1.Analysis of laparoscopic modified ventrifixation in treating famale uterine prolaps

Objective To discuss the safety and efficacy of laparoscopic modified ventrifixation for women with uterine prolapse.Methods Thirty-seven women with Ⅱ-Ⅳ degree uterine prolapse were undergent laparoscopic modified ventrifixation in the the First Hospital of Qinhuangdao and Anzhen Hospital of Beijing from January 2008 to June 2012.The midpart of a noabsorble PROLENE soft mesh was sutured to the anterior cervical fascia.Two back ends of the mesh were passed through extraperitoneal channels through ligamentum latum uteri and sutured to the abdorminal wall to fix uterus.The effect and complications were observed.Results The objective and subjective success rate were 100% and 91.9% respcetively at 6 months after operation.All patients were followed up for 6 to 48 months.Prolapse recurrence rate was 10.8% (4/37).Conclusion Laparoscopic modified ventrifixation is effective,safe and mini-invasive in the treatment of uterine prolapse.The surgery may be a satisfactory procedure for women with uterine prolapse hoping for uterine preservation.

AIM To study the effects of PDB on blood glucose. METHODS 3 groups of health mice and 3 groups of model mice were administered with high, mean and low doses of PDB compatibility groups (combined with glibenclamide 0.66 mg?kg -1) respectively for 15 days. The blood goucose variation of health and model mice was observed. RESULTS A low dose of PDB compatibility group decreased obviously blood glucose in the health mice. 3.Diagnostic value and safety analysis of thoracoscopic lung biopsy in diffuse parenchymal lung disease

Objective To investigate the clinical value and safety of thoracoscopic lung biopsy in the diagnosis of diffuse parenchymal lung diseases. Methods 35 patients with diffuse parenchymal lung disease underwent thoraco-scopic lung biopsy were reviewed in order to obtain clinical and pathological data. Results 31 cases were diagnosed by pathology, and the total positive diagnostic rate was 88.6 % (31/35), 10 cases as usual interstitial pneumonial (UIP), 6 cases as Tuberculosis, 4 cases as alveolar cell carcinoma, 3 cases as nonspecific interstitial pneumonia (NSIP), 3 cases as crypto-genieorganizingpneumonia (COP), 2 cases as pulmonary alveolar proteinosis (PAP), 1 case as Wegener's granulomatosis (WG), 1 case as acute interstitial pneumonia (AIP), 1 case as lymphangioleiomyomatosis (LAM). 3 cases (8.6 %) were occurred with postoperative complications, 2 cases as pulmonary infection combined with respiratory failure, 1 case as idiopathic pulmonary fibrosis with acute exacerbation, and 2 cases (5.7 %) were complicated with death. Conclusions Thoracoscopic lung biopsy is a safe and effective method which helps final di-agnosis of diffuse parenchymal lung disease.
4.Effect of information system management combined with PDCA on negative emotions and quality of life in patients with Parkinson's disease

Objective To explore the effect of information system management combined with PDCA on negative emotion and quality of life in patients with Parkinson's disease.Methods Totally of 232 patients with Parkinson's disease who were hospitalized in Affiliated Hospital of Ji'ning Medical University from July 2014 to June 2016 were selected and divided into observation group and control group according to the random number table, with 116 cases in each group. The control group was given routine mental health education, while the observation group was treated with the combination of PDCA and nursing intervention on the basis of routine nursing. The scores of depression, cognitive function and quality of life were measured and evaluated at three months after admission.Results At the time of admission, scores of depression showed no significant difference between two groups (t=0.965,P=0.102); after the intervention, the score of depression in the observation group was significantly lower than that in the control group, and the difference was statistically significant (t=3.359, P=0.001). Before the intervention, the score of cognitive assessment scale showed no significant difference between two groups (t=1.254,P=0.068); after the intervention, cognitive function score in the observation group was significantly higher than that in the control group, and the difference was statistically significant (t=4.021, P<0.001). The scores of physiological function, psychological function, social function, environmental adaptability and total quality of life in the observation group were improved significantly higher than those in the control group, and the differences were statistically significant (t=5.274, 4.667, 4.277, 4.148, 3.957;P<0.01).Conclusions Information system management combined with PDCA mode nursing intervention can improve the negative emotions and the quality of life of patients with Parkinson's disease.
5.Diagnostic value of Tamm-Horsfall protein and osteopontin in serum and 24-hour urine for urolithiasis

Objective To investigate the diagnostic value Tamm-Horsfall protein(THP)and osteopontin(OPN)in serum and 24-hour urine for urolithiasis.Methods A total of 101 patients with urolithiasis who underwent flexible ureteroscopy lithotripsy at the Urology Department of Civil Aviation General Hospital from April 2020 to March 2023 were included as the stone group,and 50 healthy individuals were enrolled as the control group.The samples of serum and 24-hour urine samples were collected from both the groups,and the levels of THP and OPN were measured using enzyme-linked immunosorbent assay(ELISA).Logistic regression analysis was performed to evaluate the association between each biomarker and urolithiasis,and receiver operating characteristic(ROC)curves were plotted to assess their diagnostic value.Results The stone group showed significantly lower THP levels(20.13[13.12,26.03]mg/d)and OPN levels(51.24[36.72,101.37]μg/d)in 24-hour urine,and THP levels(182.01[160.91,209.20]ng/mL)and OPN lev-els(18.76[15.72,22.48]ng/mL)in serum compared to the control group(all the P<0.001).Binary Logistic regression analysis re-vealed that THP(OR=0.736,95%CI:0.606-0.895),OPN(OR=0.975,95%CI:0.958-0.993)and citrate(OR=0.067,95%CI:0.012-0.376)in 24-hour urine,and THP(OR=0.946,95%CI:0.908-0.986)and OPN(OR=0.896,95%CI:0.803-0.999)in ser-um were the protective factors for urolithiasis,while calcium level(OR=2.125,95%CI:1.243-3.633)24-hour urine was a risk factor(all the P<0.05).ROC curve analysis showed that the areas under the curve(AUCROC)for the individual diagnosis of urolithiasis were 0.846,0.809,0.786,0.823,0.748,and 0.755 for the above six biomarkers,respectively.The AUCROC for the combined diagnosis u-sing THP+OPN in serum,THP+OPN in 24-hour urine and all the six biomarkers were 0.882,0.920 and 0.984,respectively,indica-ting better diagnostic performance.Conclusion The combined detection of the THP and OPN levels in serum and 24-hour urine may have good diagnostic value for urolithiasis and serve as potential diagnostic biomarkers.
6.Analysis of clinical features and risk factors of heat stroke patients with infection

Objective:To investigate the clinical features and risk factors of infection in patients with heat stroke (HS).Methods:The patients with HS who were admitted to the emergency departments of seven hospitals, including West China Hospital of Sichuan University, the Second Affiliated Hospital of Chengdu Medical College and so on, between July 01, 2020 and September 30, 2022 were retrospectively analyzed. The basic characteristics and laboratory test results of the patients were collected. The patients were divided into infected group and non-infected group according to the etiological results, and the differences in clinical characteristics between the different groups were compared. Multivariate logistic regression was used to screen variables, and the predictive model was established and ROC curve was performed to compare the predictive efficiency with SOFA score.Results:During the study period, a total of 183 patients with HS were included, and 156 patients were finally included for analysis according to the inclusion and exclusion criteria. Among them, 58 cases (37.2%) were in the infected group. The mortality of infected group was significantly higher than that of non-infected group (41.4% vs. 25.5%, P < 0.05). Compared with the patients in non-infected group, the infected gourp serum procalcitonin, direct bilirubin, alanine transaminase (ALT), creatinine, lactate dehydrogenase (LDH), creatine kinase isoenzymes, myoglobin, activated partial thromboplastin time (APTT) and respiratory support ratio at admission were significantly higher and albumin level was lower in infected group (all P < 0.05). Multivariate logistic regression analysis showed that myoglobin, ALT, APTT and respiratory support required at admission were independent risk factors for HS co-infection. A prediction model was constructed based on these four indicators, and the area under ROC curve of the prediction model was 0.846, which was better than SOFA score (0.732, P < 0.05). Conclusions:The prognosis of patients with heat stroke complicated with infection were obviously poor. Myoglobin, ALT, APTT and need for respiratory support at admission are risk factors for HS complicated with infection. The prediction model based on these four indicators is better than SOFA score for the early identification of the HS patients with infection.
7.Effect of ultra-early nursing intervention on postoperative cognitive impairment in patients with cerebral hemorrhage

Objective:To explore the effect of ultra-early nursing intervention on postoperative cognitive behavior and prognosis of patients with cerebral hemorrhage.Methods:Using the convenient sampling method, a total of 82 patients with cerebral hemorrhage who were admitted to the First Affiliated Hospital of Zhengzhou University from March 2019 to January 2021 were selected as the research objects. According to the random number table method, they were divided into the control group and the observation group, with 41 cases in each group. Both groups received surgical treatment. The control group received routine nursing, while the observation group received ultra-early nursing intervention on the basis of routine nursing when vital signs were stable 48 hours after operation. The differences in neurological function (National Institutes of Health Stroke Scale, NIHSS) , cognitive function (Loewenstein Occupational Therapy Cognitive Assessment, LOTCA) and activity of daily living scores were compared between the two groups at 48 hours and 1 month after operation.Results:The NIHSS score at 1 month after surgery in both groups was lower than that at 48 hours after surgery and the NIHSS score at 1 month after surgery in observation group was lower than that in control group ( P<0.05) . The LOTCA dimension scores of the two groups at 1 month after operation were higher than those at 48 hours after operation and the LOTCA dimension scores of the observation group at 1 month after operation were higher than those of the control group ( P<0.05) . The activity of daily living scores of the two groups at 1 month after operation were higher than those at 48 hours after operation and the activity of daily living score of the observation group at 1 month after operation was higher than that of the control group , the differences were statistically significant ( P<0.05) . Conclusions:Ultra-early nursing intervention can effectively promote the recovery of postoperative neurological function and cognitive function in patients with cerebral hemorrhage and improve the activity of daily living of patients.
8.The investigation and analysis on the Chinese Medicine courses for the students major in western medicine

Objective To investigate the evaluation and suggestion of the courses of Chinese Medicine for clinical students majoring in medical college. Methods In January 2017, at the end of the Chinese Medicine courses, a total of 35 clinical students of Capital Medical University were investigated through a questionnaire, mainly related to students' understanding of Chinese medicine; evaluation and suggestions for Chinese Medicine education and teaching. Results The majority of students believed that Chinese medicine was safe and effective. The proportion of believing worthing learning that the Chinese medicine accounted for 94.3% (33/35), and who believed Chinese medicine helpful for the clinical work accounted for 82.9% (29/35). However, the proportion of satisfaction with the current teaching method was 60% (21/35). Conclusions Most of the clinical students in this investigation have recognized the role of Chinese medicine, and have shown great interest in studying the course, but they are not satisfied with the current teaching. To improve the teaching quality of Chinese Medicine through the reform should be the direction of teachers' efforts in the future.

Objective:To explore the current status of surgery for portal hypertension to grasp current status and future development of surgery in China.Methods:This study is jointly sponsored by China Hepatobiliary & Pancreatic Specialist Alliance & Portal Hypertension Alliance in China (CHESS).Comprehensive surveying is conducted for basic domestic situations of surgery for portal hypertension, including case load, surgical approaches, management of postoperative complications, primary effects, existing confusion and obstacles, liver transplantation(LT), laparoscopic procedures and transjugular intrahepatic portosystemic shunt(TIPS), etc.Results:A total of 8 512 cases of portal hypertension surgery are performed at 378 hospitals nationwide in 2021.Splenectomy plus devascularization predominated(53.0%)and laparoscopy accounted for 76.1%.Primary goal is preventing rebleeding(67.0%) and 72.8% of hospitals used preventive anticoagulants after conventional surgery.And 80.7% of teams believe that the formation of postoperative portal vein thrombosis is a surgical dilemma and 65.3% of hospitals practiced both laparoscopy and TIPS.The major reasons for patients with portal hypertension not receiving LT are due to a lack of qualifications for LT(69.3%)and economic factors(69.0%).Conclusions:Surgery is an integral part of management of portal hypertension in China.However, it is imperative to further standardize the grasp of surgical indications, the handling of surgical operation and the management of postoperative complications.Moreover, prospective, multi-center randomized controlled clinical studies should be performed.
