1.Total gastrectomy D_2 and D_2~+ procedure for upper portion carcinoma of the stomach(a report of 46 cases)

Conclusions The total gastrectomy D 2 or D 2 + with Roux en Y anastomisis procedure for the upper portion carcinoma of stomach can resect the lymph nodes completely, and postoperative complications are reduced. If the indications and techniques could be standerized, the postoperative 5 years survival rate might be increased.
2.Study on quality standard of YurongXiaocuo granule

Objective: To establish a quality control standard for YurongXiaocuo granule.Methods: TLC was used to identify Radix Puerariae,Fructus Forsythiae,Bulbus Fritillariae Thunbergii,while the content of tanshinoneⅡA was determined by HPLC.Result: The methods of qualitation and quantitate was simple,accurate and with strong specifi city,the good linear rang of tanshinoneⅡA was 3.2-32?g and the average recovery was 99.51%(RSD=2.74%).Conclusion: This method can control effectually the quality of YurongXiaocuo granule.
3.Study on MRI expression of spinal tuberculosis and brkucellare spondylitis

Objective To expore the difference between magnetic resonance image ( MRI) expression of spinal tuberculosis and brku-cellare spondylitis. Methods Retrospectively analyzed the clinical data of 10 patients with tuberculous spondylitis and 12 patients with bru-cellar spondylitis from Jan. 2012 to Oct. 2013. All the patients were scanned by MRI, and the expression difference of MRI were compared. Results The vertebral body of spinal tuberculosis was destroyed severely, and it often accompanied by the kyphosis and multiple-level para-vertebral abscess, and even adjacent organs tuberculosis. The vertebral body of brucellar spondylitis was destroyed lightly,and the abscess is often limited. Conclusion We can distinguish spinal tuberculosis and brucellar spondylitis by the typical difference of the expression of MRI.
4.Comparison of CE-MRA, T 1WI and 3D-T 1WI sequence in evaluation of brain tumor blood supply

Objective:To evaluate the diagnostic efficiency of contrast-enhanced magnetic resonance angiography (CE-MRA), T 1WI and three-dimensional thin-layer T 1WI sequences (3D-T 1WI) in evaluating the blood supply of brain tumors, so as to improve the enhanced magnetic resonance imaging (MRI) scanning scheme of brain tumors. Methods:After the contrast agent was injected, 29 patients with brain tumors were scanned with CE-MRA, T 1WI and 3D-T 1WI. The imaging manifestations of " observing the enhancement of tumor substance" , " observing the small vessels in the tumor body" and " observing the small vessels around the tumor" were qualitatively classified, and the diagnostic differences of the three sequences were analyzed by McNemar test. Results:⑴ In the aspect of " observing the contrast medium in the tumor body" , CE-MRA, T 1WI and 3D-T 1WI sequences diagnosed as grade A1 were 0 cases, 8 cases (27.59%) and 19 cases (65.52%) respectively. The diagnostic efficiency of 3D-T 1WI sequences was better than that of T 1WI sequence ( P<0.05), and T 1WI sequence was better than that of CE-MRA sequence ( P<0.05). ⑵ In the aspect of " observing the small vessels in the tumor" , CE-MRA, T 1WI and 3D-T 1WI sequences diagnosed as grade B1 were 8 cases (27.59%), 5 cases (17.24%) and 16 cases (55.17%), respectively. The diagnostic efficiency of 3D-T 1WI was better than that of T 1WI and CE-MRA ( P<0.05). ⑶ In the aspect of " observing the small vessels around the tumor" , CE-MRA, T 1WI and 3D-T 1WI sequences diagnosed as grade C1 were 18 cases(62.07%), 5 cases (17.24%) and 14 cases (48.28%) respectively. The diagnostic efficiency of CE-MRA and 3D-T 1WI wre better than that of T 1WI ( P<0.05). Conclusions:The combination of 3D-T 1WI and CE-MRA sequence can comprehensively evaluate the blood supply of the tumor, which is of great significance for the localization, characterization and treatment of brain tumors.
5.Comparison of the Clinical Efficacy and Safety of Different Restorations for Deciduous Teeth Defect

Objective:To investigate the curative effect of different restorations for deciduous teeth defect of children.Methods:67 children with teeth defecting seriously and being unable to fill restore were enrolled from January 2011 to January 2015 and randomly divided into three groups,one group of patients accepted metal crown restoration (Group A,n=22),one group adopted ceramic inlay restoration (Group B,n=22),and the last one accepted silver amalgam filling (Group C,n=23),the curative effect and incidence of adverse reactions among three groups in the period follow-up were compared.Results:In the period of 18-month follow-up,6 patients of toothache,5 patients of food impaction and crevices between restorations and teeth,3 cases of gingival congestion,2 cases of secondary caries were observed in group C;3 cases of toothache,1 case of food impaction,gingival congestion,crevices between restorations and teeth and secondary caries were observed in group B;2 cases of toothache and food impaction,1 case of gingival congestion were observed in group A.The incidence of adverse reactions in group A and group B was lower than those in group C (P<0.05),which was lower in group A than that in group B,but no significant difference was found between group A and group B (P>0.05).Conclusion:The restoration of metal crown and ceramic inlay deserved popularization had better curative effect and safety for deciduous teeth defect of children than silver amalgam filling.
6.The progress in estimation of life expectancy in elderly prostate cancer patients

Prostate cancer is a common malignancy in elderly men. Assessment of life expectancy is crucial for the treatment decision. However, accurate evaluation of individual life expectancy is still a challenge for us. By now, the life expectancy assessment methods mainly include query life table, online prediction questionnaire, gait speed monitoring, considering complications, and comprehensive prediction model. The first three evaluation methods are more convenient and have strong clinical feasibility. Complication calculation and comprehensive model are less convenient but may improve the accuracy of life expectancy prediction. The application of comprehensive prediction model is recommended at present. The prediction methods need further optimization and verification.
7.Clinical features of traumatic thoracolumbar fractures

Objective To analyze the clinical features of traumatic thoracolumbar fractures,and to improve the level of prevention and treatment of traumatic thoracolumbar fractures.Methods A review of 527 patients with traumatic thoracolumbar fractures admitted to our hospital from January 2006 to December 2015 was performed to analyze their clinical features,who were divided into different groups according to the age,fracture level and etiology.Results Of the 527 patients,238 patients aged form 40 to 59 year-old,which accounting for about 45.2%.Combined thoracolumbar and other segments fracture were presented in 76 patients,accounting for about 14.4%,among which combined thoracolumbar and lumbar spine fracture in 42 patients (8.0%),thoracolumbar and thoracic spine fracture in 22 patients (4.2%),thoracolumbar and cervical spine fracture in 7 patients and thoracolumbar(1.3%),cervical spine,thoracic spine and lumbar spine fracture in 5 patients(0.9%).The sex ratio was 136∶49 (2.8),incidence of neurological deficit was 50.3%,incidence of combined injuries was 38.9% in the 20 to 39 year-old patients.High fall,low fall and road traffic accident were the most common etiologies,accounting for 43.1%,23.3% and 14.6% respectively,other etiologies accounting for 19.0%.In the patients caused by struck by object group,the sex ratio was 52∶3 (17.3),incidence of neurological deficit was 60.0%,incidence of combined injuries was 47.3% and mean ISS score was (21.4 ± 12.7) point,the values were significantly larger than the other patients (P < 0.05).According to ASIA neurologic grading system,72 patients in grade A,17 patients in grade B,37 patients in grade C,95 patients in grade D,68 patients recovered 1 or more grades during hospitalization,which accounting 12.9% of all the patients.Conclusion The epidemiological survey of traumatic thoracolumbar fractures reveals that the age from 40 to 59 year-old and high fall was the most common age and etiology for injuries of spinal fracture.The 20 to 39 year-old patients group and struck by object group presented with the highest sex ratio,incidence of neurological deficit and combined injuries,the prevention and treatment should be designed according to the injury features of different individuals.
8.Unilateral short-segment pedicle screw fixation for treatment of thoracolumbar fractures

Objective To assess the operation results of unilateral and bilateral short-segment pedicle screw fixation combined with in-termediate screws in thoracolumbar fractures .Methods A total of 73 patients with thoracolumbar fracture were included in our study .Among the 73 patients, 48 cases were treated by bilateral short-segment pedicle screw fixation and the other 25 cases were treated by unilateral short-segment pedicle screw fixation,with a mean follow-up of 24.6 months.Surgical time,surgical blood loss,surgical draining loss,hospital stays, hospitalization cost,Cobb’s angle,kyphosis of the vertebral body ,anterior height of the fracture vertebral body ,VAS and ODI scores between the two groups were compared .Results There were significant differences in the surgical time and hospitalization cost between two groups (P<0.01).The differences in the Cobb’s angle,kyphosis of the vertebral body, and anterior height of the fracture vertebral body before and after surgery between two groups were all significant (P<0.05).There was no significant difference in the loss of the correction at the last fol-low-up between the two groups(P>0.05).Conclusion Unilateral short-segment pedicle screw fixation combined with intermediate screws can significantly correct the kyphosis and achieve the clinical effect of bilateral short -segment pedicle screw fixation technique .Meanwhile , the unilateral short-segment pedicle screw fixation technique can significantly reduce the surgical time and hospitalization cost ,which is an ef-fective method for thoracolumbar fracture .
9.Regulatory effect of bFGF and TGF-βfor the proliferation of mesenchymal progenitor cells derived from prima-ry osteoarthritis cartilage

Objective To observe the regulatory effect of bFGF and TGF-β1 for the proliferation of mesenchymal progenitor cells ( MPCs) derived from primary osteoarthritis cartilage, and to provide theoretical evidence in preventing and curing primary OA. Methods Different concentrations of bFGF and TGF-β1 ( alone or combined) were used to treat primary OA cartilage and their effects on proliferation of MPCs were tested by MTT method. Results Either bFGF (10. 0~50. 0 ng/mL) or TGF-β1 (0. 1 ~1. 0 ng/mL) alone can significantly promote the proliferation of MPCs derived from primary OA cartilage (P<0. 05). But with their increased concentration,the proliferation rate was of no significant changes (P>0. 05). The combination of 10. 0 ng/mL bFGF and 1. 0 ng/mL TGF-β1 significantly increased the prolifer-ation of MPCs from primary OA (P<0. 05). Conclusion Both bFGF and TGF-β11 play important roles in the proliferation of MPCs in primary OA cartilage,and they can increase the proliferation in different degree with different concentrations. There must be feasible methods of gene technology to promote cell proliferation and differentiation of MPCs for repairing articular car-tilage injury.
10.Effects of Different Dose Ambroxol Hydrochloride on Lung Protection of Patients in Perioperative Thoracoto-my

OBJECTIVE:To investigate the effect of different doses ambroxol hydrochloride on lung protection of patients in perioperative thoracotomy. METHODS:60 patients in perioperative thoracotomy were randomly divided into control group(30 cas-es)and observation group(30 cases). Control group received 30 mg Ambroxol hydrochloride injection from 1 week before surgery to 1 week after surgery,adding into 100 ml 0.9% Sodium chloride injection,intravenously,twice a day. Observation group re-ceived 300 mg Ambroxol hydrochloride injection from preoperative 1 week to postoperative 1 week,adding into 100 ml 0.9%Sodi-um chloride injection,intravenously,twice a day. All patients received the second-generation cephalosporin antimicrobial drugs for anti-infection after surgery. lung surfactant-associated protein A(SP-A),intercellular adhesion molecule(ICAM-1)levels preopera-tive 1 week and postoperative 1 week,and incidence of postoperative complications in 2 groups were observed. RESULTS:Preop-erative 1 week,there were no significant differences in the SP-A and ICAM-1 in 2 groups(P>0.05). Postoperative 1 week,SP-A and ICAM-1 in control group and ICAM-1 in observation group were significantly higher than preoperative 1 week,but SP-A and ICAM-1 in observation group were lower than control group,the differences were statistically significant(P<0.05),there were no significant differences in SP-A in preoperative 1 week and postoperative 1 week(P>0.05). The incidence of postoperative compli-cations in observation group was significantly lower than control group,the difference was statistically significant(P<0.05). CON-CLUSIONS:The effect of high-doses ambroxol hydrochloride on lung protection of patients in perioperative thoracotomy is signifi-cantly superior to small dose,which can reduce the incidence of lung complications.
