1.The effects of dexmedetomidine and ketamine on breast lumpectomy in women patients

Objective To observe the effects of dexmedetomidine and ketamine on breast lumpectomy in women patients .Meth‐ods Thirty women who underwent breast lumpectomy were randomly allocated into two groups (n=15) .Group A received mid‐azolam 2 .0 mg ,fentanyl 0 .1 mg and ketamine 2 .0 mg/kg ,everyone required supplementary propofol 5 .0 mg?kg -1 ?h-1 .In group B ,dexmedetomidine 0 .5 μg/kg was administered as a loading dose during 10 minutes followed by a continuous infusion of 0 .2μg?kg -1 ?h-1 ,Ketamine 2 .0 mg/kg was administered along with the dexmedetomidine loading dose .The recovery time ,the VAS and Ramsay at the end of the first hour of postoperation were recorded along with side effects .Meanwhile ,hemodynamic parameters , like heart rate(HR) ,mean blood pressure (MAP) and SPO2 were also monitored .Results Compared with group A ,the HR and MAP were significantly lower(P<0 .05) ,and no significant changes in SPO2 were noted (P>0 .05) in group B;there was no sig‐nificant difference in VAS and Ramsay scores among two groups ,the recovery time was shorter in group B(P<0 .05) .However ,the incidence of respiratory arrest and glossocoma were greatly higher in group A(P<0 .05) .Conclusion Dexmedetomidine and ket‐amine can offer an effective sedoanalgesia and lower hemodynamic discrepancy without causing any significant side effect on breast lumpectomy in women patients .

OBJECTIVES:
To explore the value of three-dimensional fast gradient echo sequence (3D-GRE) in observation of the craniocervical junctional ligament.
METHODS:
A total of 21 healthy volunteers underwent 3D-GRE imaging. The imaging data was imported into the post-processing workstation. The structures of the ligaments in the craniocervical junctional area were observed and evaluated by multiplanar reconstruction technique.
RESULTS:
The features of ligaments in the craniocervical junction were shown clearly for all the 21 cases of volunteers. The scan time was 267-294 s. After the treatment with the three-dimensional reconstruction technique, the signal characteristics and the running structure of the transverse ligament, the alar ligament, the serrated ligament and the lamina could be effectively displayed.
CONCLUSIONS
The 3D-GRE can evaluate the three-dimensional data of craniocervical junctional ligament in a short period of time. Post-processing reconstruction technique can clearly evaluate the structure characteristics of each ligament, which can lay a foundation for further application in craniocerebral trauma patients.

4.Isolation and culture of adipose-derived mesenchymal stem cells from inbreed line miniature pig of Wuzhishan and their biological characteristics.

To establish a method for isolation, culture and identification of adipose-derived mesenchymal stem cells (ASCs) from the inbreed line miniature pig of Wuzhishan (ILMW).
Methods: A total of 100 g adipose tissues were obtained from subcutaneous tissues of neck in six-month old healthy ILMW (3 samples, male). ASCs from ILMW (ILMW-ASCs) were isolated from adipose tissues through 0.1% collagenase digestion. The cells at the 3rd, 5th, 8th, 13th passages were collected. Cell morphology, size, phenotype, cell cycle, and apoptosis were monitored. Cell differentiation was induced and cell proliferation curve was drawn.
Results: The ILMW-ASCs, fibroblast-like or whirlpool-like, began the adherence at 36 h and entered a logarithmic phase in the 5th day. Eighty percent of them were fused in the 7th day. The average diameter and volume of ILMW-ASCs were (17.00±0.54) µm and (2.58±0.24)×10-9 L, respectively. The expressions of CD29, CD44 and CD90 were positive, and there was no significant difference between the different passages (all P>0.05). The expressions of CD45, CD8a and HLA-DR were increased with the increase in passages after the 3th passage (all P<0.05). The adipogenic induction of ILMW-ASCs was observed by positive oil red O staining, and the osteogenic induction of ILMW-ASCs was determined by positive alizarin red staining. Apoptosis and senescence occurred in the 13 passage of ILMW-ASCs, and the proportion of S phase of cell cycle was lower than that in lower passages (all P<0.05).
Conclusion: ILMW-ASCs are one of the best choice for porcine ASCs, which might provide a source of candidate stem cells for therapy of large animal disease models and tissue or organ repairment.

5.Diagnostic Value and Clinical Significance of G6PD Activity for the Mediterranean Anemia.

OBJECTIVETo investigate the diagnostic value and clinical significance of the glucose 6-phosphate dehy-drogenase (G6PD) activity for mediterranean anemia (MA), so as to provide the reference for early clinical diagnosis and treatment of MA.
METHODSThe peripheral blood was collected from 100 healthy persons and 168 patients with MA, then the agarose gel electrophoresis, MA gene detection, blood routine examination, serum ferrium levels and G6PD activity assay were performed, and the results of evaluating MA were comparatively analyzed.
RESULTSThe G6PD activity in all type MA patients was obviously higher than that in healthy controls (P < 0.01), the MCV value in all type MA patients was significantly lower than that in healthy controls (P < 0.01). The detection of G6PD activity showed that the sensitivity, specificity, positive and negative likelihood ratio, diagnostic index and Youden index of G6PD for MA patients were 85.12%, 68%, 2.66, 0.219, 1.53 and 0.53 respectively, which suggest the better efficacy of G6PD value for diagnosis of MA.
CONCLUSIONThe G6PDS activity of patients with MA in different subtypes is higher than that of healthy persons, the G6PD level has a certain diagnostic value for MA, but there is an optimal range.

6.Textual Research of Classical Prescription Da Jianzhongtang

Da Jianzhongtang, which is from Synopsis of Golden Chamber(《金匮要略》), is one of the hundred ancient classical formulas. In this textual research, a total of 46 records on this prescription from 42 ancient books and 207 records from modern literature were screened out. Through systematic research on its name, dosage, processing, evolution of the meaning on prescription compatibility, indications, decocting methods, usage, and precautions, this paper aims to lay a literature basis for further research on Da Jianzhongtang. The results show that the converted dosages for the components are 3 g of Zanthoxyli Pericarpium, 12 g of Zingiberis Rhizoma, 6 g of Ginseng Radix et Rhizoma, and 30 g of Saccharum Granorum. The efficacy can be labeled on the prescription instruction as warming the middle and tonifying deficiency, descending adverse Qi and relieving pain. According to modern literature, the main indications include diseases in digestive system, circulatory system, orthopedic system, urinary system, and ophthalmic system, particularly the intestinal obstruction, gastric ulcer, chronic gastritis, and duodenal ulcer in the digestive system. The core pathogenesis of the above diseases is "deficiency-cold of middle energizer and internal exuberant cold". Precautions are as follows. ① For the processing of Zanthoxyli Pericarpium, stir-frying can alleviate its toxicity and increase the decoction rate of effective ingredients. ② For the dosage, overdosage will lead to dry cough, edema, etc. ③ As to the contraindications after the medication, raw, cold, or indigestible food should not be taken after medication. ④ For the syndrome taboos, this prescription should be used with caution for patents with Yin deficiency, congealing cold, and Qi stagnation, and be forbidden for patients with spleen and stomach dampness-heat. The above results lay a basis for the research and development of the compound preparation for Da Janzhongtang and provide a reference for its instructions.
7.Expression of miR-106b-5p in children with primary immune thrombocytopenia and its correlation with T cells.

OBJECTIVES:
To study the expression level of plasma miR-106b-5p in primary immune thrombocytopenia (ITP) and its correlation with the levels of T helper 17 cell (Th17) and regulatory T cell (Treg) and the Th17/Treg ratio.
METHODS:
A total of 79 children with ITP (ITP group) and 40 healthy children (control group) were selected as subjects. According to the treatment response, the 79 children with ITP were divided into three groups: complete response (n=40), partial response (n=18), and non-response (n=21). Quantitative real-time PCR was used to measure the expression level of miR-106b-5p. Flow cytometry was used to measure the frequencies of Th17 and Treg, and the Th17/Treg ratio was calculated. The correlation of the expression level of plasma miR-106b-5p with the frequencies of Th17 and Treg and the Th17/Treg ratio was analyzed.
RESULTS:
Compared with the control group, the ITP group had significantly higher levels of miR-106b-5p, Th17, and Th17/Treg ratio (P<0.05) and a significantly lower level of Treg (P<0.05). After treatment, the ITP group had significant reductions in the levels of miR-106b-5p, Th17, and Th17/Treg ratio (P<0.05) and a significant increase in the level of Treg (P<0.05). Compared with the partial response and non-response groups, the complete response group had significantly lower levels of miR-106b-5p, Th17, and Th17/Treg ratio (P<0.05) and a significantly higher level of Treg (P<0.05). The correlation analysis showed that in the children with ITP, the expression level of plasma miR-106b-5p was positively correlated with the Th17 level and the Th17/Treg ratio (r=0.730 and 0.816 respectively; P<0.001) and was negatively correlated with the Treg level (r=-0.774, P<0.001).
CONCLUSIONS
A higher expression level of miR-106b-5p and Th17/Treg imbalance may be observed in children with ITP. The measurement of miR-106b-5p, Th17, Treg, and Th17/Treg ratio during treatment may be useful to the evaluation of treatment outcome in children with ITP.

8.Adolescent Tuberculosis Reported in Haikou Region and Factors Influencing the Outcome in 2018 - 2021

Objective To analyze the adolescent tuberculosis reported in Haikou region between 2018 and 2021 and factors influencing the outcome. Methods The data on adolescent tuberculosis cases reported in Tuberculosis Information Management System in the Haikou Regional Centre for Disease Control and Prevention (RCDC) from 2018 to 2021 were collected. A follow-up survey was carried out until 31 December 2022. The changes in reported incidence of adolescent tuberculosis in Haikou was analyzed, and multivariate Cox proportional hazards regression models were used to identify independent factors influencing the outcome of adolescent tuberculosis. Results A total of 265 cases of tuberculosis in adolescents were reported between 2018 and 2021, of which 55 were reported in 2018, 74 in 2019, 67 in 2020, and 69 in 2021. The total number of reported cases of tuberculosis was 140 among male adolescents, which was slightly higher than 125 among female adolescents. The number of reported cases of tuberculosis was 134 in adolescents ≥15 years of age and 131 in adolescents <15 years of age. No statistically significant difference was reported between good outcome group and poor outcome group in age, gender, place of residence, nationality, and domicile place (P>0.05), whereas difference was found in the history of tuberculosis between two groups (P<0.05). Multivariate Cox proportional hazards regression model denoted that retreated tuberculosis (HR: 2.172, 95%CI: 1.483-3.007), complicating underlying disease (HR: 1.451, 95%CI: 1.080-1.985), and number of sputum smears (HR: 2.617, 95%CI: 1.531-3.458) were the risk factors for adverse outcomes in adolescent tuberculosis. Conclusion From 2018 to 2021, the number of reported adolescents of pulmonary tuberculosis in Haikou increased, suggesting that the management and prevention of adolescents should be strengthened to improve the prognosis rate of adolescents.
9.Predictive Value of Acute Phase Proteins on the Prognosis of Patients with Acute Myeloid Leukemia.

OBJECTIVE:
To analyze the predictive value of acute phase proteins (APPs) on the prognosis of patients with acute myeloid leukemia (AML).
METHODS:
293 AML patients who met the study requirements from January 2015 to April 2021 were collected, their clinical characteristics and pre-treatment APPs levels [including albumin (ALB), fibrinogen (FIB), C-reactive protein (CRP), Ferritin (FER)] were followed up and investigated. Pearson correlation coefficient was used to analyze the correlation between APPs. Logistic regression was used to analyze the risk factors for mortality in AML patients. ROC curve was used to analyze the predictive value of APP for mortality in AML patients, and Kaplan-Meier survival analysis was used to compare the effect of APPs on complete remission (CR) rate, overall survival (OS), disease-free survival (DFS), and progression-free survival rate (PFS) of AML patients.
RESULTS:
Pearson correlation analysis showed that there were negative correlations between ALB and CRP (r=-0.134, P=0002), as well as ALB and FER (r=-0.148, P=0.001). There were correlations between FER and CRP (r=0361, P<0.001), as well as FER and FIB (r=0.293, P<0.001). Logistic regression analysis showed that advanced age (>50 years) (OR=1.87, 95% CI=1.25-2.15, P<0.001), relapse after treatment (OR=2.11, 95% CI=111-3.18, P=0.003), FLT3-ITD mutation (OR=2.59, 95% CI=1.10-4.12, P<0.001), CRP≥524 mg/L (OR=1.21, 95% CI=1.02-2.14, P=0.024), CFA (CFA=CRP*FIB/ ALB)≥3 (OR=2.41, 95% CI=1.65-6.47, P<0.001), and FER≥1145.58 mg/ml (OR=1.67, 95% CI=1.15-3.75, P<0.001) were the risk factors for the survival of AML patients. ROC curve analysis showed that FER (AUC=0.752, 95% CI=0.681-0823, P<0.001, the best cut-off value=1220.56 mg/ml) and CFA (AUC=0.804, 95% CI=0.741-0.868, P<0.001, the best cut-off value=3.00) had higher predictive value for the survival of AML patients. The remission rate, PFS, DFS, and OS in the low CFA group (CFA≤3) were significantly higher than those in the high CFA group (CFA>3), and the overall mortality rate was lower than that in the high CFA group; the remission rate, PFS, DFS, and OS in the low FER group (FER≤1220.56 mg/ml) were significantly higher than those in the high FER group (FER>1220.56 mg/ml), while the overall mortality rate was lower than that in the high FER group, and the difference is statistically significant.
CONCLUSION
The CFA value and FER level before treatment in AML patients can independently predict the prognosis of patients, and high levels of CFA and FER are associated with poor prognosis of AML patients.

10. Analysis of clinical value of high-risk factors combined with stratification analysis in predicting clinical prognosis of N2-3M0 nasopharyngeal carcinoma patients

Objective:
To evaluate the clinical value of high-risk factors in combination with stratification method in predicting the clinical prognosis of patients diagnosed with N2-3M0 nasopharyngeal carcinoma (NPC).
Methods:
A total of 226 N2-3M0 NPC patients who underwent IMRT from November 2013 to May 2015 were enrolled in this study. The relationship between tumor volume, cervical metastatic lymph node characteristics (necrosis and fusion) and T and N staging was analyzed. The high-risk factors that affected the survival were identified. The value of high-risk factors combined with stratification method in predicting the clinical prognosis was assessed.
Results:
N3 staging, Vn≥47.15cm3 and lymph node fusion (LNF) were the high-risk factors for distant metastasis in patients with stage N2-3M0 NPC. All patients were classified into the low-risk, medium-risk, high-risk and extremely high-risk groups according to high-risk factors. For patients in the low-risk, medium-risk, high-risk and extremely high-risk groups, the 3-year overall survival rates were 84.2%, 76.7%, 58.7% and 36.4%(all
