1.Clinical Studies on Emotional Disorder in Patients After Acute Stroke

[Objective] To investigate the occurrence rate, manifestations and influencing factors of emotional disorder in patients after acute stroke. [Methods] Emotional disorder in 107 cases of acute stroke was assessed by 17-item Hamilton Depression Scales (HAMD), 14-item Hamilton Anxiety Scales (KAMA), Mini-Mental State Examination (MMSE) and Barthel index (BI), and the relationship between emotional disorder and influencing factors were also analyzed. [Results] The occurrence rate of emotional disorder after acute stroke was 41.12% , in which depression was 18.69% , anxiety 9.35% and concurrence of depression and anxiety 13.08% . Emotional disorder usually occurred in the dominant hemisphere of young patients with lower BI after repeated attacks of stroke; its manifestations relieved with the cure of primary diseases. Its dominant syndrome pattern was classified as stagnation of liver-Qi. [ Conclusion ] Emotional disorder, possibly being a stress reaction, is the common complications of acute stroke and usually classified as stagnation of liver-Qi; early effective rehabilitation is the best preventive method for emotional disorder acute stroke.
2.Sequence analysis of ribosomal DNA internal transcribed spacer 2 of sym-patric populations of Anopheles sinensis of different feeding preferences

Objective To investigate the existence of genetic divergence of sympatric populations of Anopheles sinensis of different feeding preferences based on the rDNA-ITS2 sequence differences. Methods A large number of wild anopheles popu-lations were trapped all night by man-baited net and calf-baited net that had been set up between high-density natural villages of An. sinensis populations and vector-breeding sites,from which two groups of An. sinensis were separated by morphological iden-tification and brought back to the lab for conventional breeding. A large closed greenhouse which temperature and humidity was appropriate was selected as research settings of mark-release-recapture methods by female mosquitoes ,in the center of which above An. sinensis populations baited by man and calf and respectively correspondingly marked by red and yellow phosphors were released in together,in each side of which An. sinensis were recaptured simultaneously by man-baited net and calf-baited net. An. sinensis populations trapped by man twice were brought back to the lab and bred with man-blood,correspondingly ones trapped by calf with calf-blood. Man-preferring and calf-preferring strains were screened respectively from An. sinensis which had been baited by man and calf by the mark-release-recapture methods after parent and F1 mosquitoes,and sequencing and aligning of both rDNA-ITS2 were conducted via PCR amplification. Results The recapture ratios of wild parental mosquitoes An. sinensis of man-preferring group by man-baited net and calf-baited net were 54.07%(339/627)and 45.93%(288/627)re-spectively,and ones of calf-preferring group by man-baited net and calf-baited net were 58.01%(409/705)and 41.99%(296/705)respectively. Two groups of parental mosquitoes trended towards selecting the original blood hosts in host-seeking prefer-ence(χ2=19.42,P<0.01). The recapture ratios of F1 mosquitoes An. sinensis of man-preferring group by man-baited net and calf-baited net were 63.43%(765/1 206)and 36.57%(441/1 206),and ones of calf-preferring group by man-baited net and calf-baited net were 68.22%(1 039/1 523)and 31.78%(484/1 523). Two groups of F1 mosquitoes had more significant characteris-tics of selecting the original blood hosts in host-seeking preference(χ2=271.69,P<0.01)and showed the genetic differentia-tion phenomenon,but the results of sequencing and aligning of the rDNA-ITS2 via PCR amplification showed no difference in base sequence between the two strains and both were 469 bp. Conclusions The genetic divergence based on the rDNA-ITS2 se-quence does not happen in An. sinensis sympatric populations of different feeding preferences.
3.Clinical study of the umbilical blood stem cells transplantation in the treatment of chronic liver failure

Objective To observe the safety and clinic effect of umbilical blood stem cell transplantation for the patients with chronic liver failure (CLF).Methods 44 patients with CLF were included in the research and divided into two groups,22 in control group received internal medicine treatment,the other 22 in treatment group received umbilical blood stem cell transplantation in addition to internal medicine treatment.The biochemical index,MELD scores,clinical symptoms,survival situation and adverse reaction of the patients were observed within 2,4,12 and 24 weeks.Results Albumin and prothrombin activity of treatment group were higher than those of control group,the MELD scores of the treatment group was lower than that of control group,the survival rate was higher than the control group,and the difference is significant between the two groups (P < 0.05).There was no significant difference between the two groups in terms of alanine aminotransferase and total bilirubin (P > 0.05).After 4 weeks treatment,fatigue,inappetite,abdominal distention and ascitic fluid of the treatment group were better than that of control group,the difference was statistically significant (P < 0.05).Besides,the patients of the both groups did not have any adverse reaction or hepatocellular carcinoma.Conclusion Umbilical blood stem cell transplantation is safe and effective for the patients with CLF and can improve the survival rate of the patients.
4.Impact of intervention of weight management on readmission rate of patients with congestive heart failure

Objective To explore the impact of intervention of weight management (WM)on readmission rate of patients with congestive heart failure(CHF).Methods sixty-six CHF patients were divided into the control group(34 cases)and the intervention group(32 cases).The patients in the intervention group received the weight management intervention for 6 months after discharge,while the control group was carried on the conventional education and follow-up.The ability of WM and readmission rate of patients due to onset of CHF were compared between the two groups.Results Six months after the intervention,significant differences were found on the scores of weight monitoring,WM related-knowledge,belief and practice in the intervention group,but only WM related-practice was improved in the control group.The readmission rote due to onset of CHF was reduced in the intervention group.Conclusions WM intervention could significantly improve the ability of WM and reduce readmission rate due to CHF onset of CHF patients.
5.Study on the Mechanism of Aurantii Fructus Immaturus and Its Main Active Ingredients in Promoting Gastrointestinal Motility of Model Rats with Spleen Deficiency

OBJECTIVE:To study the mechanism of Aurantii fructus immaturus(AFI)and its main active ingredients in pro-moting gastrointestinal motility of model rats with spleen deficiency. METHODS:170 rats were randomly divided into blank group (10 rats) and modeling group (160 rats),rats in modeling group was induced models with spleen deficiency by bitter cold diar-rhea+irregular diet. After modeling, rats were randomly divided into model group, naringin (NA) low-dose, medium-lose, high-dose groups(3.267,6.535,13.070 mg/mL),neohesperidin(NE)low-dose,medium-lose,high-dose groups(3.865,7.730, 15.460 mg/mL),synephrine(SY)low-dose,medium-lose,high-dose groups(0.252,0.504,1.008 mg/mL),compatibility groups with 3 monomer ingredients (NA-NE-SY) low-dose,medium-lose,high-dose and AFI water decoction low-dose,medium-lose, high-dose groups(0.104,0.208,0.416 g/mL,calculated by crude drug),ig,once a day,10 mL/kg,for 7 d. After the last admin-istration,gastrin (GAS) in serum,and acetylcholine (ACh),motilin (MTL),substance P (SP),vasoactive intestinal peptide (VIP)levels in plasma were detected. RESULTS:Compared with blank group,GAS level in serum and ACh,MTL,SP levels in plasma in model group were reduced(P<0.01),VIP level in plasma was increased(P<0.05). Compared with model group,ex-cept for the GAS level in serum showed no obvious change in NA high-dose group and SY doses groups,other medicine groups were obviously increased (P<0.05 or P<0.01);the ACh levels in serum were obviouly increased in NE high-dose group,SY high-dose group and AFI water decoction low-dose group(P<0.01). MTL levels in plasma were obviously increased in NE medi-um-dose,high-dose groups,SY high-dose group,compatibility low-dose,medium-dose groups and AFI water decoction medi-um-dose,high-lose groups (P<0.05);SP levels in plasma were obviously increased in NA low-dose,medium-dose groups and NE doses groups(P<0.05 or P<0.01);VIP levels were reduced in NA low-dose group,SY high-dose group and AFI water decoc-tion low-dose,medium-lose groups(P<0.05). CONCLUSIONS:AFI may promote the gastrointestinal motility of model rats with spleen deficiency by promoting the secretion of GAS,ACh,MTL,and inhibiting the secretion of VIP;there are differences be-tween AFI and the 3 monomer ingredients in regulation of gastrointestinal hormones.
6.Analysis of the hemolysin and virulence-related genes of incomplete hemolysis Staphylococcus aureus

Objective To analyze hemolysin and virulence -related genes in incomplete hemolytic Staphylococcus aureus.Methods Fifty strains of incomplete hemolytic Staphylococcus aureus were isolated from patients admitted in the Second Affiliated Hospital of Soochow University during 2013 and 2014, and the isolates with complete hemolytic phenotype were also collected at the same period as the control strains . All the strains were inoculated and subcultured on four kinds of sheep blood agar plates supplied by different manufacturers to compare their hemolytic phenotype .The relative mRNA expressions of hemolysin genes (hla, hlb, hlc, hld) in standard strain, complete and incomplete hemolytic phenotype strains were detected by real-time quantitative polymerase chain reaction (RT-qPCR), and valued by 2 -△△Ct method.t test was used to compare mRNA expressions of hemolysin genes .Western blot was performed to analyze the expression of α-hemolysin.Antibiotic susceptibility test of incomplete hemolytic strains was performed using broth microdilution method.Resistant gene mecA and virulence genes pvl, tst were detected by PCR.Results The steady and hereditary incomplete hemolysis was observed in 50 strains of incomplete hemolytic Staphylococcus aureus on the sheep blood agar plates from different suppliers .Taking mRNA expression of hla, hlb, hlc, hld in standard strain as 1, the relative mRNA expressions of hemolysin genes in incomplete hemolytic strains were 0.02, 7.51, 0.06 and 0.12 respectively, there were statistical differences between standard strain and incomplete hemolytic strains (t =8.46, -56.40, 8.12 and 7.61, all P <0.05).And the expression of α-hemolysin was decreased in incomplete hemolytic strains .All the strains were identified as methicillin resistant Staphylococcus aureus (MRSA).Three strains exhibited different minimum inhibitory concentrations of teicoplanin and linezolid after subcultured , but the differences had no impact on the final results of antibiotic susceptibility test .mecA, pvl and tst genes were positive in incomplete hemolytic strains . Conclusion Staphylococcus aureus with incomplete hemolytic phenotype is methicillin resistant with higher expression of β-hemolysin and lower expressions of α-hemolysin, γ-hemolysin and δ-hemolysin.It carries plv and tst virulence genes and is of high virulence .
8.Lipid peroxidation and anti-lipid peroxidation and the skin pigment in skin of C57BL/6 mice exposed to arsenic

Objective To study the effect of lipid peroxidation and anti-lipid peroxidation and the pigment in skin of C57BL/6 mice exposed to arsenic.Methods Forty male C57BL/6 mice were divided into four groups via the random number table method,ten mice in each group,and the mice were fed ad libitum drinking water containing arsenic at 0,1,5 and 25 mg/L concentrations for a period of 6 weeks,respectively.Twelve days before the end of the experiment,procedure of depilation was performed to induce anagen of hair cycle.On the 30th day of experiment,the activity of superoxide dismutase (SOD) was determined by nitrite method,the activity of glutathione peroxidase (GSH-Px) was determined by dithiobisobenzoic acid method,the content of malondialdehyde (MDA) in skin of C57BL/6 mice was determined by thiobarbituric acid method.Melanin was measured by NaOH dissolution method.Results No significant difference was found in body weight and water intaken between groups (F =0.119,0.363,P > 0.05).The activity of SOD [(16.00 ± 5.05),(13.96 ± 2.02),(10.46 ± 3.14) U/mg prot] in 1,5 and 25 mg/L arsenic groups were all significantly lower than that in control group [(20.36 ± 4.82) U/mg prot,P < 0.05].GSH-Px activity in 1,5 and 25 mg/L arsenic groups [(98.14 ± 23.92),(87.18 ± 10.87),(53.56 ± 19.97) U/mg prot] were all significantly lower than that in control group [(119.34 ± 33.14) U/mg prot,P < 0.05].While MDA levels in 5 and 25 mg/L arsenic groups [(9.09 ± 2.04),(11.48 ± 2.21) nmol/mg prot] were significantly higher than that of control group [(6.19 ± 0.56) nmol/mg prot,P < 0.05] and that of 1 mg/L arsenic group [(6.52 ± 1.67) nmol/mg prot,P < 0.05).No significant difference was found in melanin levels (P > 0.05).Conclusions Lipid peroxidation and the decreasing of antioxidation may be one of the mechanisms of arsenic-induced skin damage.Arsenic-induced skin melanin content change is not found.
9.Data Mining Research on Academic Thought of Treating Chest Pain from Famous Medicalists since the Han Dynasty

The analysis on regularity of comparability among famous physician's prescriptions for treating chest pain since the Han Dynasty will be helpful to treat chest pain and develop new herbs.The Chinese Medicine Auxiliary System (V2.5) was used to analysis the regularity of comparability by inputting the most famous 101 prescriptions for the treatment of chest pain from the Han Dynasty to Qing Dynasty.The results showed that the most common single drugs were cassia twig,40 times (39.6%).The most common couplet medicines were licorice and poria cocos,21 times (20.8%).The most common triangle medicines were ginger,pinellia,and cassia twig,13 times (12.9%).The most common drug combination based on association rules was the combination of cassia twig,peucedani radix→ginger (confidence level of 0.909 1).The core drugs based on complex network was cassia twig,licorice,and etc.New prescription combinations based on the entropy method were 11 new prescriptions including allium,cassia twig,poria,and almonds.It was concluded that there were regularity of comparability in ancient prescriptions for treating chest pain.Approaching and developing medicine for the treatment of chest pain should be from the following 6 points.The priority choice of single drug was cassia twig,and etc.The priority choice of couplet medicines was licorice and poria cocos,etc.The priority choice of triangle medicines was ginger,pinellia,cassia twig,and etc.The combination of high correlation should be chosen in treating chest pain,such as cassia twig,peucedani radix→ginger,and etc.The core drugs are necessary.In the warming yang method,cassia twig is the core drug and ginger is the auxiliary drug,and etc.New prescription combination,such as allium,cassia twig,poria,almonds,and etc.should be applied.
10.Effect of high-throughput sequencing for the prevention and control of thalassemia.

OBJECTIVE:
To assess the value of next generation sequencing (NGS) for the prevention and control of thalassemia.
METHODS:
NGS was used to sequence 3083 clinical blood samples suspected for thalassemia during initial screening. Retrospective analysis was conducted on blood samples detected with rare genotypes of thalassemia and abnormal hemoglobin.
RESULTS:
NGS analysis of the 3083 samples has found 1089 subjects with thalassemia genotypes (alpha-thelassemia genotype: 26.01%, beta-thalassemia genotype: 6.71%, and alpha-compound-beta genotype: 2.59%), which yielded a positive detection rate of 35.32%. Rare alpha-thalassemia genotypes including HBA2 c.123delG, HBA1 c.354_355insATC and Fusion gene, and rare beta-thalassemia genotypes including HBB c.-100G>A and HBB c.316-90A>G, were discovered. In addition, 19 patients were found to have abnormal hemoglobin, mainly including Hb Hamilton, Hb Hekinan II, Hb Shizuoka, Hb Owari, Hb New York, Hb J-Bangkok and Hb Port Phillip.
CONCLUSION
NGS can play a crucial role for improving of the prevention and control of thalassemia and formulating a screening system with better efficacy.
