1.Expression of tumor stem cell markers Lgr5 and CD44 in esophageal carcinoma and its clinical significance

Objective To analyze the expressions of tumor stem cell markers Lgr5 and CD44 in esophageal carcinoma and its clinical pathological significance.Methods A total of 108 cases of patients with esophageal cancer, 35 cases of stage Ⅱ, 40 cases of stage Ⅲ, 4 cases of stage Ⅳ. After tumor specimens were stained, the expressions of Lgr5 and CD44 in esophageal cancer tissue were detected and its relationship with the clinical features were analyzed.Results There were relations between expressions of Lgr5 and CD44 in esophageal cancer tissue and clinical staging, the later the stageing, the higher expression rate of Lgr5 and CD44, but there was no significant differece with Chi-square test. The Spearman correlation analysis showed there was no correlation between Lgr5 or CD44 and esophageal cancer condition. Conclusion CD44 and Lgr5 are highly expressed in esophageal cancer tissue, but the correlation between CD44 and Lgr5 expression and esophageal cancer condition is uncertain.
2.The diagnosis value of ultrasound elastography applied in prostate cancer

Objective To evaluate the efficacy and safety of ultrasound elastography in the diagnosis of pros-tate cancer.Methods 83 patients with prostate cancer were selected and diagnosed by transrectal ultrasound elastog-raphy (TRTE)and transrectal ultrasound(TRUS).The surgical pathology of the patients was used as diagnosis basis, and the efficacy of the two methods was evaluated.Results There were 102 nodules of all the 83 patients,44 patients'surgical pathology were benign,a total of 49 nodules,39 cases were vicious,a total of 53 nodules.47 nodules were gland nodules,accounting for 46.08% and 55 nodules were outer gland nodules,accounting for 53.92% of all the 102 nodules.There were 54 focis,the TRTE detected 36 focis,the detection rate was 66.67%,and TRUS detected 24 focis,the detection rate was 44.44%.The sensitivity,specificity and accuracy rates of TRTE were 66.67%, 68.65% and 67.64%,which were higher than 44.44%,64.58% and 53.92% of TRUS,and the differences were statistically significant (χ2 =10.710,P <0.05;χ2 =3.879,P <0.05;χ2 =4.084,P <0.05).The differences of pathological findings and TRTE,TRUS findings of the benign and malignant prostate nodules were not statistically significant (P >0.05 ).Conclusion It has higher diagnostic sensitivity,specificity and diagnostic accuracy for ultrasound elastography in the diagnosis of prostate cancer,which can improve the clinical diagnostic performance and worthy of clinical application.
3.Nosocomial Infection in 1659 Patients with Hematological Disorders:A Retrospective Analysis

OBJECTIVE To investigate the incidence and pathogens of infection in 1659 consecutive cases in single center hematological unit.METHODS The incidence,pathogen,and outcome of infection in 1659 hospitalized patients with hematological disorders from 1999 to 2006 were retrospectively analyzed.RESULTS The overall incidence of infection was 24.4% according to the person-times of hospitalization,which included 22.1% of nosocomial infection and 2.3% of community acquired infection.Most of the pathogenic bacteria of the nosocomial infection were Gram-negative.The most common bacteria in the sputum samples included Enterobacter cloacae(19.3%) and Pseudomonas aeruginosa(14.8%).The most common bacteria in the blood samples were coagulase negative Staphylococcus(CNS,39.3%),the next was Pseudomonas aeruginosa and Escherichia coli.There were 4.21% bacteria resistant to most of antibiotics in nosocomial infection.There were 114 fungi isolated.Candida albicans was accounted for 35.1%.The mortality due to nosocomial infection was 7.4%.CONCLUSIONS The patients in hematology ward are susceptible to infection.The pathogens of nosocomial infection are most likely G-bacteria.Some bacteria are resistant to almost all antibiotics.The incidence of fungal infection is increasing in the near 8 years.
4.The outcome of monoclonal gammopathy of undetermined significance in elderly patients

Objective To explore the outcome of monoclonal gammopathy of undetermined significance (MGUS). Methods The data from 14 MGUS patients in our hospital including clinical features, outcome and change of M protein concentration were analyzed retrospectively. Results The MGUS didn't have the clinical manifestations of multiple myeloma (MM), the time of outcome from MGUS to MM was about 4-20 years (mean time, 10 years). The most types of MM were IgA and IgG, 6 cases were IgA type, 6 cases were IgG type and 2 cases were light chain type. The concentration of immune globulin in general showed an upward trend year by year. A few showed fold lines ascend. Conclusions The elevated monoclonal immunoglobulin may develop into MM after many years. We must follow up frequently to avoid error diagnosis and missed diagnosis.
5.Expression and diagnostic value of 34βE12, Galectin-3 and HBME-1 in thyroid nodules

[ABSTRACT]OBJECTIVETo study the expressions of 34βE12, Galectin-3 and HBME-1 in thyroid nodules, and to explore its diagnostic value for papillary thyroid carcinoma (PTC).METHODSEn VisionTM immunohistochemical technique was used to detect the expression of 34βE12, Galectin-3 and HBME-1 in 352 thyroid lesions. The correlation between the expressions of the 3 protein markers and clinicopathological characteristics was evaluated. The receiver operating characteristic area under the curve (ROC-AUC) and their index for diagnosis evaluation were also calculated.RESULTSThe positive rates of 34βE12, Galectin-3 and HBME-1 in 246 PTC lesions were significantly higher than those in benign nodules (P<0.001). There was no relationship between the expression of the 3 protein markers and clinicopathological characteristics (eg. gender, age, numbers of lesions, tumor size, capsular invasion, lymph node metastasis, TNM staging). The ROC-AUC of 34βE12, Galectin-3 and HBME-1 for diagnosis of PTC was 0.936, 0.915 and 0.898 respectively. The sensitivity of 34βE12, Galectin-3 and HBME-1 for diagnosis of PTC was 94.3%, 95.5% and 91.1% respectively, while the specificity was 81.1%, 71.7% and 83.0% respectively, and the diagnostic accuracy rate was 90.3%, 88.4% and 88.6% respectively.CONCLUSION The expressions of 34βE12, Galectin-3 and HBME-1 are statistically different between PTC and benign lesions, but no associations are found with clinicopathological characteristics, indicating the three protein markers have important diagnostic value for PTC.
6.Correlation between myocardial infarction and carotid atherosclerosis in older adult patients

Objective:To investigate the correlation between myocardial infarction and carotid atherosclerosis in older adult patients.Methods:A total of 98 older adult patients with myocardial infarction who received treatment in Lishui People's Hospital from June 2020 to June 2021 were included in this study. They were divided into three subgroups: mild ( n = 25), moderate ( n = 43) and severe ( n = 30) groups according to the severity of myocardial infarction. An additional 50 healthy subjects who concurrently received physical examination in the same hospital were included as controls. All participants underwent color Doppler ultrasound to evaluate the degree of carotid stenosis and its correlation with the severity of myocardial infarction. Results:In the myocardial infarction group, 327 carotid plaques were detected in 88 out of 98 patients, with the detection rate of 89.8%. In the control group, 85 carotid plaques were detected in 17 out of 50 healthy subjects, with the detection rate of 34.0%. In the myocardial infarction group, most plaques were mixed and soft, accounting for 34.8% (114/327) and 51.4% (168/327), respectively. In the control group, most plaques were calcified, accounting for 62.3% (53/85). There was significant difference in plaque property between the two groups ( χ2 = 102.23, P < 0.05). There was significant difference in the degree of carotid lumen stenosis between control and myocardial infarction groups ( χ2 = 60.07, P < 0.05). The degree of carotid lumen stenosis increased with the aggravation of myocardial infarction ( χ2 = 15.17, P < 0.05). Carotid intima-media thickness in the severe group was (1.49 ± 0.26) mm, which was significantly greater than (1.28 ± 0.24) mm in the moderate group, (1.15 ± 0.21) mm in the mild group and (0.82 ± 0.16) mm in the control group ( t = 5.21, 7.42, 14.29, all P < 0.05). Plaque score in the severe group was (2.56 ± 0.51) points, which was significantly higher than (2.33 ± 0.45) points in the moderate group, (1.58 ± 0.39) points in the mild group, and (1.12 ± 0.36) points in the control group ( t = 3.00, 11.23, 14.77, all P < 0.05). Correlation analysis showed that the severity of myocardial infarction was positively correlated with the degree of carotid artery lumen stenosis, intima-media thickness and plaque score ( r = 0.41, 0.33, 0.28, all P < 0.01). Conclusion:The severity of myocardial infarction in the older adults is correlated with carotid atherosclerosis, and carotid lumen stenosis can be used as a predictor of myocardial infarction.
7.Coexistence of Subcutaneous Panniculitis-Like T-Cell Lymphoma and Dermatomyositis in a 12-Year-Old Boy

Subcutaneous panniculitis-like T-cell lymphoma (SPTL) is an extremely rare, indolent skin malignancy that can be difficult to distinguish from autoimmune disease-associated panniculitides. Here, we describe a 12-year-old boy who was diagnosed at age 7 years with dermatomyositis with classical manifestations, including poikiloderma, Gottron’s sign, and symmetric muscle weakness. Recently, the boy presented multiple subcutaneous nodules and fever. Histopathological examination and immunohistochemical staining revealed coexistence of SPTL. To our knowledge, this is the first case of dermatomyositis accompanied with SPTL. This case alert clinical physicians of the possibility of SPTL should be considered when a patient with dermatomyositis has new lesions presenting as nodules and unknown fever.
8.Correlation analysis between phonographic cardiac function and uterine artery resistance index in pregnant women with hypertension of different conditions during pregnancy

Objective:To analyze the correlation between phonographic cardiac function and uterine artery resistance index (RI) in pregnant women with hypertension of different conditions during pregnancy by color Doppler ultrasonically.Methods:A total of 100 pregnant women with gestational hypertension admitted to Lishui People′s Hospital from January 2020 to February 2021 were selected and divided into gestational hypertension group (40 cases), mild pre-eclampsia group (30 cases) and severe pre-eclampsia group (30 cases) according to the severity of their disease. Another 30 normal pregnant women who underwent physical examination during the same period were selected as the control group. The left ventricular function parameters isovolumetric relaxation time (IVRT), left ventricular ejection fraction (LVEF), left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume(LVESV), left atrial volume index (LAVI) and uterine arter RI in 4 groups were quantitatively analyzed by ultrasound during 28 to 38 weeks of gestation. The correlation between cardiac ultrasound parameters and of pregnant women RI were analyzed.Results:As the severity of the disease, the levels of IVRT, LVEDV, LVESV, LAVI were gradually rised, and the level of LVEF was gradually decreased, there were significant differences among the 4 groups ( P<0.05). The uterine arter RI in the control group, gestational hypertension group, mild pre-eclampsia group and severe pre-eclampsia group was 0.42 ± 0.04, 0.50 ± 0.05, 0.54 ± 0.11, 0.58 ± 0.06, there was significant difference ( F = 34.11, P<0.001), and as the severity of the disease , the level of uterine arter RI was gradually rised, there were significant differences among the 4 groups ( P<0.05). Pearson correlation analysis showed that IVRT had certain correlation with RI in the 4 groups ( r = 0.331, 0.450, 0.531, 0.582; P<0.05). In mild and severe pre-eclampsia group, there were a certain correlation between LVEDV, LVESV and RI (in mild pre-eclampsia group: r = 0.360 and 0.411, P<0.05; in severe pre-eclampsia group: r = 0.383 and 0.451, P<0.05). Conclusions:The left heart function of pregnant women with different conditions evaluated by ultrasound showed a downward trend, and the uterine artery RI increased gradually in pregnant women with severe pre-eclampsia, and RI was closely related to the cardiac ultrasound parameter IVRT.
9.Efficacy observation of mesenchymal stem cells derived from human umbilical cord for therapy of hepatitis B patients with decompensated cirrhosis

Objective To investigate short-term efficacy and security of transplantation of human umbilical cord mesenchymal stem cells (HUCMSCs)in the course of treatment of decompensated cirrhosis.Methods Ninety-six Hepatitis B patients with decompensated cirrhosis were enrolled,among which 50 were sbject to transplantation of HUCMSCs in addition to routine therapy (the treatment group) and the rest patients were underwent routine therapy(the control group).We observed the efficacy and security at 2,4,6 and 12 week of the treatment course.Results The symptoms of atigue,anorexia,bloating and edema were greatly relieved in both groups after 4 weeks,and sustained remission after 4-12 weeks.The overall survival rate was 96% after 12 weeks of the treatment group,2 patients were died after 8 and 12 weeks of the HUCMSCs transplantation due to hepatic encephalopathy repecticely.Compared to the baseline,ALT,AST,TBil and ALB(albumin) were improved after 2,4,6 and 12 weeks in both groups(P < 0.05),and the averages of ALB(albumin) of treatment group were higher than the control group.PT(prothrombin time) and PTA(prothrombin time activity) were improved after 4,6,12 weeks in the treatment group (P < 0.05),however,there were no differences in the control group during the 4,6 and 12 weeks(P >0.05).During the course of HUCMSCs tranplantation,the security was satisfied and no special side effects were observed.Conclusion The transplantation of HUCMSCs for therapy of Hepatitis B patients with decompensated cirrhosis is a safety and effective therapy,and can improved ALB,PT,PTA,liver function and clinical symptoms within a short period,which is an alternative method of treatment recommended.
10.Effect of lateral position ventilation combined with vibration sputum drainage on patients with acute respiratory distress syndrome: a prospective randomized controlled trial

Objective To investigate the curative effect of lateral position ventilation combined with vibration sputum drainage on the patients with acute respiratory distress syndrome (ARDS). Methods A prospective randomized controlled trial was conducted. The patients with ARDS undergoing mechanical ventilation (MV) admitted to intensive care unit (ICU) of Linyi Central Hospital from January 2013 to June 2017 were enrolled, and they were divided into simple ventilation group and combined treatment group according to random number table. The patients in both groups received etiological treatment, protective ventilation strategy, sensitive antibiotics for anti-infection, and calefacient and humidifying treatment. The patients in the simple ventilation group received bilateral discontinuous alternative lateral position ventilation [pressure controlled ventilation (PCV), tidal volume (VT) ≤ 6 mL/kg, the inhaled oxygen concentration (FiO2) and positive end expiratory pressure (PEEP) was adjusted to maintain the airway plateau pressure (Pplat) ≤ 30 cmH2O (1 cmH2O = 0.098 kPa)], and those in the combined treatment group received lateral position ventilation combined with vibration sputum drainage, twice a day, 15 minutes each time. The parameters of respiratory function and inflammation as well as excretion of sputum before and after treatment were compared between the two groups, and the complication and prognosis were recorded. Results A total of 200 patients with ARDS were included, 4 patients were excluded because of severe pneumothorax, massive hemorrhage of the digestive tract, or elevated intracranial pressure, so 196 patients were finally enrolled in the analysis. There were 98 patients in the simple ventilation group and the combined treatment group, respectively. There were no significant differences in parameters of respiratory function and inflammation as well as excretion of sputum before treatment between the two groups. After treatment, the above parameters in both groups were improved, and the effect of combined treatment was more significant. Compared with the simple ventilation group, white blood cell count (WBC), procalcitonin (PCT), C-reactive protein (CRP) at 24 hours of treatment in the combined treatment group were significantly decreased [WBC (×109/L):9.1±1.6 vs. 11.8±3.6, PCT (μg/L): 14.5±2.4 vs. 22.7±3.2, CRP (mg/L): 32.2±6.3 vs. 67.2±7.2, all P < 0.01], the quantity of sputum excretion was significantly increased (mL: 49.3±12.5 vs. 36.8±11.8, P < 0.01); 72 hours after treatment, the oxygenation index (PaO2/FiO2) in the combined treatment group was significantly increased [mmHg (1 mmHg = 0.133 kPa): 278±28 vs. 238±39, P < 0.01], and PEEP, FiO2, airway resistance (Raw) were significantly lowered [PEEP (cmH2O): 5±2 vs. 7±3, FiO2: 0.40±0.11 vs. 0.49±0.12, Raw (cmH2O): 12.8±0.7 vs. 16.2±0.8, all P < 0.01]. There was no significant difference in static lung compliance (Cst) or Pplat between the two groups. Compared with the simple ventilation group, the duration of MV (days: 5.5±3.0 vs. 8.6±2.1), the length of ICU stay (days: 7.5±5.4 vs. 11.3±4.6) and the extraction time of artificial airway (days: 6.6±2.8 vs. 9.4±3.3) in the combined treatment group were significantly shortened (all P < 0.01). However, there was no significant difference in the 28-day mortality rate [4.1% (4/98) vs. 3.1% (3/98)], the rate of tracheotomy [2.0% (2/98) vs. 3.1% (3/98)] or the incidence ofⅠ-Ⅱ skin pressure sore [1.0% (1/98) vs. 2.0% (2/98)] between simple ventilation group and the combined treatment group (all P > 0.05). No artificial airway disappearance, pneumothorax, grade Ⅲ or Ⅳ pressure sore was found in the two groups. Conclusions Compared with lateral position ventilation alone, the treatment of combined with vibration sputum drainage is more effective for improving the respiratory function of patients with ARDS, reducing infection action, shortening the duration of MV and the length of ICU stay, and improve the recovery, promote the rehabitation of patients.
