1.Quantitative detection of the loop mediated isothermal amplification gene in the loopmediated isothermal amplification of Japanese encephalitis and dengue fever

Objective To establish a loop-mediated isothermal amplification(LAMP) quantitative method for rapid detection of Japanese encephalitis and dengue fever virus.Methods According to the LAMP principle,design primers for LAMP detection and reaction system,establish LAMP detection method,and to evaluate the linear relationship between initial copy number and the specificity,sensitivity,repeatability and the reaction time(fluorescence signal value of 1×104 corresponding time).Results One sets of LAMP primers could be used to complete the detection work in 0.5 h.The sensitivity of LAMP detection technology was 10 times higher than that of the classical PCR technology,and no cross reaction with other viruses,and the coefficient of variation of the average test was less than 5%.There was a good linear relationship between cycle threshold and template concentration.Conclusion This method has high specificity,sensitivity,simple operation,which is easy to get the results,low equipment requirements and rapid,suitable for primary health institutions and the field inspection agencies for wide applications.
2.Efficacy of anesthesia with different doses of dexmedetomidine combined with propofol and remifentanil in patients undergoing abdominal surgery

Objective To evaluate the efficacy of anesthesia with different doses of dexmedetomidine combined with propofol and remifentanil in patients undergoing abdominal surgery.Methods Ninety ASA Ⅰ or Ⅱ patients of both sexes,aged 18-64 yr,weighing 45-80 kg,undergoing abdominal surgery,were randomly assigned into 3 groups ( n =30 each):control group (group C ),dexmedetomidine 0.25 μg· kg-1 · h-1 group ( group D1 ) and dexmedetomidine 0.50 μg· kg-1·h-1 group (group D2 ).Dexmedetomidine was infused at a rate of 0.25 μg·kg-1 ·h-1 and 0.50 μg·kg-1 ·h-1 in groups D1 and D2 respectively until extubation after a loading dose of 0.5 μg/kg over 15 min.In group C,normal saline was infused intravenously at a rate of 10 ml/h.Anesthesia was induced with TCI of propofol with the target plasma concentration (Cp) of 1.0 μg/ml,iv injection of cisatracurium 0.2 mg/kg and TCI of remifentanil with Cp of 3 ng/ml.The patients were tracheal intubated and mechanically ventilated.PETCO2 was maintained 35-40 mm Hg and SpO2 was maintained ≥95%.Anesthesia was maintained with TCI of remifentanil with the target Cp of 5 ng/ml,iv infusion of cisatracurium 0.1 mg· kg-1 · h-1 and TCI of propofol.The target Cp of propofol was increased to maintain Narcotrend index of 37-46.The amount of remifentanil,cisatracurium and propofol consumed,extubation time and eye-opening time,complications during operation and during recovery from anesthesia were recorded.Results There was no significant difference in the amount of remifentanil and cisatracurium consumed and extubation time among the three groups ( P > 0.05).Compared with group C,the eye-opening time was significantly prolonged,the incidence of hypertension and tachycardia during operation,and restlessness,vomitting,hypertension,and tachycardia during recovery from anesthesia was significantly decreased in groups D1 and D2,and the amount of propofol consumed was significantly decreased in group D2 (P < 0.05).Compared with group D1,the eye-opening time was significantly prolonged,the incidence of hypertension during operation,and restlessness,hypertension,and tachycardia during recovery from anesthesia was significantly decreased in group D2 ( P < 0.05).Conclusion When combined with propofol and remifentanil,dexmedetomidine infused at a rate of 0.50 μg·kg-1 · h-1 can provide satisfactory efficacy for abdominal surgery.
3.Impact of steatotic liver grafts on early liver function and prognosis of Hver transplantation recipients

Objective To evaluate the impact of steatotic hepatic grafts on postoperative early liver function and prognosis.Methods Clinical data of 35 cases receiving steatotic liver grafts in our hospital were retrospectively analyzed.The donor liver grafts were divided into 3 groups based on the degree of fatty infiltration(<20%group,20%~40%group,≥40%group).Results were compared with that from control normal grafts(41 cases).Liver function parameters including AST,ALT,TB,PT on the day of surgery and 1,2,3,7 days later were monitored among the 4 groups,as well as the incidence of delayed nonfunction (DNF),and 3 and 6 months patient survival rate.Analysis of the correlation between degree of donor steatosis and liver function after operation was carried out.Resuits There was significant difference in postoperative ALT、AST、TB、PT level between≥40%group and 20%~40%group,and between 20%~40%and group<20%group.There was no significant difference among the groups in DNF occurrence rate and 3 month.6 month patient survival rate.Correlation analysis suggested that the degree of donor steatosis was positively correlated with the damage of liver function.Conclusions Grafts with≤20% steatosis was safe,whereas≥20% especially ≥40% steatosis carries a high risk of posttransplant liver dysfunction even delayed nonfunction.
4.Research and development of genetic diagnostic method of Staphyloccocus aureus based on loop-mediated isothermal amplification

Objective To develop a method of loop-mediated isothermal amplification(LAMP) to Staphyloccocus aureus rapidly, specifically, sensitively and simply suited for the primary health agency. Methods According to conserved nucleotide of Staphyloccocus aureus and principle of LAMP, we designed a set of LAMP primers and set up an LAMP reaction system. We evaluated the specificity, sensitivity and re-peatability of the method. In addition,we evaluated the linearity between initial template copies 1g value and reaction time (the time when the fluorescent value is 1×10~4). Results The optimal assay showed that it was no cross-reaction with other closely related members of pathogens, and was 10 times more sensitive than PCR. The coefficient of variance between tests was less than 5% ,and the kinetics curves showed a good line-arity between initial template copies lg value and reaction time(r~2=0. 9501). The detection activity could be finished within 1 h with the sensitivity of LAMP was 100% and the specificity was 94.4%, and the accuracy was 96.6%. Conclusion These findings demonstrated that the LAMP had the potential clinical application for detection and differentiation of Staphyloccocus aureus in the public health agency for its sensitive, specific and simple feature.
5.Analysis on mental health status of health system staffs in a district of Chongqing municipality

Objective To investigate the mental health states of health system staffs in a district of Chongqing municipality. Methods Seven hundred and seventy-three health system staffs were taken as the research subjects for conducting the investigation by the Symptoms Checklist-90 (SCL-90).With the total SCL-90 score >160 as the positive psychological symptom,the total mean scores of SCL-90 and the 9 items of factor score of somatization,obsession,personal relations,depression,anxiety,hostility,dread, paranoid and psychotics were performed the statistical analysis.Results The positive rate of psychological symptoms in 773 health system staffs was 17.46% (135/773),among them which in the clinical nurses,clinicians and nonclinical staffs were 23.20%(58/250),15.63%(50/320)and 13.30%(27/203)respectively.The somatization and obsession scores of the health system staffs in this district were obviously higher than those of the national norm (P <0.05),while their personal relations and paranoid scores were obviously lower than those of the national norm(P <0.01).The paranoid scores of the staffs in the district health units were higher than those of the staffs worked in town health units(P <0.05),and other 8 items of factor score and the total mean scores were significantly higher than those in the town health unit staffs (P <0.01).The scores of obsession,depression,anxiety,hostility and the total mean scores of the clinical nurses were significantly higher than those of the clinicians (P <0.01),while scores of so-matization,personal relations,paranoid and psychotics had no significant differences between them(P > 0.05).The total mean scores of the clinical nurses were higher than those of the non-clinical staffs(P <0.05).Conclusion The scores of multiple factors had significant differences between the health system staffs in this district and the national norm.The psychological health status in the clinicians was poorer than that in the non-clinical staffs,especially the psychological health status in the clinical nurses were much poorer.
6.Effect of continuous nursing intervention in tumor patients with PICC

8.Prenatal diagnosis of two Turner syndrome fetuses with 46,X,i(X)(q10)

Objective To summarize the prenatal diagnosis and genetic counseling of Turner syndrome fetuses with 46,X,i(X)(q10).Methods Two gravidas admitted to the Obstetrics and Gynecology Hospital of Dalian were enrolled in this study.One gravida,who was admitted in October 2016,was classified as high risk of Down syndrome based on prenatal serologic screening and systematic ultrasonography,which found remarkably shorter humeri and femora than fetus of the same gestations.The other was suggested to be monosomy X after non-invasive prenatal testing and admitted in November 2017.Fluorescence in situ hybridization (FISH) and karyotyping were performed for prenatal diagnosis.Peripheral blood karyotyping was also offered to the two women and their partners.Results FISH test for amniotic fluid did not find numerical abnormality in 13,18,21,and sex chromosomes in these two fetuses.Karyotype analysis showed that the two fetuses were both 46,X,i(X) (q10),while their parents were normal.Both cases were terminated after genetic counseling.Conclusions Prenatal serological screening,systematic ultrasonography and non-invasive prenatal testing may help to identify Turner syndrome fetus of 46,X,i(X) (q10).Timely and accurate prenatal diagnosis may prevent the affected fetus from being born.
9.Effects on female depression treated with the combined therapy of acupuncture and the five-element music therapy.

OBJECTIVE:
To observe the clinical effects and safety on female depression treated with the combined therapy of acupuncture and the five-element music therapy.
METHODS:
A total of 120 female patients with depression were randomized into an electroacupuncture (EA) group, an acupuncture group, a five-element music therapy group and a group of the combined therapy of acupuncture and the five-element music therapy (combined therapy group), 30 cases in each one. In the EA group, the electric stimulation of acupuncture was applied at Baihui (GV 20), Shenting (GV 24), Neiguan (PC 6), Shenmen (HT 7), Sanyinjiao (SP 6) and Taichong (LR 3), with disperse-dense wave, 10 Hz/50 Hz, and 0.5 ms in wavelength. The needles were retained for 30 min. In the acupuncture group, the acupoints were same as the EA group, but without electric stimulation. In the five-element music therapy group, the music of tune was adopted in the treatment. In the combined therapy group, the acupuncture treatment and the five-element music therapy were same as the acupuncture group and the five-element music therapy group, in which the music therapy was followed after acupuncture treatment. The treatment was given once a day, at 2 days of interval after every 5 treatments in a week, totally for 8 weeks. The Hamilton depression scale (HAMD), Hamilton anxiety scale (HAMA) before and after treatment and the treatment emergent symptom scale (TESS) after treatment were observed in each group. The clinical effects and safety were compared among the groups.
RESULTS:
The total effective rate was 86.7% (26/30) in the combined therapy group, better than 66.7% (20/30) in the EA group, 63.3% (19/30) in the acupuncture group and 53.3% (16/30) in the five-element music therapy group (<0.01, <0.05). After treatment, HAMD and HAMA scores were all reduced obviously in the patients of the 4 groups (all <0.01), and the scores in the combined therapy group were lower than the other 3 groups (all <0.01). The TESS score in the combined therapy group was lower than the other 3 groups (all <0.01) and the score in the five-element music therapy group was lower than the EA group and the acupuncture group (both <0.01).
CONCLUSION
In the female patients, the combined treatment with acupuncture and the five-element music therapy obviously reduces the severity of depression and anxiety, as well as the adverse reactions. The effects are better than the simple application of EA, acupuncture or music therapy.

10.Porphyromonas gingivalis outer membrane vesicles activate Toll-like receptor 2 to promote osteoclast differentiation by carrying lipopolysaccharide

Objective:To investigate the effects of Porphyromonas gingivalis derived outer membrane vesicles (Pg OMV) on osteoclast differentiation of macrophages and its underlying mechanisms. Methods:The morphology and the size distribution of Pg OMV were analyzed by transmission electron microscopy and nanoparticle tracing analysis, respectively. The osteoclast precursors were treated with 1, 3 and 10 mg/L Pg OMV (1, 3 and 10 mg/L OMV treatment group) or phosphate buffer solution (PBS)(control group). The formation of osteoclasts was analyzed by tartrate-resistant acid phosphase (TRAP) staining and F-actin staining and real-time quantitative PCR (RT-qPCR) were used to detect the expression of Fos and matrix metallopeptidase 9 (MMP9). Polymyxin B (PMB) was used to block lipopolysaccharide (LPS) and then Pg OMV was used to treat osteoclast precursor (PMB-OMV treatment group), and OMV treatment group was used as control. TRAP and F-actin staining were used to observe the formation of osteoclasts and actin rings. The effect of Pg OMV on the expression of Toll-like receptor (TLR) 2 and TLR4 in preosteoclasts was detected by Western blotting. The osteoclast precursors were pretreated with 10, 50, 100 and 200 μmol/L C29, an inhibitor of TLR2, and then treated with Pg OMV(OMV+10, 50, 100 and 200 μmol/L C29 treatment group) and OMV treatment group without C29 pretreatment was control. TRAP and F-actin staining were used to observe the formation of osteoclasts and actin rings. The osteoclast precursor cells were treated with OMV (OMV treatment group) and OMV incubated with PMB (PMB-OMV treatment group) and the expression of TLR2 in osteoclast precursor was detected by Western blotting.Results:Pg OMV showed classical vesicular structures, and the average particle size of Pg OMV were 179.2 nm. A large number of actin rings were observed in the 3 and 10 mg/L OMV treatment groups. The percentages of TRAP-positive osteoclast area in 3 mg/L OMV treatment group [(22.6±2.1)%] and 10 mg/L OMV treatment group [(32.0±2.3)%] were significantly increased compared with control group [(4.9±0.5)%] ( P<0.001). Compared with the control group (1.000±0.029), the mRNA relative expression of Fos in 3 mg/L OMV treatment group (1.491±0.114) and 10 mg/L OMV treatment group (1.726±0.254) was significantly increased ( P=0.013, P=0.001). Compared with the control group (1.007±0.148), the mRNA relative expression of MMP9 in the group of 10 mg/L OMV (2.232±0.097) was significantly increased ( P<0.001). Actin ring formation was less in PMB-OMV treatment groups than in OMV treatment groups. The proportion of TRAP-positive osteoclasts area [(14.8±3.8)%] in PMB-OMV treatment group was significantly lower than OMV treatment group [(31.5±6.7) %] ( P=0.004). The relative expression of TLR2 in OMV treatment group (1.359±0.134) was significantly higher than that in the control group (1.000±0.000) ( t=4.62, P=0.044). Compared with the OMV treatment group [(29.4±1.7)%], 50, 100 and 200 μmol/L C29 significantly decreased the formation of osteoclasts [(24.0±1.7)%, (18.5±2.1)%, (9.1±1.3) %] ( P=0.026, P<0.001, P<0.001). TLR2 protein expression in PMB-OMV group (0.780±0.046) was significantly lower than that in OMV group (1.000±0.000)( t=8.32, P=0.001). Conclusions:Pg OMV can promote osteoclast differentiation by carrying LPS, TLR2 plays an important role in Pg OMV mediated osteoclast differentiation.
