Objective To discuss the clinical features and related factors of psychosis in patients with Parkinson's disease (PD) and assess the prevalence of psychosis.Methods A total of 82 patients with Parkinson's disease and 69 healthy controls were evaluated by Brief Psychiatric Rating Scale and neuropsychiatric questionnaire.For the PD group,the age at PD onset,type and dose of PD medications,and rapid eye movement (REM) sleep behavior disorder (RBD) were recorded.Unified Parkinson's disease rating scale (UPDRS,parts Ⅱ and Ⅲ) and Hoehn-Yahr staging were used for the evaluation of the severity of motor dysfunction,Hamilton depression scale (HAMD) for emotional state,montreal cognitive assessment (MoCA) for cognition,and scales for outcomes in Parkinson's disease-SLEEP (SCOPA-SLEEP) for quality of sleep.Results The prevalence of psychosis was 36.59％ in PD patients and 2.90％ in the control group.The difference between the two groups was statistically significant (P＜ 0.001).Among the 30 PD patients with PD psychosis meeting the NINDS/NIMH criteria,patients experienced illusions,false sense of presence,hallucinations and delusions was 22.0％,14.6％,29.3％,and 7.3％,respectively.Univariate analysis showed that age,levodopa equivalent dosage,SCOPA-SLEEP scores,and prevalence of RBD were significantly higher in thePDP group [(71.1±6.18),(628.7±283.2),(15.3±7.28),60.00％] than in the PDNP group [(66.6±9.25),(461.1±317.2),(9.7±7.64),28.85％] (all P＜0.05).Multivariate analysis showed that Hoehn-Yahr staging was a protective factor (OR =0.039,95％ CI:0.004-0.402),while sleep disorder was a risk factor (OR=1.157,95％CI:1.015-1.320).The PD patients with more psychotic symptoms were much older and with higher SCOPA-SLEEP scores,which had a statistically significant difference (P =0.0042,P =0.0112).Conclusions The prevalence of psychosis is much higher in PD patients than in healthy controls.PD psychosis is correlated with older age,larger levodopa equivalent doses,higher SCOPA-SLEEP scores,and prevalence of REM sleep behavior disorder.

Objective To investigate the effects of anti-parkinsonian drugs on cytokines(IL-6 and TNF-?) produced by PBMC in PD patients. Methods 25 subjects were divided into 3 groups : normal controls(NC),pergolide and Madopar treated PD patients(P+M group),Madopar treated PD patients(M group). PBMC were cultured in the presence of pure RPMI-1640,RPMI-1640 with ConA and RPMI-1640 with pergolide,respectively. The amount of IL-6 and TNF-? in the culture media was measured by enzyme-linked immunosorbent assay (ELISA). Results IL-6 production amount by PBMC when stimulated with ConA was significantly lower in group P+M and M than in group NC( P 0.05). In group P+M,the concentration of TNF-? was significantly higher in the presence of ConA than in the presence of pure RPMI-1640 and in the presence of pergolide( P 0.05). Conclusions The ability of amount by PBMC of PD elderly patients to excrete IL-6 was significantly decreased in the presence of ConA. Pergolide might arise the excretion of IL-6 by PBMC during the treatment process of PD and at the same time does not arise the excretion of TNF-?,which might be neurotoxic.

BACKGROUND:The pathogenesis of knee osteoarthritis is stil not very clear, and there is a lack of early diagnosis methods. Because it is more common in middle-aged women, it is suggested that the pathogenesis of knee osteoarthritis may be closely related to serum estradiol levels in the human body. OBJECTIVE:To explore the correlation between the level of serum estradiol and the severity of osteoarthritis in female patients with knee osteoarthritis. METHODS:Seventy-six female patients with knee osteoarthritis admitted in the First Affiliated Hospital of Guangxi Medical University from February 2012 to December 2013 were enroled, and divided into premenopause group (n=42) and postmenopause group (n=34). According to Kelgren-Lawrence (K-L) grading, both two groups were subdivided into K-L 0, II and IV groups. The knee function was evaluated with Hospital for Special Surgery (HSS) Knee Score Standard in USA, the serum estradiol levels were detected with electrochemiluminescence immunoassay. Then, the correlation analysis was performed. RESULTS AND CONCLUSION: The serum estradiol levels of the premenopause group and postmenopause group were decreased significantly with the increase of K-L grade and age (P < 0.05). Under the same K-L grade, the serum estradiol levels in the postmenopausal patients were significantly lower than those in the premenopausal patients. Linear regression analysis showed that there was a negative relationship between age and serum estradiol level (R2=0.222,P < 0.05), and there was a positive correlation between HSS scores and serum estradiol levels (R2=0.415,P < 0.05). The severity of knee osteoarthritis in female patients shows a negative correlation with the level of serum estradiol but a positive correlation with age.

Objective To evaluate the therapeutic effect of percutaneous transhepatic biliary drainage (PTBD) with different approaches in patients with hilar cholangiocarcinoma type Ⅱ-Ⅳ.Methods The short-term clinical effect and longterm survival rate were analyzed retrospectively for a group of 97 patients with hilar cholangiocarcinoma type Ⅱ-Ⅳ receiving PTBD with different approaches,i.e.unilateral and bilateral biliary drainage,each including stent placement and drainage tube implantation.Results No significant difference was found in the decreasing of TBIL and DBIL,the survival rate and survival Curve between unilateral and bilateral biliary drainage.The median survival time was 7.5 months in unilateral and 6.7 months in bilateral biliary drainage,6.0 months in single stent and 6.5 months in single drainage tube,and 4.3 months in bilateral stents,respectively.The post operation complications mainly occurred in single drainage tube.Conclusion Single biliary drainage is able to decrease bilirubin for hilar cholangiocarcinoma type Ⅱ-Ⅳ,while stent implantation should be taken as the first choice.

Effects of total saponins from Prunella vulgaris (PVS)on acute myocardial infarction and hypertension of anesthetized rats were assessed. Pretreatment with PVS at a dose of 40mg/kg ip lowered Lg PVC, Lg (VT+VF) and ischemic arrhythmic scores; at a dose of 20mg/kg ip,resulted in a significant reduction of infarct size as compared with the controls; 2. 5mg/kg iv showed an hypotensive action to both systolic and diastolic blood pressure.

To explore the safety of ropinirole in the treatment of Parkinson′s disease( PD).A total of 221 PD patients participated in a multi-center,12-week randomized,bromocriptine-controlled,double-blind, double-dummy and parallel-group trial.The safety was assessed on the basis of adverse reactions , blood pressure,pulse,laboratory parameters and electrocardiography recordings.The incidence of adverse reaction was 34.9%in ropinirole group and 34.8% in bromocriptine group.And the frequency of adverse reactions had no inter-group statistical significance (χ2 =0.000,P=0.995).Ropinirole has an excellent profile of safety in the treatment of Chinese PD patients.

One hundred patients with Parkinson's disease (PD) aged 60 and above treated with levodopa were enrolled in this cross section study.The general conditions,medication,unified Parkinson's disease rating scale (UPDRS) scores and the incidence of levodopa-induced dyskinesia (LID) were documented.The incidence rate of LID in this group of PD patients was 37.0％ (37/100).The incidence was significantly higher in patients with levodopa treatment ≥ 4 years than that in patients with levodopa treatment ＜ 4 years (55％ vs.26％,x2 =8.770,P =0.003).The incidence rate ofpeak dosage dyskinesia in tremor-dominant PD patients was lower than that in rigidity-dominant PD patients(x2 =4.399,P =0.036).The incidence rate of LID was correlated with the duration of levodopa therapy.Amantadine may reduce the incidence of off dystonia.

ObjectiveTo analyze cerebral hemorrhagic infarction (HI) and relative factors with the database of Standard Stroke Registry (SSR).Methods1 487 consecutive patients with acute ischemic stroke admitted within 3 days after onset were analyzed with SSR.ResultsHI was observed in 11% of patients, of whom 51% were diagnosed as having cardiogenic embolism. In patients with supratentorial infarction of cardiac origin, 28% had HI (mild 67%, moderate 23, hematoma 10%). On multiple logistic regression analysis, independent factors related with HI were found to be age, prosthetic cardiac valve and NIHSS scores at admission. Patients with more severe HI were associated with a poorer outcome at discharge. Clinical outcome tended to be better in patients receiving thrombolytic agents than in those without receiving agents, while the incidence of HI was slightly more frequent in the former.Conclusion It is confirmed that the SSR database is useful for understanding and analyzing the status of stroke diagnosis and management throughout the nation, and revisions are needed in some formats.
