1.Preparation of vulnerable plaques in abdominal aorta: an experimental study in rabbits

Objective To establish an animal model of vulnerable plaque in abdominal aorta by pharmacological triggering of atherosclerosis in rabbits. Methods Thirty New Zealand white rabbits were randomly and equally divided into 3 groups: group A, receiving balloon injury together with high lipid diet; group B, receiving high lipid diet only, and group C, receiving regular diet only. After feeding for 12 weeks, all the rabbits in three groups underwent pharmacological triggering with Chinese Russell's viper venom (CRVV) and histamine to induce the rupture of the plaque and the formation of thrombosis. Results In 8 survival rabbits of group A, ten lesions with the stenotic degree over 50% were found, which had large lipid core with the thickness of fibrous cap being thinner than 65μm, by which these ten lesions were judged as vulnerable plaques. In 9 rabbits of group B, only 3 vulnerable plaques could be determined, while no single vulnerable plaque could be identified in the rabbits of group C. Conclusion For establishing an animal model of vulnerable plaque in abdominal aorta in rabbits, balloon injury combined with high lipid diet is a feasible and reliable method.

3.Clinical outcomes of Cyberknife in brain metastases

Objective To retrospectively analyze clinical outcomes of brain metastases treated with Cyberknffe. Methods From Aug. 2006 to Aug. 2007, 40 patients with brain metastases treated with Cy-berknife in Tianjin Cancer Hospital were included. Totally 68 brain metastatic lesions were treated. The maximal diameter was 0.4 -7.5 cm (average 1.9 cm). The primary tumor of 35 patients was partial re-ponse or stable to the previous treatment at the diagnosis of brain metastasis. The geometric center of target volume was set as the isocenter and the nominal standard dose point. More than 95% target volume was cov-ered by 80% isodose surface. Hypofractionated radiation was 18 -36 Gy given in 1 -5 fractions of 5 -25 Gy. Results The Follow-up rate was 95%. The clinical symptom remission rate (including complete and partial remission) at one week after Cyberknife treatment was 90% (36/40). The therapeutic effective rate (CR + PR + SD) was 94% (64/68) after three months follow-up, which was evaluated by contrast-en-hanced CT or MRI. The 3-month local control and 1-year survival rates were 78% (53/68) and 68% (27/40). New brain metastatic lesions outside the radiation field occurred in 14 patients within 3 months, which was independent of age, whole brain irradiation, number or maximal diameter of original lesions, frac-tion dose, frequency or total dose of Cyberknife treatment. Conclusions Cyberknife, used as the primary treatment of brain metastases, can achieve good clinical outcomes.
4.Experimental study on preventing obesity by compound rhubarb preparation in rats

AIM: To investigate the effect of compound rhubarb preparation (Kintop) in preventing obesity in rats and its probable mechanism involved. METHODS: Twenty-six newborn SD rats were randomly grouped as rhubarb preparation plus high-energy forage group( n= 8), high-energy forage control group( n= 8) and ordinary forage control group( n= 10). The rats in rhubarb preparation plus high-energy forage group and high-energy forage control group were fed with high-energy forage and those in ordinary forage control group were fed with ordinary forage. The rats in rhubarb preparation plus high-energy forage group were administered by compound rhubarb preparation (40 mg?100 g -1 body weight?d -1 ) from 9th to 17th week. The dynamic changes in body weight, celiac fat weight and adipocytes size were measured. Immunohistochemical analysis of leptin in celiac adipocytes (ABC method) and measurement of serum leptin level (RID method) were performed. RESULTS: The body weight and the wet weights of celiac fat were lower, their adipocytes were smaller and immunohistochemical stainings of leptin were weaker in rhubarb preparation plus high-energy forage group than those in high-energy forage control group. Objective To eveluate the clinical curative effects of combination of Ma Yinglong She Xiang Zhi Chuang Gao,Jin Xuan Zhi Ke Xun Xi Xan,Diosmin,Macrogol 4000 powder in treating postoperative complications of ring mixed hemorrhoids.Methods Ninty cases of postoperative ring mixed hemorrhoids patients were divided into two groups randomly from January 2008 to June 2009.Experimental group:From the first day on Jin Xuan Zhi Ke Xun Xi San 55 g and the 1000 mL boiling water were added flushing,and Maying Long She Xiang Zhi Chuang Gao 2.5 g,2 times daily.The Macrogol 4000 powder 10 g and water 200 mL were admistrated orally,Diosmin 1.0 g orally,2 times daily.Oral administration of two kinds of medications was done each two hours.Control group:Using 1:100 Sterile warm salt water hip bath,and Ma Yinglong she xiang zhi chuang gao 2.5 g,2 times daily;Phenolphthalein tablets 100 mg orally,2 times daily.Results The experimental group surpassed the control group in the anus ache,the hemorrhage,edema (P<0.05).The heal time reduced obviously(P<0.01).Conclusion To combination of Ma Yinglong She Xiang Zhi Chuang Gao,Jin Xuan Zhi Ke Xun Xi San,Diosmin,Macrogol 4000 powder has the distinct improvement in the anus ache,the hemorrhage,dropsy of ring mixed hemorrhoids and reduces the injured area heal time obviously.
6.The clinical value of contrast-enhanced ultrasound in superficial lymphadenopathy qualitative diagnosis

Objective To evaluate the value of contrast-enhanced ultrasound in superficial lymphadenopathy. Methods Ninty-four superficial enlarged lymph nodes were studied by 2-dimensional, color Doppler ultrasound, and contrast-enhanced ultrasound. Then the contrast-enhanced images were analyzed by Philips Q-LAB software. All the results were compared with pathological diagnosis. Results For the 94 superficial lymph nodes examined,44 were benign,33 were metastases and 17 were lymphomas. The sensitivity, specificity,and accuracy of contrast-enhanced images were 84% ,74% and 790//oo respectively. Contrast-enhanced ultrasound examination showed intense homogeneous enhancement in 39 of 44 benign lymph nodes; high or low homogeneous enhancement in 25 of 33 and 7 of 33 in metastases respectively;intense homogeneous, and scarce enhancement in 6 of 17 and 9 of 17 in lymphomas respectively. Time-intensity curves showed that compared with metastasis lymph nodes and lymphomas, benign lymph nodes had higher peak intensity and larger area under the curve (P<0.01). Conclusions The diagnosis accuracy was significant increased when contrast-enhanced ultrasound was test against conventional ultrasound. The character of contrast agent enhancement and Q-LAB time-intensity curves provide valuable diagnosis information for differential diagnosis of benign,metastasis lymph nodes and lymphomas.
7.A prospective comparison cohort study between baseline serum uric acid level and coronary artery disease in first-degree relatives and non-first-degree relatives of type 2 diabetes males

Objective To investigate the relationship between baseline serum uric acid and the severity of coronary artery disease ( CAD ) in the first-degree relatives or non-first-degree relatives of men with type 2 diabetes. Methods Three hundred and eighty-one men with negative coronary angiography for the first time were divided into diabetes and non-diabetes groups and followed-up for 5 years. The primary outcome was acute coronary syndrome suspected during subsequent 5 years, and the coronary angiography was conducted simultaneously. The severity of CAD was assessed by the coronary stenosis index ( CSI) and the number of coronary lesion vessels. Results In normal blood glucose group, serum uric acid was higher in the first-degree relatives of diabetics compared with non-first-degree relatives(P<0. 01), along with higher morbidity of CAD, CSI, and coronary lesion vessels (all P<0.01). Correlation analysis showed that CSI(r=0. 250, P=0. 041) and coronary lesion vessels(r=0. 252, P=0. 040) in non-diabetics group were associated with baseline levels of serum uric acid. Conclusion The elevation of serum uric acid was closely related to subsequent CAD, especially in first-degree relatives of male with type 2 diabetes, which could be used as an early indicator for CAD prediction.
8.Effect of stellate ganglion block on cellular immune function in diabetic rats

Objective To evaluate the effect of stellate ganglion block (SGB) on cellular immune function in diabetic rats.Methods Healthy male Sprague-Dawley rats,aged 3 months,weighing 240-280 g,were used in this study.Diabetes mellitus was induced by intraperitoneal 1% streptozotocin 60 mg/kg and confirmed by blood glucose ≥ 16.7 mmol/L 3 days later.Forty-eight rats with diabetes mellitus were randomly divided into 2 groups (n=24 each) using a random number table:diabetes mellitus group (group DM) and group SGB.Another 24 healthy rats,aged 3 months,were selected and served as control group (group C).At 1 week after successful establishment of the model,unilateral transection of cervical sympathetic trunk (TCST) was performed in group SGB,while the right cervical sympathetic trunk was only exposed in C and DM groups.Before TCST (T0) and on 1,3,7 days after TCST (T1-3),6 rats were randomly selected from each group,and blood samples were collected from the inferior vena cava for determination of the blood glucose,plasma norepinephrine (NE) concentrations (by enzyme-linked immunosorbent assay),and levels of T lymphocyte subsets CD3+,CD4+ and CD8+ in whole blood (using FACSCalibur flow cytometer).C D4+/CD8+ratio was calculated.The rats were weighed before sacrifice,and the rats were sacrificed to obtain the thymus which was weighed.The thymus index (thymus weight/body weight) was calculated.Results Compared with group C,the blood glucose was significantly increased,and the levels of CD3+ and CD4+ in whole blood,CD4+/CD8+ ratio,and thymus index were significantly decreased at T0-3 (P<0.05),and no significant change was found in CD8+ levels in DM and SGB groups (P>0.05),the plasma NE concentrations were significantly decreased at T1-3 in group SGB (P<0.05),and no significant change was found in plasma NE concentrations in group DM (P>0.05).Compared with group DM,the blood glucose and plasma NE concentrations were significantly decreased,and the levels of CD3+ and CD4+ in whole blood,CD4+/CD8+ ratio,and thymus index were significantly increased at T1-3 (P<0.05),and no significant change was found in CD8+ levels in group SGB (P>0.05).Conclusion SGB can improve the cellular immune function in diabetic rats.
9.A model of early intervention for cerebral palsy that combines clinical, health care and rehabilitation

目的探索脑瘫患儿早期发现、早期干预的工作模式。方法临床、保健、康复相结合 ,采用产前检查、孕妇学校、产时儿科医生进产房、新生儿行为神经评定测定、高危儿抚触训练室、婴儿期儿童保健监测、各街道防治站常规体检筛查转诊等多种形式结合早期发现并干预脑瘫高危儿。结果此工作模式较模式前显著提高了脑瘫的早期诊断率及康复疗效。结论临床、保健、康复相结合的方式整合了医疗资源,起到了促进脑瘫早发现、早诊断、早治疗的作用 ,值得推广。
