Repeat prescription is adopted in Singapore as a convenient measure for patients with chronic diseases.On the one hand, it can avoid patients''inconvenience caused by frequent visits to hospitals.On the other hand, it reduces the waste of medical resources.In recent years, Singapore has also been using modern technology and measures to expand the medication refill service actively in order to enhance work efficiency, reduce waiting time and promote drug safety, thus to improve patients'' satisfaction.With the increasing medical pressure and the deepening of medical reform, the repeat prescription model in Singapore shows positive enlightenment to China.
3.Clinical analysis of 129 Young patients with lung cancer

Clinical data of 129 young pafienm with lung cancer were analyzed retrospectively.Adenocarcinoma was the main pathologic type of lung cancer(n=62,48.1%),followed by undifferentiated small cell carcinoma(n=35,27.1%),squamous carcinoma(n=20,15.5%).Forty four cases were misdiagnosed at first consultation with a misdiagnosis rate of 34.1%.To avoid misdiagnosis seems to be the key issue for improvement of prognosis in young patients with lung cancer.
4.Clinical studies of sodium ferulate,deproteinized calf serum and high pressure oxygen on treating acute carbon monoxide poisoning

Objective Discuss the treatment effect of sodium ferulate,deproteinized calf serum and high pressure oxygen on acute carbon monoxide poisoning.Methods Seventy-five cases with coma acute carbon monoxide poisoning were randomly divided into two groups:the treated group (37 cases) and the control group (38 cases).The two groups were treated with high pressure oxygen,high flow oxygen uptake and support treatment,The treated group were treated with sodium ferulate and deproteinized calf serum.The control group were treated with citicoline act medicaments.Result The occurrence and injury degree of myocardial damage and delayed encephalopathy were markedly attenuated (P < 0.05),the time of coma was obviously decreased (P <0.01),the death rate was obviously difference (P <0.05) in the treated group compared with the control group.Conclusion Sodium ferulate,deproteinized calf serum injection possess obviously effect on acute carbon monoxide poisoning.
5.Investigation of soft contact lens related knowledge and wearing behaviors

Objective To investigate the soft contact lens related knowledge and wearing behaviors.Method Two hundred outpatients in the ophthalmologic clinic wearing SCL involved in the survey by the questionnaire to investigate the SCL-related knowledge and the wearing behaviors.Results In the 200 SCL wearer,73.50% did not make any examinations by specialists and only 18.00% of them took regular examinations.The wearers were good at the knowledge of selecting SCL diopter and positive and negative lens.The wearers with a history of less than 6 months were better than those wearing SCL more than 6 months in respect of hand cleaning,lens check,lens wearing, and lens cleaning.The wears with the education of two-year vocational training were significantly better than those with the education of three-year or more collegiate training in respect of wearing duration,sleep with lens,hand cleaning and lens cleaning(all P<0.05).Conclusion Contact lens wearers know less about SCL related knowledge and their wearing behaviors needs improvement.
6.Development and Application of Motivation-enhancing Self-management Program for Rural Aged with Hypertension.

OBJECTIVE: The purpose of this study was to develop a motivation-enhancing self-management program and apply to rural aged with hypertension to evaluate the applicability of the Program. METHODS: The motivation-enhancing self-management program was on the evidence of Orem's self-care theory and Ford's Motivational system theory which consists of group education and tele-coaching conducted 12-week. The program evaluation was done by structured questionnaires of motivation (self-efficacy, perceived benefit, perceived barrier and emotional salience), self-care and blood pressure. There were totally 31 aged with hypertension in 2 rural areas enrolled in the program. RESULTS: There was only perceived benefit statistically significant between the intervention and control group (t=2.05, p=.04) and self-care level was no difference between the two groups (t=0.21, p=.84) after program. But there are statistically significant differences of systolic blood pressure between the intervention and control group(t=-2.62, p=.01). CONCLUSION: The motivation-enhancing self-management program is necessary for the rural aged with hypertension, but it's needed modify and revise in the future study.

7.The influence of treating course on the Hpid-regulating effect of Xuezhikang and Simvastati

ObjectiveTo observe the influence of treating course on the lipid-regulating effect of Xuezhikang and Simvastati.MethodsTwo hundred and two patients with unstable angina pectoris(UAP) were randomly enrolled and divided into Simvastatin group(n = 98)and Xuezhikang group(n = 104).The TC, TG,LDL-C, HDL-Clevelsweremeasuredbeforethe treatment and atthe3rd, 6thmonthafter the treatment.According to the suitable threshold of blood lipids in normal Chinese people, the patients who had abnormal blood lipids were picked out from the two groups for the further observation.ResultsThe lipidregulating effects of Simvastatin and Xuezhikang were showed up at the 3rd month, and the effective rate were 40.6% and 60.9% for TC regulation,51.2% and 84.5% for LDL-C regulation,22.2% and 61.5% for HDL-C regulation.We found significantly better effects of Xuezhikang than that of Simvastatin(x2 = 6.38,17.05,4.04;P < 0.05, P < 0.01, P < 0.05) .However, at the 6th month the effective rate of Simvastatin and Xuezhikang were 89.9% and 95.4% for TC regulation,89.1% and 97.2% for LDL-C regulation,83.3% and 84.6% for HDL-C regulation, which showed no significant differences (x2 = 1.81,3.57, 0.01 ; Ps > 0.05).The effective percentages of reduced TG level of Simvastatin and Xuezhikang were 9.5% and 24.5% at the 3rd month,51.4% and 68.4% at the 6th month, which showed significant differences (x2 = 6.45,5.13 ; Ps < 0.05) between the two groups at both time points.Conclusion The lipid-regulating effects of Xuezhikang and Simvastati were influenced by the treating period.Xuezhikang shows lipid-regulating effect more quickly ,and it is recommended in treating CHD patients,especially for the patients with hypertriglyceridemia.
8.Relationship of serum insulin and peptide C levels with inflammatory reaction in patients with severe multiple trauma

Objective To approach the changes of serum insulin and peptide C and determine their relationship with inflammatory reaction in patients with severe multiple trauma. Methods The serum insulin, peptide C, tumor necrosis factor (TNF)-α, interleukin (IL)-10, C reactive protein (CRP) were detected in 30 patients with severe multiple trauma at days 1,3 and 7 after trauma to analyze the dynamic changes of serum insulin and peptide C and their correlations with Acute Physiology and Chronic Health Evaluation Ⅲ ( APACHE Ⅲ), cytokines and CRP. The changes of cytokines and CRP were also compared with those of serum insulin and peptide C in patients with different outcomes. The changes of serum insulin and peptide C of 35 healthy subjects were detected and used as control. Re-sults The serum insulin and peptide C levels of patients were higher than those in control group at each time point after trauma, with remarkably positive correlation with APACHE Ⅲ. Whether the serum glu-cose and age were controlled or not,serum insulin and peptide C were positively correlated with IL-10 at each time point and with TNF-α and CRP at days 3 and 7. The levels of serum insulin, peptide C and IL-10 were decreased with time in both groups with different outcomes. Meanwhile, the levels of TNF-α and CRP were decreased in the survival groups but increased in the death groups. The levels of above indices in death group were significantly higher than survival groups at the same time point. Conclusions The increasing of serum insulin and peptide C is correlated with inflammatory reaction after severe multiple trauma. The dynamic changes of both indices can either reflect injury severity or be used as an effective index in dynamically monitoring anti-inflammatory degree of the organism.
9.Dynamic observation on optimal time of coronary angiography performed using 1 28-MSCT after taking nitroglycerin and its significance

Objective:To investigate the optimal time of coronary computed tomography angiography (CCTA) after taking nitroglycerine,and to provide basis for improving the image quantity of CCTA .Methods:43 patients underwent CCTA were scanned with coronary artery calcium score (CACS)after taking nitroglycerin 0,3,5 and 10 min.Then the diameters of the same coronary artery from the same anatomic location and the expanding rates were measured,and the change regular was analyzed with single factor analysis of variance of SPSS 17.0 software. Results:The average coronary artery expanding rate was 8% 3 min after taking nitroglycerin, and the difference was significant compared with 0 min (P<0.05);but there were 5 patients whose coronary artery didn’t expand. The coronary artery expanded obviously 5 and 10 min after taking nitroglycerin compared with 0 and 3 min (P<0.05);the maximum expanding rates at 5 and 10 min were both 38%.There were 13 and 28 patients whose coronary artery reached the maximum expanding rates (17% and 19%)5 and 10 min after taking nitroglycerin, respectively;the coronary artery of 2 patients expanded at a same level at 5 and 10 min.The average expanding rates of coronary artery had no significant differences between 5 and 10 min (P>0.05 ).Conclusion:Taking nitroglycerin can significantly expand the diameter of coronary arteries.It is necessary to perform CCTA during 5-1 0 min after taking nitroglycerine.

The operating room is the main place of surgery,but there exist many ethical problems when developing quality nursing service in operating rooms.These problems mainly related to the imperfect preoperative and postoperative visit systems,the lack of effective communication and protection of patients privacy during the operations,and the lack of self-discipline spirit.To solve these problems,we should improve the preoperative and postoperative visit system,strengthen the psychological counseling for patients,cultivate the nurses' self-discipline spirit,respect and protect the privacy of patients,so as to improve the quality of nursing service in the operation rooms,and better service for patients.
