1.Effect of berberine on expression of vascular endothelial growth factor in diabetic nephropathy rats

Aim To investigate the renoprotective effect of berberine in diabetic nephropathy rat model. Methods The rat model of DN was induced by intra-peritoneal injection of streptozotocin ( STZ ) after fed with high sugar and high fat diet for six weeks. The rats were divided into 5 groups randomly, i. e. normal control group, model group, BBR ( 50 mg · kg-1 ) , BBR ( 100 mg · kg-1 ) and BBR ( 200 mg · kg-1 ) treatment group. The fasting blood glucose ( FBG) was evaluated at 2, 4, 6,8 week respectively. The patho-logical changes in the kidney were determined by PAS staining. The expression of vascular endothelial growth factor ( VEGF) was detected by immunohistochemistry and Western blot respectively. Results Compared with normal control group, the value of FBG, SCr, BUN and UTP of model group were sharply increased.
Compared with model group, the value of FBG in ber-berine different dosage treatment groups were signifi-cantly decreased to various degrees, and berberine dif-ferent dosage treatment could decrease the levels of SCr, BUN and UTP in different degree. Berberine could surpress the alterations of pathological changes in the kidneys and downregulate the expression levels of VEGF in the kidney of diabetic rats with nephropathy. Conclusion Berberine could significantly ameliorate the biochemical indicators and renal injury of the model rats through affecting the abnormal expression levels of VEGF in the kidney.
2.Management of polypoid lesions of the gallbladder: an analysis of 748 patients

Objective To investigate the risk factors of gallbladder neoplastic polyps,to determine the proper surgical indications,and to understand more about the clinical characteristics of gallbladder adenoma.Methods The clinical data of 748 patients diagnosed to have polypoid lesion of gallbladder (PLG) and underwent cholecystectomy from January 1998 to December 2012 in the Second Affiliated Hospital of Zhejiang University School of Medicine were retrospectively reviewed.Results Among 748 patients,340 had abdominal symptoms.Postoperative histopathology showed non-neoplastic polyps (n =659),gallbladder adenoma (n =68),gallbladder cancer (n =15) and no polyps (n =6).The mean diameters of the nonneoplastic lesions and the neoplastic polyps were (9.38 ± 3.44) mm and (14.55 ± 5.71) mm,respectively (P < 0.01).The average age of the patients with non-neoplastic lesions was (44.14 ± 11.42) years and (47.39 ± 12.82) years in those with neoplastic polyps (P < 0.05).The proportion of solitary PLG was 41.4% (253/611) and 59.59% (47/79) for the non-neoplastic lesions and the neoplastic polyps respec tively (P < 0.01).The size of PLG (13.34 ± 4.18 mm vs.20.07 ± 8.19 mm,P < 0.05) and the age of the patient (45.78 ± 11.66 years vs.54.13 ± 15.82 years,P <0.05) between the gallbladder adenoma and gallbladder cancer groups were significantly different.Gallbladder adenoma or dysplasia was identified in 66.7% (10/15) of gal1bladder cancer specimens.Conclusions Patients with PLG and with abdominal symptoms,large size (≥ 10 mm),old age (≥50 y) and solitary polyp are indications for cholecystectomy.Gallbladder adenoma may develop to gallbladder cancer within 10 years.
3.Nosocomial Infection During Radiotherapy with Concomitant Weekly Paclitaxel for Advanced Nasopharyngeal Carcinoma: A Clinical Investigation

OBJECTIVE To investigate the risk factors of nosocomial infection in patients with advanced nasopharyngeal cancer during radiotherapy with concomitant weekly paclitaxel treatment. METHODS Twenty six cases of advanced nasopharyngeal cancer patients in our hospital were analyzed retrospectively. RESULTS There were 20 cases of nosocomial infection from inpatients.The infection rate was 76.9 % and isolates of bacteria were found,of which Candida albicans accounted for 46.2%,Gram-negatives were 38.5%,Gram-positives were 11.5%.The main risk factors were long time hospitalization,oral mucosal lesion,leukocytopenia,improper application of antibiotic and glucocorticoids. CONCLUSIONS To prevent and control nosocomial infection in advanced nasopharyngeal cancer during radiotherapy and concomitant chemotherapy,measures such as shortening hospitalization days,reinforcing oral nursing,preventing the leukocytopenia,reasonable using of antibiotics and glucocorticoids should be adopted.
4.A application study of dose constraint template in intensity modulated radiation therapy planning for early stage nasopharyngeal carcinoma

Objective To investigate a feasibility of using dose constraint template (DCT) to increase conformity index (CI) of planning target volume (PTV) and improve intensity modulated radiation therapy (IMRT) planning efficiency for early stage nasopharyngeal carcinoma. Methods Ten patients with pathological diagnosed and treated by IMRT were selected for this study. Target volumes were delineated with Corvus 6.3 of treatment planning system, two dose limiting regions(DLR) around PIN were added by extending from PIN,each DLR was 1 cm thick. We created three plans:Plan0,Planl and Plan2. PianO was without DLR and DCT, Planl without DLR but with DCT, Plan2 with both condition;but to compare dose distribution in PLTV and normal tissue using three plans. Results Three plans could fill equal request of dose distribution in PLTV and normal tissue, and their difference was not statistical significant. CI of Plan2 was increased and planning time was decreased significantly compared with Piano and Planl. Conclusloa Usage of DCT together with DLR can increase CI of PTV and improve IMRT planning efficiency for early stage nasopharyngeal carcinoma, planning time is shortened significantly.
5.Effect of berberine on the expression of nephrin, podocin and intergrin α3β1 in diabetic nephropathy rats

Aim To investigate the effect of berberine on the expression of nephrin, podocin and intergrinα3β1 in diabetic nephropathy ( DN ) rat model, and further probe in to the renoprotective effects of berber-ine and its potential mechanisms. Methods The rat model of DN was induced by intraperitoneal injection of streptozotocin ( STZ ) after fed with high-sugar and high-fat diet for six weeks. The rats were assigned into 6 groups randomly: normal control group, DN model group, BBR (50,100 and 200 mg·kg-1 ) treatment group and enalaprilat positive control group ( 1 mg · kg-1 ) . The distribution and expression of kidney podocyte related proteins nephrin, podocin and interg-rinα3β1 were detected by immunohistochemical meth-od following electron microscopy observation ( × 1000 ) and high magnification observation( × 400) and West-ern blot. Results The podocyte related protein neph-rin, podocin and intergrin α3β1 were mainly distribu-ted in podocyte, but slightly different. Compared with normal control group, the expresion of podocyte related protein nephrin, podocin and intergrin α3β1 was de-creased obviously; compared with model group, BBR (100 and 200 mg·kg-1 ) treatment group could sig-nificantly suppress the abnormalities of pathological changes of the kidney and upregulate the expression levels of podocyte specific protein nephrin, podocin and intergrin α3β1 in the kidney of diabetic rats with nephropathy. Conclusions Berberine could alleviate the abnormalities of kidney pathological changes and proteinuria production in the DN model rats, which may be related to the upregulation of the expression of the podocyte proteins nephrin, podocin and intergrinα3β1.
6.Application of Rectal Prolapse Constipation Balloon in Single Auxiliary Defecation.

OBJECTIVE:
To explore the application value of rectal prolapse constipation balloon in single auxiliary defecation.
METHODS:
Forty-one patients with moderate or severe rectocele were treated with a rectocele constipation balloon through the vagina. The defecography and VAS scores were compared before and after implantation.
RESULTS:
There was a significant difference between the anorectal angle, rectocele, and VAS scores before and after intervention in defecography (<0.01).
CONCLUSIONS
A single assisted defecation of the rectocelicular constipation balloon is feasible.

7.Method and practice of integrating online and offline hybrid teaching of Animal Immunology into curriculum ideology and politics

Objective:In order to truly realize the deep integration of ideological and political education and professional edu-cation in the course of Animal Immunology and effectively improve the comprehensive quality of students.Methods:The experiment adopts the teaching method of integrating the online and offline hybrid teaching of Animal Immunology into the ideology and politics of the course,studies the course design concept,teaching objectives,teaching design and teaching implementation,and will conduct an evaluation of the teaching effect from the examination results and ideological and political effect.Results:The results show that the teaching model has stimulated students'interest in learning and improved their overall ability.Conclusion:It shows that the teaching effect of this method is good and the basic task of moral education is realized.
8.Cinobufagin Combined with Thalidomide/Dexamethasone Regimen in the Treatment of Patients with Newly Diagnosed Multiple Myeloma of Phlegm and Stasis Obstruction: A Retrospective Study

ObjectiveTo investigate the efficacy and safety of cinobufagin tablets combined with thalidomide/dexamethasone (TD) regimen in the treatment of newly diagnosed multiple myeloma (NDMM) with phlegm and stasis obstruction. MethodsThe clinical data of 50 patients with NDMM of phlegm and stasis obstruction who were hospitalized at the Jiangsu Province Hospital of Chinese Medicine from June 1st, 2015 to July 31th, 2019 were retrospectively analyzed, and they were divided into a control group (bortezomib/dexamethasone-containing regimen, 27 cases) and an observation group (cinobufagin tablets combined with TD regimen, 23 cases). The clinical efficacy and safety were compared between the two groups after two or three courses of treatment. The primary outcomes were clinical remission rate including overall response rate and deep remission rate, one-year and two-year overall survival rate, and adverse effects. The secondary outcomes were the proportion of plasma cells in bone marrow, hemoglobin, β2-microglobulin, lactate dehydrogenase, serum creatinine, blood urea nitrogen, bone pain score, and KPS functional status score (KPS score) before and after treatment. ResultsIn terms of clinical efficacy, there was no statistically significant difference (P>0.05) in the overall response rate [the observation group 69.57%(16/23) vs the control group 70.37% (19/27)] and deep remission rate [the observation group 56.52% (13/23) vs the control group 55.56% (15/27)] between groups after the treatment. The one-year overall survival rates of the observation group and the control group were 90.9% and 92.4%, and the two-year overall survival rates were 81.8% and 80.9% respectively, with no statistically significant differences between groups (P>0.05). During the treatment, no renal function injury occurred in both groups. The incidence of peripheral nerve injury in the observation group was 8.70%, which was lower than 48.15% in the control group (P<0.01). After the treatment, the proportion of myeloma plasma cells, β2-microglobulin, serum creatinine level, and bone pain score decreased, while the hemoglobin level and KPS score increased in both groups (P<0.05 or P<0.01). Compared between groups after treatment, the bone pain score of the observation group was lower than that of the control group, while the KPS score was higher than that of the control group (P<0.05). ConclusionThe clinical efficacy of cinobufagin tablets combined with TD in the treatment of NDMM is equivalent to bortezomib/dexamethasone-containing regimen, but the former is more helpful in relieving the pain and improving the quality of life, and has better safety.
