1.Epidemiological analysis of esophageal cancer in Kashgar region in 2015

Objective To investigate the incidence and epidemiological characteristics of esophageal cancer in Kashgar region,and to provide theoretical basis for the comprehensive prevention and treatment of esophageal cancer.Methods A total of 1 815 cases of esophageal cancer were collected from January 2015 to December 2015.The morbid feature was analyzed and fitting-conditional Logistic model multivariate analysis was used to infer the influence factors.Results In 1 815 cases of esophageal cancer,the sex ratio was 2.4/1.0,and there were more men than women (x2 =5.90,P =0.015).Age of onset was mainly focused on 51-70 years old.The incidence rate of Shule County was the highest.Logistic model multivariate analysis showed that esophageal cancer was closely related to the three factors:fast food intaking speed (x2 =17.756,P < 0.001),hot eating habit (x2 =8.234,P =0.003) and history of stomach diseases (x2 =14.421,P < 0.001).Conclusion The morbidity of esophageal cancer maintains a high level in Kashgar region,which may be related to the local residents life habits,history of stomach diseases and other factors.Comprehensive strategy of prophylaxis and treatment must be taken.
2.Effect of fluid acupuncture therapy combined with spectrum therapy for relieving pain due to chronic soft tissue injury

BACKGROUND: Simple acupuncture therapy often fails to produce significant relief of pain induced by chronic soft tissue injury, but liquid acupuncture therapy, with its longer action on the acupoint and the synergistic effect of the drugs, may, at least theoretically, have better effect.OBJECTIVE: To observe the effect of fluid acupuncture therapy with compound Danshen injection solution combined with spectrum therapy for relieving pain caused by chronic soft tissue injury.DESIGN: A case-control observationSETTING: Health Clinic, College of Engineering of Nanjing Agricultural UniversityPARTICIPANTS: Ninety-four outpatients with chronic soft tissue injury treated in the Health Clinic of College of Engineering, Nanjing Agricultural University between May 2001 and November 2004 participated in this experiment on a voluntary basis. The patients were divided into liquid acupuncture therapy combined with spectrum therapy group (Combined treatment group)and acupuncture control group, with 47 patients in each group.METHODS: In combined treatment group, the acupuncture was administered on the acupoints offengchi (GB 20), tianzong (SI11), quyuan (SI13),ashixue, beishuxue, and jiajixue. Compound Danshen injection solution was injected into each acupoint 2 to 3 cm. Two acupoints were chosen from each region of pain for injection every other day, for 30 days for a treatment course. After the injection, the sites of injection were subjected to spectrum therapy for 30 minutes every other day, for 30 days for a treatment course. One course of acupuncture and spectrum therapy was administered separately. In the acupuncture control group, the above acupoints were chosen for simple acupuncture therapy in identical treatment course.Visual anologue score (VAS) was used for evaluation of the therapeutic effects before and 1 course after the therapy. Remarkable effect was defined by reduction rate of VAS >80% after therapy, improvement by VAS reduction by 20% and 80%, and no effect by VAS reduction by < 20%.MAIN OUTCOME MEASURES: VAS score of the patients in the twogroups before and 1 course after therapy, and curative effect 1 treatment course after therapy.RESULTS: VAS of the patients in the acupuncture control group and combined therapy group was significantly decreased after 1 course of therapy in comparison with that before therapy (1.30±0.90 vs 6.57±1.21 for the former and 0.50±0.40 vs 6.64±1.13 for the latter, t=23.958 and 35.116, P < 0.01).Reduction rate of the score was significantly higher in combined therapy group than in the control group [(93.14±10.12)% vs (80.13±11.08)%,t=8.764, P < 0.01], and so was the total effective rate (remarkable effect rate and improvement rate, 93.6% vs 78.7%, χ2=4.644, P < 0.05 )].CONCLUSION: Fluid acupuncture with compound Danshen injection solution in combination with spectrum therapy can significantly relieve the pain caused by chronic soft tissue injury, with better effect than simple acupuncture therapy.
3.Kv7 channels in modulation of central monoaminergic neurotransmission

The KCNQ gene family encodes five Kv7 channel sub-units(Kv7. 1-5) and four of these (Kv7. 2-5) channels are ex-pressed in the central nervous system ( CNS ) . Neuronal Kv7 channels participate in both pre-and post-synaptic modulation of neurotransmissions. Activation of neuronal Kv7 channels inhibits neuronal excitability and limits the release of monoaminergic neu-rotransmitter. Kv7 channel openers provide novel therapeutic op-tions in the treatment of disease states characterized by overactiv-ity of monoaminergic neurons ( e. g. schizophrenia, anxiety and drug abuse) . This review summarizes the studies on expression and functional role of Kv7 channels in monoaminergic neurotrans-mission in the CNS and the potential therapeutic effect of Kv7 channel openers on the disease characterized by overactivity of monoaminergic neurons.
4.Clinical Observation of Fludarabine、 Intermediate-dose Ara-C and G-CSF in Treatment for Refractory or Relapsed Acute Myeloid Leukemia(26 Cases)

Objective To observe the therapeutic effects and side effects of FLAG regimen(fludarabine, Ara-C, and granulocyte-colony stimulating factor( C-CSF) for refractory or relapsed acute myeloid leukemia( AML). Methods Twenty-six AML were treated with fludarabine plus intermediate-dose Ara-C and G-CSF,of whom 15 cases belonged to refractory and 11 cases belonged to relapsed. Results After two courses of treatment, 14 cases were completely relieved (53. 8% ) and 5 cases were partially relieved (19.2% ). The overall effective rates was 73.1%. The main side effects were severe myelosuppression and non-hematological toxicity was mild. Conclusion FLAG regimen was very effective for refractory or relapsed acute myeloid leukemia and was well tolerated. The treatment-related mortality rate was low,so it provided a treatment choice for these patients.
6.The expression of miR-451 and its clinical significance in gastric cancer

Objective To investigate the expression of miR-451 and its clinical significance in gastric cancer. Meth?ods The miR-451 expression was detected in 92 samples of gastric cancer and 47 sapmles of normal stomach mucosa. The correlations of miR-451 expression with clinicopathologic characteristics, overall survival and disease-free survival were an?alyzed in patients with gastric cancer. Results The overexpression rate of miR-451 was 38.04%in gastric cancer, which was significantly lower than 76.60%in adjacent normal tissues (χ2=18.503,P<0.001). The overexpression rate of miR-451 was significantly lower inⅢ-Ⅳstages of gastric cancer than that inⅠ-Ⅱstages (27.3%vs 54.1%, P=0.009). The expres?sion of miR-451 was also lower in patients with lymph node metastasis than that in patients without lymph node metastasis (27.1%vs 57.6%, P=0.004). However, there were no significant differences in miR-451 expressions between different age, gender, histological grade and T stage groups (P>0.05). The overall survival rate was significantly higher in patients with high expression of miR-451 than that in patients with low expression of miR-451(69.2%vs 39.1%, P=0.003). Conclusion miR-451 is associated with the malignant transformation, progression, and prognosis of gastric cancer, and which can be used as a potential target of therapy and a biomarker of diagnosis and prognosis of gastric cancer in the future.
7.Comparison between interventions of point-injection integrated with electric acupuncture and simple oral medication on primary osteoporosis

BACKGROUND: There are many clinical applications of oral medication in treatment of primary osteoporosis, but there are few reports on integration of point injection and electric acupuncture in treatment of primary osteoporosis.OBJECTIVE: To observe the effects of intervention with point injection and electric acupuncture on the basis of western drug on primary osteoporosis and to compare the therapeutic effects with simple western medical treatment.DESIGN: Case comparison was designed.SETTING: Hygiene Clinic of Engineering College of Nanjing University of Agriculture and First Affiliated Hospital to Nanjing University of Medical Sciences.PARTICIPANTS: Totally 46 cases of primary osteoporosis were selected from the outpatients in Hygiene Clinic of Engineering College of Nanjing University of Agriculture from April 2000 to June 2004 and all of the cases were in the observation voluntarily. Two groups were randomized according to visiting sequence, named acupuncture+drug group (23 cases), aged (67±4) years and drug group (23 cases), aged (66±5) years.METHODS: ① In acupuncture+drug group, on the basis of medication of alfacalcidol and calcium carbonate, the integrative therapy of point injection and electric acupuncture was applied. Pont injection: Xue-saitong zhusheye was injected on zusanli (ST 36), iaixi (KI 3), sanyinjiao (SP 6) and xuanzhong (GB 39) 1.67-3.33 cm in depth. Two points were selected each time and 2 mL on each point, once/day and the points were injected bilaterally and alternatively. Electric acupuncture:The points were pishu (BL21), shenshu (BL 23) and qihaishu (BL24).Electric apparatus was attached on the points after arrival of qi, with continuous wave, 2 Hz, 30 minutes, once a day. One month treatment for either point injection or electric acupuncture was taken as one course.The second course was followed afterbreak for 5 days. Totally, 6 months were required. Western medical treatment: α Vit. D3 0.5 μg per day was administrated orally, onceaday and calcium carbonate, 1 tablet, once a day,continuously for 6 months. ② Drug control: Only western drugs were administrated orally and the usage was same as the acupuncture +drug group. The patients in two groups were treated for 6 months. Bone mineral density (BMD) of L2-4 and upper end of femur was assayed with X-Ray absorptiometry.MAIN OUTCOME MEASURES: BMD of L2-4 and upper end of femur before and 6 months after treatment in two groups.RESULTS: All of 46 cases of primary osteoporosis entered result analysis, no dropped out case. ① Comparison of BMD of lumbar vertebras before and 6 months after treatment in two groups: After 6-month treatment,in acupuncture +drug group, BMD of L2-4 was improved significantly [(0.829±0.049), (0.791±0.043) g/cm2, (t=2.80, P < 0.01)] compared with that before the treatment. In drug control, BMD of L2-4 was improved significantly [(0.813±0.051), (0.788±0.048) g/cm2, (t=1.71, P < 0.05)]compared with that before the treatment. Concerning to the variations of BMD between two groups, the variation in acupuncuture+drug group was more than that in drug control [(0.038±0.020), (0.025±0.025) g/cm2 (t=1.95,P < 0.05)]. ②) Comparison of BMD of femur before and 6 months after treatment in two groups: There was no significant difference in BMD before and after treatment in two groups (P > 0.05).CONCLUSION: Compared with simple western drug treatment, the integrative therapy of point injection and electric acupuncture improves very significantly BMD of lumbar vertebras, aiming at effective treatment of primary osteoporosis. But the change in BMD of femur is not obvious before and after treatment, which is probably associated with lack of adequate blood circulatiou in femoral region.
8.Clinical application of DKK-1,TFF3 and CA72-4 detection in diagnosis of gastric cancer

Objective To explore the clinical application of DKK-1,TFF3 and CA72-4 detection in the diagnosis and treatment of gastric cancer.Methods Seventy-five cases(gastric cancer group) of gastric cancer admitted to our hospital from January 2013 to May 2015 were selected.Seventy cases of benign gastric disease(benign gastric disease group) and 70 persons undergoing the physical examination(healthy control group) were selected as the research subjects.The concentration of CA72-4 in each group was detected by the electrochemiluminescence analyzer.The serum DKK-1 and TFF3 levels in each group were detected by enzyme linked immunosorbent assay(ELISA).The receiver operating characteristic(ROC) curve was drawn for evaluating the diagnostic efficiency of each index in each group.The sensitivity,specificity,positive predictive rate and negative predictive rate of 3 indicators for diagnosing gastric cancer before operation were compared.The concentration change of various indexes after gastric cancer radical resection were compared..Results The concentrations of DKK-1,TFF3 and CA72-4 in the gastric cancer group were significantly higher than those in the benign gastric disease group and healthy control group,the difference was statistically significant(P<0.05).The area under ROC curve of DKK-1,TFF3 and CA72-4 were 0.876(95%CI 0.803-0.950),0.944 4(95%CI 0.894-0.975) and 0.818(95%CI 0.726-0.884) respectively.The sensitivity of CA72-4 was 78.6% and the specificity was 84.3%,the sensitivity of DKK-1 was 93.6% and the specificity was 87.1%,the sensitivity and specificity of TFF3 was 92.4% and 90.1% respectively.The concentrations of DKK-1,TFF3 and CA72-4 after radical resection in the gastric cancer group were significantly reduced,the difference was statistically significant compared with before treatment(P<0.05).Conclusion The detection of DKK-1,TFF3 and CA72-4 has a certain clinical value for the diagnosis of gastric cancer.The combined detection of these 3 indicators is conducive to improve the specificity and sensitivity of gastric cancer diagnosis.
9.Analysis of depression rates among elderly hypertensive patients and healthy residents and its risk factors in Fuzhou

Objective To investigate the rates of depression and its risk factors in elderly hypertensive patients and healthy residents in Fuzhou.Methods 882 elderly primary hypertensive patients aged 60 years and over and 1230 healthy elderly residents were assessed by the geriatric depression scale (GDS) and a self-made general information questionnaire, and SPSS 16.0 was used to analyze data.Results The average score of depression and the incidence of depression were higher in elderly hypertensive patients than in healthy elderly residents [(12.43±5.01) vs.(11.53±5.13), 64.4% vs.55.3%, t=-3.849, x2=16.386, P<0.01].Compared with healthy controls, scores of depression were higher for all age groups in elderly married male patients, with low levels of education (junior middle school or below), a monthly income less than 2000 yuan, and partial or no medical insurance coverage (P< 0.05 for all).Scores of depression were higher in elderly hypertensive patients who had smoking or drinking habits, social activity, hobby, or physical exercise than in healthy controls, (P<0.05 for all).Scores of depression were also higher in elderly hypertensive patients who had family support, barely enough or adequate family expenses or family support than in healthy controls, regardless of living conditions (P<0.05 for all).Multiple stepwise regression analysis showed that risk factors for depression in elderly hypertensive patients were family support, age, medical insurance coverage, family expenses, living conditions, hobbies, and gender, while risk factors for depression in healthy elderly residents were family support, interests, medical insurance coverage, and age.Conclusions The incidence of depression is higher in elderly hypertensive patients than in health elderly people in communityin Fuzhou.Family support, hobbies, medical insurance coverage and age are the common risk factors for depression in both elderly hypertensive patients and healthy residents.Family expenses, living conditions and gender are important risk factors for depression in elderly hypertensive patients.
10.Effects of hydrogen rich water on the expression of Nrf 2 and the oxidative stress in rats with traumatic brain injury

Objective To investigate the effects of hydrogen rich water on the expression of nuclear factor erythroid 2-related factor 2 (Nrf2) and oxidative stress in rats with traumatic brain injury (TBI).Methods Ninety healthy male Sprague-Dawley (SD) rats were randomly divided into sham operation group, TBI group and hydrogen rich water treatment group (HW group), with 30 rats in each group.TBI model was reproduced by the modified Feeney weight dropping method.The skulls of rats in sham operation group underwent only craniotomy without direct hit.The rats in HW group received brain injury by hitting after craniotomy, followed by injection of hydrogen rich water (5 mL/kg) intraperitoneally once a day for 5 days after successful reproduction of the model.The rats in sham operation group and TBI group were given an equal amount of normal saline in same manner.Six rats from each group were sacrificed at 6, 12, 24, 48 hours and 5 days after evaluating neurological severity scores (NSS).The brain tissue in injured ipsilateral cortex was harvested.The activity of catalase (CAT), glutathione peroxidase (GSH-Px), and content of malondialdehyde (MDA) were determined by spectrophotometry.The expressions of mRNA and nucleoprotein of Nrf2 were determined by quantitative real-time polymerase chain reaction (RT-qPCR) and Western Blot.The pathological changes were observed with microscopy after hematoxylin and eosin (HE) staining.Results ① NSS score:compared with TBI group, NSS in HW group at 12, 24, 48 hours and 5 days were significantly decreased (12 hours: 9.83±2.32 vs.13.17±2.71, 24 hours: 9.83±2.79 vs.13.50±2.43, 48 hours: 7.50±2.07 vs.11.83±2.14, 5 days:5.50 ± 1.87 vs.10.50 ± 2.43, all P < 0.05).② Compared with sham operation group, the activity of GSH-Px and CAT in TBI group were markedly declined after operation, while the MDA content was elevated significantly, especially at 24 hours [CAT (kU/g): 1.080±0.312 vs.3.571 ±0.758, GSH-Px (kU/g): 9.195±3.173 vs.32.385± 10.619, MDA (μmol/g): 12.282±2.896 vs.4.349± 1.511, all P < 0.01].Compared with TBI group, the parameters in HW group were improved, and they were similar as sham operation group.③ RT-qPCR: no significant difference was found in the expression of Nrf2 mRNA at each time point in three groups.④ Western Blot: the expression of Nrf2 nucleoprotein (gray value) in TBI group was apparently higher than that in sham operation group, and peaked at 24 hours (0.703 ± 0.262 vs.0.238 ± 0.120, P < 0.05), and the expression in HW group was obviously higher than that in TBI group, especially at 24 hours (1.110 ± 0.372 vs.0.703 ± 0.262, P < 0.05).⑤ HE staining: the brain structure in sham operation group was found to be intact.However, there were different degrees of pathological changes at each time in TBI group, especially at 24 hours.The pathological damage of brain tissue in HW group was significantly milder.Conclusions Hydrogen rich water can up-regulate the expression of Nrf2, and reduce oxidative damage of traumatic brain injury in rats.Nrf2 can up-regulate the expression of its downstream antioxidant enzymes, which may be the mechanism of the upregulation expression of Nrf2 in the study.
