Objective To investigate the risk factors and effective nursing for patients with benign prostatic hyperplasia(BPH)within 3 days after operation.Methods The risk factors for restlessness of patients with BPH in 2006 were analyzed and the nursing points were summarized.Results Age of patients,surgical style and sleeping time before the operation were evidently related to the incidence of restlessness.Conclusion Strengthening of psychological nursing,appropriate surgical style and insurance of abundant sleeping time before operation could reduce the incidence of restlessness of patients after operation.
4.Correlation analysis of lower respiratory tract bacterial colonization and airway inflammation in stable COPD patients

Objective To detect sputum quantitative bacterial culture and sputum cell counts in stable chro -nic obstructive pulmonary disease (SCOPD) patients, and explore the correlation between lower airway bacterial colo-nization(LABC) and airway inflammation.Methods 47 patients with SCOPD were chosen ,all patients induced spu-tum specimens to sputum quantitative bacterial culture and sputum cell counts ,sputum and serum IL-8 levels were al-so detected.Results There were 13 patients whose bacterial colonization of the lower respiratory tract in patients was higher than 107CFU/mL.Serum IL-8 levels of 13 patients was ( 70.65 ±19.89 ) pg/mL, sputum IL-8 levels was (364.65 ±74.19)pg/mL,all significantly higher than those whose bacterial colonization of the lower respiratory tract lower than 107CFU/mL,(15.87 ±3.01)pg/mL and (146.65 ±28.11)pg/mL,there were statistically significant difference(t=13.43,9.76,all P<0.05).After 1-year follow-up,FEV1 decline(0.071 ±0.016)L in patients whose bacterial colonization of the lower respiratory tract≥107CFU/mL,it was significantly higher than those whose bacterial colonization of the lower respiratory tract <107CFU/mL;the frequency of acute exacerbation was (2.93 ±1.07) times,significantly more than those whose bacterial colonization of the lower respiratory tract <107CFU/mL,the differ-ences were statistically significant (t=5.43,6.54,all P<0.05).Conclusion Lower respiratory tract bacterial colo-nization in patients with SCOPD can aggravate the inflammatory response and increase degree of airflow limitation .
5.Application of lumbo-iliac fixation in pelvic ring reconstruction following communited Denis Ⅲ sacral fractures

Objective To study the anatomic morphology of posterior column of ilium(PCI)and valne of lumbo-iliac fixation(LIF)in pelvic ring reconstruction following communited Denis Ⅲ sacral fractures.Methods A total of 17 hemi-sectional pelvis and dry ilia were selected to observe the length,width and height(safety and standard)of PCI,when a detailed description was done on the methods determining entry point and operation techniqus for iliac screws anchoring.Nine patients with eommunited sacral fractures involving Denis Ⅲ areas combined with neurologic deficits were decompressed for recovery of neurological function and reconstruction of pelvic ring by using LIF.Postoperative follow-up was done on role of LIF and result of neural rehabilitation.Results PCI was mean 110.57 mm in length,24.24 mm in width,14.54 mm in standard height and 31.46 mm in safe height.Nine patients could turn over in bed immediately following operation and walk one month later.A follow-up for 13-37 months in eight patients showed stable reconstructed pelvic ring,sound function of movement and bone graft fusion,which met the target of keeping stability of fractured extremity and pelvis.Conclusions pCI can provide anatomic support for LIF.When decompression and fixation is needed for sacral fractures combined with anterior pelvic ring disruptions,LIP can provide stable fixation and play an important role in reconstruction,nursing and rehabilitation of pelvic ring.
6.Bioprotein glue combined with gelatin sponge for prevention of epidural adhesions after surgical operation

Objective To prove the prophylactic function of absorbable gelatin sponge and bioprotein glue for prevention of epidural and nerve root scar adhesions after laminectomy. Methods We excised the L6 vertebral plates of 36 adult New Zealand male rabbits,to make vertebral plate excision model and to find nerve root. Then divided them into 3 groups based on different materials:gelatin sponge and bioproten glue (group A),gelatin sponge (group B) and saline solution (group C). The morphological changes of the scars were observed grossly and histologically at 2 and 8 weeks after operation. We also measured the orbit of the nerve root. Objective To evaluate the effects of patient controlled analgesia(PCEA)on the perioperative changes of circulatory and pulmonary function of elderly with hypertensions after abdominal surgery.Methods Twenty-eight patients of ASAⅡ-Ⅲ aged more than 60 years undergoing uratomy were randomly divided into two groups:control group and PCEA group.Preoperative and postoperative circulatory and pulmonary functions were measured with noninvasion circulatory monitor and pocket lung function meter respectively.Results In control group,the systolic pressure,diastolic pressure,and heart rate increased by 19%,17% and 19%,respectively,as compared with preoperation.The percentage of forced vital capacity(FVC%),percentage of forced expiratory volume in first second to forced vital capacity(FEV1%) and percentage of maximal ventilatory volume(MVV%) of postoperation in control group were significantly decreased compared with preoperation(P

Objective To analyze the characteristics of breast medullary carcinoma in CEUS and to compare with pathologic features.Methods Morphologic characteristics of 13 breast medullary carcinomas in CEUS were analyzed.The diameter of mass before and after CEUS were compared.Parameters from time-intensity curves of masses were analyzed in contrast with peripheral breast parenchyma.All the results from CEUS analysis were compared with pathological manifestations.Results Breast medullary carcinoma was characterized as irregular shape (n=10),clear margins (n=11) and uniform enhancement (n=11) in CEUS.These characteristics were in accordance with their morphologic characters in pathology.The diameter of mass before and after CEUS had no significant defference (P=0.61),which was in accordance with expansive growth in pathology.In contrast with peripheral breast parenchyma,the arrival time and time to peak of breast medullary carcinoma were significantly shorter (P=0.034,0.021),and peak enhancement intensity was significantly stronger (P=0.005),which were in accordance with the increased vascular density and their uniform distribution,big arteries at the margin of masses in pathology.Conclusion Breast medullary carcinoma has distinguished characteristics in CEUS,which are in accordance with characters in pathology,and can be used as the basis in clinical diagnosis and differential diagnosis of breast medullary carcinoma.
9.Application of Ultrasound Guided Drainage Combined with Different Hardeners for Treatment of Simple Breast Cysts

Purpose To investigate the clinical effect of ultrasound guided drainage combined with different hardeners for simple breast cysts,and to further provide reference for individual treatment.Materials and Methods Clinical data of 117 cases breast simple cysts were analyzed retrospectively,of which 42 cases were routine ultrasound-guided puncture combined with fluid injection of hypertonic sugar (group A),40 cases were routine ultrasound guided drainage combined with ethanol injection therapy (group B),and 35 cases were routine ultrasound guided therapy alone (control group).The clinical efficacy and adverse reactions were compared among the three groups after treatment.Results The total effective rate of group A was 95.24%,group B was 97.50%,and control group was 82.86%.Total effective rate of group A and B had no significant difference (P>0.05),which were both higher than control group (P<0.05).The adverse reactions rate of group B was higher than group A and control group,which had statistical significance (P<0.05).Conclusion Ultrasound guided injection of hypertonic sugar or ethanol after drainage treatment for simple breast cysts can both obtain satisfactory clinical results,but adverse reactions of the former one are mild,it should be widely applied.
10.Preparation and In Vitro/In Vivo Evaluation of Celecoxib Self-microemulsifying Drug Delivery Systems

Objective To investigate celecoxib self-microemulsifying drug delivery system (CXB-SMEDDS) that was developed to increase the dissolution rate and oral bioavailability of celecoxib.Methods The formulation of CXB-SMEDDS was optimized by pseudo-ternary phase diagrams analysis.The appearance, morphology, particle size distribution and in vitro drug release behavior of CXB-SMEDDS were investigated after diluted by water.The bioavailability of CXB-SMEDDS was determined by oral administration to rats compared with CXB suspension.Results An optimized formulation was selected: Medium chain triglycerides as oil phase, Tween 20 as surfactant, Transcutol HP as cosurfactant.The ratio of oil phase, surfactant and cosurfactant was 2∶9∶9.Upon mixing with water, CXB-SMEDDS formed a clear and transparent microemulsion solution with homogeneous small spherical under transmission electron microscopy.For particle size of CXB-SMEDDS was found to be (57.6±14.2) nm.The in vitro dissolution test indicated a significant improvement in release characteristics of CXB.The AUC of CXB-SMEDDS and CXB suspension were (5.54±0.94) and (3.32±0.59) mg·L-1·h, respectively.The relative bioavailability was 166.9%.Conclusion The SMEDDS can significantly increase celecoxib dissolution in vitro and bioavailability in vivo.
