2.The efficacy of wet compress with flos lonicerae for cetuximab correlative erythra

Objective To evaluate the efficacy of wet compress with herbs for cetuximab correlative erythra. Methods Forty-two patients received radiochemotherapy combined with cetuximab were randomly divided into two groups. The 23 patients in the experimental group received one-week wet compress with 5g/100ml flos lonicerae twice to five times per day. While the 19 patients in the control group were given wet compress with tepid water. The efficacy on day 3 and day 7 were observed. Results The efficacy on erythra was better in the experimental group than that of control group P<0.05. Conclusion The wet compress with flos lonicerae is effective,safe and economical for the treatment of cetuximab correlative erythra,which is deserved to be applied in clinical practice.
3.Effects of Seven-step Complex Decongestion Therapy on Upper Limb Lymphedema after Operation for Breast Cancer

Objective To explore the effects of seven-step complex decongestion therapy (CDT) on post-operative upper limb lymphede-ma of breast cancer. Methods From August, 2015 to September, 2016, 71 patients with upper limb lymphedema after breast cancer surgery accepted CDT for 20 days, including skin care, opening of lymphatic pathway, relief of scar tissue, manual lymphatic drainage, bandage compression, air pressure wave therapy and functional exercise. The circumference of eight sites of both limbs was measured and the differ-ences were calculated before treatment, and one, five, ten, fifteen and twenty days of the treatment. Results The differences of circumfer-ence increased one to 15 days of the treatment (Z>2.03, P<0.05), and decreased 20 days of the treatment (Z=3.01, P<0.01). Conclusion CDT is effective on lymphedema after breast cancer surgery for 20 days of a course, but may worsen in the first 15 days, which may be relat-ed to acute stress response or redistribution of lymph.
4.Effect of Manual Lymphatic Drainage on Preventing Postoperative Axillary Web Syndrome of Breast Cancer

Objective To explore the effect of manual lymphatic drainage on prevention of postoperative axillary web syndrome of breast cancer. Methods From June to December, 2015, 400 breast cancer patients after modified radical mastectomy were randomly divided into control group (n=200) and intervention group (n=200). The control group accepted routine functional exercise on affected upper limbs, while the intervention group accepted manual lymphatic drainage in addition. The incidence of axillary web syndrome in both groups was compared one, two and three months after operation. Results The incidence of axillary web syndrome was less in the intervention group than in the control group in all the time points after operation (χ2>6.17, P<0.05). Conclusion Manual lymphatic drainage can effectively pre-vent axillary web syndrome in patients with breast cancer after modified radical mastectomy.
5.Application of improved submental island flap in hypopharyngeal cancer reserved laryngeal function surgery.

OBJECTIVE:
This study aimd to evaluate the application and clinical effect of improved submental island flap in hypopharyngeal cancer reserved laryngeal function surgery.
METHOD:
A retrospective review of clinical data was performed on 38 patients of hypopharyngeal cancer reserved laryngeal function using sumental island flaps, by the way of improving in design of vascular pedicle, reconstructive mode of laryngeal and hypopharyngeal function and closing of wound of neck following hypopharyngeal cancer resection. Meanwhile, the effect and prognosis was comprehensively assessed on patients with hypopharyngeal cancer reserved laryngeal function using improved submental island flaps.
RESULT:
The submental flaps kept alive in all 38 cases. During the follow-up period, 18 cases were dead, and of them, 7 cases died of the second primary carcinoma, included 4 cases of esophagus cancer, 1 case of cancer of soft palate, 2 cases of nasopharyngeal carcinoma; and 5 cases died of cervical or parapharyngeal lymph nodes recurrence; 2 cases died of hepatic metastasis; and 4 cases died of pulmonary metastasis. The overall 5-years survival rate was 52.6%.
CONCLUSION
Improved submental island flap repairing postoperative defect of hypopharyngeal cancer reserved laryngeal function has many advantages including higher success rate, more security, easy and simple to operate as well as good clinical effects, and is worth to widespread using.

6.Diagnosis and clinical phenotype analysis of a case with large fragment homozygous deletion of rare β gene cluster.

OBJECTIVE:
To report on a case with homozygous deletion of large β gene cluster and its clinical characteristics.
METHODS:
A total of 71 001 peripheral blood samples were subjected to capillary electrophoresis and conventional testing for common thalassemia mutations. The genotypes of suspected β gene cluster deletions were analyzed by Gap-PCR and multiplex ligation-dependent probe amplification (MLPA). Their hematological characteristics were compared by statistical analysis R software.

The carrier rate for large fragment deletions of β gene cluster in Huizhou region is rather high, for which the value of HbF is significantly increased. Attention should be paid to screening and diagnosis of rare genotype to prevent missed diagnosis and/or misdiagnosis.

7.The study of erythrocyte fragility and morphological changes caused by roller pump in vitro.

To observe the erythrocyte fragility and morphological changes of erythrocytes caused by roller pump. Ten tests were divided into two groups, Polystan pediatric pump group A (n = 5) and COBE pump group B (n = 5). Ten whole blood samples (each 400 ml) were circulated in the roller pump for 16 h. Erythrocyte fragility and free hemoglobin were measured before pumping and at every 2 hours during pumping. The possible morphological changes of erythrocytes caused by roller pump were observed by scanning electron microscope. The electron microscopic observation was made before pumping and at every 4 hours throughout pumping. Results showed that the erythrocyte fragility of two groups was not increased during a long period of pumping. The number of acanthocytes of two groups was 1.77/1.81% in the samples before pumping and 6.12/7.13, 9.18/8.73, 13.21/12.89, 16.53/17.21% at 4 h, 8 h, 12 h, and 16 h respectively. The free hemoglobin level of two groups was increased linearly during a long duration of pumping and the index of hemolysis of two groups was 0.296 mg/L/h and 0.3993 mg/L/h respectively. The result shows: 1. the erythrocyte fragility was not increased during a long period of pumping; 2. the erythrocyte membrane was injured or broken by roller pump directly; 3. the morphological changes of erythrocytes would be the basis of post operative hemolysis.

8.Clinical analysis of 23 patients with ossifying fibroma of paranasal sinuses.

OBJECTIVE:
To investigate the clinical features and treatment options of ossifying fibroma of paranasal sinuses.
METHOD:
A retrospective evaluation of twenty-three patients with ossifying fibroma of paranasal sinuses was presented. The choice of surgical operations on ossifying fibroma of paranasal sinuses was mainly decided by the location and area of ossifying fibroma. Radical operations were performed in twenty-one patients, ten of them through a lateral rhinotomy approach, eight through nasal endoscopic approach, four through Caldwell-Luc approach, one through coronal approach.
RESULT:
Two patients were performed partial resection by nasal endoscopic surgery. Diagnoses of all cases were confirmed by pathology. All patients outcomes were successful, no serious complication from the surgical technique occurred. Twenty cases were followed-up for six months to nineteen years. Two patients recurred.
CONCLUSION
Earlier diagnosis, CT scan, proper surgery, and radical resection are the keys to the treatment of ossifying fibroma of paranasal sinuses.

9.Comparison of side effects of intensity modulated radiotherapy and conventional radiotherapy in 69 cases with nasopharyngeal carcinoma.

OBJECTIVE:
To compare the side effects of intensity modulated radiotherapy(IMRT) and conventional radiotherapy in nasopharyngeal carcinoma.
METHOD:
Sixty nine cases of nasopharyngeal carcinoma were random selected by stages,with 32 cases in IMRT group and 37 cases in conventional radiotherapy group. The target areas in IMRT group were nasopharyngeal carcinoma, parapharyngeal space and neck lymphatic area with the fractional dose of 2.00-2.12 Gy per time, for 33-35 times. The cases in conventional radiotherapy group were given facio-cervical field radiation, DT 40-60 Gy per time, for 20-30 times. The reinforcing dosage in lateral facial field increased to DT 70 Gy in the nasopharyngeal area. The prophylactic irradiation dose of the neck was DT 50-55 Gy.
RESULT:
The incidence of dry mouth one year after radiotherapy in the IMRT and conventional radiotherapy groups were 9.38% (3/32) and 94.59% (35/37) respectively, with a significant difference between the two groups (P < 0.01). The incidence of difficulty in opening mouth in the IMRT and conventional radiotherapy groups were 6.25% (2/32) and 72.97% (27/37) respectively, with a significant difference between the two groups(P < 0.01).
CONCLUSION
Compared with the conventional radiotherapy, IMRT may improve the control rate and obviously de creases the side effects. It could be recommended for the radiotherapy of nasopharyngeal carcinoma.

10.Phenotypic and genotypic analysis of 45 cases with Hemoglobin Q-Thailand.

OBJECTIVETo explore hematological and molecular characteristics of Hemoglobin Q-Thailand in Huizhou area of Guangdong Province.
METHODSA total of 34 977 samples were screened by capillary and agarose gel electrophoresis. Samples suspected with HbQ strips were subjected to blood cell count and DNA sequencing. Twenty three common mutations associated with α- and β-thalassemia were identified by liquid phase chip and diversion hybridization technique.
RESULTSThe carrier rate of Hb Q-Thailand in Huizhou area was 0.13%. Pedigree analysis indicated that the Hb Q-Thailand allele is linked with a leftward single a-globin gene deletion (-α). Hematological index (HGB, MCV, MCH, HbA, HbA, HbQ) of 45 heterozygous carriers of Hb Q-Thailand were (130.25±17.37) g/L, (79.81±4.97) fl, (26.38±1.48) pg, (71.37±5.07)%, (1.65±0.45)%, (26.87±4.95)%, respectively. A statistical difference was also found in their hematological index of HbA and HbA compared with 408 heterozygous carriers of -α mutation (P<0.05).
CONCLUSIONHb Q-Thailand has a high detection rate in Huizhou area. The allele is mainly in a heterozygous status and linked with -α. The Hb Q strip can be detected by hemoglobin electrophoresis. When combined with other types of thalassemia, the heterozygotes will show unique hematological parameters.
