Objective To investigate the effect of rhein on endothelial plasminogen activator inhibitor-1 (PAI-1) mRNA expression and protein synthesis induced by transforming growth factor ?1 (TGF-?1), and explore the protective mechanism of rhein on human endothelial cells. Methods A human umbilical endothelium derived cell line (ECV-304) from ATCC was used in this study. The endothelial PAI-1 mRNA expression and protein synthesis were detected by Northern blot and flowcytometry analysis respectively. The activity of phospho-p44/p42 MAPK induced by TGF-?1 was determined by immunoprecipitation analysis and Western blot. Results TGF-?1 (2?g/L) could increase rapidly endothelial PAI-1 mRNA expression and persist for 24 hours. The PAI-1 mRNA expression induced by TGF-?1 in endothelial cells was inhibited by incubating with rhein. Rhein could also inhibit the endothelial PAI-1 protein synthesis. Further study revealed rhein showed a significantly inhibitory effect on the activity of phospho-p44/p42 MAPK induced by TGF-?1 in human endothelial cells. Conclusions Rhein shows a protective action on endothelial cells by inhibiting PAI-1 overexpression, and this may provide beneficial effects on the treatment of diabetes mellitus and its complications.

Objective To explore the changes and its clinical significance of frontal lobe magnetic resonance spectroscopy(1H-MRS) in patients with Parkinson's disease(PD).Methods 1H-MRS localized to both frontal lobe,was carried out in 36 patients with PD(14 patients with Hoehn-Yahr stageⅠ,8 patients with Hoehn-Yahr stage Ⅱ,8 patients with Hoehn-Yahr stage Ⅲ,and 6 patients with Hoehn-Yahr stage Ⅳ) and 20 age-matched healthy control subjects.The metabolite ratios of N-acetylasparatate(NAA)/creatine(Cr),and choline-containing compounds(Cho)/Cr in patients with PD on different stage were compared with those in normal control group.Results NAA/Cr ratios were reduced significantly and Cho/Cr ratios were increased significantly in both frontal lobe in patients with Hoehn-Yahr stageⅢ and Ⅳ compared to those in normal control group(P

OBJECTIVE: To study the possible pathway of the effect of musk on brain disorder, distributing into the brain through blood brain barrier. METHODS: We used the musk ketone (muscone), a main composition of musk, to inject through the tail vein of the rats into the blood and took the brain and other organs at different times to make samples. Then gas chromatography was used to measure the distribution of muscone in the brain and other organs. RESULTS: Muscone could pass through the normal rat's blood brain barrier into the brain and soon reached the highest peak and remained in higher concentration, and more slowly metabolized as compared with other organs. CONCLUSION: Musk distributing into the brain through blood brain barrier provides the basis for its effect in treating brain disorders. Chromatography is an effective method to study the active composition of Chinese herbal medicine distributing through the blood brain barrier into the brain.

Objective To investigate the diagnosis and treatment of major arterial injuries of the extremities. Methods In the period between September 1999 and June 2002, 129 patients with 176 arterial injuries were admitted to our hospital. 122 patients of them with 162 arterial injuries underwent microsurgical repair. Results The limbs of 116 patients with 154 arterial injuries survived. The total survival rate of affected extremities was 89.9% ; the blood vessel recanalization rate was 95.06% . Conclusion Careful physical examination and advanced vascular Doppler equipment can avoid missed diagnosis, and microsurgical technique in vascular repair can successfully save limbs.

Objective To evaluate the value of measurement of the width of pars compacta of substantia nigra(SNc) and SNc's ratio to midbrain diameter in diagnosing Parkinson's disease(PD) and differentiating PD from vascular parkinsonism(VP) on routine MRI.Methods The width of SNc, midbrain diameter, and SNc's ratio to midbrain diameter were measured in 60 patients with PD (38 patients with Hoehn-Yahr stage Ⅰ～Ⅱ, 14 patients with Hoehn-Yahr stage Ⅲ, and 8 patients with Hoehn-Yahr stage Ⅳ), 60 patients with VP (26 patients with Hoehn-Yahr stage Ⅰ～Ⅱ, 20 patients with Hoehn-Yahr stage Ⅲ, and 14 patients with Hoehn-Yahr stage Ⅳ), and 60 age-matched heathly control subjects by means of routine axial T 2-weighted MRI. Objective To explore the postoperative care to hemodialysis patients with hypocalcemia after parathyroidectomy with refractory secondary hyperparathyroidism. Methods The nursing measures included timely correction of hypocalcemia, nursing of muscular spasm, dietary nursing and rational medication. Five of them developed numbness in the limbs, which was alleviated by administration of venous transfusion of calcium gluconate. After six months, the blood calcium in 6 patients resumed to normal level. For 1 case with lower blood calcium, the level of blood calcium maintained at 1.8~2.0mmol/L after supplement with high dose of calcium and high -calcium hemodialysis. Conclusions The hemodialysis patients with refractory secondary hyperparathyroidism after parathyroiddectomy are susceptible to hypocalcemia. The timely correction of hypocalcemia and related nursing measures are of significance for their recovery.

Objective To explore the correlation between serum uric acid levels and uric acid excretion indexes in children with primary nephrot?ic syndrome(PNS),and to reveal the prevalence of hyperuricemia and its possible causes in PNS. Methods The clinical data in 74 cases of pe?diatric PNS were retrospectively analyzed. These patients were grouped as hyperuric acid(HUA)or non?hyperuric acid(NUA),and their clinical features and laboratory findings were compared between two groups,including age,serum uric acid,serum creatinine,urea,cystatin C,triglycer?ide,cholesterol,albumin,creatinine clearance rate,24 hours urine uric acid,serum uric acid/serum creatinine,uric acid clearance,and the frac?tional excretion of uric acid and other indicators of differences. In addition ,a correlation analysis was carried out for the excretion of uric acid index. Results Several factors were lower in HUA group than that of NUA group ,such as the serum albumin and fractional excretion of uric acid(P1=0.034,P2=0.025);while the serum cystatin C and serum uric acid/serum creatinine ratio(P1=0.038,P2=0.001)was higher in HUA group. The uric acid excretion was negatively correlated with serum uric acid levels in all 74 children with PNS(r=-0.43,P=0.016),and the serum uric acid/creatinine ratio was positively correlated with serum uric acid(r=0.486,P=0.001). Conclusion Both the increase of blood uric acid production and excretion reduction were observed in children with PNS ,and the serum uric acid levels and renal tubular function is closely related.

Objective To assess the value of 16-slice spiral CT perfusion in the diagnosis of hyperacute cerebral infarction.Methods Routine and perfusion CT of brain were performed in 20 cases suspected with hyperacute cerebral stroke within 6 hours of symptom onset,the images of plain scan,rCBV,rCBF,MTT and TTP were observed and evaluated. All cases were followed-up with CT after one week.Results Ischemic lesions were found in routine CT in 5 cases,15 cases had negative results at routine CT, and ischemic lesions were showed in all of them by perfusion CT images.rCBV,rCBF distinctly decreased, MTT and TTP obviously prolonged in infarction area,and rCBF decreased, MTT and TTP prolonged and rCBV was normal or up lightly in the penumbra area. The infarction lesions were detected in 15 cases by follow-up CT.Conclusion 16-slice spiral CT perfusion in diagnosis of hyperacute cerebral infarction may be a valuable tool in predicting clinical prognosis.
