1.Clinical Observation on 50 Hz Frequency of Electricity Stimulating Finger Extensor Tendon for Hand Dysfunction Caused by Incident Storke

Objective] To study the effect of 50 hz frequency of electricity stimulating finger extensor tendon for hand dysfunction caused by incident storke. [Methods] 67 cases diagnosed as hand dysfunction of incident storke patients randomly divided into treatment group and control group(33 cases in treatment group, 34 cases in control group). The control group was given Bobath method for hand function recovery training. The treatment group was given 50 hz frequency of electricity stimulating finger extensor tendon on the basis of Bobath rehabilitation treatment, 5 times every week, for 4 weeks. Evaluate patients' symptoms improvement, before and after, with Fugl-Meyer grade(hand part) and modified Barthel index evaluation, and then make statistical analysis of the results. [Result] After the treatment,the grades, Fugl-Meyer grade(hand part) and modified Barthel index, of two groups both increased significantly, with statistical significance(P<0.01); Both of the grades of treatment group were significantly higher than that of control group, with statistical significance(P<0.01). [Conclusion] 50 hz electrical stimulating finger extensor tendon can significantly improve hand dysfunction caused by incident storke, improve their abilities of daily life.
2.IMMUNE RESPONSES OF MICE VACCINATED WITH RECOMBINANTS CODED DIFFERENT STAGE GENES FROM FCC 1/HN STRAIN OF PLASMODIUM FALCIPARUM

Aim To search the effective malaria mixed gene vaccine and to explore the immune response of vaccinated host. The recombinant pcDNA3-EBA175/HRP Ⅱ was injected alone or mixed with pcDNA3-Pfs25 of Plasmodium falciparum into mice by intramuscular route. The muscle of injected parts were treated previously, e.g. injecting 50μl of 0. 5%bupivacaine into the muscle of left latter leg, with 2mm depth. To observed the changes of IgG antibody value, the splenic lymphocyte proliferation, the ratio of CD4+/CD8+ subgroups and NK cell killing activity. After injecting recombinant pcDNA3-EBA175/HRP Ⅱ alone or mixed with pcDNA3-Pfs25 into mice by intramuscular route, it showed that sera IgG value increased, the splenic T lymphocyte proliferation stimulated by specific antigen of Plasmodium falciparum increased, the ratio of CD4+/CD8+ decreased and NK cell activity increased. Enhanced immune injection could improve host's immune reaction.It is suggested intramuscular injection is an effective immune route, and mice inoculated with coding two gene recombinant alone or that mixed with sexual stage gene recombinant could all induce increased humoral and cellular immune response, and NK cell activity.
3.Clinical investigation on the effect of combined prism lenses for juvenile myopes control and prevention

Objective To investigate the effect of combined prism lenses for the treatment of juvenile myopes. Methods 622 cases of juvenile myopes ranging from - 1. 0D to - 5. 0D, aging from 6 - 18ages, were divided randomly into two groups. The control group(320 cases) wear general glass and the research group(302 cases) wear combined prism lenses. The juvenile myopes of both groups were checked their vision every 6 months and data were collected at the end of the fourth 6 months and analyzed by SPSS software. Results Ther eyes of increasing diopter in research group were 38(6.3%of all eyes) ,33 eyes were less than 1. 0D and 5 eyes were more than 1. 0D among them;but 294(46.0% of all eyes) in the control,201 eyes were less than 1.0D and 93 eyes were more than 1.0D among them. There were significant differences between two groups(P<0.01). Conclusion Combined prism lenses for juvenile myopes (lower and middle) .compared with the common glass, had good clinical effect and it was a reliable and practicable method,to wear combined prism lenses for the treatment of juvenile myopes.

Objective To investigate methods of weaning acute brain stem hemorrage patients with respiratory failure from mechanical ventilation.Methods This was a retrospective analysis of 45 acute brain stem hemorrage patients with respiratory failure requiring mechanical ventilaiton.Results 15(33.3%)patients were successfully weaned from mechanical ventilation for the first time;the other 30(66.7%)patients failed in the first place,then succeeded after 3~7 days of and occasional disconnection from ventilation.The time of mechanical ventilation varied from 6 to 35 days.Conclusion It is difficult to wean acute brain stem hemorrage patients with respiratory failure from mechanical ventilation.Occasional disconnction from ventilaiton,antiinfection and nutritional supplementation might contribute to successful weanning.
6.Comparison of changes of intestinal microecology between different age groups in the healthy people

Objective To analyze the diversity of intestinal microecology in different healthy age groups and explore the relationship between age and the composition of intestinal microflora.MethodsFecal samples were collected from the children,adult and elderly groups.The main intestinal microflora were cultured and quantified,and the qualitative and quantitative changes in the level of category were determined by fluorescence quantitative PCR.The Bifidobacteria/Enterobacteriaceae (B/E) ratio reflexing intestinal colonization resistance was also calculated.Results The germiculture of fecal samples indicated that the counts of Bifidobacteria and Lactobacillus (P<0.01 and P<0.05,respectively) were significantly lower in the adult and aged groups than in children group.The number of Bacteriodes and Enterobacteriaceae in the elderly group were higher than in the children and the adult groups (all P<0.01).The counts of Enterococcus were increased in the adult and the aged groups (P<0.05 and P<0.01) compared with the children group,and were higher in the aged group than in the adult group (P<0.05).The B/E ratio was significantly decreased in the aged group (0.88-±-0.13) as compared with the children (1.15±0.15) and the adult groups ( 1.01± 0.20) ( both P < 0.01).The diversities of Bifidobacteria,Bacteriodes,Enterobacteriaceae and B/E ratio by fluorescent quantification PCR were consistent with the results by germiculture technology.The diversity of Lactobacillus (P<0.01) between the children and the aged groups as well as between the children group and the adult group significantly improved (P<0.01 and P<0.05 ) in PCR compared with the germiculture technology. ConclusionsThe numbers of intestinal Bifidobacterium and Lactobacillus are decreased with lower colonization resistance,while the numbers of Bacteriodes,Enterobacteriaceae and Enterococcus are increased with aging.
7.Free fibula and flap graft for reconstruction all the first metatarsal bone

Objective To evaluate the clinical effects of free fibula and flap grafts on the repair of all the first metatarsal bone at one stage.Methods There were 9 cases with the first metatarsal bone defect from Janurary 2003 to December 2009 that treated with free vascularized fibular bone and free vascularized flap at one stage.In which 6 cases reconstructed at the primary stage and 3 cases reconstructed at the second stage.Seven cases reconstructed by free vascularized fibular combined with ALTPF,two cases reconstructed by free vascularized fibular combined with TAPF.The free vascularized fibular and flap restored the first metatarsal bone and the soft tissue defects respectively.Vascular anastomosis was the artery of flap anastomosis with anterior tibial artery and the vein of the flap anastomosis with great saphenous vein,the peroneal artery and accompany vein anastomosis with artery and vein of the flap.Results The grafted tissues survived smoothly in 8 cases,vein crisis happened in I case and the ALTPF necrosis after blood vessels expedition.So the TAPF was changed to cover the soft defect and survived smoothly.Followiy-up were done from 6 to 36 months in 9 cases.There were no ulcer on flaps and no fracture again,the fibulas had been bone healing.Evaluated by Maryland standards,six cases were excellent,two cases were fine,one case was good.Conclusion The fibula combined flap grafts provide a relatively better alternative to repair the first metatarsal bone compound tissue defects at one stage.In addition,the procedure decreased frequency of operations and short the course of treatment.Sensory function reconstruction of fibula flaps should be given full attention.As fine function of the reconstructed foot,it is a effective method for reconstruction the burdened area of the foot.
8.Replantation and functional reconstruction of severed palm caused by crush injury

Objective To report microsurgical treatment of severed palm caused by crush injury. Methods From 1998 to 2002, 14 patients with severed palm and destructive crush injury were treated in our hospital. According to the specific severity and level of different injuries, replantation of severed palm with vessel transfer, replantation of fingers to the distal forearm with resection of wrist and palm, replantation of severed palm combined the thumb reconstruction were applied to reserve a maximal part of hand or fingers. Results Of the 14 cases of replantation, 3 experienced death of middle or index finger and the others all survived. The follow up of 11 cases lasted more than 6 months. After functional exercises, 10 cases recovered the function of pinch, and 1 case nearly regained normal functions. All of them regained sensation and the two point discrimination was 8 to 10 mm. Conclusion Reasonable replantation for severed palm with crush injury is still a method to regain part of hand functions.
9.Stimulatory effect of purified mycobacterium tuberculosis peptide antigen on human ??T lymphocyte proliferation

Objective:To observe the stimulating effect of C main Peptide purified from Mycobacterium tuberculosis peptide antigens(Mtb Ag) on human ??T cells, and the effect of C main peptide on the Mtb Ag activated ??T cells in vitro. Methods:C main peptide was used to stimulate fresh ??T cells of normal subjects in different doses for 10 d and the responded cells were phenotyped by flow cytometry. Furthermore,C main peptide was used to restimulate the activated ??T cells ,and then analyse the expression of early activation marker molecule CD69 in ??T cells by flow cytometry and the activity of proliferation of ??T cells by MTT assay. Results:C main peptide could stimulate the proliferation of fresh ??T cells predominantly, and it also could promote the expression of CD69 in activated ??T cells and enhance its activity of proliferation. Conclusion:C main peptide purified from Mtb Ag is a kind of specific stimulator of human ??T cell in vitro.
10.Large mitochondrial DNA deletions in ultraviolet B-induced cutaneous photodamage

Objective To analyze the association between mtDNA mutations and photodamagc after ultraviolet B (UVB) irradiation. Methods Primary human skin fibroblasts (HSF) and primary human epi- dermal keratinocytes of adult (HEKa) were irradiated by sub-lethal doses of UVB thrice a day for 4-5 days. Thereafter, genomic DNA was extracted from irradiated cells and conventional PCR was applied to detect the frequency rates of 4977 bp and 3895 bp mtDNA deletion. To quantitatively analyze the mutation levels, SYBR Green real-time PCR method was performed. Results In both cell lines, the frequency rates and relative copy number of deletions increased with the cumulative doses of UVB exposure (P<0.05). The prevalence rate of 3895 bp deletion peaked 53.3% and and relative copy number reached (49.63±4.38)×10-5, showing a more intense response to the accumulation of UVB radiation than 4977 bp deletion. In HSF, the minimum cumu- lative dose of UVB radiation was 150 mJ/cm2 for the induction of 3895 bp deletion, and 200 mJ/cm2 for the induction of 4977 bp deletion. It seemed that mtDNA deletion was more readily to be induced by UVB radia- tion in HSF than in HEKa. Conclusions The development and accumulation of mtDNA mutation are intimately related with cumulated UVB dose received by skin cells, and the 3895 bp deletion is more reliable in moni- toring the photodamage caused by UV than 4977 bp deletion. Therefore, the 3895 bp deletion may serve as a biomarker for the detection of photodamagc in skin cells. HSF appear to have an increased susceptibility to UVB radiation, which results in a higher frequency and level of mtDNA mutations compared with HEKa.
