1.Thrombolysis for Treating Acute Deep Vein Thrombosis in the Lower Limbs

Objective:To observe the effect of thrombolysis in treating acute deep vein thrombosis (DVT) in the lower limbs.Methods:For the 14 patients diagnosed by colour Doppler ultrasonographic examination,intravenous drip of urokinase,dextran and mailuoning was ministered alternatively through the superficial venosus dorsalis pedis in the involved side.Meanwhile,oral intake of aspirin and dipyridamole was also prescribed.Results:Ten patients healed and four significantly improved.Conclusion:The efficacy of thrombolysis for treatment of DVT in the lower limbs is valid.This intervention is especially suitable for those patients who missed the optimum time window of operation.
2.Development of Hidden-pathogen Theory---Proposition and Demonstration of Emotional Hidden-pathogen Doctrine

Hidden-pathogen theory is always a leading theory of febrile disease etiology from the proposal until the Ming Dynasty and it plays a critical guiding role on development and maturity of febrile disease theory. However, along with the change of times and drift of disease spectrum, this theory is facing unprecedented challenges. The gradually in-creased clinical experiences and facts have been difficult to be explained and induced using the already existed hidden-pathogen theory. Since hidden-pathogen theory has its own theoretical status and value, which cannot be lightly given up, the question of how to develop this theory in order to better reveal the pathogenesis in the guidance of diagnosis and treatment has become the focus of attention. This paper tried to proposeemotional hidden-pathogenas an example to provide a possible paradigm to develop hidden-pathogen theory and systematically expound main ideas and reasoning processes of emotional hidden-pathogen doctrine and to explore its theoretical significance and application value.
3.The constituents of breast diseases and an analysis preponderantage of discovery of each disease: an epidemiological study

Objective To study the epidemiology of breast diseases based on a vast amount of data collected from a number of hospitals. Methods The pathological data of breast diseases collected from 14 hospitals were statistically analyzed. Grouping was made according to the patients′ age, i.e. every 10 years from 0 to 69 as a group, and the patients aged 70 and over as another group. A database was established using software SPSS (10.0). A statistical analysis was made on the types of breast diseases, the incidence was calculated, and curves were plotted up based on the types of disease and the contribution of patients′ age in different hospitals. Results A total of 35948 cases, including 46 diseases of breast diseases, were analyzed. The common diseases were fibroadenoma, carcinoma, adenosis, hyperplasia of male breast, cystic hyperplasia and accessory-breast, which made up 37.99%, 21.49%, 20.60%, 4.54%, 3.60% and 2.80%, respectively, of the total cases analyzed. Conclusion The number of patients suffering from breast diseases is proportional to the size of the hospital. It has been found that different pathology of the breast had its peak preponderant age of discovery. In all the cases analyzed, tumors were found in 23 167 cases, accounting for 64.45% of all patients, among which benign and malignant lesions constituted 66.13% and 33.74%, respectively. Fibroadenoma was the predominant tumor among benign tumors (89.14%), while the breast cancer constituted 98.82% of malignant tumors. The incidences of the both types of tumor, were similar in different hospitals. No significant difference was found on the peak ages of different breast diseases among the hospitals. 12 781 cases of nontumorous lesions accounting for 35.55% for all the cases showed marked differences in age distribution among hospitals, implying that there were different diagnostic criteria for such lesions.
4.Relationship between the clinical onset of cerebral infarction in anterior cycle and properties of carotid atheromatous plaque

Objective To investigate the relationship between the clinical onset of cerebral infarction in anterior cycle and properties of carotid atheromatous plaque.Methods A retrospective analysis was carried out on clinical data of 624 cases of cerebral infarction in anterior cycle from December 2010 to December 2013 in our hospital.The carotid artery plaque Crouse score,index,and intima-media thickness (IMT) were used to evaluate carotid atherosclerosis and plaque lesions.The National Institute of Health stroke scale (NIHSS) questionnaire was used to evaluate neural function defect.Results Compared to the non-lesion side,the lesion side carotid artery plaque Crouse score was decreased and IMT was increased (P < 0.05) ; the lesion carotid artery plaque Crouse was lower,the higher NIHSS scores (r =-0.546,P =0.004) ; IMT score was higher,higher NIHSS score; carotid artery atheromatous plaque Crouse was negatively related to NIHSS score; IMT score was positively related to NIHSS score (r =0.672,P =0.003).Conclusions The clinical onset of cerebral infarction in anterior cycle was correlated with the carotid sclerosis degree.The Crouse score and IMT of the lesion have certain significance in predicting prognosis of cerebral infarction in anterior cycle.
5.Large Sample Multicenter Study on Syndrome Differentiation Standard of Premenstrual Syndrome with Liver-qiInvasion Pattern

This study was aimed to solve the problem of overall syndrome differentiation information collection on patient’s main complain of traditional Chinese medicine (TCM) four diagnostic methods, in order to make syndrome differentiation standard of premenstrual syndrome (PMS) with liver-qi invasion pattern. The method of cluster sampling was applied in the investigation. The appropriate structural equation modeling (SEM) was established according to multi-dimension indexes of case features and TCM theories. The results of the “information acquisition form for doctor” structural equation showed that emotion dimension, body-symptom dimension, organism condition dimension load coefficients were 0.715, 0.574 and 0.904, respectively. The results of the “information acquisition form for patient” structural equation showed that emotion dimension, body-symptom dimension, organism condition dimension load coefficients were 0.705, 0.945 and 0.848, respectively. There were strong relativities among all dimensions. Combined with TCM theories, the main symptom of identified PMS with liver-qi invasion pattern was “irritable and angry”. And the secondary symptoms contained headache, breast tenderness, distention and pain of lower abdomen and stomach. The combining symptoms were insomnia, dreaminess, swelling of extremities, fatigue, inefficiency for work and housework, hard to focus the attention. It was concluded that the combination of information acquisition method from both the doctor and patient established a relative comprehensive information acquisition system. It established the syndrome differentiation standard of PMS with liver-qi invasion pattern for the first time based on epidemiologic investigation. It provided important reference for the study of PMS subtype.

Editor's Notes: The basic theory of traditional Chinese medicine is the theoretical foundation of Chinese medicine , modernization of Chinese medicine needs of the modern theory of Chinese medicine urgently . However , it is always the important issues about how modern Chinese medicine theory should be created , and what kind of theory should have been established concerned and explored by domestic and foreign counterparts in tradi-tional Chinese medicine . To lead the development of modern Chinese medicine theory , Modern Basic Theory of Traditional Chinese Medicine series of symposiums are set up specially in the current Review column, and discussed thoroughly and systematically using three consecutive pats respectively. Among them, part I contains 3 articles about new concepts, new theories, new disciplines; part II contains 5 articles about new ideas, new norms, how to clear concept, how to verify hypotheses, why to evaluate theory; part III contains 12 articles about concept of wholism updating, treatment based on syndrome differentiation development, viscera-state doc-trine innovation, pathogeny, etiology, pathogenesis, prevention, principle of treatment, knowledge structure, cogni-tion structure , monographic study , 5-year outlook . Published article on this issue is the first one belong to new concept of part I. In this paper, we want to describe the denotation and connotation of the new concept of the modern basic theory of traditional Chinese medicine from three aspects of a concept: a label , a theoretical definition , and an operational definition learning from the international academic understanding of scientific concepts, demonstrate the necessity of theoretical premise to the research and establishment of the modern basic theory of traditional Chinese medicine and the need to build on , illustrate the inevitable trend toward modern science . 8.Titanium wiring for fixation of distal clavicle fractures and acromioclavicular joint dislocation in 37 cases

AIM: To observe the curative effect of titanium wiring in treatment of the fractures of Neer Ⅱ type distal clavicle and dislocation of Tossy Ⅲ type acromioclavicular joint. METHODS: From March 2004 to December 2006, 37 cases with Neer Ⅱ type distal clavicle fractures or Tossy Ⅲ type acromioclavicular joint dislocation were treated with titanium wiring in Changzheng Hospital, Second Military Medical University of Chinese PLA. Titanium wiring (produced by Sofamor-Danek Company in USA) had good histocompatibility, intensity of tension and flexibility. The broken coracoclavicular ligament was repaired directly by titanium wiring in all cases. The outcomes were evaluated according to Herscovici's criteria. RESULTS: All the cases were followed up for 6 to 24 months. All the dislocated acromioclavicular joints returned to normal 2 weeks after operation. Excellent and good rate of shoulder function was 100% according to Herscovici standard. X-ray imaging showed all clavicle fractures united, acromioclavicular joint had no redislocation or titanium wiring breaking. The incisions of all cases had no obvious swelling and exudation. CONCLUSION: When Neer Ⅱ type distal clavicular fracture and Tossy Ⅲ type acromioclavicular joint dislocation are treated with titanium wiring, the acromioclavicular joint can exercise earlier and recover quickly.
9.A case report of Guillain-Barré syndrome associated with subarachnoid hemorrhagic aneurysm embolization

A case of a 47-year-old female patient who had a severe headache for 2 days was reported. Spontaneous subarachnoid hemorrhage (SAH, hunt-Hess classification Ⅱ level)-anterior communicating aneurysm rupture was diagnosed by brain CT and digital subtraction angiography (DSA) examination.Intravascular interventional aneurysm embolization was performed, and the patient appeared progressive limb weakness 5 days after operation. Physical examination revealed grade Ⅱ upper limb muscle strength and grade Ⅰ lower limb muscle strength. The muscle tone of the extremities was decreased,and the tendon reflex disappeared. The pinprick pain was decreased below the double wrist joints and the ankle joints. After neuroelectrophysiology test and cerebrospinal fluid assay, the patient was diagnosed as Guillain-Barré syndrome associated with subarachnoid hemorrhagic aneurysm embolization.
