Chinese Journal of Clinical Infectious Diseases 2020;13(5):335-340
Objective:To explore the risk factors of bacterial infections in patients with decompensated hepatitis B cirrhosis and to construct a risk prediction model. Methods:The clinical data of 198 patients with decompensated hepatitis B cirrhosis admitted in Zhejiang Provincial People’s Hospital from January 2014 to April 2020 were retrospectively analyzed. There were 86 patients with bacterial infection (infection group) and 112 patients without bacterial infections (non-infection group). The risk factors of bacterial infections were analyzed by multivariate Logistic regression. R language was used to establish a nomogram model to predict the risk of bacterial infection in patients with decompensated hepatitis B cirrhosis. Receiver operating characteristic (ROC) curve was used to explore the prediction efficiency of the nomogram model for bacterial infection. Results:Multivariate logistic regression analysis showed that previous smoking history, prothrombin time, neutrophil count and hypersensitive C protein were independent risk factors for bacterial infection in patients with decompensated hepatitis B cirrhosis ( P<0.05 or <0.01), while regular antiviral treatment and high-density lipoprotein were protective factors ( P<0.05 or <0.01). ROC curve showed that the area under the curve (AUC) of risk prediction model for bacterial infections was 0.872 (95% CI 0.820-0.924, P<0.01), and AUC of MELD score for predicting bacterial infections was 0.670 (95% CI 0.599-0.735, P<0.01); the risk prediction model was superior to MELD score in prediction ( Z=4.89, P<0.01). Conclusions:The established risk prediction model in this study can more accurately predict the occurrence of bacterial infections in patients with decompensated hepatitis B cirrhosis.

Objective To explore the effect of topical application of sodium hyaluronate on preventing perivascular adhesion of the vein grafts in rabbits. Methods Thirty-six male New Zealand white rabbits, aged 5 months, were randomly and equally divided into 2 groups: groups A and B. Arterial defect model was established by cutting about 1cm artery from the middle part of the dissected left common carotid artery. A section about 3cm was cut from the right external jugular vein, and the harvested vein was inverted and anastomosed end-to-end to the artery defect. After the anastomosis, the adventitia and two anastomoses of the grafted veins in group A were coated locally with 0.2ml sodium hyaluronate. The grafted veins were obtained 1, 2 and 4 weeks after the operation, with the perivascular adhesion of the vein grafts being examined macroscopically before the resection. HE staining and Masson staining were preformed for histological changes of grafted vein wall and the perivascular adhesion of the vein grafts. At 2, 4 weeks postoperation, the perivascular adhesions of the vein grafts were graded by the grading criteria of adhesion in macroscopic evaluation and histological evaluation. Result At 1, 2 and 4 weeks postoperatively, the macroscopic and histological observation found that the perivascular adhesions in group A were looser than those in group B. The macroscopic grade and histological grade were lower in group A than in group B, there was a significant difference between the two groups at 2 and 4 weeks postoperation (P<0.05). Conclusion Topical application of sodium hyaluronate can reduce the perivascular adhesion and is an ideal treatment strategy for preventing perivascular adhesion of vein grafts.

Objective:To explore indicators related to visceral fat index by constructing a random forest model. Methods:In this cross-sectional study, the laboratory measures and body composition analysis records of 617 hospital employees (in-service and retired) who underwent physical examination in Heilongjiang Provincial Hospital Health Management Center from March to September 2021 were selected. The subjects were divided into a training set ( n=411) and a test set ( n=206) with the ratio of 2∶1. A total of 110 predictors were included in the model. The model was constructed with the training set and was evaluated with the test set. The optimal number of nodes and decision trees were selected to evaluate the prediction performance of the optimal model. And the top 10 relatively important factors were selected for further investigation. The 617 participants were further divided in to groups according to the visceral fat index: the normal or high visceral fat index group, and the differences of the top 10 relatively important factors were further compared between the two groups. Results:The optimal number of nodes of the final random forest model was 39 and the number of decision trees was 300. The accuracy, precision, sensitivity and specificity of the model was 83.3%, 73.9%, 89.4% and 78.7%, respectively. The area under the receiver operating characteristic curve and 95% confidence interval of the model was 0.881 (0.832-0.931). The top 10 relatively important factors in the model were body mass index, gender, age, serum uric acid, red blood cell count, monocyte cell count, C-peptide, carcinoembryonic antigen, glycosylated hemoglobin and glutamyl transpeptidase. There were significant differences in the up-mentioned 10 indicators between the subjects with normal and high visceral fat index (all P<0.05). Conclusions:The random forest model built in this study has good performance in predicting visceral fat index, and visceral fat is related with changes in liver function, pancreas function and immune function.

Chinese Journal of Radiological Medicine and Protection 2017;37(9):690-695
Objective To evaluate the effect of setup errors on the 2D image projection and image registration, and then propose an improved registration method based on mutual information. Methods An anthropomorphic head phantom was used to simulate the rotational and translational setup errors. The geometric disparities were reflected by the changes of mutual information. Known setup errors were intentionally introduced to twenty cases divided into two groups demarcated by 3 mm translation error and 3° rotation error: ten cases with larger errors and ten with smaller errors. Then the anterior-posterior and lateral portal images were captured by the electronic portal imaging device ( EPID ) , based on which the setup errors were calculated using two mutual information registration method respectively: the vender provided one, and the improved method as proposed. The calculated errors were compared with the actual setup errors to evaluate robustness of the method. Results For the ten cases with smaller setup errors, the average translational registration disparities using the conventional method were 0. 3, 0. 4, and 0. 3 mm in x, y and z directions respectively. The rotational disagreements were 0. 4° in both x and z directions. The average time consumption was 28. 7 s. The corresponding discrepancies analyzed using the improved method were 0. 3, 0. 4, 0. 3 mm, 0. 5° and 0. 4°, respectively. On average, 31. 1 s was needed for registration. For the ten cases with larger setup errors, the mean disparities of the conventional method were 0. 9, 0. 7, 0. 8 mm, 0. 9° and 0. 8°, 29. 9 s taken on average. The corresponding result of the improved method was 0. 5, 0. 4, 0. 5 mm, 0. 6° and 0. 5°, 33. 2 s taken on average. Conclusions Regarding smaller setup errors, the two methods showed little difference and both had good performance in imageregistration accuracy. For larger setup errors, however, the improved mutual information registration method exhibited significantly higher accuracy than the conventional method, at cost of clinically acceptable registration time.
