1.Efficacy of ambroxol combined with budesonide inhalation in the treatment and intervention of neonatal pneumonia

Objective To explore and analyze the effect of ambroxol hydrochloride combined with budesonide inhalation in the treatment of neonatal pneumonia and intervention treatment. Methods May 2014 to March 2017, Taizhou Hospital for treatment of pneumonia in infants, a total of 100 cases of the research object, according to the different treatment methods of the infant patients into the control group and the study group, 50 cases in each group. The study group were treated with ambroxol combined with budesonide inhalation treatment methods, patients in control group were treated with budesonide inhalation treatment methods, two patients were given nursing intervention on in the process of treatment, observation and comparison of two groups of children were observed two different quality methods the therapeutic effect. Results The children of patients with treatment effect was stronger than the control group, the difference is statistically significant(P<0.05). The study of the recovery time and hospitalization time were significantly less than the control group, with statistically significant difference (P<0.05). The degree of satisfaction with nursing is significantly better than the control group(P<0.05). Conclusion The treatment method of neonatal pneumonia ambroxol combined with budesonide inhalation, and implement the corresponding nursing intervention, can effectively improve the clinical symptoms of patients, improve lung function in patients, improve the therapeutic effect, shorten the patient's symptoms disappeared time and hospitalization time, children can not only improve the standard of living of the patients, but also enhance the degree of recognition of the hospital and nursing care of patients thus, play a role in the promotion.
2.Dynamic changes of serum neuron-specific enolase level in patients with transient brain ischemic attack

BACKGROUND: Neuron-specific enolase, γtype isoenzyme that is specially present in the cytoplasm of neurons and neuroendocrine cells, is considered as a sensitive predictor for neuronal damage.OBJECTIVE: To observe the changes of serum neuron-specific enolase in patients with transient brain ischemic attack, so as to explore its relationship with the degree of neuronal damage.DESIGN: Case-control observation.SETTING: Department of Neurology, Jinan No. 4 People's Hospital.PARTICIPANTS: A total of 29 patients who were hospitalized in the Department of Neurology, Jinan No. 4 People's Hospital, due to transient brain ischemic attack (all called for emergent medical treatment within the onset of 6 hours) between March 2002 and May 2004 were enrolled in this study. There were 18 males and 11 females with the average age of(60.36t11.67) years. According to the duration of neural functional deficits, all subjects were divided into two groups, namely, transient-symptom group (≤ 6 hours) of 19 cases and lasting-symptom group (> 6 hours)of 10 cases. At the same time, 25 healthy controls, 15 males and 10 females with the average age of (62.34±9.65) years, rere selected from those who came for routine health examination.METHODS: Fasting elbow venous blood of 1 mL was collected only once from the subjects in control group; the same amount of blood was collected from the patients in transient ischemic attack group immediately after hospitalization, and at days 2, 3, 4 and 5. Roche Elecsys 2010 automatic analyzer was used to detect serum neuron-specific enolase. Neuronal damage was assessed with neurological deficit scale (defined as practical recovery if scores were reduced by 90%-100%; remarkable improvement if scores were reduced by 46%-89%; improvement if scores were reduced by 18%-45%; ineffective if scores were reduced by less than 17% or even the disease aggravated).MAIN OUTCOME MEASURES: The daily changes of serum neuronspecific enolase.RESULTS: All the54 subjects remained in the final result analysis. [1]Comparison of neuron-specific enolase density: It was significantly higher in transient brain ischemic attack group than in control group [(23.53±12.35) vs(14.29±6.83) μg/L, t=2.678, P < 0.01]. [2] Curve of neuron-specific enolase changes during the acute stage: It began to increase at the early stage,reached the peak level on the next day, and gradually declined to the normal level in 4-5 days. [3] The level of serum neuron-specific enolase in the two groups with various durations of neurological deficit symptoms: It was obviously higher in transient-symptom group than in control group [(19.24±8.95)vs (14.29±6.83) μg/L, t=1.893, P < 0.05], and higher in lasting-symptom group than in control group [(28.87±13.15) vs (14.29±6.83) μg/L, t=4.367,P < 0.001]. [4] The level of neuron-specific enolase was positively correlated with the duration of neuronal damage (r=0.815, P<0.01).CONCLUSION: Serum neuron-specific enolase increases within a short term after transient brain ischemic attack and reaches the peak level at around 24-36 hours, suggesting that the detection of serum neuron-specific enolase has a guiding value in assessing the severity of transient brain ischemic attack.
3.Coping strategies and influence factors among primary glaucoma patients

Objective To explore coping strategies and influence factors in primary glaucoma patients through describing characteristics of their coping strategies.Methods From November 2007 to June 2008,data were collected from 120 primary glaucoma patients in three hospitals in Guangzhou and Shenzhen.The final sample consisted of 100 patients and the data underwent analysis.Results Among different coping styles of primary glaucoma patients,facing was lower than norm,avoiding and yielding were higher than norm.The factors related to coping style were age,different cohabitant,different economic condition,different occupation,different educational level and health payment mode.Conclusions Primary glaucoma patients usually took avoiding or yielding coping styles.Nurses should help patients with low income and education background to face the disease positively.Besides,fully consideration should be given to patients with different age,at the same time,marriage and family support should also exert active function.
4.Research in relationship of personality traits and coping strategies among primary glaucoma patients

Objective The purposes of this study were to explore the relationship between personal-ity traits and coping strategies among primary glaucoma patients. Methods 100 patients with primary glaucoma participated the investigation adopting Eysenck Personality Questionnaire (EPQ) and the Medical Coping Methods Questionnaire (MCMQ). Data underwent analysis using descriptive and Spearman rank correlation methods. Results L(lie scale) was (57.83±4.13). The primary glaucoma patients usually took different coping style from norm, facing was (16.91±2.21), avoiding was (15.69±2.01), yielding was (11.54±2.03), all was significantly different than those of norm. L was evidendy negatively correlated with facing, positively correlated with avoiding. Conclusions The personality traits of primary glaucoma pa-tients are different from norm people. They usually take avoiding and yielding coping style, while facing coping style is rarely adopted. Patients with exocentric personality and easy to communicate are apt to adopt positive coping style, while in patients who are identical in emotion and lie, they may take avoiding and yielding coping styles.
5.Molecular characteristic of dominant serotypes of Vibrio parahaemolyticus isolated from foodborne disease outbreaks in Guangdong province

ObjectiveTo study the toxin genes and pandemic group distribution as well as the genetic correlation between the major serotypes( O3:K6,O1:Kut,O4:K8 ) of Vibrio parahaemolyticus (VP) isolated from the outbreaks of Guangdong province.MethodsThe tdh and trh genes,GS-PCR and orf8 gene were detected on the 62 isolates sourced from patient and seafood occurred in the 23 outbreaks during 2008-2010.44 isolates of which were analyzed on PFGE digested by Not Ⅰ enzyme.ResultsToxin genes distributions suggested that 96.8% (60/62)isolates were tdh+,trh-.Three tdh+ isolates sourced from seafood were found.Pandemic group distribution suggested that 97.2% (35/36) O3:K6,5.88% (1/17) O4:K8,66.7% (8/9)O1:Kut serotype was GS-PCR+ and/or orf8+,respectively.PFGE analysis suggested that 44 isolates were separated into 3 clusters,of which the similarity of PFGE profile was 80.5% in the pandemic group cluster constructed by 28 isolates,the similarity between pandemic group and non-pandemic group was 59.5%.Pandemic group of O3:K6,O1:Kut as well as 04:K8 isolated on some outbreaks were processing the same PFGE profiles.ConclusionThe characteristic of toxin genes of major serotypes VP isolated in the outbreaks of Guangdong province form 2008-2010 was tdh- present and trh- absent.Within the pandemic group,O3:K6 and O1:Kut were the major serotypes.In single outbreak,isolates belongs to pandemic group but with different serotype seem to be close correlations.
6.Expression of trefoil peptide (TFF1 ,TFF3) in colonic polyps and its relationship with clinicopathological parameters

Objective To evaluate the expression of trefoil peptides (TFF1 ,TFF3) in colonic polyps and its relationship with clinicopathological parameters.Methods 120 cases of colon polyps were selected as the colon polyps group ,including 40 cases of hyperplastic polyps and 80 cases of adenomatous polyps ,and 30 cases of co-lon cancer (colon cancer group) and 20 cases of normal colonic mucosa (normal colon mucosa group) were se-lected as controls.The expression of TFF1 and TFF3 in various tissues were detected by q-RT-PCR ,Western Blot and immunohistochemistry ,and their relationship with clinicopathological parameters was analyzed.Re-sults The positive rates of TFF1 in normal colon mucosa ,hyperplastic polyp tissue ,adenomatous polyp and colon cancer were 0 ,0 ,53.8% and 80.0%,respectively and the differences were statistically significant (χ2=66.614 ,P<0.05).The positive rates of TFF3 in normal colon mucosa ,hyperplastic polyp tissue ,adenomatous polyp and colon cancer were 90.0%,77.5%,55.0% and 30.0%,respectively.and the differences were statisti-cally significant (χ2=24.688 ,P<0.05).The differences of TFF1 mRNA ,TFF3 mRNA ,TFF1 protein ,TFF3 protein expressions in the four groups were statistically significant.TFF1 protein expression in moderate and severe dysplasia polyp tissues increased significantly compared with those in mild dysplasia polyp tissues (t=2.760 ,P=0.009) ,while the expression of TFF3 significantly decreased (t=2.556 ,P=0.015) ;the expression of TFF1 protein in villous adenomatous polyp tissues increased significantly compared with that in tubular ad-enoma tissues (t=2.549 ,P=0.013) ,while the expression of TFF3 protein decreased significantly (t=2.108 , P=0.038).Conclusion The expression of TFF1 and TFF3 is closely related to the process of malignant co-lonic cancinogenesis ,and can be used as a biomarker for the differential diagnosis of colon benign diseases and early diagnosis of colon cancer.
7.Sequencing Analyses of the Hypervariable Region within the VP2 Gene of a Strain of the Aleutian Mink Disease Virus.

To analyze the molecular mechanisms of cross-host transmission of the Aleutian mink disease vi rus (ADV), the hypervariable region fragment of the VP2 gene of the ADV in Jilin Province (China) was amplified. Sequencing analyses showed diversity at residue 174 by comparison with other VP2 genes in GenBank. The phylogenetic tree indicated that the ADV-JL strain had a close relationship with the highly pathogenic strain from Denmark: ADV-K. Results implied that residue 174 may be associated with ADV infectivity.

8.NORTRIPTYLINE DELAYS DISEASE ONSET IN HUNTINGTON'S DISEASE MICE

Huntington's disease (HD) is an autosomal dominant neurodegenerative disease. A cardinal histopathologic feature of HD is the progressive loss of striatal medium spiny neurons. As there is no effective treatment for this fatal disease so far, we explore the therapeutic potential of nortriptyline to identify drugs that might be effective treatments for HD. N548mu [ 1955-128] huntingtin stable ST14A cell line was cultured and incubated in the presence or absence of serial concentrations of nortriptyline. Then R6/2 transgenic HD mice were treated with nortriptyline from five to twenty-one weeks of age. Nortriptyline protected striatal cells expressing mutant huntingtin when shifted to a nonpermissive temperature. Nortriptyline delay the disease onset to 127 d in R6/2 mice as compared with 102 d in saline-treated controls, but nortriptyline did not significantly delay mortality. As a gross marker of lack of systemic toxicity, there was no significant difference in the weight of the treated and control R6/2 mice. The results demonstrate that clinically reasonable doses of one of the identified drugs, nortriptyline, delays disease onset in a mouse model of the disease more than any previously identified compound. The most desirable features of a drug for HD are minimal toxicity and the ability to extend symptom-free living. Nortriptyline appears to be one such good candidate.
9.The effects of continuous glucose monitoring used in septic shock patients with different tissue perfusion

Objective To compare the consistency between interstitial fluid glucose and arterial blood glucose in septic shock patients with different tissue perfusion levels.Methods A prospective investigative study was conducted. Sixty-one septic shock patients with ages above 18 years old admitted to the Department of Critical Care Medicine of Peking Union Medical College Hospital, Chinese Academy of Medical Sciences from April 2013 to December 2013 were enrolled. The real-time continuous glucose monitoring system (RTCGMS) and arterial blood gas analyzer were used to measure the patients' interstitial fluid glucose and arterial blood glucose, and according to the criteria of International Organization for Standardization (ISO) and the median of relative absolute difference (Median RAD), the consistency between interstitial fluid glucose and arterial blood glucose was calculated. Based on the lactate (Lac) level and pulse oxygen perfusion index (PI), the septic shock patients were divided into groups with different degrees of tissue perfusion, the consistency between the interstitial fluid glucose and arterial blood glucose among septic shock patients with different degrees of tissue perfusion was compared by using Bootstrap re-sampling technique.Results Negative correlation existed between PI and Lac (r= -0.272,P < 0.001), which showed the opposite change tendency of organism tissue perfusion. In patients with Lac > 8 mmol/L, their consistency between interstitial fluid glucose and arterial blood glucose was better than that in those with Lac > 2-4 mmol/L, and the 95% credibility intervals (CI) of ISO standardized deviation value was 0.026-38.710 (P < 0.05). In patients with PI ≤ 0.7%, their consistency between interstitial fluid glucose and arterial blood glucose was better than that in those with PI > 0.7%-1.4%, the 95%CI of median RAD difference value was 0.002-0.076, and the 95%CI of ISO standardized deviation value was -27.000 to -0.583 (allP < 0.05); in patients with PI > 3.0%, their consistency between interstitial fluid glucose and arterial glucose was better than that in those with PI ≤ 0.7%, PI > 0.7%-1.4% and PI > 1.4%-3.0%, and the 95%CI of ISO standardized deviation values were 3.322-28.302, 11.988-40.265 and 5.170-33.333 respectively (allP < 0.05).Conclusions When septic shock patients were under low tissue perfusion (Lac > 8 mmol/L or PI ≤ 0.7%), the worse the tissue perfusion, the better the consistency between interstitial fluid glucose and arterial blood glucose; when septic shock patients were under normal local tissue perfusion (PI > 3.0%), the better the local tissue perfusion, the better the consistency between interstitial fluid glucose and arterial blood glucose.
10.Effects of naloxone at different doses on neurons of cerebral cortex in rats

BACKGROUND: Naloxone has a significant arousal effect on many types of comas. It is usually believed that this is because its inhibition on endogenous opioid peptides. But depth of coma is not necessarily positively correlated to endorphin (EP).OBJECTIVE: Based on existing findings on direct stimulating effect of naloxone on cerebral cortex, further studies need to be done to explore whether it is dose-dependent or not.DESIGN: Single-factor design based on cells.SETTING: Neurology department in a university hospital and the neurology department in a hospital of a military medical university of Chinese PLA.MATERIALS: This study was completed in the Laboratory Center of Tongji Medical College, Huazhong University of Science and Technology. Thirty healthy new born Wistar rats, regardless of their gender, aging 8 - 12 days and weighing 150 -250 g, were selected.METHODS: The experiment was performed at room temperature. The perfusion slot were placed on the microscope stage, and cells with smooth surfaces, triangle or pyramidal shapes, strong refraction and more than one neurites were selected for patch clamp experiment. Patch clamp whole-cell recording technique was used to measure the pyramidal cells of the frontal lobe immediately after separated from the Wistar rats, and to investigate the fluctuations of their membrane potential of cerebral cortex neurons and the frequencies of their spontaneous electric activities after administration of naloxone at different doses.MAIN OUTCOME MEASURES: The neural excitatory reaction rate, depolarization amplitude and increasing rate of spontaneous electric activities after administration of different doses of naloxone were selected as main outcome measurements.RESULTS: The excitatory reaction rates of cerebral cortex neurons immediately after separation to doses of naloxone(100, 50, 10, 1, 0. 1 μmol/L)were 83%, 67%, 86%, 71% and 33%; while the depolarization amplitude of them were 9. 8, 9.6, 8.4, 5.2 and 1. 3 mV respectively; and the corresponding spontaneous electric activity were increased by 587% , 375% ,291%, 125% and 69%.CONCLUSION: Naloxone can induce excitatory reactions in cerebral cortex neurons directly, and the reactions have proved to be dose-dependent.
