObjectiveTo evaluate the parenteral nutrition formula's character of all-in-one-mixed preparation and pre-mixed preparation applied for hospitalized patients in China.MethodsWe collected parenteral nutrition (PN) prescriptions in hospitalized patients with various diseases at dispensing centers of 6 hospitals in different areas of China. Statistic analysis was made by nutrients application, total liquid, nonprotein calories, total nitrogen, ratio of non-protein calories and total nitrogen, and ratio of glucose and fat. A comparison was made between all-in-one-mixed and pre-mixed preparation as to all the indices.Results The nutrients supply in all-in-one-mixed PN preparation, as well as in the pre-mixed PN preparations of 3 different energy degrees can meet the basic nutrition requirements of most patients. The total nitrogen supply in all-in-one-mixed PN prescriptions meets nutrition requirements in most patients, but the ratio of non-protein calories and total nitrogen[( 180-250) : 1]and the ratio of glucose and fat (0. 56-1.26)were significantly different from what a doctor would recommend[( 100-150) : 1, 1.0]. While the ratios in the pre-mixed PN preparation on 3 different energy degrees are more consistent with recommeded values,which are at 167 and 0. 8 respectively.ConclusionsAll-in-one-mixed and pre-mixed PN preparation have their own individual advantages and limits, and both of them can meet the requirements of most hospitalized patients. More attention should be paid to the nutrients proportion when doctors prescribe all-in-one-mixed PN preparations.
5.Anterior cervical disc replacement and anterior cervical decompression and fusion for treating single segment cervical disc herniation:which has greater effects on adjacent segment degeneration?

BACKGROUND: Studies showed that both anterior cervical disc replacement and anterior cervical decompression and fusion can achieve good clinical result in cervical spondylosis. However, it is not conclusive about which kind of surgical method has an advantage in avoiding the adjacent segment degeneration. OBJECTIVE: To compare the effect on adjacent segment degeneration of single segment cervical disc herniation treated with anterior cervical decompression and fusion and anterior cervical disc replacement. METHODS: We col ected clinical data of 178 patients with cervical disc herniation and receiving anterior cervical disc replacement or anterior cervical decompression and fusion from January 2009 to December 2012. A retrospective analysis was performed. There were 116 cases in the anterior cervical decompression and fusion group and 62 cases in the anterior cervical disc replacement group. RESULTS AND CONCLUSION: (1) Evaluation: visual analogue scale score, Japanese Orthopaedic Association Scores and neck disability index were improved significantly in both groups during final fol ow-up compared with that pre-treatment (P < 0.05). (2) No significant difference in range of motion of operation segment, adjacent upper segment and adjacent lower segment was detected between final fol ow-up and pre-operation in the anterior cervical disc replacement group (P > 0.05). At 3 months after surgery, in the anterior cervical decompression and fusion group, surgical segment was confluent, and range of motion lost. During final fol ow-up, range of motion of adjacent upper segment and adjacent lower segment was significantly increased, and the increased range of motion in the upper segment was bigger than that of the lower segment (P < 0.05). (3) During final fol ow-up, X-ray films and MRI images revealed the number of degenerated adjacent segment was more in the anterior cervical decompression and fusion group than in the anterior cervical disc replacement group (P <0.05). The number of degenerated middle and upper segments was more than that of the lower segment in both groups (P < 0.05). (4) The findings confirmed that anterior cervical disc replacement or anterior cervical decompression and fusion for treating cervical disc herniation could effectively relieve nerve symptoms of patients. However, compared with the anterior cervical disc replacement, adjacent segment degeneration occurs more commonly after anterior cervical decompression and fusion.
6.Role of orexin in sleep disorder after isoflurane anesthesia in rats

Objective To evaluate the role of orexin in the sleep disorder after isoflurane anesthesia in rats.Methods Sixty-four male Sprague-Dawley rats,aged 10-12 weeks,weighing 280-320 g,were randomly divided into 2 groups (n =32 each) using a random number table:control group (C) and isoflurane group (Ⅰ).Group Ⅰ inhaled 1.2% isoflurane from 8:00 to 13:30 to induce anesthesia,followed by 0.5 h of recovery.Group C received no anesthesia and the other procedures were similar to those previously described in group Ⅰ.The induction time and awakening time were recorded.Eight rats were randomly chosen to record the movement condition (locomotor time and activity) from 14:00 to 8:00 the next morning.Before beginning of anesthesia,at 4 h after beginning of anesthesia,and at 4 and 10 h after the end of anesthesia,6 rats were randomly chosen in each group to count the orexin/c-fos double-labeled neurons in hypothalamus.The ratio of activated orexin neurons (orexin/c-fos double-labeled neurons to orexin positive neurons) was calculated and plasma orexin-A concentration was detected.Results The induction time was (2.14 ± 0.17) min,awakening time was (8.7 ± 0.5) min,and EEG showed that there was no typical burst and suppression patterns in group Ⅰ.There was no significant difference in the number of orexin positive neurons between the two groups (P > 0.05).Compared with group C,the time for locomotor activity was significantly prolonged,and the activity was increased during the night (P < 0.01),the number of activated neurons,ratio of activated orexin neurons and plasma orexin-A concentration were decreased at 4 h after beginning of anesthesia in group Ⅰ (P < 0.01).The plasma orexin-A concentration was lower at 4 h after beginning of anesthesia,while higher at 10 h after the end of anesthesia than before beginning of anesthesia in group Ⅰ (P < 0.05).The number of activated neurons was significantly larger and ratio of activated orexin neurons was higher before beginning of anesthesia and at 10 h after the end of anesthesia and the plasma orexin-A concentration was higher at 4 and 10 h after the end of anesthesia than at 4 h after beginning of anesthesia in group Ⅰ (P < 0.05 or 0.01).Conclusion The development of sleep disorder after isoflurane anesthesia during the day time in rats is related to the regulatory role of orexin in it.
7.Compound flap transplantation in the treatment of 20 patients with scar of functional position after burns

AIM: To observe the restorative effect of compound flap transplantation on the scar of functional position after burns. METHODS: Twenty inpatients with scar of functional position after burns treated by compound flap transplantation were selected from the Department of Burns and Plastic Surgery, First Hospital Affiliated to Liuzhou Medical College between May 2002 and December 2005, and all patients knew and agreed with the items. All patients were removed of the scar of functional position under whole-anesthetization to transplant the acellular allogenic dermis (It was manufactured by Beijing Dieyalaifu Biotechnological Co., Ltd of Guangxi Beisheng Pharmaceutical Group, and it was characteristics of soft, flexible with better tenacity. Besides, it was milk white in net with two kinds of surfaces: smooth surfaces and rough surfaces). Acellular allogenic dermis of rough surface closely adhered to the wound with smooth surface up toward, and the wound was sutured. Autologous skin grafts (obtained from scalp, trunk or extremities) were transplanted into acellular allogenic dermis, and the flap gaps shall be kept as small as possible, so as to reduce the exposure of acellular allogenic dermis. Wounds were dressed at 2 weeks postoperation, and the follow-up was conducted regularly. The survival rate, contraction rate, the smooth and glossy degree, skin elasticity, joint function, and healing of donors were observed from 2 weeks to one year after the surgery. RESULTS: Twenty patients were followed after compound flap transplantation with the time ranged from one month to one year, including 4 patients within 1-3 months, 3 patients within 3-6 months, and 13 patients within 6 months to one year. Thirty functional positions out of 31 healed in the first period. While the flap in another position was influenced by exposed acellular allogenic dermis due to over large gaps among autoallergic flaps. The survival rate of transplanted flap was 93.00%, and the flaps transplanted were smooth in appearance with proper elasticity and less contraction, and the joint range of motion was better. The skin of donor site recovered better. CONCLUSION: Compound flaps can significantly ameliorate the functional disturbance and deformities caused by scar of functional position after burns with high survival rate.
8.Study on quality standard of YurongXiaocuo granule

Objective: To establish a quality control standard for YurongXiaocuo granule.Methods: TLC was used to identify Radix Puerariae,Fructus Forsythiae,Bulbus Fritillariae Thunbergii,while the content of tanshinoneⅡA was determined by HPLC.Result: The methods of qualitation and quantitate was simple,accurate and with strong specifi city,the good linear rang of tanshinoneⅡA was 3.2-32?g and the average recovery was 99.51%(RSD=2.74%).Conclusion: This method can control effectually the quality of YurongXiaocuo granule.
9.Pulmonary ventilation/perfusion scintigraphy in assessing residual pulmonary hypertension after pulmonary endarterectomy

Objective:To analyze the relationships between baseline percentage of pulmonary perfusion defect scores (PPDs%) and residual pulmonary hypertension after pulmonary endarterectomy in chronic thromboembolic pulmonary hypertension (CTEPH) patients.Methods:A total of 37 CTEPH patients (21 males, 16 females, age (50±12) years) who had pre- and post-pulmonary endarterectomy ventilation/perfusion (V/Q) scans from January 2016 to January 2019 at Fuwai Hospital were retrospectively collected. Residual pulmonary hypertension was defined as post-surgery mean pulmonary artery pressure (mPAP) higher than 30 mmHg (1 mmHg=0.133 kPa). Semi-quantitative index PPDs% was calculated to represent the extent of impaired perfusion in the whole lung. Pre- and post-surgery pulmonary hemodynamic parameters including pulmonary arterial pressure and resistance and lung perfusion were compared. Factors that may be related residual pulmonary hypertension were analyzed using logistic regression analysis. Paired t test and independent-sample t test were also used. Results:Pulmonary endarterectomy significantly decreased mPAP ((50.22±11.72) vs (26.41±10.61) mmHg; t=12.599, P<0.001) and pulmonary vascular resistance((10.06±5.48) vs (2.89±1.58) Wood unit; t=8.086, P<0.001). The number of defected lung segments (13.79±2.11 vs 5.52±2.82; t=11.593, P<0.001) was significantly reduced. Patients who had residual pulmonary hypertension ( n=11) exhibited significantly higher PPDs% before the surgery compared to those who were without residual pulmonary hypertension ( n=26; (57.48±5.88)% vs (47.77±11.09)%; t=-3.458, P=0.002). Baseline PPDs% was an independent factor for predicting residual pulmonary hypertension after pulmonary endarterectomy (odds ratio ( OR)=1.106, 95% CI: 1.006-1.216, P=0.036). Conclusions:Pulmonary V/Q scan can assess the therapeutic effects of pulmonary endarterectomy. Extent of impaired lung perfusion assessed by V/Q scan is the influencing factor for post-surgery residual pulmonary hypertension.
10.Evaluation on effectiveness of prevention in clinical epidemiology teaching for the students in clinical medicine

Objective To study the teaching effect of prevention in clinical epidemiology teaching.Methods 187 clinical medical students of Grade 2010 from China Medical University were selected as the research objectives.2 teaching hours of prevention content was increased in the clinical epidemiology teaching,and the anonymous questionnaire survey was used to assess the teaching effectiveness.A total of 187 questionnaires were issued and 187 valid questionnaires were collected.All the data were analyzed by SPSS 16.0.Results 82.9% (155 people) of the students believed that the addition of preventive content was necessary,40.1% (75 people) of the students believed that there was a significant increase in the content of the prevention.For the understanding of the content,only 11.2% (21 people) of the students said they had a complete understanding of the class,but after teaching 51.3% (96 people) of the students expressed a clear understanding of the content.82.4% (154 people) of the students thought that the teaching contents of prevention was tightly combined with the clinical,and 74.9% (140 people) of the students thought that 2 hours setting was appropriate.Students' demands for prevent content also included:1) how to strengthen prevention in daily life;2) tumor disease prevention,including current international popular tumor vaccine;3) of emergent public health event instance;4) occupational disease prevention measures,etc.Conclusion Increasing the contents of prevention in the clinical epidemiology teaching has made students of clinical medicine change their clinical concept and realize the key role of prevention in clinical treatment.At the same time,this study understands students' needs for the content of prevention,and provides a basis for the setting and op-timization of clinical epidemiology course in the future.
