1.The value of taking surgical margin specimens during transurethral resection of bladder tumor in the diagnosis and treatment of non-muscle invasive bladder cancer.

OBJECTIVETo evaluate the value of taking surgical margin specimens during transurethral resection of bladder tumor(TURBT) in the diagnosis and treatment of non-muscle invasive bladder cancer.
METHODSThe data of 356 patients with non-muscle invasive bladder cancer from June 2009 to January 2014 were analyzed retrospectively. A standardized protocol were performed during TURBT in 176 patients(surgical margin group), by taking surgical margin specimens from tumor base and 'normal'-appearing margin sites. The other 180 cases merely received general TURBT (general group). To observe the positive rate of surgical margin specimens and whether the diagnosis and treatment plan changed in the surgical margin group. Using Chi-square test to compare the recurrence and progression rates between surgical margin and general groups. To compare their recurrence-free survival time by Mann-Whitney U test. Results Of these 176 surgical margin specimens, the positive rate was 19. 3% (34/176),which consists of tumor base 11. 9% (21/176) and tumor normal-appearing margin 7. 4% (13/176). Following with urothelial carcinoma Ta stage in 1. 7% (3/176), T1 stage in 5. 7% (10/176), T2 stage in 8. 0% (14/176), carcinoma in situ (Tis) in 4. 0% (7/176). Among these 176 patients final diagnosis were changed in 10. 8% (19/176) patients due to the specimens results, and 18. 2% (32/176) patients altered their treatment plans. All 356 patients with a mean follow-up of 36. 8 months (6 to 60 months). Two groups of patients(surgical margin group vs. general group) recurrence rates respectively were 22. 2% (39/176) vs. 35. 6% (64/180), recurrence-free survival time were 33. 0 months vs. 23. 5 months and progression rates were 5. 7% (10/176) vs. 10. 6% (19/180). Compared with general group, patients who were taken additional surgical margin specimens showed significantly lower recurrence rate (χ2 = 7. 677, P = 0. 007) and longer recurrence-free survival time (U = 12 605,P = 0. 001). While the progression rate showed no statistical difference between them (χ2 = 2. 825, P = 0. 121).
CONCLUSIONTaking additional surgical margin specimens during transurethral resection of bladder tumor is helpful for pathological diagnosis and the planning of further treatment.

2.Effect of Passive Smoking on Clara Cell and CC16 in Rat Lung Tissue

Objective To observe the effect of passive smoking on Clara cells and secretion of Clara cell secretory protein (CC16) in rat lung tissue and to find some relations between passive smoking and chronic inflammation in the lung. Methods 40 male rats were randomly divided into 4 groups: passive smoking for 1 month (group 1), 2 months (group 2), and 3 months (group 3) and control group (n=10). The change of Clara cell ultrastructure was detected through transmission electron microscope and the lung pathology was examined by HE stain. The expression of CC16 mRNA in the lung was detected by RT-PCR. The number of Clara cells and synthesis of CC16 were detected by immunohistochemistry, the CC16 level in BALF was detected by ELISA. Results The ultrastructure of Clara cells in group 3 was destroyed greatly and the formation of pneumatocele was observed. 3.Studies on the chemical constituents from the mycelia of marine fungus Aspergillus versicolor

Objective To study the chemical constituents from mycelium of marine fungus Aspergillus versicolor.Methods The compounds were separated by column chromatography and their structures were elucidated based on chemical and spectral analysis. Results Seven compounds have been isolated from the acetone and methanol extracts from the mycelia of Aspergillus versicolor. Their structures were determined as sterigmatocystin (I), 6-methoxysterigmatocystin (II), averufin (III), tyrosine (IV), 3-methyl-pyrrolopiperazine-2, 5-dione (V), 3-isopropyl-pyrrolopiperazine-2, 5-dione (VI), carbamide (VII). Conclusion Compounds IV,V and VI were isolated for the first time from this genus of marine fungi.
4.Association of genetic polymorphism of CCNE1 and RIP2 with bladder cancer risk

Objective To evaluate the relationship between the CCNE1 or RIP2, identified at a single nucleotide poly?morphism, and the risk, clinic stage and pathological grade of bladder cancer. Methods Peripheral venous blood samples were obtained from 176 patients with bladder cancer and 210 controls without cancer. DNA was extracted. Polymerase chain reaction (PCR) method was used to detect CCNE1 (rs8102137) and RIP2 (rs42490) polymorphism. According to the postoper?ative pathological results, patients with bladder cancer were determined the grading and staging. The genotype differences of medium gene and the distribution gene were analyzed and compared in bladder cancer group and control group. The relation?ship of CCNE1 (rs8102137) and RIP2 (rs42490) genotypes and clinical data of patients with bladder cancer was analyzed, and the relationship of them with the genetic susceptibility to bladder cancer was also analyzed. Results The genotype dis?tribution was with good group representative in control group. The frequency of CCNE1(rs8102137) variant allele was signifi?cantly higher in bladder cancer group (40.91%) than that of control group (30.95%,OR=1.54,95%CI:1.02-2.45, P<0.05). The frequency of RIP2 (rs42490) variant allele was significantly higher in bladder cancer group (72.73%) than that of control group (62.38%, OR=1.61, 95%CI:1.04-2.48, P<0.05). There were no significant differences in gene polymorphisms of CC?NE1(rs8102137) and RIP2 (rs42490) between different pathological grades and different clinical stages of bladder cancer. Conclusion The CCNE1 (rs8102137) and RIP2 (rs42490) polymorphism have interaction in occurrence of bladder cancer process. There is higher risk of bladder cancer in individuals carrying mutant alleles than that of individuals carrying wild type.
5.Investigation on maternal and infant health literacy among lying-in women

Objective:
To investigate the level of maternal and infant health literacy and its influencing factors among lying-in women, so as to provide insights into formulating maternal and infant health education and promotion strategies.
Methods:
Lying-in women were sampled from Children's Hospital of Shanxi using a cluster sampling method from March to September 2022. Demographic characteristics, maternal and infant health literacy, and health education demands were collected through a questionnaire survey. Factors affecting maternal and infant health literacy among lying-in women were identified using a multivariable logistic regression model.
Results:
A total of 1 099 questionnaires were allocated, and 1 059 valid questionnaires were recovered, with an effective rate of 96.36%. Participants had a mean age of (30.93±4.01) years, 930 urban residents (87.82%), and 706 pluripara (66.67%). The overall prevalence of maternal and infant health literacy was 35.51% among lying-in women, and the prevalence rates of basic knowledge and concept, healthy lifestyles and behaviors and basic skills were 47.69%, 83.10% and 38.81%, respectively. Multivariable logistic regression analysis showed that educational level (diploma or undergraduate degree, OR=3.916, 95%CI: 1.250-9.031; master degree and above, OR=4.557, 95%CI: 1.498-11.460), occupation (company employees, OR=2.701, 95%CI: 1.385-5.268; medical staff, OR=2.981, 95%CI: 1.289-6.893), pluripara (OR=5.649, 95%CI: 3.919-8.142), participating in health education activities (OR=2.332, 95%CI: 1.524-3.570), and participating in schools for pregnant women (OR=2.252, 95%CI: 1.541-3.291) were promoting factors for maternal and infant health literacy; while gestational hypertension (OR=0.255, 95%CI: 0.133-0.488) and gestational diabetes (OR=0.318, 95%CI: 0.211-0.478) were inhibiting factors for maternal and infant health literacy. Most participants acquired health knowledge through online media (62.42%), and hoped to participate in health education activities through online media publicity (68.18%).
Conclusions
The maternal and infant health literacy level of lying-in women in this survey is associated with education, occupation, parturition frequency, participation in health education activities, participation in schools for pregnant women, gestational hypertension and gestational diabetes.
6.Effect of bone mesenchymal stem cells on toll-like receptor 2/4 expression in lungs of rats with acute hemorrhagic necrotizing pancreatitis

Objective To investigate the effect of bone mesenchymal stem cells on Toll-like receptors (TLR) 2/4 expression in the lungs of rats with acute hemorrhagic necrotizing pancreatitis (AHNP). Methods Seventy SD male rats were randomly divided into sham-operation group (n=10), AHNP group(n=30) and MSCs-treated group(n=30). Masc rate of BMSCs with surface mark were measured by flow cytometer. TLR2/4mRNA expression in the the lung were measured by RT-PCR, and The ratio of Wet/dry and lung histological changs were observed. Results TLR2/4 mRNA could be detected in the lungs with low values in sham-operation group, markedly increased in 3 h, and peaked in 12 h in AHNP group (P<0.05). Lung injuries were aggravated and the levels of TNF-α in the lung were increased (P<0. 05) . Treatment with MSCs could effectively inhibit TLR2/4 mRNA expression and relieve lung injuries. The levels of TNF-α in the lung were decreased (P<0.05). Conclusions The expression of TLR2/4 mRNA is increased in the lungs in AHNP and the lung injuries are aggravated. MSCs could markedly inhibit TLR2/4 mRNA expression in the lungs in AHNP, which would lead to relief of lung injury.
7.Diagnosis and surgical treatment of the Brucellar spondylitis

Objective To study the clinical,laboratory,imaging characteristics and surgical outcomes of Brucellar spondylitis.Methods The clinical,laboratory,imaging characteristics and surgical outcomes of 27 patients with Brucella spondylitis were analyzed in our department from September 2000 to December 2010.There were 18 males and 9 females,aged from 23 to 57 years(average,32.8 years ).There were 3 cases in cervical vertebra,8 cases in thoracolumbar vertebra,16 cases in lumbosacral vertebra respectively.Before admission,17 patients were misdiagnosed as tuberculosis,2 ones were as postoperative change of lumbar disc herniation.Eight patients underwent posterior debridement,autologous bone grafting and instrumentation,19 cases were treated with anterior debridement,autologous bone grafting and instrumentation.Streptomycin was placed in localized lesions for 24 patients,except for 3 cases whose streptomycin skin test positive.External fixation brace were used for 4 to 6 weeks,oral medication was used for a half month to 3 months.Clinical evaluation included clinical cure rate,pain visual analog scale (VAS) and the fusion rate.Results All of 27 patients were followed up for an average of 19.7 months (range,6 to 48 months).According to evaluation standard for Brucella spondylitis,24 patients were cured,3 patients got improvement.Three patients underwent the second operation owing to wound infection and sinus formation.Conclusion The pain of spine with intermittent fever,the imaging features as well as positive serum agglutination test can confirm the diagnosis of Brucellar spondylitis.Operation should be considered in patients with persistent pain and neurologic disorders as well as systemic symptoms.Instrumentation is effective and safe in the treatment of patients with spinal brucellosis following debridement,autogenous bone grafting,combined with antibiotic therapy.
8.Effects of Aqueous Extracts of Codonopsis Radix on MicroRNA Expression Profiling of D-galactose Induced Kidney of Aging Mice

Objective To explore the effects of aqueous extracts of Codonopsis Radix onD-galactose induced aging model mice; To discuss the anti-aging molecular mechanism of Codonopsis Radix.Methods Subcutaneous injection ofD-galactose solution was used to establish aging models. 100 Kunming mice were divided into normal control group, model group and low-, medium-, high-dose of Codonopsis Radix interventional groups randomly, 20 mice in each group. Low-, medium-, high-dose of Codonopsis Radix interventional groups were given relevant dosage for gavage, while normal control group and model group were given the same volume of NS by gavage for 42 d. After treatment for 42 days, the BUN and CREA in mouse serum were examined; the AffymetrixmiRNA 4.0 microarray was employed to identify the differentially expressed microRNA (miRNA) related with these processes; the bioinformatic tools were also used to further analyze the cluster of miRNA microarrys and the pathways which the target genes of miRNA were involved in.Results Compared with normal control group, the levels of BUN and CREA in mouse serum increased in model group (P<0.05); compared with model group, the levels of BUN and CREA in Low-, medium-, high-dose of Codonopsis Radix interventional groups decreased, in which high-dose of Codonopsis Radix could most significantly inhibit the level of BUN (P<0.05); the cluster analysis showed the miRNA expression profilings of high dose of Codonopsis Radix interventional group and normal control group were brought together, while the profiling of model group was clearly divided with the other groups. In model group vers normal control group, 36 differentiated expressed miRNA showed, which predicated in 10 main biological functions. In high-dose of Codonopsis Radix interventional group vers model group, 34 differentiated expressed miRNAs in high-dose of Codonopsis Radix interventional group showed, and the analytical results of biological functions of target genes were the same as those of model group vers normal control group.Conclusion In the anti-aging process, Codonopsis Radix can not only influence the miRNA expression profiling, but also influence its functional environment.
9.The clinical value of multiple bladder biopsies in the diagnosis and treatment of non-muscle invasive bladder cancer

Objective To evaluate the clinical value of multiple bladder biopsies in the diagnosis and treatment of non-muscle invasive bladder cancer (NMIBC) during transurethral resection of bladder tumor(TURBT).Methods The data of 408 NMIBC patients from January 2009 to December 2013 was analyzed retrospectively.There were 302 males and 106 females.The mean age of these 408 patients was 65 years old from 33 to 86 years.Bladder multipoint biopsies were performed in 216 patients (biopsy group),and were not performed in the other 192 patients (control group).The multipoint biopsies were taken from right and left bladder walls,anterior and posterior walls,dome,trigone,prostatic urethra and abnormal mucosa.There were 127 males and 89 females in the biopsy group,with a mean age of 64 years old (from 18 to 87 years).In the control group,118 males and 74 females aged between 15 and 92 years have an average age of 66 years old.There was no statistically significant difference in regard to gender and age between the two groups.The positive rate of biopsy and whether the diagnosis and treatment plan changed in the study group were recorded and the recurrence and progression rates were compared between study and control groups.Results Of these 216 multiple mucosa biopsies,the total abnormal detection rate was 48.1% (104/216).There were urothelial carcinoma in 12.5% (39/216),carcinoma in situ in 5.6% (12/216),dysplasia in 9.7% (21/216),cystitis in 20.4%.The final diagnosis were changed in fifteen patients (6.9%) due to the biopsy results,and 38 patients(17.6%) treatment plans were changed.The 1-,3-,and 5-year recurrence-free survival rates (RFS) of biopsy group and control groups were 96.3% vs.85.4%(x2 =14.955,P=0.000),85.2% vs.69.8% (x2 =13.183,P =0.000) and 69.9% vs.64.1% (x2 =1.574,P =0.245);progression-free survival(PFS) were 99.1% vs.96.3% (x2 =8.253,P =0.006),94.0% vs.87.0% (x2 =5.901,P=0.017) and90.3% vs.85.4% (x2 =2.273,P=0.169).The 1-and 3-year RFS and PFS of biopsy group were higher than control group.There was no significant difference in the 5-year RFS and PFS between the two groups.Conclusions Multiple bladder biopsies could be helpful for pathological diagnosis and the post-TUR treatment of NMIBC.Furthermore,it may reduce the early recurrence and progression rates of NMIBC,but have no effect on long-term prognosis.
10.A study on the relationship between biomarkers and left ventricular myocardial native T 1 value in patients with lone atrial fibrillation

Objective:To investigate the relationship between serum procollagen Ⅲ amino terminal peptide (PIIINP), collagen I carboxyl terminal cross-linking peptide (CTXI), high-sensitivity C-reactive protein (hs-CRP) and cardiac magnetic resonance (CMR) T 1 mapping value in patients with lone atrial fibrillation (AF). Methods:Fifty-five patients with lone AF in Beijing Anzhen Hospital from July 2017 to June 2018 were prospectively enrolled. Another 20 healthy volunteers were examined at the same time to provide normal reference range. All patients completed PIIINP, CTXI, hs-CRP and CMR examination within one week. CMR examination including cine, pre-contrast T 1 mapping, and late gadolinium enhancement sequences. We used t test, Mann-Whitney U test or chi square test to compare the difference of the above indexes between AF patients and the control group. Spearman correlation analysis was used to determine the associations between left ventricular native T 1 value and blood biomarker in AF patients. Results:All the patients were paroxysmal AF with an average age of (48±10) years, of which 46 (83.6%) were male. The PIIINP, CTXI, hs-CRP, left ventricular native T 1 value of AF patients were 5.83 (3.52, 12.40) ng/ml, 4.63 (3.31, 6.82) μg/ml, 3.41 (1.72, 6.61) mg/L and (1 261±23) ms, respectively, which all significant higher than those in healthy subjects (all P<0.05). The native T 1 value of left ventricular myocardium was positively correlated with PIIINP ( r=0.492, P<0.05) and hs-CRP ( r=0.516, P<0.05), but not with CTXI ( r=0.021, P>0.05). Conclusions:The PIIINP, CTXI and hs-CRP increased in patients with lone AF, PIIINP and hs-CRP were correlated with elevated native T 1 value of left ventricular myocardium.
