1.Effect of retention enema with Huanghu Decotion on infantile rotavirus enteritis

Objective To study the effect of retention enema with Huanghu Decotion on infantile rotavirus enteritis and summarize the nursing strategies.Method One hundred and sixty nine infants with infantile rotavirus enteritis were randomly divided into observation group(n=86)and control group(n=83).On the basis of conventional treatment,the observation group was treated with retention enema with Huanghu Decotion and the control group with Smecta 3 d for a course of treatment.The two groups were compared in terms of the total effective rate.Results There was significant difference in the total effectives rate between the two groups(P<0.05).The rate in the observation group was highter than that in the control group.Conclusions Retention enema with Huanghu Decotion is superior to that by Smecta in treating infantile rotavirus enteritis.The comprehensive nursing care is helpful for the improved curative compliance and therapeutic effect.

Objective To study the pathogenesis of avascular necrosis of femoral head(ANFH) in young adults.Methods The data of DSA of femoral head(113 femoral heads) with ANFH proved by clinic or radiology and treated by interventional radiology and 45 normal peoples(46 femoral head) were comparatively analysed.Results The variation of artery of femoral head in interventional treating group was appeared in 28 cases(35.9%),51 arteries involved,including 17 internal iliac arteries variation(33.3%),29 medial femoral circumflex arteries variation(56.9%),4 lateral femoral circumflex arteries variation(7.8%),1 lateral and medial femoral circumflex arteries common bole variation(2.0%).The variation of artery of femoral head in control group was appeared in 5 cases,5 arteries involved.There was significant difference between the two groups(P
3.Modern Literature-based Analysis of Regularities in Analogous Points for Acupuncture and Moxibustion Treatment of Restless Legs Syndrome

Objective To analyze regularities in selection of analogous points for acupuncture and moxibustion treatment of restless legs syndrome in modern Chinese literature. Three large Chinese databases, China National Knowledge Infrastructure (CNKI), Wan Fang database (Wanfang) and VIP Database (VIP), were searched by computer to retrieve literature on acupuncture and moxibustion treatment of restless legs syndrome. A database of acupuncture and moxibustion prescriptions was set up. The characteristics of and regularities in selection of analogous points for acupuncture and moxibustion treatment of restless legs syndrome were analyzed by use of data mining techniques. Results A total of 107 related articles were included, involving 105 selected acupoints. The main analogous points for modern acupuncture and moxibustion treatment of restless legs syndrome are Zusanli, Sanyinjiao, Chengshan and Yanglingquan. The main meridians that the analogous points belong to are the Bladder Meridian of Foot-Taiyang, the Gallbladder Meridian of Foot-Shaoyang and the Spleen Meridian of Foot-Taiyin. Most of these acupoints are distributed on the lower limb and the lumbodorsal region. The most frequently used specific points that the analogous points belong to are Five Shu points and lower He-Sea point. Association rule analysis shows that the points that are most highly correlated are Yanglingquan and Zusanli. Attention is paid to acupoint selection based on syndrome differentiation and compatible application of local acupoints.Conclusions Selection of analogous points that have the same or similar indications for modern acupuncture and moxibustion treatment of restless legs syndrome makes acupoint combinations produce a synergistic effect and can improve the clinical therapeutic effect.

ObjectiveTo study the correlation with Genes Coding forESBLs and ClassⅠIntegron in ESBLs-producing Escherichia coli from children.MethodsPCR was used for gene typing detection of 100 strains of ESBLs-producingEsche-richia colistrains. While detection of class I integrase gene in the 100 strains ESBLs-producingEscherichia coli and 100 strains of non-ESBLs producingEscherichia coli were separately performed by PCR. It provides the solid base not only to reveal the relationship between class I integron and drug resistance, but also the relationship between class I integron and ESBLs-producing. ResultsThe most frequently genotyping from ESBLs-producingEscherichia coli in children isCTX-M (84%), followed by TEM-1(63%). The predominant distribution of genotype in ESBL- producing strains isTEM-1 +CTX-M (45%), followed by CTX-M (34%). Class I integrase gene detected in ESBLs- producing and non- ESBLs producing strain were 100 cases (100%) and 25 cases (25%) separately, the difference was statistically signiifcant (P<0.05); drug resistance in class I integron positive strains were signiifcantly higher than in class I integron negative strains, especially in Ciprolfoxacin, Levolfoxacin, and Amino-glycoside (86.4%, 88%, and 80%).ConclusionsThe distribution of Class I integron in ESBLs-producingEscherichia coli is signiifcantly higher than that in non-ESBLs-producing strains, It is rational that Class I integron highly correlate with strong drug resistance in ESBLs-producing strains.
5.Mini-invasive surgical treatment of acute Achilles tendon rupture by Achillon

Objective To evaluate the clinical efficacy of Achillon,a novel guide for Achilles tendon suture produced by Newdeal Ltd in France,in the mini-invasive surgical treatment of acute Achilles tendon rupture. Methods Between December 2010 and March 2011,7 patients(7 feet) with acute Achilles tendon rupture were treated in our department.They were 5 males (5 feet) and 2 females (2 feet),with an average age of 34.2 years (range,21 to 53 years).The interval between injury and operation ranged from 1 to 11 days(3.8 days on average).A longitudinal incision approximately 2.0-3.5 cm in length was made around the ruptured Achilles tendon for minimally invasive repair after insertion of the Achillon.Postoperative rehabilitation was carried out. Results All the wounds healed at the first stage.All patients were followed up for 5 to 11 months, with an average of 7.2 months. All patients regained normal weight-bearing walking 3 months after operation without any re-rupture. According to the ankle-hindfoot scoring system of American Orthopaedic Foot and Ankle Society (AOFAS),they scored 93.2 points.By the visual analogue scale(VAS),they scored 1.2 points at the operation site. Conclusion The surgical treatment of acute Achilles tendon rupture with Achillon has advantages of mini-invasion,a low rate of incision problems and quick function recovery.
6.Correlation between peripheral nerve injury and evoked electroneurographic changes in early stage

BACKGROUND: The injury of peripheral nerve may cause electroneurographic changes. However, what is the correlation between electroneurographic changes and the extent of nerve injury?OBJECTIVE: To probe into the changes of evoked electroneurography in early stage after nerve injury for clinical application and reference.DESIGN: Paired t-test in three groups of samples.SETTING: Department of Traumatic Orthopaedics People' s Hospital of Beijing University.MATERIALS:The trial was conducted in the Animal Center of the People's Hospital of Beijing University from August 2003 to December 2003. The subjects were 8 adult New Zealand rabbits of SPF grade(weighing 1.5 kg) and either gender from the Animal Center of Medical College of Beijing University.METHODS:The model of nerve injury was made in the 16 tibial nerves of the 8 rabbits, in which one third or two thirds of each nerve was ligated at the same transverse part. The samples were thus divided into three groups: non-injury (normal) group, 1/3 nerve injury group and 2/3 nerve injury group.MAIN OUTCOME MEASURES: The indices of evoked electroneurography, i. e. motor nerve condution velocity, negative M wave amplitude, negative M wave area, maximal M wave amplitude and maximal M wave area were measured in the three groups. The correlation between the extent of nerve injury and the 5 indices was analyzed.RESULTS: All the 5 indices of the 3 groups were significantly different( P< 0.05). The mean values of the 5 indices were 1.00 ±0.0,0.97 ±0.18,1.01 ±0.22;0.98 ±0.16 and 0.98 ±0.19 in normal group, 0.76 ±0.12,0.41 ± 0.24, 0.47 ± 0.30, 0.48 ± 0.27 and 0.50 ± 0.29 in 1/3 nerve injury group, and 0. 62±0. 10, 0. 11 ±0. 11, 0. 11 ±0. 10, 0. 12 ±0. 14 and 0.13± 0. 12 in 2/3 nerve injury group. The coefficient of correlation between the 5 indices and the injury were 0. 902, 0. 938, 0. 936, 0. 907, and 0. 914, respectively.CONCLUSION: In the early stage after peripheral nerve injury, the electroneurographic indices negatively correlate with the extent of nerve injury. The negative M wave amplitude and area are relatively more closely correlated with the extent of injury.
7.Role of PPARγ in the hematogenous spread of hepatocellular carcinoma after hepatic ischemia reperfusion in mice

Objective To investigate the role of peroxisome proliferator-activated receptor γ (PPARγ) in hematogenous spread of hepatocellular carcinoma after hepatic ischemia reperfusion in mice and its mechanism.Methods One hundred and sixty mice were randomly divided into 4 groups,sham,control,Rosiglitazone(R group) and Rosiglitazone + GW9662 (R + GW).The mice models with hepatic ischemia reperfusion combined with portal vein metastasis of hepatocellular carcinoma were well established.Serum ALT level,expressions of MMP-9,NF-κB and PPARγ,hepatic replacement area (HRA) and survival of mice were compared.Results (1) The median survival in sham group was 16.3 d,R group 12.1 d,control group 9.6 d,R + GW group 8.7 d.(2) Impact on portal venous metastasis:compared with left hepatic lobe (ischemic hepatic lobe) of control group,the HRA was significantly decreased in the left hepatic lobe of sham group (29.1% vs.13.2%,P <0.05).Tumor load was higher in control group than R group (29.1% vs.13.0%,P < 0.05).(3) Serum ALT level:after 2 h,8 h and 24 h hepatic ischemia reperfusion injury (HIRI),the ALT levels in control group [(1 134.2 ± 320.5) U/L],R group [(1 017.3 ± 365.9)U/L] and R + GW group [(1 344.0 ± 304.3) U/L] were all higher than sham group [(20.6 ± 7.8) U/L],P <0.05.With 8 and 24 h HIRI,ALT levels were highest in R + GW group [(4 101.7 ± 462.2) U/L,(3 730.8 ± 582.7) U/L],following by control group [(3 649.1 ± 440.1) U/L,(2 226.7 ± 442.7) U/L],andRgroup [(1691.9±398.6)U/L,(1 109.2±237.4)U/L],P<0.05.(4) MMP-9 expression:after 8 h HIRI,MMP-9 expression level was predominantly elevated in control group than R group [(41.3 ± 10.7) vs.(4.7 ± 1.1),P < 0.05].Similarly,MMP-9 expression was higher in R + GW group than both control and R groups [(166.9 ± 7.9) vs.(41.3 ± 10.7) and (4.7 ± 1.1),P < 0.05].(5) Expressions of PPARγand NF-κB:in the control group,PPARγ expression emerged after 2 h HIRI,and reached the peak with 8 h HIRI,decreased significantly with 24 h HIRI.NF-κB expression elevated with time,and at the peak with 24 h HIRRI.In R + GW group,the PPARγ expression was similar to control group and high expression of NF-κB were detected at all three endpoints.In R group,marked expression of PPARγ was observed after 2 h HIRI,and reached to peak after 24 h HIRI.NF-κB showed weakly positive expression after 2 h HIRI.Conclusions Rosiglitazone could significantly reduce hematogenous spread of hepatocellular carcinoma after hepatic ischemia reperfusion in mice.This may be attributed to NF-κB expression inhibition by PPARγ up-regulation and decreased MMP-9 production after pretreatment with Rosiglitazone.

AIM: To explore the cytokines level and the discrepancy of reaction to dexamethasone (Dex) in ALI rats induced by hydrochloric acid (HCl) and lipopolysaccharide (LPS). METHODS: Ninety-six SD rats were divided into six groups at random (n=16 in each group): NS group, HCl group, LPS group, NS+Dex group, HCl+Dex group and LPS+Dex group. Every group was divided into two subgroups: the bronchoalveolar lavage (BAL) subgroup and no bronchoalveolar lavage (NBAL) subgroup. The total leukocytes, PMN%, macrophage%, lymphocyte%, total protein in BALF and the wet/dry of the lung weight were measured. The concentrations of TNF-?, IL-1?, IL-4 and IL-10 in serum and BALF in every group were compared. 9.Long-term outcome of neoadjuvant chemoradiotherapy based on the depth of invasion in mrT3 low rectal cancer

Objective:To investigate the prognosis of cT3 and the subgroups of low rectal cancer patients who underwent neoadju-vant chemoradiotherapy (CRT) and evaluate whether all patients with cT3 low rectal cancer should undergo CRT. Methods:A total of 223 patients with cT3 low rectal cancer treated in the Department of Colorectal Surgery of Fujian Medical University Union Hospital from January 2008 to December 2012 were divided into neoadjuvant chemoradiotherapy group (CRT group) (115 cases) and no neoad-juvant chemoradiotherapy group (nCRT group) (108 cases) according to whether the patients underwent CRT. Afterward, the patients were retrospectively divided into three subgroups (mrT3a, mrT3b, and mrT3c) according to the proposed criteria of the Radiologic Soci-ety of North America (RSNA) by measuring the depth of mesorectal invasion (DMI) (DMI<5, DMI=5-10, and DMI>10 mm). The prog-noses of the two groups and their subgroups were compared. Results:The CRT and nCRT groups revealed no significant differences in the 3-year disease-free survival rate and the local recurrence rate for all the mrT3 patients (78.2%vs. 71.9%, P=0.608;4.4%vs. 8.5%, P=0.120) and mrT3a patients (82.4%vs. 81.8%, P=0.837;5.8%vs. 5.9%, P=0.658). On the contrary, for the mrT3b patients, the CRT and nCRT groups revealed significant differences in the 3-year disease-free survival rate (84.4%vs. 42.4%, P=0.032) and local recurrence rate (0.0%vs. 18.2%, P=0.014). For the mrT3b,c patients, the CRT and nCRT groups revealed no significant difference in the 3-year dis-ease-free survival rate (72.8%vs. 42.4%, P=0.060) but revealed a significant difference in the local recurrence rate (2.4%vs. 18.2%, P=0.021). COX regression analysis was utilized for 3-year disease-free survival, DMI and circumferential resection margin (CRM) were significant in the univariate analysis. Additionally, the multivariate analysis indicated that CRM is an independent impact factor (OR=2.249, CI 1.067-4.742, P=0.033). Conclusion:CRT can improve the prognosis of patients with mrT3b,c low rectal cancer but may not significantly influence the prognosis of patients with mrT3a and CRM-negative low rectal cancer;surgical treatment can be performed in these patients without CRT.
10.Serum levels of insulin-like growth factor-1 and insulin-like growth factor-binding potein-3 in Henoch-Schonlein purpura

Objecfive To investigate the role of serum Insulin-like growth factor(IGF)-1,insulinlike growth factor-binding potein(IGFBP)-3 in children with Henoch-Schonlein purpura(HSP).Methods The serum concentration of IGF-1,1GFBP-3 was measured by enzyme-linked immunosorbent assay(ELISA)method in 45 acute SHP patients,40 recoverv patients and 30 healthy controls.Results The serum levels of IGF-1 [(452±183)μg/L],IGFBP-3 [(13 897±3124)μg/L] and C-reactive protein(CRP)[(20±8)mg/L]in acute phase were significantly higher than those in healthy controls(P<0.0 1)and higher than those during recovery period.The serum level of IGF-1,IGFBP-3 for the HSP patients dropped back slowly and their levels during recovery period were the same as those in healthy controls(P>0.05).The serum levels of IGF-1[(621±253)μg/L] and IGFBP-3[(18 763±3173)μg/L] were higher in the renal damage group than in the non-renal damage group(P<0.01).and the same in patients with gastrointestinal symptoms group as in patients without gastrointestinal symptoms group(P>0.05).whereas the serum level of CRP was not significantly different(P>0.05).The serum levels of IGF-1,IGFBP-3 showed positive correlation with the level of CRP(r=0.624,0.672,P<0.01).Conclusion The IGF-1 and IGFBP-3 may play an important role in the pathological mechanism of HSP.The level of IGF-1 may be used as an indicator for HSP disease activity and progression.IGF-1 mav have a close relation with the damageof renaJ system in HSP.
