Autophagy is a vacuolar process of cytoplasmic degradation by lysosome which ubiquitously occurring in all eukaryotic cells. The researches of autophagy have made great progress with the development of the yeast model and genetic technology. This review will summarize the determination of autophagy, its relationship with apoptosis and its role in the tumor treatment in order to give a comprehensive understanding of the function of autophagy.
2.Assessment of intervention measures on trihalomethane in finished water by interrupted time series analysis

Background The Qingcaosha Reservoir is facing issues of algal blooms and eutrophication, and the resulting increase in the level of chlorination disinfection by-products in the water has been a major concern. Objective To evaluate the impact of "Algae Monitoring and Control Program in Qingcaosha Reservoir" (hereinafter referred to as the program) on the control of trihalomethanes (THMs) in conventional finished water. Methods From 2011 to 2019, water samples were collected from the Lujiazui Water Plant once per season, one sample each time, and the concentrations of four THMs (trichloromethane, dichlorobromomethane, monochlorodibromomethane, and tribromomethane) were measured in the samples. Using 2014 when the program was implemented as a cut-off point, the entire study period was divided into two phases: pre-implementation (2011–2013) and post-implementation(2014–2019). Segmented linear regression with interrupted time series analysis was applied to assess the concentrations and trends of THMs in the finished water before and after the program launch. Results The concentration of total THMs in finished water increased by 1.561 µg·L−1 (P=0.010) for each season of time extension before launching the program. The change in the concentration of total THMs in finished water was not statistically significant after the program launch, but the THMs concentration showed a decreasing trend as the slope was −0.626 (P=0.001). From 2017 until the end of 2019, the average concentration of THMs in finished water of Lujiazui Water Plant dropped to 10 μg·L−1 or less. Conclusions The algae and eutrophication control measures in Qingcaosha Reservoir have achieved good results, controlling THMs in finished water at a low level, and the trend of THMs has changed from a yearly increase pattern before the program to a yearly decrease pattern after the program.
3.Effect of stapledtransanal in the treatment of severe rectal obstructive constipation

Objective To evaluate the clinical efficacy stapledtransanal in the treatment of severe rectal obstructive constipation. Methods Eighty patients with rectal obstructive constipation and hospitalized in Tangshan City Hospital of TCM were enrolled in the study and randomly divided into the treatment group and control group,40 cases in each group. The treatment group was treated with stapledtransanal and the control group was treated with transvaginal repair of longitudinally cut transverse suture operation. The clinical symptoms of two groups were observed and quantified. The operation time,intraoperative blood loss and wound healing time were recorded, Wexner constipation scores were collected at 1, 3, 6 and 12 months after the operation; Postoperative bleeding,pain,infection,anal distention,rectovaginal fistula and other complications were observed. The clinical efficacy of the two groups was compared. Results The total effective rate in the treatment group was 100% (40/ 40),which was significantly higher than that in the control group (90. 00%(36/ 40)) (χ2 = 0. 60,P= 0. 027). There were significant differences in the clinical symptom scores of the two groups at 7 days (treatment group(3. 58±1. 61) points,control group(4. 12±1. 52)points))and 3 months after treatment(treatment group (4. 23±2. 13)points,control group (5. 33±1. 12)points) compared with those before treatment (treatment group (13. 23±3. 08)points,control group (14. 45±3. 01) points ) (P<0. 05);There was a statistically significant difference in clinical symptom quantification score between the treatment group and the control group at 3 months after treatment (P<0. 01). In the treatment group,the operation time ((15. 65±1. 61) min),intraoperative bleeding ((10. 77±5. 23) ml) and wound healing time ((9. 28±1. 41)) d were better than those of the control group ((25. 89±3. 71) min,(43. 31±8. 11) ml,(14. 87±1. 92) d) (t = 15. 632,14. 266,15. 518,P<0. 01). The scores of Wexner in the two groups were lower than those of the same group at 1,3,6 and 12 months after operation. The scores of Wexner in the treatment group were lower than those of the control group at 1,3,6 and 12 months after operation (P<0. 05). In terms of postoperative complications,the pain and anal distention in the treatment group were better than those in the control group ( P < 0. 05) . Conclusion Stapledtransanal is effective for the treatment of outlet obstructive constipation.
4.Application of rotational skin flap in repairing skin and soft tissue defects in the perineum

Objective:To explore the application effect of rotating skin flap in the repair of skin and soft tissue defects in the perineum.Methods:A retrospective analysis was conducted on the data of 9 patients with perineal soft tissue defects treated in the Department of the First Affiliated Hospital, Wenzhou Medical University from January 2022 to March 2024. Among them, 8 cases were soft tissue defects after extensive resection of Paget′s disease in the perineum, and 1 case was soft tissue defects after treatment of severe urethral stricture. The defect area ranged from 4 cm×1.5 cm to 30 cm×35 cm, and all wounds were repaired with rotating skin flaps, For patients with excessive area, rotating skin flaps were used to cover important areas, combined with autologous skin and artificial skin to cover the remaining wounds. Patients with urethral stricture were treated with free oral mucosal reconstruction of the urethra combined with rotational skin flap coverage. The donor site was directly sutured or autologous skin was transplanted.Results:After surgery, the recipient skin flaps of 9 patients survived and the donor area healed. After a follow-up of 9-15 months, the skin flap survived well, with skin color approaching normal, and the donor site wound healed well. Eight patients with Paget′s disease did not show any recurrence, among which one patient with extensive wound expansion had good recovery of the perineal skin flap coverage area, and small-scale chronic ulcers appeared in the skin graft area. The skin flap of the patient with urethral stricture had recovered well, and the shape of the penis was good, without any urethral stricture, urinary fistula, sinus tract, etc.Conclusions:The rotational skin flap has a clear therapeutic effect on perineal soft tissue defects and is suitable for repairing perineal skin and soft tissue defects.
5.Role and research progress of transient receptor potential vanilloid-1 in acute respiratory distress syndrome

Acute respiratory distress syndrome(ARDS)is a severe critical respiratory disease characterized by refractory hypoxemia,which is caused by intrapulmonary and extrapulmonary factors.It has a rapid onset,and high morbidity and mortality.With the global prevalence and mutation of respiratory viruses,the diagnosis and treatment of ARDS have become more complicated,requiring exploration into the molecular mechanisms and effective therapeutic methods of the occurrence and development of ARDS in clinical practice.Researchers have found that the pathogenesis of ARDS involves the interaction of multiple factors,including imbalances in inflammatory responses and redox reactions,dysregulation of endothelial cells,disruption of alveolar-capillary barrier and abnormalities in coagulation function.Although advancements in molecular biology techniques such as genomics and proteomics have provided new insights into the pathogenesis of ARDS,there is still a lack of early diagnostic biomarker and effective drugs targeted for ARDS.At present,more comprehensive and in-depth basic and clinical research is still needed.Increasing evidence suggests that transient receptor potential vanilloid-1(TRPV1),also known as the capsaicin receptor,plays a crucial role in respiratory system diseases.TRPV1 is widely distributed in the upper respiratory tract,airway smooth muscle,alveoli and pulmonary blood vessels,participating in mediating airway dilation and constriction,cough reflex,and release of inflammatory mediators related to inflammation and pain,as well as sensing and transmitting various biological signals related to temperature,chemical substances and mechanical stress stimuli in the respiratory system.The widespread distribution and diverse physiological functions of TRPV1 make it a research hotspot in the occurrence and development of respiratory system diseases such as pneumonia,pulmonary edema,cough,asthma and acute lung injury.This article reviews the correlation and molecular mechanisms between ARDS caused by sepsis,traumatic brain injury and respiratory viruses with TRPV1,aiming to summarize the positive effects of regulating TRPV1 expression on the pathogenesis of ARDS and provide reference for strengthening early diagnosis and effective intervention measures for ARDS.
